Ovarian cancer (OC) is an immunogenic tumor and among the first where measures of anti-tumor immunity correlated with improved survival. Thus, immunotherapy could be a viable OC treatment modality. Nonetheless, clinical OC immunotherapy trials have demonstrated only modest successes at best, and there is currently no Food and Drug Administration (FDA)approved OC immune therapy despite recent successes in other carcinomas and lymphomas and FDA-approved immunotherapy agents for them. New data suggest specific impediments to effective de novo and treatment-induced anti-OC immunity and support the concept that effective, tolerable OC immunotherapy could be developed based on these newer insights. This chapter reviews the history of OC immunotherapy, highlights important discoveries in OC-related immune dysfunction, covers promising recent developments, highlights newer and ongoing clinical trials, and speculates on future directions that could lead to improved OC immunotherapy approaches.

Although most patients with obsessive-compulsive disorder (OCD) attain adequate symptomatic relief with conventional first-line treatments, a meaningful fraction remain incapacitated despite numerous medication trials and intensive behavioral therapy (Garnaat et al., 2014). A subset of these patients may be candidates for surgical treatment. Stereotactic neurosurgical lesions have been used successfully since the 1960s to treat patients with especially severe, chronic, treatment-refractory OCD. This chapter briefly reviews the history of psychiatric neurosurgery and the development of the four current neurosurgical approaches to OCD. It discusses issues related to the careful selection of appropriate surgical candidates. It then reviews available outcomes data for each of the four surgical procedures. The putative mechanisms of action of these ablative neurosurgeries, in the context of our current understanding of the pathophysiology of OCD, are explored. The chapter concludes with future directions in which psychiatric neurosurgery may evolve to address current limitations and harness more fully its therapeutic potential for patients with OCD.

The coexistence of ADHD, tic disorders (TD) and obsessive-compulsive behaviour (OCB) can appear in any combination as a ‘clinical dyad or triad’. The possible combinations are well above what would be expected by chance alone. Overlap and relationship, specifically in the long-term course, between these problem areas creates different psychopathological profiles at different time points along the lifetime. In this chapter we explore the phenomenology of the possible co-occurrences of ADHD, TD, and OCB. Specifically, the impact of TD and/or OCB on the clinical presentation of ADHD will be described. The co-occurrence of ADHD with TD and/or OCB and their multifaceted interactions are a challenge for clinical assessment and treatment having a developmental psychopathological view in mind.

Obsessive-compulsive disorder (OCD) is defined by the presence of obsessions and/or compulsions. In general, obsessions trigger anxiety or discomfort, whereas compulsions relieve it. Unfortunately, this transient relief can reinforce obsessions, compulsions, and the association between them. This feedback reinforcement has been described as the “OCD cycle.” Many of the specific issues explored throughout this text can be conceptualized in relation to this cycle. The heterogeneity of OCD symptomatology means there are many variations on this scheme. Despite these variations, the idea of a feedback cycle between obsessions and compulsions is broadly applicable and heuristically useful. Most treatments can be thought of as efforts to interrupt this cycle of reinforcement. This chapter explores the complexities of this feedback, and how it may be initiated and maintained. It is probable that different individuals with OCD can enter the cycle of maladaptive reinforcement via different routes.

Abstract:
This chapter discusses the use of deep brain stimulation (DBS) for intractable OCD. In contrast to ablative procedures, DBS has the advantages of being reversible (explantable) and adjustable. In 2010, the FDA approved a Humanitarian Device Exemption (HDE) for ventral capsule/ventral striatum DBS in intractable OCD, based upon the device’s safety and its probable benefit, for up to 4,000 people a year in the US. DBS is only suitable for adult patients who remain severely ill despite multiple medication trials and adequate treatment with cognitive behavioral therapy. Further research is needed to further test the efficacy of DBS in OCD, compare targets, identify predictors of response, and optimize programming. Future DBS systems may take advantage of closed loop technology in which feedback from the brain is used to adjust stimulation automatically according to the patient’s changing needs.

This chapter gives a broad overview of the “habit hypothesis” of obsessive-compulsive disorder (OCD). Most patients with OCD recognize that becoming trapped in seemingly never-ending streams of repetitive ritualistic behaviors defies reason. Importantly, this recognition is not enough to put a halt to these behaviors. It has been proposed that these compulsions are “bad habits”: that external cues trigger an urge to perform a familiar response, which the patient cannot resist. The chapter presents the basics of what habits are, and how they relate to what we call “goal-directed control” over action. Next, an in-depth analysis of a series of empirical investigations that tested this hypothesis will be presented. In the final section, the habit hypothesis of OCD will be put into the broader context of “compulsivity” as a putative trans-diagnostic trait that is relevant for many psychiatric disorders.

