Abstract:
The majority of today's elderly people were primarily infected with Mycobacterium tuberculosis at a time when no effective chemotherapeutic treatment was available. With the progressive decline in cell mediated immunity that accompanies aging, the potential to reactivate a dormant lesion, or to be re-infected increases. The latter particularly applies in areas of high density living e.g. homes for the elderly. The incidence of pulmonary tuberculosis in whites in South Africa is very similar to that in industrialized countries (approximately 16/100 000). In a survey of old age homes in East London (South Africa) involving 809 white subjects the prevalence rate was found to be 1403/100 000; clusters were found in individual homes where up to 10% of residents had tuberculosis. The age specific incidence in the community for whites was 86/100 000, and in homes for the aged the incidence in 648 elderly subjects followed for 2 years was 1080/100 000. It is concluded that the elderly living in high density accommodation constitute a high-risk group for the development of the disease. The diagnosis of pulmonary tuberculosis in the elderly may be complicated by the high prevalence of atypical radiographic changes, difficulty in obtaining sputum, and the high false-negative rate of the tuberculin skin test. Thus, the value of haematological and biochemical abnormalities in 93 elderly tuberculotics, 113 elderly non-tuberculotics and 264 young tuberculotics were investigated. The results in the elderly tuberculotic patients were: Normochromic normocytic anaemia (70%), leucocytosis (55%), thrombocytosis (33%), rapid ESR in 90%, lymphopenia (22%) and monocytopenia (37%); hyponatraemia (60%), hypokalaemia (42%) and hypoalbuminaemia (83%), serum bilirubin (20%) and alkaline phosphatase, aspartic transaminase and lactic dehydrogenase are elevated in approximately 2/3 of patients. In comparison with the younger group (mean age 48 years) with cavitating tuberculosis, the prevalence of elderly patients (with generally mild and non-cavitating disease) with elevated bilirubin, alkaline phosphatase and liver enzymes was approximately 50% higher. When the results of liver enzyme elevations in the elderly tuberculotics were compared retrospectively with elderly patients with non-tuberculotic destructive lung disease, the former group had significantly higher values. The sensitivity (76%), specificity (48%) and positive predictive value (60%) suggest that liver enzyme abnormalities may provide useful contributory data in the non-invasive diagnosis of pulmonary tuberculosis in the elderly. The chest radiographs in 93 consecutive cases of bacteriologically proven pulmonary tuberculosis showed infrequent apical involvement (7%), with the most frequent abnormality being opacification of the middle and lower zones of the lungs; half the cases had a pleural reaction. cavitation occurs in only 1/3 of patients, and was sited equally in the apical zones and in the mid and lower zones. These findings contrast with the pattern of cavitating apico-posterior disease commonly seen in reactivated tuberculosis in younger adults. A series of 21 patients was studied to compare the yield of sputum smear examination with sputum culture for M. tuberculosis. Sputum production in non-cavitating disease was found to be infrequent and unpredictable and the number of bacilli is usually scanty. Repeated Culture of sputum for M. tuberculosis is required to improve the likelihood of obtaining a positive bacteriological diagnosis. On the basis of this study at least 4 negative sputum cultures are required to exclude the disease. In a study of 10 patients the impact of 4-drug therapy on the viability of M. tubercle in their sputum was assessed. Viable tubercle bacilli continue to be excreted in patients with cavitating pulmonary disease on treatment for up to 9 weeks. It is suggested that patients with cavitating disease should probably not be allowed to return to high density accommodation for the elderly until their sputum is clear of acid fast bacilli on sputum smear examinations. The usefulness of using annual tuberculin skin reaction (Mantoux) tests as a screening procedure was evaluated in 648 residents in old age homes. The criteria for further investigation for pulmonary tuberculosis was either recent conversion to positive (reaction equal to 10 mm or more) or a year-on-year increase of greater than 12 mm, or any reaction> 20 mm. 206 subjects were identified as "possibly having the disease" and of these the diagnosis of pulmonary tuberculosis confirmed in 13 cases. 10/13 patients had Mantoux reactions of greater than 20 mm and 3/13 between 10 mm and 19 mm. As a result of this study the recommendation is made that a yearly Mantoux test is a useful screening procedure, and will help identify a population who should be further investigated with chest radiographs and sputum cultures.

