Full textINTRODUCTION: Guided bone regeneration (GBR) in combination with implant placement has become a routine treatment. However, there is still a lack of evidence regarding critical aspects of such GBR procedures, especially the ones related with primary stability of the graft materials. Accordingly, the present investigation was designed to analyze the influence of flap manipulation and guided bone regeneration materials with respect to the volume stability of the augmented region during suturing of mucosal flaps. MATERIAL AND METHODS: Peri-implant box-shaped bone defects were created in pig mandibles. Three procedures were tested in the first part of the investigation: - Granulate (particulated xenograft + collagen membrane) - Granulate + Pins (I) (particulated xenograft + collagen membrane + fixation pins) - Solid block (block xenograft + collagen membrane) Two procedures were tested in the second part: - Granulate + Pins (II) (particulate xenograft applied buccally + collagen membrane + pins) - L-shaped soft-block (particulate xenograft applied buccally + L-shaped soft-block xenograft applied buccally and occlusally + collagen membrane + pins) Cone beam computed tomography scans were obtained prior and after blinded wound closure. The horizontal thickness (HT) of the augmented region was assessed at the implant shoulder (HT0mm) and at 1 mm to 5 mm apical to the implant shoulder (HT1mm - HT5mm). Repeated measures ANOVA were used for statistical analysis. RESULTS: Wound closure induced a statistically significant change of HT0mm and of HT1mm in all the treatment groups (p≤0.05). The use of fixation pins or a solid block performed better than a particulated xenograft plus collagen membrane alone (p≤0.05). The addition of a L-shaped block statistically improved stability (p≤0.05). CONCLUSION: Wound closure induced significant displacement of the bone substitute, especially in the coronal portion of the augmented site. The stability of the bone substitute and collagen membrane was enhanced by the application of fixation pins and by the use of block bone substitute instead of particulated bone substitute. However, still a 20% of graft displacement was detected at the coronal level. Finally, the addition of a L-shaped soft-block substantially improved coronal stability.

Full textSealing evaluation of endodontic fillings in vitro leads to irreproducible or contradictory results which preclude concluding to the superiority of one method or material. This thesis is intended to improve knowledge of evaluating techniques for endodontic sealing and to develop a new method to evaluate the permeability of root canal fillings using the measure of a nitrogen flow.In the first part, we perform a critical analysis of evaluation methods of sealing commonly used in endodontics from a review of literature. In the second part we describe the gas permeability method, its physical process and its application in endodontics. In the third part, we treat water permeability by comparing the technique of fluid filtration and nitrogen permeability. Also, we model with physical laws the penetration process of dyes in the endodontic interface with protocols corresponding to dye leakage studies. In the fourth section we describe the adaptation of the method of gas permeability to the study of dental implant connections tightness

Full textRevisa el procedimiento quirúrgico para la colocación de implantes. Siendo fundamental el conocer la anatomía del maxilar y mandíbula aplicada a la implantología. En las áreas donde el tejido óseo disminuye es importante realizar procedimientos quirúrgicos de colocación de implantes cortos, y en regiones posteriores es comparable a la de las regiones anteriores, además se pueden alcanzar la misma tasa de éxito clínico con implantes cortos si se compara con los de mayor longitud, siendo su uso una alternativa terapéutica predecible a corto y medio plazo. Es importante conocer los límites anatómicos para la colocación de implantes, a través de tomografía computarizada Cone Beam, puesto que el análisis volumétrico dará la certeza de los reparos anatómico.

Full textThe present investigation has tried to analyze and compare two implant planning and placement treatment concepts from a holistic approach. Conventional and, more frequently, computer-assisted implant planning and placement protocols have been repeatedly investigated in terms of implant placement accuracy. However, the implant placement precision is only one of the relevant aspects when evaluating an implant planning and placement procedure. Other aspects such as treatment’s predictability, complications or unexpected events derived, treatment outcomes, patients’ perception, influence on the patient’s quality of life, or time and economic costs derived, are identically relevant when assessing the treatment’s value. Therefore, the aim of this study was to investigate all of these parameters in order to quantify the treatments’ efficiency.

Full textTitanium is a material of choice for orthopedic and dental applications. To confer with titanium surfaces antibacterial properties and improve its potential for osseointegration LBPS team proposed the functionalization of polymers "bioactive". Our work fits into this theme, we have studied both the adherence of S. Aureus and the response of osteoblasts on titanium surfaces grafted with poly (styrene sulfonate) polyNaSS. Bioactive titanium surfaces were performed by grafting polyNaSS obtained by radical copolymerization Three types of titanium surface were used for biological tests: non-modified titanium, oxidized titanium, titanium grafted. The bacterial adhesion was investigated on titanium surfaces grafted and ungrafted, pre-adsorbed plasma proteins or not. Counting of bacteria was followed by labeling bacteria with fluorescein and quantification by image analysis. Indeed, we showed that the modification of titanium surfaces by grafting a polymer "bioactive" such as poly (styrene sulfonate) can inhibit (> 70%) adherence of S. Aureus. This property probably involves inhibiting the amount and / or conformation of adsorbed proteins that may mediate bacterial adhesion. In parallel, we explored the response using osteoblastic cells in fetal rat calvaria in culture. The results showed that the grafting of poly (styrene sulfonate) does not alter the number of adherent cells on titanium, or morphology. It does not interfere either with proliferation, as evidenced by the kinetics of cell proliferation similar on all three surfaces tested. However, the specific activity of alkaline phosphatase on the grafted surface was significantly higher (p <0. 01) than those observed on control surfaces, mineralization nodules were visible on the three titanium surfaces tested. However, the sum of the areas on the titanium surface grafted was significantly (p <0. 05) greater than that observed on the surface of oxidized titanium and unmodified. In conclusion, the grafting of titanium surfaces with these bioactive polymers sulfonates one hand inhibits the adhesion of S. Aureus and the other not only does not interfere with the adhesion and proliferation but stimulates differentiation of osteoblastic cells. These surfaces could be interesting for clinical application in coatings of implants, particularly in the field of orthopedics to prevent peri-implant infections

