La réduction des risques liés à l’utilisation des rayons ionisants est une préoccupation constante des praticiens mais aussi des législateurs qui ont transposé cette règle éthique dans le Code de la Santé Publique. Cette réduction des risques s’exprime à travers l’un des principes fondamentaux en radioprotection : la justification de tous les actes de radiodiagnostic. En application directe de ce principe, l’État français a publié, en 2006, un guide des indications des examens radiologiques en odontostomatologie. Néanmoins, celui-ci n’abordait que très partiellement le domaine de l’orthodontie, domaine dont les techniques radiologiques en pleine évolution rendent nécessaire une actualisation permanente des recommandations.L’objectif de cet article est donc, à partir d’une analyse systématisée et critique de la littérature, de proposer des recommandations actualisées en imagerie orthodontique.

Abstract:
The objective was to study the epidemiological, clinical and radiological aspects of patients with mandibular fractures in the odontostomatology department at the General hospital Idrissa Pouye in Senegal. This was a retrospective study of the records of patients who came for a mandibular fracture during the period from February 2007 to June 2019 to the odontostomatology department of the General hospital Idrissa Pouye in Dakar. The inclusion criterion was any patient file containing complete information and presenting a mandibular fracture received in the odontostomatology department. We used sociodemographic, clinical and radiographic variables. Of 128 patients who came for consultation with a mandibular fracture, 115 were male and 13 were female. The age group most affected was between 21 and 30 years. Workers (25.2%), pupils and students (25.2%) were the most represented. The most frequent cause of mandibular fractures was road traffic accidents (36.71%). In 41% of the road accidents, Jakarta mopeds were involved. The diagnosis was confirmed by radiological examination, which consisted of an orthopantomogram in 83.5% of cases. The mandibular symphysis was the most frequent location with 40.3% of fracture sites. Total continuity of the lines was observed in 21.9% of cases.

The odontostomatology consultation is a medical visit which, according to Hippocrates, involves the following four stages: observation, palpation, percussion and auscultation. The aim was to study the reasons for consultation in Odontostomatology. Methodology: Retrospective and descriptive over three months from 1er January to 31 March 2022. Results: The age group 15 to 44 years was the most represented, i.e. 72% of cases. The female sex constituted 59%; the household profession was the most represented, i.e. 43% of cases, and the marital status was married (70%) of cases. Dental pain was the most frequent cause of consultation (98%), followed by dental mobility (2%), and self-medication was used by 37% of our patients. Dental caries was the most common diagnosis, accounting for 77.87% of cases. Conclusion: Early consultation and appropriate management can prevent the most serious complications of oral diseases.

ABSTRACTSupernumerary teeth (ST) are odontostomatologic anomaly characterized by as the existence excessive number of teeth in relation to the normal dental formula. This condition is commonly seen with several congenital genetic disorders such as Gardner's syndrome, cleidocranial dysostosis and cleft lip and palate. Less common syndromes that are associated with ST are; Fabry Disease, Ellis-van Creveld syndrome, Nance-Horan syndrome, Rubinstein-Taybi Syndrome and Trico–Rhino–Phalangeal syndrome. ST can be an important component of a distinctive disorder and an important clue for early diagnosis. Certainly early detecting the abnormalities gives us to make correct management of the patient and also it is important for making well-informed decisions about long-term medical care and treatment. In this review, the genetic syndromes that are related with ST were discussed.

Introduction: According to the World Health Organization (WHO), the determinants of health facility attendance are "personal, social, economic and environmental factors that determine the state of health of individuals or populations". The objective of this study was to study the determinants of attendance at CHU-CNOS. Methodology: The study took place within the CHU-CNOS. This was an analytical cross-sectional study that took place over a period of 6 months (July to December 2020). Patients who received treatment at CHU-CNOS constituted the study population. All patients who received treatment at the CHU-CNOS and who agreed to participate in the study were included in this study.The sample size was n = 420 patients. The questionnaire survey and the questionnaire were respectively the technique and the data collection tool. The questionnaire was administered face to face to patients to collect data. Data entry and analysis were performed on EPI INFO 2000 version 7 and SPSS software version IBM SPSS Statistics 2, respectively. Results: More than half of those surveyed was 52.1% female. The sex ration was 0.92. Bride and groom represented 66% of the study population. Of the people surveyed, 35.7% were not in school. Civil servants made up 21.9% of respondents. The majority of respondents (90.2%) found that the costs of care at the CHU-CNOS were affordable. The average cost of care was 3,965 FCFA with a standard deviation of 1,245 FCFA (3,965 ± 1245 FCFA). The 30.4% of those surveyed said that the costs of prescribed drugs were high. The average cost of prescribed drugs was 6,635 ± 1,400 FCFA. The satisfaction rate of the people surveyed was 80.7%.. Among the unsatisfied patients surveyed, 63% had the waiting time in front of the office as a reason for dissatisfaction and 18.5% felt that the reception was poor.There was a statistically significant relationship to satisfaction with the following variables: level of listening to the patient by the practitioner (p <0.001)