This chapter reviews standard pharmacological treatments for OCD and the evidence supporting them. Selective serotonin reuptake inhibitors (SSRIs) remain the pharmacological treatment of choice and are associated with improved health-related quality of life. Improvements are usually sustained over time as long as treatment is continued. Discontinuation is associated with relapse and loss of quality of life, implying that treatment should continue long-term. A substantial minority of patients who fail to respond to SSRIs may benefit from dose elevation, switch to clomipramine, or adjunctive antipsychotic, though long-term trials validating the effectiveness and tolerability of these strategies are relatively lacking.

Compulsive hoarding is a prevalent and chronic problem that is associated with a profound public health burden. Individuals with hoarding symptoms who have been included in OCD treatment outcome trials have responded poorly to standard pharmacological and psychological treatments. Interventions based on a cognitive-behavioral model specific to compulsive hoarding have shown more promise, according to recent studies. This chapter reviews assessment practices, current evidence regarding biological and psychological treatments for hoarding, and challenges associated with treating hoarding. The cognitive-behavioral treatment for hoarding developed by Steketee and Frost (2007) is described, followed by research questions regarding how to improve treatment outcomes for hoarding.

This chapter reviews current research regarding nutraceutical substances that have been systematically investigated through either case studies, open-label and/or randomized placebo-controlled trials and found to have some evidence of efficacy in the treatment of obsessive compulsive and related disorders (specifically, trichotillomania, compulsive nail biting, and excoriation). The substances to be considered are myo-inositol, St. John’s wort, milk thistle, borage, glycine, sarcosine, psilocybin, and N-acetylcysteine. A description of each substance is presented, together with relevant biochemistry and interpretation regarding the current evidence of efficacy and safety/tolerability as well as what can be determined about factors that may moderate its efficacy, including OCD symptom severity, and advice in regard to concomitant antidepressant use. Conclusions and recommendations for future research are also presented.

Hoarding Disorder (HD), new in DSM-5, is remarkably prevalent, affecting 2% to 5% of the population. Hoarding symptoms were long considered an aspect of OCD, but it has been increasingly recognized that they differ, phenomenologically and epidemiologically; the new DSM-5 diagnosis formalizes this recognition. HD is a complex disorder consisting of problems with attachments to possessions that lead to difficulty discarding and organizing them. Together, these features lead to severely cluttered living spaces that can pose serious health and safety threats. The vast majority of those with HD acquire excessively, mostly through buying or collecting things that others have discarded. Hoarding behaviors appear early in life and typically get progressively worse over the life span. Cognitive behavioral therapy specifically designed to treat hoarding has been shown to be effective, though many still suffer from symptoms after treatment. Several medications have shown promise, but no controlled clinical trials have been conducted.

This essay explores notions of alienation and estrangement, contrasting Kafka’s The Trial with the classical accounts offered by Hegel and Marx. A sense of impossibility emerges in Kafka’s work that is new and at the same time tragic. We are estranged from the very conditions of recognizing the conflicting forces that define experience—that is, estranged from the possibility of reconciliation that is clearly necessary, but in no way experienced as real, in spite of the inextinguishable hope that such reconciliation may nevertheless be possible. “Oh, [there is] plenty of hope, an infinite amount of hope—but not for us,” as Kafka famously put it. References are made to Adorno’s and Benjamin’s writings on Kafka. The references to tragic ambiguity, initially presented in relation to the work of Vernant and Naquet, are buttressed philosophically by a reflection on the peculiar modality of the “unreal possibility” of overcoming estrangement.

El ejercicio contemporáneo de la investigación en las diferentes ciencias sociales implica pensar constantemente en los procesos metodológicos, su naturaleza, la supremacía del problema de investigación sobre los límites disciplinares y la incidencia de su trabajo en el presente, esto es, su política. Estos cuatro planos conforman un volumen que se ajusta, a su vez, en función de las fuentes, las preguntas de investigación, sus objetivos, la situación de quien investiga… Así, pensar el método involucra una combinatoria permanente de elementos que desborda los aspectos procedimentales y, por tanto, una falta de resguardo metodológico, disciplinar y ontológico, pues ni siquiera las comunidades con que dialogamos o las fuentes con que trabajamos nos garantizan de antemano su forma o su estabilidad a largo plazo. Esta triple falta de garantía nos pone a la intemperie, ante el devenir de un trabajo que nunca promete resultados definitivos, o al menos duraderos, pero que sí hace apuestas políticas, como ocurre con los ocho capítulos que reúne Investigar a la intemperie, producidos por investigadoras del Instituto de Estudios Sociales y Culturales Pensar. Estos trabajos presentan reflexiones de métodos tomadas del oficio cotidiano de investigación en ámbitos como la migración, las políticas públicas, la producción de espacialidades, de saberes formales y no formales, de cuerpos definidos por las especificidades locales del conflicto armando colombiano, y desde perspectivas críticas como los estudios feministas, decoloniales, culturales y de las migraciones.

