Full textObstruktivt sömnapnésyndrom (OSAS) innebär upprepade andningsuppehåll under natten tillsammans med extrem dagen efter-trötthet. Fortfarande finns det många personer med ett odiagnostiserat OSAS. Syftet med litteraturstudien var att belysa hur personer med obstruktivt sömnapnésyndrom upplever sin livskvalitet. I metoden har 18 artiklar bearbetats. Resultatet visar på en försämrad livskvalitet hos personer med OSAS. Livskvaliteten påverkas av fysiska, psykiska och sociala aspekter. Personer med OSAS påverkades i sitt vardagliga liv då tröttheten begränsar deras livsstil. En efterfrågan av relevant information kan upptäckas i denna patientgrupp. Genom CPAP-behandling kan livskvaliteten öka. Sjuksköterskan kan genom extra stöd och undervisning i samband med diagnostisering och behandling, ytterligare öka patientens livskvalitet. Mer kunskap behövs hos sjukvårds¬personalen om OSAS och CPAP-behandlingar. Detta för att kunna ge undervisning och stöd till patient och anhöriga. Genom en utvecklad sömnanamnes skulle fler patienter med OSAS kunna upptäckas och få hjälp. Det finns ett behov av mer forskning om OSAS ur ett sjuksköterskeperspektiv samt om patienters upplevelser av OSAS.

Full textAim: To evaluate the long-term effect of treatment with an oral appliance in patients withobstructive sleep apnea syndrome (OSAS) at the Department of Orofacial Pain and JawFunction, Faculty of odontology, Malmö University, and to investigate which subjective andclinical factors can predict a successful treatment outcome and whether side effects occur. Material and method: In a retrospective quality study, 183 consecutive patient records wereevaluated between 2017/01-2020/05. Data from the records were examined for subjectivesymptoms and clinical findings in order to answer stated questions. Data were obtained froma standardized inquiry and a clinical examination from baseline (n=183), 3 months (n=143)and a one-year follow-up (n=93). The data material was analysed with SPSS 27. Results: A good treatment effect was reported by 66% at 3 months and 64% at 1 year followup. A significant sustained improvement in morning sleepiness(p<0.000), daytime sleepiness(p<0.000), and sleepiness (p<0.000) was recorded. Sleepiness was graded according to theEpworth sleep index. The number of awakenings per night decreased (p<0.000). Compliancewas good, 71% of patients used the splint 6-7 nights a week. Most common dental sideeffects were transient pain in the masticatory system. Dental side-effects in the form ofreduced vertical and horizontal overbite occurred in 22 respectively 14 patients at the 3months follow up. No predictive factors could be found in this study. Conclusion: The majority of patients experienced a good treatment effect, had reducedmorning fatigue and daytime sleepiness and showed a good consistency of using an oralappliance. No predictive values could be identified for good treatment outcomes. Pain anddental changes were reported by one in five patients, decreased overbite and/or overjet wasrecorded in approximately one in 10 patients.

Background: Obstructive Sleep Apnea Syndrome (OSAS) is prevalent and the syndrome is associated with increased risk of health related problems. A positive effect of behavioral change programs to reduce weight is expected. More research is needed to identify barriers and facilitators to behavior change.
Aim: To identify prerequisites for eating behavior change in obese patients with OSAS.
Method: Semi-structured interviews were conducted on 15 obese patients living with OSAS. Data were analyzed using qualitative content analysis.
Result: The categories identified as barriers to change in eating behavior in the analysis were desire and craving, influenced by mental state, low self-confidence, insufficient support, social acceptance, complexity of coping with new eating behaviors in daily routines, costs, lack of knowledge, perceived helplessness and low susceptibility. Categories that were identified as facilitators to change in eating behavior were high value of outcome expectations, support and control, external premises, applicable behavior change strategies, self-worth and being triggered by weight increase.
Conclusion: To motivate patients living with OSAS to change their eating behavior it is necessary to address both barriers and facilitators. The categories low susceptibility as a barrier and high value of outcome expectations as a facilitator appears to be important for this patient group.
Sammanfattning
Bakgrund: Obstruktiv sömnapnésyndrom (OSAS) är vanligt förekommande och syndromet är associerat med hälsorelaterade problem. Det finns en förväntad positiv effekt av beteende förändrande interventioner som syftar på viktminskning. Mera forskning behövs för att identifiera hinder och underlättande faktorer.
Syfte: Att identifiera förutsättningar för att ändra matvanor hos feta patienter med OSAS.
Metod: Semistrukturerade intervjuer utfördes på 15 feta patienter med OSAS och kategorier identifierades med hjälp av en innehållsanalys.
Resultat: Följande kategorier identifierades de som hinder till förändrade matvanor i analysen; begär och sug, påverkan av sinnestillstånd, lågt självförtroende, otillräcklig stöd, social acceptans, komplexiteten i att klara av nya beteenden i dagliga rutiner, kostnader, brist på kunskap, upplevd hjälplöshet och låg mottaglighet. De kategorier som underlättade ändrade matvanor i studien var följande: högt värde av förväntningar, stöd och kontroll, externa faktorer, användbara strategier för ändringar i beteende, självkänsla och viktökning som utlösare.
Slutsats: För att framgångsrikt motivera patienter med OSAS att ändra sina matvanor måste hänsyn tas både till hinder och till faktorer som underlättar ett förändrat beteende. Kategorierna ”låg mottaglighet” som ett hinder och ” högt värde av förväntningar” som underlättande faktor tycks vara viktiga för denna patientgrupp.

Full textMany patients with obstructive sleep apnea syndrome (OSAS) treated with Continuous Positive Airways Pressure (CPAP) interrupt their treatment at an early stage. Not having adherence to the treatment implies various health risks for the patient that can have serious consequences. Therefore, the communicationsituation at the initial testing visit may affect whether the patient does not follow up and completes the treatment. The purpose of this study was to illustrate how healthcare professionals describe their experiences of the communicationsituation at the initial visit testing CPAP. A qualitative interview study was conducted with 23 people who all meet this group of patients. As an analytical method, inductive content analysis was used. In the analysis of the material three categories was revealed; Build a Relationship, Take an Educational Approach and Support the Patient's Participation. These three categories emerged in a theme that demonstrated how the communication situation was described as Creating a prerequisite for learning for the patient. The study highlights the importance of a person-centered approach, a co-productive care where the patient is seen as a partner in the decisions and how the healthcare professionals' pedagogical approaches affect the communicationsituation in different ways. The experiences described in the study were perceived to affect the patient's ability to learn in the situation for an increased adherence to the CPAP treatment.
