Abstract:
Allelic exclusion is established in development through a feedback mechanism in which the assembled immunoglobulin (Ig) suppresses further V(D)J rearrangement. But Ig expression sometimes fails to prevent further rearrangement. In autoantibody transgenic mice, reactivity of immature B cells with autoantigen can induce receptor editing, in which allelic exclusion is transiently prevented or reversed through nested light chain gene rearrangement, often resulting in altered B cell receptor specificity. To determine the extent of receptor editing in a normal, non-Ig transgenic immune system, we took advantage of the fact that λ light chain genes usually rearrange after κ genes. This allowed us to analyze κ loci in IgMλ+ cells to determine how frequently in-frame κ genes fail to suppress λ gene rearrangements. To do this, we analyzed recombined VκJκ genes inactivated by subsequent recombining sequence (RS) rearrangement. RS rearrangements delete portions of the κ locus by a V(D)J recombinase-dependent mechanism, suggesting that they play a role in receptor editing. We show that RS recombination is frequently induced by, and inactivates, functionally rearranged κ loci, as nearly half (47%) of the RS-inactivated VκJκ joins were in-frame. These findings suggest that receptor editing occurs at a surprisingly high frequency in normal B cells.

Abstract:
Background and Purpose: The impact of coronavirus disease 2019 (COVID-19) on the occurrence of ischemic stroke has been the subject of increased speculation but has not been confirmed in large observational studies. We investigated the association between COVID-19 and stroke. Methods: We performed a cross-sectional study involving patients discharged from a healthcare system in New York State, from January to April 2020. A mixed-effects logistic regression analysis and a propensity score–weighted analysis were used to control for confounders and investigate the association of COVID-19 with ischemic stroke. Similar techniques were used to detect the impact of concurrent COVID-19 infection on unfavorable outcomes for patients with stroke. Results: Among 24 808 discharges, 2513 (10.1%) were diagnosed with COVID-19, and 566 (0.2%) presented with acute ischemic stroke. Patients diagnosed with COVID-19 were at one-quarter the odds of stroke compared with other patients (odds ratio, 0.25 [95% CI, 0.16–0.40]). This association was consistent in all age groups. Our results were robust in sensitivity analyses, including propensity score–weighted regression models. In patients presenting with stroke, concurrent infection with severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) was associated with higher case-fatality (odds ratio, 10.50 [95% CI, 3.54–31.18]) and a trend towards increased occurrence of discharge to rehabilitation (odds ratio, 2.45 [95% CI, 0.81–1.25]). Conclusions: Using a comprehensive cross-section of patients from a large NY-based healthcare system, we did not identify a positive association between ischemic stroke and COVID-19. However, patients with stroke with COVID-19 had worse outcomes compared with those without, with over a 9-fold increase in mortality. Although no definitive conclusions can be reached from our observational study, our data do not support the concerns for an epidemic of stroke in young adults with COVID-19.

Abstract Background Crohn’s disease (CD) is an inflammatory disease of the gut with multiple psychosocial comorbidities, including lost wages, stress on loved ones, and psychiatric illnesses, all of which are associated with substance use disorders. While approximately 7.5% of individuals aged 12 or older in the United States have a substance use disorder, we know little about this issue among patients with CD. Using a nationally representative dataset, we examined the burden of alcohol, tobacco, and illicit drug use among patients with CD in the United States. Methods We queried the national Medicaid claims data from 2010 through 2014 using ICD-9-CM codes for CD and use of alcohol, tobacco, opioids, cocaine, amphetamine, and cannabis. We defined CD as having two or more CD diagnostic codes; all included individuals must have at least one continuous year of Medicaid eligibility. The primary outcomes of interest were the total and the substance-specific prevalence of use in patients with CD. Results During our five-year study period, there were 51,091 individuals with CD. Among them, 7.4% had at least one code to suggest substance use, and 1.3% had codes for two or more substance types. Non-users were 65% female, 39-years old at the first CD diagnosis, and had 1.7 years of median follow-up after the said diagnosis. Users were 62% female, 40-years old at the first CD diagnosis, and had 1.8 years of median follow-up after diagnosis. The frequencies of substance use codes by CD patients were tobacco (3.9%), opioids (2.2%), alcohol (1.6%), cannabis (0.4%), cocaine (0.3%) and psychostimulants (0.3%). Conclusion At least 1 in 13 patients with CD in the US Medicaid cohort noted substance use from 2010 through 2014. Our finding was comparable to the reported prevalence of substance use disorders in the general US population.