From 2005 to 2010, 20 consecutive patients with fully manifested neurofibromatosis type 1 (NF1) underwent elective neurofibroma resection at our institution (Departments of Plastic Surgery and of Odontostomatology). Specimens were photographed under optical microscope and confocal laser scanning microscopy (CLSM) with ultra-high accuracy of detail, including depth of field. Patients were followed up for a minimum of 4 years and up to a maximum of 12 years, postsurgery. While all nonrecurring lesions showed intense fluorescence, six of the seven lesions with absence of fluorescence under CLSM recurred at a mean of 5.5 years after surgical excision. Among the re-excised lesions, 3 were diagnosed as malignant at the subsequent removal. Despite the limitation of a small cohort, CLSM appears to be a simple and low-cost technique to differentiate forms of neurofibromas with low and high risk of recurrence and malignant degeneration.

Abstract:
Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is the most serious side effect in patients receiving bisphosphonates (BPs) for neoplastic disease and osteoporosis. The aim of this study is to propose a new dimensional stage classification, guiding the surgical treatment of BRONJ patients, and to evaluate the success rate of this new management. From 2004 to 2013, 203 neoplastic and osteoporotic patients with 266 BRONJ lesions were referred to the Odontostomatology Unit of the University of Bari. All patients underwent surgery after suspension of BPs therapy and antibiotic treatment. The surgical procedure was complemented by piezosurgery and followed by the application of hyaluronate and amino acids. The new dimensional staging suggests the choice of the surgical approach, and allows the prediction of postoperative complications and soft and hard tissues healing time, guiding the surgical treatment protocol. This protocol could be a successful management strategy for BRONJ, considering the low recurrences rate and the good stabilisation of the surgical sites observed after a long-term follow-up.

Abstract:
Aim. To investigate the role of dental malocclusion treatment in the outcomes of Recurrent Acute Otitis Media (RAOM). Materials and Methods. The clinical outcome (number of acute recurrences in 12 months) of 61 consecutive children treated medically for RAOM was analysed. Children underwent an odontostomatologic evaluation, a fiberoptic endoscopy, and skin-prick tests. Results. 32 children (group A) were diagnosed with dental malocclusion and treated with a mandibular repositioning plate. Dental malocclusion was ruled out in the other 29 patients with RAOM, and they were used as controls (group B). The two groups were homogeneous in terms of sex, exposure to RAOM risk factors, skin test results, and adenoid hypertrophy, while age was significantly higher in group A. Age, sex, exposure to RAOM risk factors, adenoid hypertrophy, and skin test results were not associated with RAOM outcome. Children in group A treated for dental malocclusion were strongly associated with a lower number of acute episode recurrences at both univariate (p<0.0001) and multivariate analysis (p=0.001). Conclusions. RAOM showed better outcomes in children with dental malocclusion wearing a mandibular repositioning device. Dental malocclusion in children with RAOM may play a role in the pathogenesis of Eustachian tube dysfunction.

Abstract:
Odontogenic infections represent a frequent condition that in some cases, if not treated promptly, can spread quickly to the rest of the body and turn into life-threatening infections. In this work, the case is reported of a 59-year-old woman, diabetic and overweight, who presented to the Odontostomatology and Otolaryngology Section of the Policlinic of Bari with mandibular tooth infection that had developed into a deep neck space infection leading to the involvement of cavernous sinuses and near mediastinum. The diagnosis, the surgical drainage of the phlegmon and removal of infection foci, appropriate control of the airways, and a correct antibiotic therapy made it possible to avoid a potentially fatal condition. Prompt management and early diagnosis of deep space neck infections, such as phlegmon and/or necrotizing fasciitis, with the auxilium of CT scans and tools such as LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis), NLR (Neutrophil-to-Lymphocyte Ratio), and LRINECxNLR scores (Laboratory Risk Indicator for Necrotizing Fasciitis and Neutrophil to Lymphocyte Ratio), are advised to evade delays and complications that could potentially worsen the patient’s outcome.