Abstract:
Set within the context that the world’s population is ageing at an unprecedented rate, it is argued that care of the elderly, and their everyday lived experiences are poised to become prominent concerns. In the shadow of this, the ageing population poses a myriad of challenges not only for the elderly but also for policy makers who put in place systems for the provision of services within residential care homes. By virtue, given that communities of elderly consumers voices are often muted within many academic analyses of social policy and service provision this study illuminates and distils communities of elderly consumers understandings of residential care homes. Given the absence of suitable literature within the fields of marketing and interpretive consumer research, this study turns to the sociological and anthropological literature of Hall and Jefferson (1976) and Levi-Strauss (1966). In doing so, communities of elderly consumers within residential care homes can be theorised as a cultural community of ‘bricoleurs’ within a ‘cultural map of meaning’. Furthermore, viewed through this lens, such bricoleurs can be seen to understand their meaningful everyday lived experiences within, and through, the use of ‘bricolage’. Emanating from eight existential-phenomenological interviews, a rich picture emerges wherein bricoleurs understandings of residential care homes can be seen to be embedded not only within, but also through, such things as the body, leisure trips, noise disturbances, death, large items of furniture, small hand-sized objects, mobility aids, quality of care and social interaction. Moreover, in the light of the resultant interpretations common themes can be seen to emerge within communities of bricoleurs social and material understandings of residential care homes, namely the notion of cultures of dependency, trauma and comfort. This research contributes to marketing knowledge in that it argues that communities of ‘elderly bricoleurs’ within residential care homes can be seen to be held together by unique understandings of cultures of dependency, trauma and comfort. Furthermore, it is also argued that elderly bricoleurs address themselves to a relatively limited amount of bricolage that enables them to keep alive actual, desired, imagined and fictional community ties. Furthermore, the reality and efficacy of cultural communities of elderly bricoleurs seems to depend on their ability to address ‘whatever is to hand’ (Levi-Strauss, 1966) in order to construct and understand their cultural maps of meaning within residential care homes.

Abstract:
As the number of elderly individuals in the population continues to increase, the demand for alternatives to group care settings will also increase. Homes for adults represent an example of the diversification of long term care options by providing care to elderly individuals in a setting that is more home-like and less institutionalized than traditional nursing homes. Since previous research is often limited to nursing home residents, the study of home for adult residents offers a fresh approach to sample selection, and provides reassurance that the needs of elderly residents are indeed being met not only in terms of custodial care but also in a therapeutic sense that enhances life satisfaction. In this study, the relationship of integrity/despair, locus of control, and life satisfaction was examined in a sample of elderly persons who reside in homes for adults. The participants were 17 males and 83 females, ranging in age from 60 to 95 and reporting fair to good health. It was hypothesized that elderly individuals who felt in control of their lives would also be more satisfied with life, Additionally, it was proposed that elderly individuals who looked back on their lives and were not satisfied would be afraid to die and would feel little or no control over their lives. Death anxiety, as measured by the Death Anxiety scale (Templer, 1970) was used to define integrity/despair. The indicator of locus of control was the Mastery Scale (Pearlin & Schooler, 1978), and life satisfaction was assessed with the Philadelphia Geriatric Center Morale Scale (Lawton, 1975). Correlation analysis yielded the following results: (a) the more internal an individual’s locus of control score, the lower the death anxiety score, (b) the more internal the locus of control, the higher the life satisfaction score, and (c) the higher the life satisfaction, the lower the death anxiety score. These findings supported the hypotheses and indicate that elderly individuals who feel in control are also satisfied with their lives and are not afraid to die.

