Purpose – The purpose of this paper is to investigate the agreement between supervisors and subordinates concerning the motives of organizational citizenship behaviors (OCB) and how the supervisors’ attributions affect their OCB ratings. Design/methodology/approach – With the permission of seven large organizations in Macau, the authors conducted a survey of 500 employee-supervisor-co-worker triads. The final sample stood at 176 such triads with three hypotheses tested. Findings – First, supervisors are more accurate when judging altruistic motives of subordinates’ OCB than with egoistic motives. Second, supervisor attribution of subordinates’ altruistic motives positively affects the supervisors’ OCB ratings. Third, employees who are motivated by altruistic motives perform more OCB actions those egoistically motivated. Originality/value – The study adds to knowledge of how supervisor attribution of subordinates’ OCB motives affects their evaluation of the subordinates. It also provides evidence about the effect of OCB motives on the actual engagement in OCB. Findings of this study support the work of Organ et al. regarding the motives behind OCB and strengthen the role of attribution theory in studying OCB.

Abstract:
A bis(terpyridine)ruthenium(II) complex ([Ru]2+) was covalently connected via a floppy - OCH 2 CH 2 O - spacer to the free-base porphyrin (H) or zinc(II) porphyrin (Zn) or both, to obtain dyads ([HRu]2+, [ZnRu]2+) and triads ([HRuH]2+, [ZnRuH]2+, [ZnRuZn]2+). These compounds have been fully characterized by MALDI, UV-vis, 1 H NMR (1D and 1 H -1 H COSY) spectroscopies, and also by the cyclic and differential pulse voltammetric techniques. Absorption spectroscopy of these newly synthesized compounds shows that significant exciton coupling exists in non-polar solvents (cyclohexane and toluene) between the porphyrin ring and the bis(terpyridine)ruthenium(II) complex. Upon excitation within the Soret band of [HRu]2+/[HRuH]2+, free-base porphyrin fluorescence was found to be strongly quenched in non-polar and weakly quenched in polar solvents, probably due to ‘singlet-triplet’ energy transfer from the free-base porphyrin to the [Ru]2+ complex. Whereas, in [ZnRu]2+/[ZnRuZn]2+, zinc(II) porphyrin fluorescence was quantitatively and reasonably quenched in non-polar and polar solvents, respectively by mainly electron transfer from the zinc(II) porphyrin to the [Ru]2+ complex. The solvent plays a crucial role in the photophysical properties of these compounds, since the energy of the triplet metal-to-ligand charge-transfer (3MLCT) excited state is influenced by the polarity of the medium. Finally, [ZnRuH]2+ exhibits the combined fluorescence properties of [HRu]2+ and [ZnRu]2+ but the observed additional quenching in non-polar solvents for the zinc(II) porphyrin component is explained by energy transfer from the zinc(II) porphyrin to the free-base porphyrin and/or the bis(terpyridine)ruthenium(II) complex.

I en studie av hjärnskadevård visas hur en sådan förändring ägt rum varvid de anhöriga gavs nya sociala betydelser. I denna process skedde såväl en integrering som en aktiv distansering i relation till de anhöriga från personalens sida.

Abstract:
Abstract Discovering oral cavity cancer (OCC) at an early stage is an effective way to increase patient survival rate. However, current initial screening process is done manually and is expensive for the average individual, especially in developing countries worldwide. This problem is further compounded due to the lack of specialists in such areas. Automating the initial screening process using artificial intelligence (AI) to detect pre-cancerous lesions can prove to be an effective and inexpensive technique that would allow patients to be triaged accordingly to receive appropriate clinical management. In this study, we have applied and evaluated the efficacy of six deep convolutional neural network (DCNN) models using transfer learning, for identifying pre-cancerous tongue lesions directly using a small dataset of clinically annotated photographic images to diagnose early signs of OCC. DCNN models were able to differentiate between benign and pre-cancerous tongue lesions and were also able to distinguish between five types of tongue lesions, i.e. hairy tongue, fissured tongue, geographic tongue, strawberry tongue and oral hairy leukoplakia with high classification performances. Preliminary results using an (AI + Physician) ensemble model demonstrate that an automated pre-screening process of oral tongue lesions using DCNNs can achieve ‘near-human’ level classification performance for diagnosing early signs of OCC in patients.

