Abstract:
Oculocardiac reflex is a phenomenon caused by traction applied to the extraocular muscles, or pressure on the globes. The afferent stimulus travels via the ophthalmic branch of cranial nerve V, and the efferent stimulus travels via cranial nerve X, which can cause severe bradycardia and potentially hypotension, atrioventricular block, ventricular ectopy, and rarely asystole. Oculocardiac reflex is different from vasovagal syncope which has another pathway, and is triggered by pain or emotional upset, that results in failure of autoregulation of blood pressure. Ophthalmologists often observe oc

Abstract:
Ocular trauma in childhood is common and may cause transient or permanent visual impairment. The anesthetic management of children with penetrating eye injuries presents several unique challenges, including potential associated injuries that may take precedence over the treatment of the eye injury, the prevention of aspiration of gastric contents, the regulation of intraocular pressure (IOP), and the prevention of the oculocardiac reflex (OCR). An understanding of the mechanisms and management of these potential problems can favorably influence surgical outcome.

This chapter describes anaesthesia for paediatric ophthalmic cases. Management is generally straightforward, but is not without its problems and complications. Most procedures are day cases, but they may involve children who are unable to comply with tests when awake, children having multiple procedures, or syndromes associated with their ophthalmic condition. Ages range from the neonate to teenagers and involve procedures such as examination under anaesthesia (EUA), squint surgery, cataract surgery (and its complications), nasolacrimal surgery, oculoplastic surgery, and glaucoma surgery. Abstract Commonly performed pediatric ophthalmologic procedures performed under anesthesia include eye examination for tonometry, fundoscopy, and assessment of visual evoked potentials, cryotherapy or laser therapy for retinopathy of prematurity or retinoblastoma, strabismus repair, lens extraction in cataracts, and enucleation, which are predominantly performed in the ambulatory setting. However, special considerations are needed in premature infants with congenital disorders associated with pathology of the eye. Abstract:
Abstract The goals of ophthalmologic anesthesia include the maintenance of a stable intraocular pressure (IOP) and avoidance of oculocardiac reflex (OCR) in a comfortable, motionless patient. Regional anesthesia in ophthalmologic surgery has several advantages over general anesthesia, such as postoperative analgesia enhancement and nausea and vomiting reduction. Due to its akinetic feature, it also has some advantages over topical anesthesia. To reduce the risk of injury to the major structures in the intraconal space, recently the peribulbar block has been replacing retrobulbar anesthesia. Ho
