Abstract:
From a patient's perspective and experience endometriosis is characterized by six major challenges and stressors: 1) often the diagnosis is delayed; 2) the cause of the disease is unknown; 3) it is a chronic recurrent disease; 4) it affects young women; 5) it is accompanied by chronic pain and dyspareunia; 6) it may lead to infertility. These features create psychological needs for patients which should be responded to by an integrated basic psychosomatic care. This includes respect and empathy, patient centered communication, adequate response to negative emotions, establishment of a helpful and lasting therapeutic relationship, psychoeducation, empowerment and shared decision making. Apart from this basic psychosomatic care which can be provided by each treating physician or nurse, some conditions need more specialized psychosomatic care in the context of a multidisciplinary team: Chronic pain services, Infertility treatment teams, Sexual counselling and therapy and treatment of psychiatric comorbities or sequelae of the disease like depression, anxiety disorder etc. Psychosomatic care should thus be integrated into the biomedical diagnosis and therapy of endometriosis.

Abstract:
AbstractPostpartum depression (PPD) is the most common mental illness in mothers following the birth of a child. Since the symptoms of PPD are similar to the normal stress of healthy women following childbirth, it is often difficult for the attending gynaecologist or midwife to diagnose this illness in a timely manner and thus initiate adequate treatment and comprehensive support for the patient. Even if there are options for a screening using evaluated questionnaires and subsequent psychotherapy and/or drug therapy in the treatment of PPD which has proven effective, it is seen that, in most treatment approaches, little consideration is given to the affect-controlled interaction and the bonding behaviour between mother and child. This article presents diagnostic measures and current therapeutic approaches as well as their integration in practice in order to achieve awareness of this topic in everyday clinical practice and show the pathways of appropriate treatment. Specific multiprofessional treatment approaches which centre on the mother-child relationship demonstrate successes with regard to depression in the mothers and also on the development of a secure mother-child bond and are thus a protective factor in the development of the affected children. The now well-known effects of PPD on the fathers as well as the negative impacts of paternal depression on child development make it clear that the treatment should not focus solely on maternal depression, but also always on the family bond between the mother, child and father in the treatment.

In this work, we propose to study the theory of psychosomatics through the psychoanalytic lens applying to a life-generating context: the childbirth situation. So, changing the focus of the illness, we can comprehend that body manifestations during the childbirth process are intrinsically connected to unconscious drive of life and death. The sample for this study is comprised of 180 women in labor in a state hospital in the East Zone of Sao Paulo, mostly between 20 and 30 years old, in the first pregnancy; 90% of the births were normal and 75% of the women were together with someone of her choice. Obstetric data like weight, gestation age and others correspond of a low risk pregnancy. When we looked at the psychological characteristics of the women in the moment of childbirth, we verified that the raped women, in their vast majority, could be considered well mentalized, in other words, had mental stability to deal with the frustrations. Even with this condition, these women obtained the best obstetric results, as postulated by the French school of psychosomatics, confirming the theory that bad mentalization is a predisposition to somatization. The less experienced women in labor were the ones with more psychic resources, this let us conclude that the experience of a previous birth can lower the expectations, dreams and plans of a new birth. Thus, in this work we verified the importance of the oniric life as protection of the psychosomatics organization and consequently of the investment of the death drive in favor of the life drive<br>O termo psicossomática tem sido muito utilizado, principalmente como campo de estudo e pesquisa devido a ligação entre as ciências médicas e psicológicas. Neste trabalho, propomos estudar a teoria da psicossomática através da ótica psicanalítica aplicando-a a um contexto possibilitador do surgimento da vida: a situação do nascimento. Assim, deslocando o foco da doença, podemos compreender que as manifestações do corpo durante o trabalho de parto estão intrinsecamente ligadas a pulsões inconscientes de vida e morte. A amostra deste estudo é composta de 180 parturientes de um hospital estadual a Zona Leste de São Paulo, a maioria com 20 a 30 anos, na primeira gestação; 90% dos partos foi normal e 75% das mulheres estavam acompanhadas por alguém de sua escolha. Os dados obstétricos como peso, idade gestacional entre outros correspondem à uma gestação de baixo risco. Ao buscar as características psicológicas das mulheres no momento do trabalho de parto, foi possível verificar que as mulheres estudadas, em sua grande maioria, podem ser consideradas bem mentalizadas, ou seja, possuem recursos mentais para lidar com as frustrações. Ainda assim, essas mulheres obtiveram os melhores resultados obstétricos, como postulado pela escola francesa de psicossomática, confirmando a teoria que aponta que a má mentalização é uma predisposição à somatização. As parturientes menos experientes foram as que contaram com mais recursos psíquicos, isso nos leva a pensar que a vivência de um parto anterior pode diminuir as expectativas, sonhos e planos de um novo parto.