ObjectiveIn order to meet local mental health surveillance needs, we created multiple mental health-related indicators using emergency department data from the Colorado North Central Region (CO-NCR) Early Notification of Community Based Epidemics (ESSENCE), a Syndromic Surveillance (SyS) platform.IntroductionMental health is a common and costly concern; it is estimated that nearly 20 percent of adults in the United States live with a mental illness[1] and that more money is spent on mental illness than any other medical condition.[2] One spillover effect of unmet mental health needs may be increasing emergency department utilization. National analysis by Healthcare Cost and Utilization Project (H-CUP) found a 55% increase in emergency department visits for depression, anxiety, and stress reactions between 2006-2013.[3] Local public health agencies (LPHAs) can play an important role in reducing costs and burden associated with mental illness. There is opportunity to use emergency department data at a local level to monitor trends and evaluate the effectiveness of local strategies. ESSENCE, available in 31 states, provides near-real time observation-level emergency department data, which can be analyzed and disseminated according to local needs. Using ESSENCE data from 6 local counties in Colorado, we developed methods to estimate the overall burden of mental health and specific mental health disorders seen in the emergency department.MethodsBoulder County Public Health expanded on existing methods to develop multiple mental health queries in ESSENCE using data from the six Colorado counties that currently participate in the Colorado North Central Region (CO-NCR) SyS (i.e., Adams, Arapahoe, Boulder, Denver, Douglas, and Jefferson Counties). Our query was based solely off relevant International Classification of Disease version 10 Clinical Modification (ICD-10-CM) mental health codes: F20-F48, F99, R45.851, X71–X83, T14.91, and R45.851. We also included T36-T65 and T71 where intentional self-harm was specified. In addition to an overall mental health query we created 11 sub-queries for: anxiety disorder, conversion disorder, intentional self-harm/suicide attempt, mood disorder, obsessive compulsive disorder (OCD), dissociative disorder, schizophrenia, somatoform disorders, stress adjustment disorder, suicide ideation, and other mental health disorder). One observation could fall into multiple subcategories through inclusion of multiple discharge diagnosis (DD).One challenge of using the DD field in ESSENCE is that in Colorado, similar to other states, there can be excess of 40 unique ICD-10-CM codes listed in the DD field, and queries identify cases by searching all listed codes. For this project, that is problematic as codes may refer to historic and underlying health conditions, rather than acute cause of the ED visit. To handle this, we performed a secondary analysis to determine whether observations were “true mental health cases” based on order of codes listed in DD field, triage notes and chief complaint. We then calculated sensitivity, specificity, positive predictive value (PPV) and negative predictive value(NPV) of including observations where mental health was listed as the first (or primary) code, first or second, or first second or third code. Our analysis revealed that observations where mental health codes are listed later were less likely to be identifiable as true mental health cases, and led to our decision to only include observations with qualifying codes listed first or second.To assess the mental health burden, we developed code in SAS 9.4 that parsed ESSENCE output by discharge diagnosis, create aforementioned sub-queries, and calculated counts and age-adjusted rates (based on 2000 US Population) to summarize demographic and geographic trends.ResultsThere were 22,451 observations with mental health discharge diagnosis codes for the six Colorado counties between January and June 2018. Of these codes, 13,331 had a mental health code as the first and/or second listed DD and were counted as true mental health visits. The age-adjusted rates of any mental health visit ranged from approximately 425 per 100,000 in Douglas County to 1,026 per 100,000 in Denver County. The most common reasons for mental health visits across the region were anxiety, mood disorder, and suicide ideation (Figure 1). There was a significant spike in mental health ED visits among the 15-24 age group, followed by decreasing rates in older age groups (Figure 2). Younger age groups most commonly had ED visits for mood disorder (all age groups under 24), while in the age groups 25-34, 35-44, 65-74 and 75+ the most common reason for ED visit was anxiety. Also of note, ED visits for suicide ideation and self- harm were highest for the 15-24 age group. Males and females had similar rates of ED visits for most diagnoses, which is notable given males generally utilize healthcare services at lower rates than females.ConclusionsSyndromic surveillance is a valuable addition to available mental health surveillance. Our methods and results demonstrate the feasibility of tracking overall and specific mental health trends using the ESSENCE platform. Unlike other available mental health data, ESSENCE provides data that is local, observation level, and near-real time. Through continued collaboration with public health, medical and other stakeholders we hope this data can be pivotal in gauging disparities in mental health burden, monitoring trends, and prioritizing solutions.References[1] Mental Illness. Abstract:

Begreppet triage är centralt inom katastrofmedicin. Syftet med triage är att utnyttja de medicinska resurserna optimalt, samt att vid masskade- och katastrofsituationer differentiera drabbade med sannolik överlevnad från de med förestående död. Det är av yttersta vikt att den som utför triage är införstådd med de etiska värderingar som styr tillvägagångssättet samt det triagesystem som används. Syftet med litteraturöversikten var att beskriva hur triage används i katastrof- och masskadesituationer på skade- och uppsamlingsplats. Genom strategiskt urval har retrospektiva studier identifierats. Till resultatet har tolv katastrof- och masskadehändelser sammanställts och analyserats med fokus på förekomst av triage på skade – och uppsamlingsplats, triagesystem och över- och undertriage. Vid de välfungerade insatserna utnyttjades de medicinska resurserna effektivt, en väletablerad och genomförd triagestrategi användes. Vid de moderata insatserna förekom såväl över- som undertriage. De mindre välfungerande insatserna karaktäriserades av att ingen triage eller recusitering av de drabbade utfördes, arbetet var oorganiserat och drabbade transporterades på ett ostrukturerat sätt. Faktum är att litet finns dokumenterat kring användandet av triage på skade- och uppsamlingsplats, det existerar litet forskning samt litet eller ingen validering kring de triagesystem som används idag. Ytterligare forskning krävs för att förstå och överföra erfarenheter från reella händelser.