Hacer la presentación de un libro como este es complejo: complejo por lo que implica la tarea misma, pues expresar en un corto texto lo que otros han presentado como fruto, quizá de meses de arduo trabajo, siempre se quedará corto frente al esfuerzo intelectual de aquellos que participaron en la escritura de estas páginas. Y también se hace más complejo cuando el conocimiento es tan amplio y multidisciplinario como el que nutre los capítulos de este texto. Desde los años 60, cuando comienza a darse una revisión del concepto de ciencia contemporánea, la filosofía, las ciencias sociales y la educación se fortalecen con la apropiación social del conocimiento a través de diferentes estrategias de difusión y divulgación de la ciencia. La investigación es un proceso científico, pero también es una actividad con una triple esencia: epistemológica/gnoseológica, metodológica y tecnológica, pero actualmente, con una cuarta dimensión: la comunicación. Hoy, comunicar la ciencia es parte de la actividad investigativa para los principios de ciencia, tecnología e innovación. El investigador, además, tiene un compromiso triple: darle rigor a la investigación científica, es decir, hacerla sistemática, metódica, ordenada, abierta, dinámica, coherente y pertinente. Por otro lado, ser lo suficientemente sensible a su realidad circundante: saber que de esa realidad debe nacer su trabajo científico. Cada vez más, la realidad concreta, las necesidades sociales, los fenómenos ambientales, sociopolíticos o socioeconómicos son la principal razón de la existencia de la investigación científica, la tecnología y la innovación. La investigación tiene sentido en la medida en que da respuestas pertinentes, inmediatas y duraderas para ese contexto concreto. En su tercera dimensión, el compromiso está relacionado con esto último: el investigador o el profesional, en general, tiene un compromiso con la vida y con el contexto. Pensar investigaciones que no tengan relación con ese entorno, o con la integralidad humana, o con la vida misma, no tiene sentido. Los participantes en este libro han asumido ese triple compromiso desde sus disciplinas propias y, en diferentes casos, desde la multidisciplinariedad. Si bien la temática aglutinante es la educación humana y los campos afines a las humanidades y la sociedad, cada capítulo y apartado promueve intereses interdisciplinarios y el estudio de fenómenos sociales actuales y, en muchos casos, preocupantes para diferentes campos de estudio. Veremos temáticas como la transformación del hombre en máquina, las competencias mediáticas, la innovación, la formación emocional en los procesos educativos, el liderazgo escolar, la música como parte de la identidad cultural de los pueblos, los objetivos de desarrollo sostenible e incluso la apropiación y gestión de conocimiento en espacios concretos como las ONG. Estas búsquedas y reflexiones no solo son vigentes, sino que además son de gran utilidad para muchos docentes, estudiantes universitarios e investigadores independientes de nuestro medio. Los ocho capítulos de este libro son resultado de procesos de investigación y han sido revisados, evaluados y validados por expertos en los diferentes temas o campos de saber. Parte de la lógica contemporánea de validación del conocimiento procede de los acuerdos intersubjetivos entre pares o entre la comunidad académica de expertos, por lo que el libro mismo proviene de un ejercicio científico de verificación y construcción sistemática y metódica del conocimiento. La investigación es un género integrativo, contiene otros géneros de discurso y de producción escritural. La difusión del conocimiento científico tiene sentido en tanto se convierte en una oportunidad que se brinda a otros frente al conocimiento, la investigación y el estímulo a nuevas búsquedas. Así que este material se espera que sea una estrategia de fomento a la apropiación social, pero adicionalmente emocionará a otros autores a que hagan el ejercicio de publicar sus obras y difundir su producción intelectual tanto como validación pública, como ratificación del quehacer científico en nuestro país. Invitamos entonces a futuros autores para continuar con estos espacios y comprometerse con la investigación en sus términos actuales: gnoseología, metodología, tecnología y comunicación.

RAINBOW: A Child- and Family-Focused Cognitive-Behavioral Treatment for Pediatric Bipolar Disorder is a comprehensive, evidence-based treatment manual designed specifically for children ages 7–13 with bipolar spectrum disorders and their families. Developed by experts in pediatric mood disorders and tested in a randomized clinical trial (RCT), RAINBOW integrates psychoeducation and cognitive-behavioral therapy (CBT) with complementary techniques from mindfulness-based intervention, positive psychology, and interpersonal therapy to address the range of therapeutic needs of families affected by this disorder. Guided by the evidence on the neurobiological and psychosocial difficulties accompanying pediatric bipolar disorder, this treatment targets the child and family across seven core components: Routine, Affect Regulation, I Can Do It, No Negative Thoughts and Live in the Now, Be a Good Friend/Balanced Lifestyle for Parents, Oh How Do We Solve This Problem, and Ways to Get Support. Throughout the treatment, the child and family will learn how to identify mood states and triggers of mood dysregulation, and develop cognitive and behavioral strategies for improving mood stability. Children will build social skills, and caregivers will develop greater balance and self-care in their own lives.