Abstract:
Tooth loss is, for the majority of patients, a strong physical and psychological disorder. The objective of this study was to evaluate the individual perception of dental aesthetics among patients received for joint prosthetic rehabilitation in the prosthesis department of the CHU-OS of Bamako. This was an epidemiological, descriptive and prospective study. The survey lasted 5 months, from April to August 2013. The population studied is made up of all the people seen in the prosthesis department. The variables studied were socio-demographic (sex, age) and clinical variables (reason for consultation, color, shape and position of teeth, personal appreciation of aesthetics, smile identification, patient satisfaction). The data obtained were entered and analyzed with SPSS version 20 software and processed with Word 2013 software. We examined 218 patients and the distribution according to sex showed a predominance of women with 67% with a sex ratio of 0.49. The study highlighted the youth of our population with 34.86% of people who are between 25 and 34 years old. In this study, white and well-positioned teeth are the dental aesthetic criteria most sought after by patients with respectively 91.3% and 54.6% of cases considering the position of the tooth always in association with the color as an important element in the aesthetics of a smile. The disability of edentulism opens up a psychological dimension that the practitioner must understand. It is a global care of the patient with his doubts and anxieties.

Abstract:
Peripheral giant cell granulomas (PGCG) associated with hyperparathyroidism (HPT) are rare clinical entities. The aim of this study is to report on 21 PGCGs of the oral cavity as the first clinical sign of unknown primary HPT (PHPT) referred to the Complex Operating Unit of Odontostomatology of Aldo Moro University of Bari from 2009 to 2019. Surgical treatment consisted in conservative enucleation of the lesion, if possible, with contextual bone rim osteoplasty with piezosurgical tools and following histological examination. After histological diagnosis of PGCG, PHPT screening was performed dosing parathyroid hormone and serum calcium. In all the patients haematological investigation demonstrated elevated values of parathyroid hormone and serum calcium ruling out an unknown PHPT. Specifically, after endocrinological evaluation, patients showed PHPT related to: parathyroid adenoma (13), parathyroid hyperplasia (two, one of which occurred in a intra-thyroidal parathyroid), and parathyroid carcinoma (1) and were scheduled for surgical treatment. Considering that PGCGs could represent the first clinical sign of an undiagnosed PHPT and the screening of PHPT is a non-invasive and cheap exam, in case of histological diagnosis of a giant cell lesion, both central and peripheral, especially in patients with synchronous or history of methacronous giant cell lesions, parathyroidal screening should be mandatory.

BACKGROUND: Maxillary Lefort I osteotomy, mandibular bilateral sagittal split ramus was frequently used in correcting skeletal class III malocclusion. There was a lack of research on class III malocclusion patients’ quality of life (QoL) after bimaxillary osteotomy. AIM: Class I Intermaxillary relationship was achieved, aesthetic was significantly improved. Significant improvement in Class III skeletal patients’ quality of life was acquired. The achievement of harmonious face would be beneficial to the facial aesthetics of patients, thus improving the quality of life. METHODS: Harmonious face index is an effective criterion in assessing the surgery’s outcome. In this study was conducted on 30 patients at Hanoi National Hospital of Odontostomatology, Viet Duc Hospital, and Hong Ngoc Hospital from April 2017 to April 2018, and it was a quasi-experimental study with self-comparison, 12 months follow up. RESULTS: Orthognathic surgery effectively corrected malocclusion crossbite, dental compensation, and helped to improve facial aesthetics. 100% of patients had the quality of life improved, good quality of life consisted of 86.7%. In comparison with a harmonious facial index of Kinh ethnic in Vietnam, 70% of patients achieved skeletal harmony, 63.3% of patients achieved dental harmony, 80% achieved soft tissue harmony. CONCLUSIONS: Vietnamese harmonious facial index should be used in planning and pre-surgical simulation

Abstract:
Introduction: Noma is defined as a gangrenous ulcerative stomatitis whose starting point is endobuccal. Its exact etiology remains unknown, but many risk factors have been described (malnutrition, poor hygiene, etc.). Chronic lymphoid leukemia (LLC) is a lymphoproliferative syndrome characterized by medullary proliferation of a B lymphocyte clone. It is not considered as a risk factor for noma disease. Observation: A 43-year-old patient is admitted in the odontostomatology unit of the Dapaong Regional Hospital Centre (Togo) for a deep lesion of the left cheek. The clinical examination allows to conclude the presence of a noma. Biological examinations also show a LLC at Binet stage C. In our patient, the LLC is associated with an immunosuppression and the development of infections due to the late diagnosis of the LLC. Comment: The immunosuppression and the development of infections are described in the literature as risk factors for noma disease. An association between LLC and noma could therefore exist. However, such association has been reported to date only once in the literature in 1976. Conclusion: Our observation suggests that the LLC could be a risk factor for noma disease. However, further studies based on large samples are necessary to conclude a causal association between LLC and noma.