The concept triage is central within disastermedicine. It alludes the medical recourses optimally, and in mass casualties- or disastersituations it differentiates victims who are likely to survive from those with imminent death. It is of outmost importance that the person performing triage is involved with the ethical value that runs the procedure and the practicing triagesystem. The aim with this literaturereview was to describe how triage is performed on scene and in treatment area. Through strategic sampling retrospective studies where identified. To the result a total of twelve disaster- and mass casualties situations has been complied and analyzed focused on the presence of triage on scene or in treatment area, triagesystem, and over- or undertriage. Within the well functioning efforts the use of the medical resources where effectively, a well established and well implemented triagestrategi where used. Within the moderate efforts it occurred as well over- as undertriage. The efforts that did not functioned well where characterized by no performing triage or resuscitation of the victims, unorganized performance on scene and unstructured transportation of the victims. It is a fact that a little has been documented about performing triage on scene or in treatment area, there exists few research efforts, and little or nothing about validations of the existing triagesystem that are practiced today. More research is needed to gain understanding and alienate experiences from intrinsically events

Patientflöden till Sveriges akutmottagningar ökar varje år. För att ta emot och bedöma pati­en­ter på ett patientsäkert sätt används triage. En välfungerande triageprocess är en förut­sätt­ning för ett patientsäkert arbete på akutmottagning. Syftet med denna studie var att göra en granskning av triageprocessen på akutmottagning, uti­från sjuksköterskors och läkares uppfattningar kring utbildning, förtroende och följsam­het.  Metoden var en deskriptiv tvärsnittsstudie baserad på en webbaserad enkätundersökning ställd till sjuksköterskor och läkare som arbetade inom triageprocessen på två sjukhus i Mel­lansverige. Inkluderade respondenter uppgick till n=191. Data har analyserats med hjälp av kvantitativ- och kvalitativ metod. Enkätfrågor med slutna svarsalternativ har analyserats kvantitativt med Chi-2-test [Χ2] och presenteras med deskriptiv statistik. Fritext­svar i enkä­ten har analyserats kvalitativt med manifest innehållsanalys.  Resultatet visar att det finns brister i triageutbildningen på akutmottagningarna som deltog i studien. Framförallt uppger läkarna att de saknar utbildning i triageverktyg och triagepro­cessens rutiner. Respondenternas svar visar att det finns brister i hur väl innehållet i utbild­ningen stämmer överens med hur arbetssättet tillämpas i den kliniska verksamheten och att respondenterna inte känner sig väl förberedda inför att börja arbeta med triage i klinisk verk­samhet. Patienternas sjukdomstillstånd anses inte identifieras adekvat av triageverktyg eller under triageprocess. Utifrån aspekter kring övertriagering samt identifiering av om­vårdnads­behov och medicinska behov har SATS högre förtroende hos sjuksköterskorna än DPT. Dock uppger respondenterna på båda akutmottagningarna att omvårdnadsbehov och medi­cinska behov inte fullt ut identifieras av triageverktyg eller under triageprocessen. Få respon­denter uppger att informationsöverföring i triageprocessen fungerar väl på deras ar­betsplat­ser. Det förekommer att respondenterna avviker från triageverktyget, triageproces­sens ruti­ner samt prioriteringsordning. Respondenternas uppfattningar är att läkare frångår oftare än sjuksköterskor. Sjuksköterskor som arbetar med sjuksköterskeledd triage anser i högre grad än övriga respondenter att bristande följsamhet till triageprocessens rutiner på­verkar patient­säkerheten på deras arbetsplats.  Slutsatsen är att det finns brister i triageprocessen på akutmottagningarna i studien som skulle kunna inverka negativt på patientsäkerheten.
Inflow of patients to Sweden’s emergency departments are increasing every year. In order to receive and assess patients in a patient-safe manner, triage is used. A well-functioning triage process is a prerequisite for patient-safe work at the emergency department. The aim of this review was to study the triage process in the emergency department based on nurses’ and physicians’ perceptions of education, confidence and compliance.  The method was a descriptive cross-sectional study, based on a web-based survey sent to nurses and physicians active in the triage process in two hospitals in Sweden. Respondents included amounted to n=191. Data have been analyzed using quantitative and qualitative meth­ods. Survey questions with closed answer options have been analyzed quantitatively with Chi-squared test [Χ2] and is presented using descriptive statistics. Free text replies have been analyzed qualitatively with manifest content analysis.  The results show that there are shortcomings concerning the triage education at the emer­gency departments participating in this study. Especially physicians state that they lack ed­ucation regarding the triage tool and in the triage process procedures. Respondents’ an­swers show that there are deficiencies regarding how well education content is consistent with how it is applied in the clini­cal ac­tivity and that respondents do not feel well prepared be­fore start­ing triage in clinical ac­tivities. Patients’ disease states are not regarded to be ad­e­quately identified by the triage tool or during the triage process. Based on aspects of over triage and identification of care and medical needs nurses show higher confidence in SATS than DPT. However, respondents at both emergency departments state that care and medi­cal needs are not fully identified by the triage tool or during the triage process. Few re­spondents state that infor­mation transfer in the triage process works well in their work­places. The re­spondents may deviate from the triage tool, the triage process procedures and priority order. Percep­tions of respondents are that physicians more frequently deviate than nurses. Nurses who work with nursing led triage consider in higher degree, than other re­spondents, that lack of compliance to triage process procedures affects patient safety at their place of work.  The conclusion is that there are shortcomings regarding the triage process in the emer­gency departments included in this study that could adversely affect patient safety.