Hong Kong cannot single out herself from the global problem of increasing elderly population. The increase of elderly population like in other countries is the by-product of dwindling of birth rate and the longevity of life caused by the advanced medical care, enhancement of nutrition and low mortality rate which prolong people's life expectancy. Provision of housing as the integral part of care to the elders at their retirement age becomes a hot topic of the government to deal with. Comprehensive housing services help to develop the concept of ageing in place, care in the community and continuum services for achievement of the healthy ageing and successful ageing of the elders Moreover, government cannot cater for the needs of all elders who require residential care services when all elders becoming old and frail. Private residential care home services fills in the gap to provide such services in the elderly market. This research studies the causes of the requirement of the private residential care homes which under-went the change of family structure, change of status of elders in the traditional families due to low fertility rate, change of economic and social aspects due to urbanization and industrialization. Development of private elderly housing services provided by government and private sector from past to present and future are overviewed. Statistics on provision of private elderly care homes places in the current market are mentioned. The studies focus on the quality of services provided by the private residential care homes for the elderly. Methodology for the research is based on a mix of qualitative and quantitative method. Measurements on the level of satisfaction of the elderly respondents are identified and itemized basing on their required physical daily personal services. Face-to-face personal contact and ' in-person' dialogues with the elderly respondents and the frontline workers and operators were conducted for questionnaires and interview results. The elderly respondents were categorized into 3 ranges, namely, the young-old, old-old and the oldest-old for analysis of their level of satisfaction of services. Different age range respondents expressed different level of satisfaction with same items of services due to different requirements at various age strata. Comparisons on the services provided by sole privately operated residential care homes, NGO and Government Purchasing Scheme are examined. Results of the research are analyzed and finalized for conclusions and recommendations. The elders' willingness of ageing in place is definite yet with expectations of improvement for upgrading life quality.

Abstract:
This dissertation looks at old age homes in British Columbia between 1920 and 1960. It describes and explains how changes in institutional care for aging men and women intersected with professional ideals, welfare state economics and political imperatives. It also considers how broader developments altered the physical and cultural world of the institution and the strategies employed by the elderly.
Facilities for the elderly changed significantly during the period. In 1920 provision of residential care for older British Columbians was extremely limited, and existing institutions operated under a poorhouse model, providing only custodial care. By 1960 a sizeable network of provincially regulated facilities had developed. Medical professionals urged that these establishments be medical facilities and that the elderly receive compassionate, therapeutic care.
However, circumstances inside these facilities limited change. While some institutions were run along medical lines, new policy initiatives were modified by strong historical links with older systems of custodial care and punitive poorhouse ethics.

Abstract:
This study concerns the characteristics of fund development directors employed in selected homes for the aged. The first purpose of this study is to develop a profile of job functions, through task analysis, among development directors in charitable homes for the aged. The second purpose of this study is to develop a profile of personal characteristics of development directors of charitable homes for the aged based on the following characteristics: age, sex, educational background, experience outside development, membership in community organizations and amount of specific training in fund development. One instrument was used to gather data for the study. It was distributed to a population of 29 development directors in charitable homes for the aged in Arkansas, Louisiana, Oklahoma, New Mexico, and Texas. This population was predetermined by an initial survey form sent to 193 chief executive officers in the five states mentioned above. Twenty-nine reported formalized programs employing a full-time person with at least a part-time involvement in fund development activities. Of the twenty-nine development directors surveyed, fifteen usable instruments were received (52 percent). A program was used for the survey that included crosstabulation of social characteristics, success in fund raising, length of time in position and educational preparation. Coded responses were manually typed into the computer. To accomplish the desired analysis, percentages and frequencies were used to treat the data. These non-Parametric procedures provide an understandable overview of the data obtained and are appropriate for the research questions. These procedures permit a summarization of the data in a manageable form. Following the tabulation of frequencies and percentages, Fischer's Exact Probability Test was computed to determine if significant relationships between actual preparation and needed preparation, personal characteristics, educational preparation, and success and usefulness of experience exist.

