Full textBackground: Obstructive sleep apnea (OSA) is a common and very spread disease associated with significant health problems, comorbidity and mortality. Most adults with OSA are without diagnosis and treatment. Primary care district nurses play an important role in identifying and guiding of patients with suspected OSA. Purpose: The purpose was to describe the district nurse's experience of identifying and guiding patients with suspected OSA in primary care. Method: A qualitative approach in the form of qualitative manifest content analysis was conducted where seven district nurses were interviewed. The analysis process was conducted according to Graneheim and Lundman's content analysis. Result: The study revealed five categories: to meet the patient, to ask the right questions, relative as motivator, to take workarounds and to interact with other care. District nurses described that they met the suspect OSA patients most often by telephone counseling or at the diabetes reception. The participants in the study illuminated the importance of how to ask the right questions to patients and to know the common signs of OSA. It was also found that relatives had an important role in identifying of patients with suspected OSA and they worked as motivators. The study also identified the strain for guiding patients with suspected OSA. Conclusion: In order to facilitate work with OSA- patients in the primary care, research is needed which is focusing on district nurses and common guidelines for investigating of patients with suspected OSA adapted for primary care staff.

Full textIntroduction: Obstructive sleep apnea is an illness which means that during nighttime-sleep, the patient experiences episodes in which she cease to breath. To the affected this can result in implications for both the somatic and the mental health. Earlier research also points towards the patient’s quality of life. The aim of this study was to examine how obstructive sleep apnea affect quality of life among patients in Sweden and if these patients experience effect on their mental health and if it’s differences between subgroups. Method: A quantitative cross sectional study was made. 19 patients with obstructive sleep apnea was participating and data was collected via forms that, beside age, sex, height, weight and day-sleepiness (Epworth sleepiness scale; ESS), also was based on the health related quality of life (SF-12) and mental health (HADS). Inclusion criteria was patients with nontreated sleep apnea that attended The Center for Sleep and Breathing at Akademiska sjukhuset to try out a CPAP. Collected data was analyzed with descriptive statistics by processing it in the computer program SPSS and comparisons between groups was made with Mann Whitney U-tests. Result: In the result it became apparent that the majority of the participants of the study had an inferior physical and mental health related quality of life and that the majority experienced anxiety. Women with obstructive sleep apnea showed more depression- and anxiety symptoms and patients with obesity had an inferior physical health related quality of life. Conclusion: Obstructive sleep apnea is tied to both health related quality of life and mental health. Further research and additional work with this patient group is needed to enhance the care of patients with obstructive sleep apnea.Keywords: Obstructive sleep apnea, quality of life, mental hea

Full textThe purpose of this study was to investigate the patient’s experiences in treatment of obstructive sleep apnoea with a mandibular advancement oral appliance conducted in general dental practice. The study was conducted to study a cross-section of patients in one of the major regions of Sweden, region Scania. The study was retrospective in its design and consisted of a postal questionnaire to 1148 patients with a treatment period of more than 6 months. The questionnaire comprised 70 questions and assertions in various domains: general health/ lifestyle, use of oral appliance, changes in symptoms, sleep-related experience changes, evaluation of treatment, value of treatment, changes in life situation and daytime sleepiness. The responder rate was 64%. Treatment with oral appliance gave relief of symptoms in the majority of patients with obstructive sleep apnoea. Quality of life, somatic and cognitive symptoms changed significant positively in patients who used the treatment frequent. The daytime sleepiness was reduced for the majority of the treated patients. The satisfaction with the treatment and the will to recommend a similar treatment for a friend was high. Treatment of obstructive sleep apnoea with an oral appliance performed in general dental practice relieved the condition and its consequences according to the patient's own judgement.

Full textBackground: Obstructive sleep apnea (OSA) is a disease with repeated episodes of partially or completely blocked airways during sleep, which leads to a lower blood saturation. OSA is known to decrease cognitive abilities like memory and learning. There is a lack of studies which investigate if there is an association between cognitive function, symptoms and physical activity in patients with OSA. Purpose: To describe the cognitive function in a group of patients who are overweight and has OSA. Examine the association between cognitive function (short-term memory) and moderate to vigorous physical activity (MVPA) and examine the association between cognitive function and severity of OSA. Method: This paper is a cross-sectional study with correlational design. The severity of the disease was measured with apnea-hypopnea index (AHI). The cognitive function was measured by digit span test and physical activity with an accelerometer. The study includes 86 patients diagnosed with OSA (AHI >15), with a BMI >25 and a self-assessed moderate intensity physical activity less than 150 minutes/week. Results :The digit span test in this study showed a mean value of 15,7. There was no significant correlation (r=-0.10, p=0.38) between amount of MVPA and short-term memory in patients with OSA (p=0.38). There was neither any significant correlation (r=0.09, p=0.40) between patients AHI and the cognitive functions. Conclusion: The amount of physical activity didn’t seem to have any effect on cognitive function. There doesn't seem to be any association between AHI and patients cognitive function. Further research is needed to confirm the results from this study.

Full textBackground: Obstructive sleep apnea (OSA) is obstruction of airflow during sleep leading to respiratory arrest. OSA is most common with obesity and among men. Individuals with OSA have a higher depression rate than the rest of the population. Physical activity (PA) has proven to be an effective treatment for depression. Aim: To investigate whether change in PA correlates with change in self-assessed depressive symptoms and if there are differences regarding gender or intensity levels. Method: A retrospective, descriptive, correlative and comparative study. Data was obtained from a previous study. PA and self-assessed depressive symptoms were measured with SenseWear Pro3 Armband and MADRS-S at baseline and at 6 months follow-up. Results: There was a correlation between an increase of PA and a decrease of MADRS-S score, r = -0.27, p = 0.04. The correlation for men was stronger r = -0.35 (p = 0.02). Women had a correlation between an increase in PA and an increase of MADRS-S score, r = 0.7 not statistically significant, p = 0.16. There was no difference in MADRS-S score between the groups performing PA at moderate intensity 5 (5.5) median (interquartile), respectively high intensity 4.5 (9.25), p=0.68 Conclusion: The results showed a correlation between an increase in PA and a reduction of MADRS-S score. Regarding differences between genders or intensity levels the low number of women and physically active at high intensity meant no conclusions could be drawn.

Full textMedical research in recent years has shown that Obstructive Sleep Apnea Syndrome, OSAS, is an affliction for which both diagnosis and treatment are often inadequate. In part, this is due to the fact that many people suffering from OSAS are not aware of the problem. The disease causes recurrent complete stops of breathing during sleep and hypoxia that may bring about various complications, for instance cardiovascular disorders. The purpose of this study, carried out as a literature survey, was to determine what factors affect OSAS and what role the nurse may play in the care of patients suffering from this disease. The survey of the literature by means of the data bases PUBMED and SAMSÖK yielded ten scientific articles showing that although various medical measures constitute the treatment of choice of OSAS the nurse plays an important role in the treatment and the diagnosis of new cases. Preventive care measures such as information about appropriate sleeping positions and reduction of weight have proven to be of significant value in the treatment of OSAS.