Tidigare studier har visat vad patienten anser vara tillfredställande i samband med triage. Där framkommer att i mötet med vårdpersonalen vill patienten känna sig värdig, sedd och bekräftad. Detta ställer krav på vårdpersonalen och den kompetens de besitter. En förutsättning för patienttillfredsställelse är information om väntetider, patienternas medicinska tillstånd och en god vårdrelation. I mötet med patienten krävs kunskap om vad dessa tycker är viktigt och betydelsefullt i samband med triage. Kunskapen om vad som är viktigt är fortfarande inte tillräckligt belyst, ur ett patientperspektiv. Syfte var att beskriva patientens upplevelse av vad som var betydelsefullt för denne i samband med triage. En kvalitativ forskningsansats med en påföljande innehållsanalys valdes. 11 stycken intervjuer genomfördes på en mindre akutmottagning i väst Sverige.Informanterna som valdes ut var patienter på denna akutmottagning. I resultatet framträdde två domäner, bemötande och information. Kategorierna som följde var omvårdnad, trygghet, delaktighet, kompetens, omhändertagande, integritet, tid och kommunikation. Informanterna ansåg det vara betydelsefullt att bli bemötta på ett individuellt plan, detta innefattade allt från basal omvårdnad till att informanterna litade på triageteamets goda kompetens. Sanningsenlig information kring väntetider och prioriteringssystemet visade sig ha betydelse. Tidigare studier har visat att vården upplevdes som god om vårdpersonalen hade ett öppet förhållningssätt, visade hänsynsfullhet och lugn. Detta bekräftas i vårt resultat. Triageteamet skulle kunna tillgodose detta genom att våga uppskatta väntetiden, men också genom att vara tydliga i att informationen kan komma att förändras.

Bakgrund: Akutsjukvård sker i första hand via en akutmottagning. Vid ett akut omhändertagande på en akutmottagning är det ytterst viktigt att patienten får den vård som krävs så fort som möjligt, därför används triagering så att den som är i mest behov skall gå först, oavsett belastning. Syfte: Syftet med denna litteratur studie var att belysa patientens upplevelse av att bli omhändertagen och triagerad av sjuksköterskan på akutmottagningen. Metod: Litteraturstudien har genomförts genom att granska och klassificera 14 artiklar som både är kvantitativa och kvalitativa. Resultat: Patienter som inte har livshotande symtom får ofta vänta länge på vård. Den långa väntan leder till att patienterna får en lägre grad av tillfredsställelse av sitt besök på akutmottagningen. Att ge patienten information om hur lång eventuell väntan kan bli och varför de får vänta upplever patienten som positivt och får därigenom en högre grad av tillfredsställelse av sitt besök på akutmottagningen. Diskussion: Efter triageringen är det viktigt att informera angående hur lång väntetiden kan räknas bli. Även om patienten inte har livshotande symtom måste sjuksköterskan skapa trygghet hos dem då de ofta känner mycket oro och ovisshet. Slutsats: Att bemöta patienten korrekt samt ge information angående triagen, eventuell väntetid, vart de kan köpa mat och dryck skapar en högre grad av tillfredsställelse hos patienten.