Background: The study purpose was to examine the incidence of hypoglycemia in very low birth weight (VLBW) infants with cholestasis while on complete enteral nutrition.Methods: A retrospective study of 270 VLBW (<1500 grams) infants born between 2008 and 2012 at York Hospital with cholestasis was performed. A blood glucose concentration ≤50 mg/dl was used to define hypoglycemia, and hypoglycemic events were recorded while infants were on full enteral feeds. Characteristics of infants with cholestasis were compared with those without cholestasis.Results: Cholestasis was noted in 9.6% (26/270) of VLBW infants, four babies were excluded. Twenty-two infants with cholestasis were analyzed and compared. Among those with cholestasis, hypoglycemic episodes occurred in 12 (54.5%) infants at 17±13 days (mean ±SD) after being on exclusive enteral nutrition and at a post-conception age between 31-42 weeks. Three infants (25%) needed transient reintroduction of parenteral glucose and/or alteration of feeding regimen to correct hypoglycemia. In contrast, the incidence of hypoglycemia in the control group (VLBW infants without cholestasis on full enteral feeds) was 4.5% (3/67) (P= <0.001). Receiver operating characteristic curve analysis showed a peak direct bilirubin of >4.1mg/dl (before full enteral feeds) predicts hypoglycemia while on full enteral feeds, with a sensitivity of 100%, specificity of 50%, and negative predictive value of 100%.Conclusions: Hypoglycemia is an unrecognized complication occurring in a high percentage of VLBW infant with history of cholestatic jaundice while receiving full enteral feeds. We propose that care-givers in the neonatal ICU monitor glucose levels in this select group of VLBW infants to avoid recurrent asymptomatic hypoglycemia.

Lapse rate tropopause (LRT) heights in the extratropics have been shown to display a bimodal distribution, with one modal maxima above 15 km (typical of the tropical tropopause) and the other below 13 km (typical of the extratropical tropopause). The climatology of the tropopause is studied by characterizing tropopause bimodality and how it relates to the occurrence of double tropopauses (DTs). LRT heights are derived from Constellation Observing System for Meteorology, Ionosphere, and Climate (COSMIC) GPS Radio Occultation temperature profiles from 2006 to 2017. Tropopause bimodality occurs most frequently within a subtropical band (20°–40°) in both hemispheres. A distinct seasonality is observed as bimodality occurs most frequently in winter except for another local maximum along the northern edge of the Asian summer monsoon. The regions with a bimodal height distribution nearly overlap the regions that experience a high frequency of DTs. DTs occur most frequently in winter (50%–70% of the time) along the poleward edge of the bimodal band, and most LRT heights are within the extratropical mode (>80%), whereas DT occurrence decreases quickly toward the equatorward edge (<20%) along with fewer LRT heights in the extratropical mode (<50%). These results indicate that LRT height bimodality occurs along the equatorward edge due to the occurrences of double tropopauses, while the poleward edge is due to single tropopause profiles that are more tropical in nature.

For acquiring important corre-lations among the information, method called itemset mining plays vital role Earlier, the notion of itemset mining was used to acquire the absolute most often occurring items in the itemset. In some situation, though having utility value less than threshold it is necessary to locate such items because they are of great use. Considering the thought of weight for each and every apparent items brings effectiveness for mining the pattern efficiently. Different mining algorithms are utilized to obtain the correlations among the information items based on frequency with the items in the dataset occurs. In frequent itemset, those things which occurs frequently whereas, in infrequent itemset the items that occur very rarely are obtained. Determining such form of data is tougher than to locate data which occurs frequently. Frequent Itemset Mining (FISM) locates large and frequent itemsets in huge data for example market baskets. Such data has two properties that are not addressed by FISM; Mixture property and projection property. Here the proposed system combines both mixture as well as projection property further providing automated support thresholds.

ABSTRACT We have examined meiotic and mitotic recombination between repeated genes on nonhomologous chromosomes in the yeast Saccharomyces cerevisiae . The results of these experiments can be summarized in three statements. First, gene conversion events between repeats on nonhomologous chromosomes occur frequently in meiosis. The frequency of such conversion events is only 17-fold less than the analogous frequency of conversion between genes at allelic positions on homologous chromosomes. Second, meiotic and mitotic conversion events between repeated genes on nonhomologous chromosomes are associated with reciprocal recombination to the same extent as conversion between allelic sequences. The reciprocal exchanges between the repeated genes result in chromosomal translocations. Abstract:
The IGF-II gene normally exhibits genomic imprinting, a DNA modification that allows the expression of only one of the two inherited alleles. With loss of imprinting, there is a gain of allelic gene expression (GOAGE) due to IGF-II being expressed by both alleles. GOAGE for IGF-II has been demonstrated in a number of malignancies and in normal epithelia surrounding malignancies, but not in epithelia without associated neoplasia. We hypothesized that nonneoplastic Barrett's epithelium might have GOAGE for IGF-II that could facilitate its progression to neoplasia. Endoscopic biopsies were obtained from metaplastic esophageal, normal gastric, and normal duodenal epithelia from 43 patients with Barrett's esophagus. Genomic DNA were analyzed using PCR followed by ApaI restriction enzyme digestion or allele-specific PCR to identify an ApaI polymorphism of IGF-II. cDNA from patients with the ApaI polymorphism were analyzed for IGF-II GOAGE using exon connection PCR, followed by a secondary nested PCR and ApaI restriction enzyme digestion. We found that 13 (30%) of 43 samples of Barrett's metaplasia contained the ApaI polymorphism and were thus informative for IGF-II, and sufficient material was available for GOAGE analysis in 9 of those 13 cases. GOAGE for IGF-II was demonstrated in five (56%) of those nine cases. All patients with GOAGE in Barrett's metaplasia also demonstrated GOAGE in the gastric and duodenal epithelia. In contrast, patients without GOAGE in Barrett's metaplasia also had no GOAGE in their gastric and duodenal epithelia. We conclude that in patients with Barrett's esophagus, GOAGE for IGF-II is found frequently in the metaplastic esophageal epithelium as well as in normal gastric and duodenal epithelia.