The disease trajectory that characterises dementia is one of progressive neuro-psychiatric, physical and cognitive decline. It is family members who often bear the brunt of the suffering that these sequential 'deaths' and losses bring. As they retain strong bonds, placement of loved ones in an institution is often seen as the ultimate act of betrayal resulting in ongoing guilt. How families perceive care given in these homes, the relationships they form with staff and the new roles they take depends on multiple variables. Although this has been the subject of some studies in the USA, UK and Europe, there is at yet no research on this phenomenon in South Africa and Africa as a whole. This study utilised Interpretative Phenomenological Analysis (IPA) to explore the experience of 10 family members of 8 patients with moderately severe to severe dementia in 5 different old age homes in Cape Town. IPA seeks to give a credible account of the interpretation of the person's experience, by using a deeper analysis whilst remaining firmly anchored in the participants account. The analysis and discussion of this study were conducted by the primary researcher and corroborated by an independent Social Worker experienced in psychoanalysis. The discussion was integrated with the literature review to enhance validity and place this research within the context of broader research on the subject. Analysis of emergent themes revealed that family members enter old age homes with a prior agenda shaped by their experiences prior to admission. Home placement is an extremely traumatic event and carries with it an ongoing heavy burden of guilt. The unwillingness or inability of medical professionals to provide information significantly increases that burden. Perceptions of how staff and administrators may think or function are often negative and distrustful and may well be misconceived. None the less they are believed to be true and heavily influence the integration of families into the home in either a positive or negative way. Old racial divides entrenched during Apartheid still hold sway and influence reactions towards and from nursing staff. Ultimately families still seek to play an active role in their loved ones lives and to seek purpose in their relationships.

Demographically the aged are the fastest growing group both world-wide and in South Africa but despite this, psychologists have neglected psycho-geriatric research and its importance. In the past, old age has been studied by psychologists as a deviation from the norm of youth. The primary focus has been on the rate of cognitive decline, the process of social withdrawal and the etiology of dementia. Although these issues are of significant importance, the positive aspects of aging should also be considered; thereby challenging the stereotype that old age means disengagement, decline and degeneration. This study aimed to explore and describe the coping and life satisfaction of the retired elderly females in a residence for the aged. A quantitative exploratory descriptive research design was employed, making use of non-probability convenience sampling. The sample consisted of 40 retired females who have been living in a residence for the aged for at least 12 months. Further inclusion criterions were that the participants should have a relatively "sound/stable" neuropsychological status (i.e., they should not be suffering from any clinically diagnosed disorders, for example, dementia and/or Alzheimer's disease). The Coping Resources Inventory (CRI) was utilized to identify the coping resources used by the participants; the Orientation to Life Questionnaire (SOC-29) was utilised as a measure of the participants' sense of coherence; the Life Satisfaction in the Elderly Scale (LSES) was employed to measure the global life satisfaction of the participants, namely on the cognitive level. A biographical questionnaire collated important demographic and background information. The data was analysed through computing descriptive statistics. Following this a Pearson product-moment correlation coefficient was utilised to inter-correlate the total score of the SOC-29 and the total score of the LSES with the individual subscales of the CRI. The results of the present study indicated the following: the SOC-29 revealed a relatively high mean score for the current sample. Results of the CRI revealed relatively high mean scores for the total scale as well as the five different subscales. The highest scores were obtained for the Spiritual/Philosophical and Physical Resources Subscales, while the lowest score was obtained for the Cognitive Resource Subscale. Results from the LSES indicated relatively high mean scores for both the total and all eight subscales. The highest scores were obtained for the Mood and xii Social contacts Subscales, while the lowest scores were obtained for the Health and Finance Subscales. Significant positive correlation was evident between the total scores of the CRI, the SOC-29 and the LSES. With regard to the individual subscales of the CRI, the results indicated significant moderate relationships between the SOC- 29 total raw score, the LSES total raw score and the Cognitive, Social and Physical Subscales scores of the CRI. However, the SOC-29 and LSES total raw scores did not correlate with the Emotional and Spiritual/Philosophical subscales of the CRI.