Abstract:
Bakgrund: Syftet med triage på akutmottagningen är att säkerställa att patienter med störst behov av vård får det inom rimlig tid och på ett korrekt sätt. Triageprocessen inleds med att skapa en första kontakt mellan sjuksköterska och patient där kommunikationen är av stor betydelse. Att kunna delge andra människor information och dessutom kunna tolka och läsa av meddelanden är en del av kommunikationen samt att lyssna och ge svar på andra personers uttryck. Även icke-verbal kommunikation är en stor del av det som kommuniceras. Akutmottagningen är en stressig miljö och det finns mycket att lära angående vad som kan förbättra kommunikationen mellan sjuksköterska och patienter. Syfte: Att belysa hinder och befrämjande av kommunikation mellan sjuksköterska och patient vid triage och på akutmottagning. Metod: En allmän litteraturstudie baserad på tio kvalitativa artiklar. Resultat: Två huvudteman med sju underrubriker identifierades. Huvudtemana var: Befrämjande av kommunikation och Hinder för kommunikation. Underrubrikerna var: Verbal kommunikation, Icke-verbal kommunikation, Delaktighet i vård, Icke-delaktighet, Bristande individfokus, Tidsbrist och Informationsbrist. Slutsats: Av resultatet framgår att fler faktorer inom triage och akutsjukvård utgör hinder för kommunikation. Resultatet tyder på att patienterna saknar information om rutinerna på akutmottagningen samtidigt som sjuksköterskorna inte alltid anser sig ha tid för att ge informationen till patienterna. De faktorer som påverkar befrämjandet av kommunikationen stämmer överens med Habermas (1981/1990) teori där kommunikation baseras på öppenhjärtig grund, ärlighet och samförstånd. Klinisk betydelse: En mer patientcentrerad kommunikation och bättre information kan behövas på akutmottagningarna för att patienterna ska få bra vård och för att arbetet ska bli enklare för sjuksköterskorna.
Background: The purpose of triage in the emergency department is to ensure that patients with the greatest need of care receive it within a reasonable time and in a proper manner. The triage process starts by creating an initial contact between nurse and patient, where communication is a vital part. Being able to communicate information to other people and to interpret messages is a part of the communication and also to listen and provide answers to other people's expressions. Non-verbal communication is also a great part of what is communicated. The emergency room is a stressful environment and there is much to learn in what can make communication between nurses and patients better. Aim: To highlight what hinders and promotes communication between nurses and patients in triage and at the emergency department. Method: A general literature study based on ten qualitative articles. Results: Two main themes and seven subthemes were identified. Main themes were: Promoting communication and Obstacles for communication. Subthemes were: Verbal communication, Non-verbal communication, Participation in care, Non-participation, Lack of individual focus, Lack of time and Lack of information. Conclusion: The results of the study show that more factors in triage and emergency care became hindrance of communication. The patients lack information about the routines in the emergency department and the nurses do not have enough time to inform the patients. The factors that affect promotion of communication consist with Habermas (1981/1990) theory where communication is based on open-hearted grounds, honesty and understanding. Clinical implication: A more patient-centered communication and better information may be needed at the emergency departments to secure good care of the patients and thereby facilitate the nurses work.

Introduktion: En viktig del i vårdprocessen är kommunikation. Därför är det viktigt att den fungerar tillfredställande. En mycket bra och beprövad kommunikationsmodell är SBAR. Ambulanssjuksköterskan ska kunna överrapportera på ett adekvat och patientsäkert sätt. Triage används för att få ett bra flöde på akutmottagningen och patientsäker vård. Syfte: Att kartlägga hur ambulanspersonalen överrapporterar patienten till akutens personal. Samt hur ambulanspersonalens triage används av akutens personal. Metod: En observationsstudie med kvantitativ design samt kompletterande enkät av kvalitativ karaktär där personalen med fritext kunde beskriva vad som var positivt samt negativt med överrapporteringen den hade en kvalitativ karaktär. Resultat: i resultatet framkom det i samtliga observationerna att det finns en klar och tydligfältdiagnos samt att bakgrund finns med. Vitalparametrar som rapporteras varierar från inga alls till korrekt rapporterat. Att ge en rekommendation till mottagande enhet sker sällan. När det gäller ESS så rapporterades det i mindre än hälften av fallen. Arbetets betydelse: Det finns en ganska stor förbättringspotential när det gäller överrapportering från ambulanspersonal vad gällande SBAR och hur den ska användas. Samtidigt en förbättringspotential från akutens personal gällande att vara intresserad av vad ambulanspersonalen överrapporterar.

Background: Triage is the sorting of patients according to the severity of medical assessment. Triage has a long history of use in Anglo-Saxon countries but is a fairly new method in Sweden. Researchers have described the Manchester triage group method (MTS) and a literature review of the concept; working contentment and competence focusing on triage. Aim: The ambition with this study was to describe a number of triage nurses working conditions. The research questions were; working with MTS- does it bring work contentment? What kind of competence is needed? Method: Data was collected from 74 triage nurses by a questionnaire containing 37 questions and two questions provided the nurses the opportunities to describe their triage experiences. Findings: The nurses found their job interesting and stimulating, but some reported unsatisfactory work environment due to lack of education, competence, support and high work load. The informants ask for more education; to solve and discuss patient scenarios and to participate in the developing of MTS. Sixty-three percent thought MTS was a difficult method with some disadvantages. The majority found triage to be a nurse task.