Realiza una comparación entre dos métodos de enseñanza: la clase magistral y el aprendizaje basado en problemas (ABP). La comparación se realiza a partir del rendimiento académico de los alumnos del diplomado de implantología de la UNMSM. La pregunta fundamental que inspira la presente investigación está relacionada a si existen diferencias significativas en el rendimiento académico de los alumnos que realizan sus estudios con el método de ABP respecto a los que cursan con la clase magistral. La hipótesis planteada señala que sí se encuentran diferencias en el rendimiento académico. La investigación es cuasi experimental, para el presente trabajo los alumnos son divididos en dos grupos, un grupo recibe la clase magistral, mientras que otro recibe el método de aprendizaje basado en problemas. Se realiza un pre test y post test sobre el tema de “Perimplantitis” y se aplica a ambos grupos. La principal conclusión está referida a que sí se encuentran diferencias significativas en el índice académico del grupo de estudiantes con los cuales se utiliza el ABP, por tanto la investigación recomienda priorizar dicho método por permitir un mejor aprendizaje.

Taking into consideration the different types of problems faced in the surgical procedures related to planning, this study presented a new possibility with the use of 3D prototyping, aiming to assist the dental professionals in the dental implant management and other procedures. This research evaluated the use of prototyping as an auxiliary tool for a safe surgical planning, considering that it provides the most faithful reproduction of the dental arches. The methodology evaluated the creation of prototypes using tomographic image les manipulated by InVesalius software, which processes 3D images for the generation of three dimensional objects to be printed by material extrusion. The objective was to create models to facilitate the management of surgical procedures in Implant Dentistry, with consequent minimization of errors and greater comfort for the patient. From images acquired in the public image bank www.dicomlibrary.com a three-dimensional model was created and printed. Following a dental implant procedure was performed on the printed model, following all step sof the procedure that s hould be done on the patient. As a result practical applicability will denote several possibilities, that increasingly digital technology nds and performs variables that in the past seemed impossible. Technological evolution implies an ever more complete future in relation to the diagnosis, planning and health are a treatment.
Levando em consideração os diversos tipos de problemas enfrentados nos procedimentos cirúrgicos, relacionados ao planejamento, neste estudo apresentou uma nova possibilidade com o uso da prototipagem 3D, objetivando auxiliar os trabalhos dos profissionais da área odontológica, no caso da gestão de implantes dentários e demais procedimentos. Essa pesquisa avaliou a utilização da prototipagem como ferramenta auxiliar para um planejamento cirúrgico seguro, considerando que esta, proporciona a reprodução mais fiel das arcadas dentárias. A metodologia avaliou a criação de protótipos utilizando arquivos de imagens tomográficas manipuladas pelo software InVesalius, que processa imagens 3D para geração de objetos tridimensionais a serem impressos por extrusão de material. O objetivo foi criar modelos para facilitar o gerenciamento dos procedimentos cirúrgicos em Implantodontia, com consequente minimização de erros e maior conforto para o paciente. A partir de imagens adquiridas no banco público de imagens www.dicomlibrary.com um modelo tridimensional foi criado e impresso. Na sequência um procedimento de implante dentário foi executado no modelo impresso, seguindo todos os passos do procedimento que deverá ser feito no paciente. Como consequente aplicabilidade prática denotará possibilidades diversas, que cada vez mais a tecnologia digital encontra e realiza variáveis que no passado parecia impossível. A evolução tecnológica implica num futuro cada vez mais completo com relação ao diagnóstico, planejamento e tratamento das áreas da saúde.

Full textThis systematic revision of articles is about topical application of Alendronate in odontology into the ambient of private dental clinic. It is reviewed in three parts. The first one is reminding onto basics about bisphosphonates and the bone cycle to understand this study; the second one is about the relationship between bone and bisphosphonates and the third one is a discussion about their use in odontology. What we can conclude after the investigation is treating different forms of periodontitis, after an extraction, or collocating implants, the Alendronate is efficient in lost bone regeneration without evidences of side effects.

Full textMagnification, which refers to the real image amplification and visual support of real images in real time, has been applied to various fields of medicine and science, over the years, and is, above all, explored in areas such as biology, optometry and investigations that require close observation of microorganisms. As the technology of microscopes develops and delivers bigger and better capabilities, health professionals perceive the benefits that these tools brings to the exercise of their métiers, and, therefore, the idea that the magnification adds value to the work of Medical dentists is being increasingly accepted amongst professional community. Taking as its starting point the need to explore in order to know deeply the magnification, we carry out a literature review with the aim to assess the importance for the development and improvement of dentists working conditions, in all areas intervention of dental science, of magnification technologies. The results of the studies analyzed seem to converge to the widespread idea that the magnification is of utmost importance, presenting itself as the future of the practice of dentistry, and the disadvantages that are inherent to it, such as high costs and the need for adaptation, seem to be outweighed by the advantages it offers, like the increasing of visual capacity and the improvement of the quality of the treatment, as well as the amelioration of ergonomics and the ease on making digital documentation and establishing communication between technicians and patients.