Sammanfattning Syftet var att beskriva skillnader i triagebeslut som förekommer bland olika personalkategorier samt att beskriva olika vitalparametrars betydelser för triage. En ökad belastning finns idag på den prehospitala akutsjukvården. För att fördela resurserna på bästa sätt, att ge vård inom rimlig tid som är nödvändig för patientens tillstånd, har triage införts. Triage innebär att bedöma och prioritera hur snabbt patienten behöver vård. Litteraturstudien hade en deskriptiv design. Vetenskapliga artiklar söktes i PubMed och Cinahl samt utifrån funna artiklars referenslistor. Tio artiklar inkluderades i studien. Det har visat sig att triage har viktig inverkan på patientens fortsatta vård. Det förekom skillnader i beslut beroende på vilken personalkategori som utförde triage. De med högre utbildning triagerade patienterna oftare till rätt avancerad vårdnivå. Ålder och vitalparametrarna andningsfrekvens, saturation och medvetandegrad hade inverkan på triagebeslutet. De patienter som undertriagerades visade på högre mortalitet. För att patientsäkerhet skall uppnås måste all sjukvårdspersonal bedöma patienten till rätt triagenivå för att minska lidande och mortalitet. Det finns en uppenbar vinst i att tidigt upptäcka sviktande vitala organfunktioner och följa hur detta utvecklas. All akutsjukvård, både prehospital och hospital vore förtjänt av ett gemensamt och standardiserat triageinstrument.

Sjukvårdsrådgivningen via telefon är väl förankrat hos Sveriges befolkning då det varje år rings många samtal dit. Avsaknaden av visuell kontakt och den begränsade tillgången till problemet gör att triage och rådgivning försvåras. Telefonsjuksköterskan kan inte ta del av den icke verbala kommunikationen såsom exempelvis ansiktsuttryck, kroppshållning eller sjukdomstecken på kroppen. Syftet med denna litteraturstudie var att belysa faktorer som har betydelse för telefonsjuksköterskans triage och rådgivning i kontakt med den vårdsökande. I litteraturstudien som grund för resultatet har metoden varit att granska fjorton artiklar med kvalitativ ansats ur ett sjuksköterskeperspektiv. Resultatet visar att telefonsjuksköterskans triage och rådgivning påverkas av olika faktorer som är relaterade till den vårdsökande, telefonsjuksköterskans egna färdigheter och erfarenheter samt faktorer relaterade till organisation. Samtalen som telefonsjuksköterskorna tar emot är varierande till innehåll. Detta ställer krav på telefonsjuksköterskans professionella kompetens. Tidigare klinisk erfarenhet, bred kunskap och en god kommunikativ förmåga är viktiga egenskaper för telefonsjuksköterskan. Omvårdnadssituationen är komplex och för att kompensera för detta använder hon sig av olika strategier. Organisatoriska faktorer som påverkar triage och rådgivning via telefon är tillgången till beslutsstöd och tillgängligheten i vården. Framtida forskning som belyser hur telefonsjuksköterskan kan hantera stress och konfliktsituationer kan underlätta för telefonsjuksköterskan i hennes utsatta arbetssituation. Specialistutbildning till telefonsjuksköterska skulle göra vården säkrare för uppringaren men även förbereda telefonsjuksköterskan inför hennes yrkesroll.

Obsessive-compulsive disorder (OCD) is a debilitating mental disorder characterized by obsessive thoughts that trigger compulsive actions. While OCD is associated with significant disability, there are currently no objective markers for screening, diagnosis, or tracking of treatment progression in OCD patients. This is especially troublesome given reliance on subjective measurements and patient-reported outcomes. Here, to advance existing assessment approaches, the authors propose the utilization of engineering tools to objectively track the physiological, kinetic, behavioral, and affective state of individuals with OCD to aid in their clinical management. The utilization of these tools to aid in human measurements is termed biobehavioral sensing. Although the data are preliminary, the authors believe that these blueprints may be implemented in the near future to aid in the screening, diagnosis, and management of OCD. Furthermore, these ideas may also be expanded to assist in the care of patients with other mental health disorders.

Despite its chequered past, modern NMD bears little similarity to historical freehand procedures. Advances in neuroimaging mean that anatomical substrates for depression and OCD are being elucidated. Ablative procedures such as ACAPS and ACING are unlikely to undergo randomized controlled trials but prospective clinical audit suggests that such procedures may offer improvement to selected patients with treatment-refractory depression and OCD. Interventions such as VNS and DBS offer the possibility of double-blind testing but as yet there is insufficient evidence to suggest that such procedures offer greater effectiveness.

This chapter focuses on horizontal cooperation agreements. Horizontal cooperation agreements are agreements that are entered into by undertakings operating at the same stage of the value chain in order to achieve a variety of efficiencies. While these agreements are generally pro-competitive, they may, however, raise anticompetitive concerns. Under EU law, horizontal cooperation agreements are generally not considered to be agreements that have as their object the restriction of competition, although there may be exceptions. Indeed, as these cooperation agreements may adversely affect the parameters of competition, competition law has taken an interest in them. Cooperation agreements are typically caught by Article 101(1) TFEU. Nevertheless, horizontal cooperation agreements that trigger anticompetitive effects, but generate efficiencies as well, can be exempted under Article 101(3) TFEU.

This concluding chapter examines some aspects of the costs, funding, and settlement of damage claims in the UK. The costs of bringing a claim can be high. The most expensive jurisdiction is England, but other procedural rules counterbalance this expense to make suing in England attractive. In some jurisdictions, reforms have been put in place to reduce litigation costs such as conditional fee arrangements and damage-based agreements that share the risks between claimants and their lawyers. After the event insurance (ATE), third party litigation funding, and lawyers’ contingent fee arrangements all reduce the costs and risks associated with litigation. However, very few civil actions go to trial and most are settled out of court through direct negotiations among the parties or through mediation or arbitration.

This chapter provides a guide to continued family problem solving. It begins with a review of skills learned earlier in treatment as well as progress with the initial family issues they have begun to address. It provides a strategy for picking the next family problem to target, with an emphasis on giving the family more independence during problem solving exercises. The chapter discusses the parents’ inadvertent potential to reinforce OCD behavior in their behaviors and in their speech. Parents who are particularly anxious may have difficulty managing their emotions in the moment, even though they understand the concept of modeling. The chapter provides strategies for helping these parents navigate OCD-related situations that may trigger their anxiety. Focus of the chapter continues to skills training in emotion regulation, including in-session practice exercises and discussion of the importance of modelling healthy responses to anxiety.

Abstract:
In glaucoma, the optic nerve head (ONH) is the site of damage to the retinal ganglion cell axons that transmit the visual information from the eye to the brain. Results of several randomized prospective trials showed that intraocular pressure (IOP), age 1, increased optic disc cupping, corneal thickness, and African ancestry 2 are independently associated with glaucomatous progression. All of these risk factors have a biologically plausible association with either the level of IOP, the severity of disease, or biomechanical properties of the ONH. Importantly, age is the only risk factor other than IOP that is independently associated with the onset and progression of glaucoma across all of the major prospective clinical trials conducted over the past twenty years. In addition, every population-based survey conducted to date has demonstrated a strong relationship between the prevalence of glaucoma with advancing age, despite almost no studies showing IOP changes with age. These findings indicate that the aging ONH becomes increasingly vulnerable to glaucomatous injury at similar levels of IOP.

Landslides are one of the main threats in maintaining pipeline integrity and depend directly on natural geological and geotechnical conditions. External factors such as weather, rainfall, and others, can trigger land movements and displace the pipeline. The Ecuadorian OCP (Heavy Crude Oil Pipeline) is a buried pipeline going in an East to West direction, crossing 485 kilometers of the Ecuadorian territory. It starts in the Amazon Region (approximately 300 meters above sea level), and then climbs the Andes Mountains (4060 meters above sea level in its tallest portion), to then descend to the shores of the Pacific Ocean. The OCP pipeline crosses many regions with varying climates, varying rainfall patterns, variable morphologies, diversity of soils, and areas affected by tectonic faults, among others. In order to prevent pipeline failures, OCP Ecuador has instituted programs to perform preventive and corrective actions in order to handle the following geological concerns: • Intervention of a specialized geotechnical team to identify and monitor critical points along the pipeline route. This team identifies unstable sites based on the observations of cracks, land movements, or other visual deformations of the pipeline route and its surroundings. • Upkeep of the preventive program. • Execution of third-level studies required to understand specific unstable zones in detail: nature of the subsoil, underground water level, geo-mechanic characteristics, stability factor, and stabilization works. • Geotechnical instrumentation used: inclinometers to search the spread of movement, shifting direction, speed, (landslide location); strain gauges for preventive control of pipeline strain, alert levels, efficiency of stabilization works; and topographic surveys to monitor superficial movements. • Data processing and mapping on GIS Software. • Annual over-flights to detect massive landslides. • Internal inspectors (online-ILI) providing a wide range of information: geometry measurements, curvature monitoring, pipeline displacement, etc. In addition, it allows detection of probable zones depicting soil movement. The purpose of this technical paper is to present the methodology applied by OCP Ecuador to prevent failure of the pipeline along its route.

Abstract:
Abstract Open hole (OH) completions are not very common in the GoM, but the area has seen an uptick in OH wells in recent years, and a few big projects have elected to use the same completion archetype. There are several different ways to complete an OH well, and one of these completion techniques involves running screens across the OH in Drill-In fluid (DIF), displacing the DIF out of the OH with brine, and then setting the packer, before pumping a filter cake breaker, designed to remove the filter cake and restore the reservoir permeability to near pre-drilling levels. A review of past open hole (OH) well completions in GoM revealed that there was an inconsistent action of the breaker on the filter-cake: sometimes the breaker would react quickly, and sometimes there was no noticeable effect. This study led to the development of a new technology to allow better displacements of the OH, with the ultimate objective of reducing initial well skin induced by the drill-in fluid (DIF) and filter cake. It was theorized that the low displacement rates would lead to poor removal of the mud from the OH, in turn leading to a poor breaker action on the DIF filter cake and a long-term impact on well injectivity and increased OPEX, as these wells tend to need an initial stimulation within a short timeframe after initial completion. The approach used was to develop a new tool to allow faster displacement rates, and test it on a trial well, to verify the results and validate this theory. To solve this problem, a new tool was proposed, developed and fully tested in a tight deadline of 6 months. The new module allows up to 9 bpm rates and up to 3,500 psi differential pressure before setting the packer, versus the previous ∼800 psi differential pressure limit, present in all tools in the market, for that casing size (7 5/8"). During the first well trial, the tool allowed a displacement of the OH at double the pump rates obtained in previous wells in the same basin, with similar OH lengths, leading to the smallest volume of contaminated fluid interface seen to date, indicating a much better displacement. Once the well was put online, it achieved an injection rate above expectations, even when the drilled OH interval penetrated significantly less net sands than originally planned. The results on this single well trial seem to corroborate the theory posed, however it is recognized that more data is required to be certain of its results, and that will only come with time, as well performance is measured and compared with other wells that did not use the same technology. The novelty of this new technology is the ability to obtain a better displacement of the OH, leading to a better breaker action and well cleanup in OH completions. Although the trial well was an injector well, the technology is equally applicable to producer wells. The paper will cover the problem description, installation procedures, development and testing of the technology, design aspects of using the technology and the successful implementation in the trial well.

Abstract This paper aims to present user experience survey results of innovative software assessment technologies available in the market aimed in evaluating risks of industrial-related ergonomic hazards. The scope covers industrial ergonomics softwares currently available for purchase where time-limited free trial is offered, particularly those that utilize 3D Motion Capture Assessment which relies on kinematic inputs aided by non-invasive computer technology and artificial intelligence, and makes use of pre-determined weightings based on biomechanical risk factors. In light of the inclusion criterion, six industrial ergonomics softwares were considered. User field trials were conducted during January to September 2020 among 10 Occupational Health Subject Matter Experts (OH SME) coming from seven oil and gas Group Companies. Each OH SME attended a product demonstration by the shortlisted software vendor, participated in software trial at their respective workplaces, and provided feedback on the software's usability by filling out a survey questionnaire. OH SME responses were then collected for further qualitative analyses. Three of the eligible softwares relied on photo snapshot capturing work activity where subsequent analysis is done through competent professional judgment of qualitative risk. Another three were dependent on 3D Motion Capture Assessment where upper and lower limb motions of employees are digitally captured, recorded, and analyzed. Two of the softwares utilized sensors attached to different parts of employee's body, while one relied on Android/Smartphone snapshot of work activity and analyzed by the software's algorithm. Analyses of OH SME feedback revealed majority of them (n = 7) preferred using 3D Motion Capture Assessment over professional judgment of qualitative risk as an effective tool in evaluation of industrial work-related ergonomic risks. 3D Motion Capture Assessment provided accurate measurements of employee joint postures and postural angles. The tool ensured consistency in risk scoring for a particular industrial-related work activity as the calculation is standardized. The tool's algorithm is aligned with globally accepted assessment tools in evaluating ergonomic risks which enhances its validity. OH SMEs have expressed concerns on use of Android/Smartphone in Critical Infrastructure and Coastal Protection Authority facilities, training time needed in learning the software, and repetitive use of motion sensors among different employees which may lead to personal hygiene issues. 3D Motion Capture Assessment is a novel ergonomics software tool that can be used in real-time and accurate evaluation of ergonomic risks arising from industrial work-related activities. It can replace observational assessment of a work activity that may be prone to professional judgment errors. However, more validation and reliability studies need to be done in future as well as determining association between ergonomics risk scores obtained from the software and prevalence of work-related musculoskeletal disorders.