This study aimed to investigate the inter-relationships between self-stigma, loneliness, and culture among older adults with mental illness residing in nursing homes. This study also explored how this population experiences self-stigma and loneliness within the context of their cultural backgrounds. A mixed-methods approach was utilised. The first phase involved a quantitative face-to-face questionnaire survey (n=16). More than half of the study participants reported low levels of self-stigma (56.3%), yet a substantial number of them scored high on the self-stigma scale (43.8%). The analysis identified a statistical relationship between stereotype endorsement and marital status (sig. =.010). No relationship was identified between Internalised Stigma of Mental illness constructs (ISMI) and age, gender, religiousness, and educational level. Loneliness was identified to be prevalent among more than half of the sample (68.8%). There was also a positive correlation between loneliness, age (sig.=.062) and religiosity (sig.=.044). The second phase involved a qualitative hermeneutic phenomenological approach involving one-to-one semi-structured interviews (n=10). Seven themes emerged: ‘social loneliness’, ‘emotional loneliness’, ‘emotional reactions’, ‘coping mechanisms’, ‘insight into illness’, ‘understanding and view towards mental illness’, and ‘behavioural reactions’. Overall, the qualitative findings supported the quantitative results but also revealed additional theoretical and conceptual insight. Most participants were collectivistic-oriented, and most experienced both social and emotional loneliness. The degree of insight into mental illness played a key role in how self-stigma was experienced, while gender and culture were found to influence how loneliness was experienced. Based on the results of both phases, a new theoretical framework is posited that explains the relationships between the concepts of loneliness and self-stigma among this population. The study also evidences and discusses a wide range of methodological issues associated with the successful recruitment of nursing homes in older adult research.

Abstract:
This thesis is an examination of the definition and implementation of ‘good practice’ in residential care for senior citizens. The central contention is that ‘good practice’ is a term that has been variously defined. Different groups define it in different ways, and their definitions have changed over time. This reflexive qualitative study explores ‘good practice’ in local authority, voluntary and private residential care homes in Scotland from the perspective of policy, practice and the experience of senior citizens who live in them. The study is based on analysis of policy documents, historical studies, and reanalysed interview and survey data from two earlier studies conducted by the author and colleagues. The thesis shows that the notion of ‘good practice’ that emerges in policy and practice documents is a confused and often conflicting set of ideas. Historically, the earliest were driven by concerns over cost. In more modern times, statements about ‘good practice’ have had a more benevolent intent but are frequently flawed by paternalistic and ageist assumptions. It is shown that staff in residential homes typically adopt a different set of attitudes: their preoccupation is with safety and the avoidance of risk. Although benevolent in intention, these interpretations of ‘good practice’ are also at variance with what residents themselves actually want. Two particular models or styles of care are examined in detail. One of these is the use of ‘keyworkers’, often implemented in ways that fail to realise its potential. The other is the ‘hotel’ model of care. The potential of this model as an alternative to the statutory model is explored. The thesis concludes that it is a model that can realise the goal of enabling residents to exercise independence, choice and privacy while meeting their needs in residential care.

Resumo: A fim de contribuir para a melhoria das instituições de longa permanência, essa pesquisa tem como objetivo analisar como três segmentos distintos ? asilados, voluntários e gestores ? constroem e ressignificam a história de um asilo e as suas histórias de vida a partir dos vínculos institucionais. A pesquisa foi desenvolvida numa instituição de longa permanência, a Vila São Vicente de Paulo, fundada em Atibaia - SP em 1924. A metodologia usada foi a pesquisaação, utilizando-se no processo de reconstrução da história institucional e da história dos sujeitos imagens fotográficas do acervo oficial associadas à história oral. A análise dos dados foi composta: a) da visão panorâmica das narrativas individuais produzidas da seleção de imagens; b) discussão dos principais temas levantados em cada segmento associando-os aos conceitos encontrados na literatura: afetividade na velhice asilada, declínio físico e finitude (asilados), valorização das atividades e dos relacionamentos sociais (voluntários) e visão administrativa do cuidar de velhos (gestores); c) interpretação das fotografias excluídas. A afetividade e a sexualidade dentro do asilo são vividas de forma emblemática, ou seja, há estratégias do cotidiano, como as festas, para poder expressá-las. A finitude é um tema constantemente presente no discurso. Os voluntários pretendem ser o vínculo com o mundo externo e proporcionar atividades sociais consideradas fundamentais. Existe a preocupação dos gestores em administrar a instituição oferecendo mais do que alimentação, cuidados de saúde e habitação, mas a maioria sente que não tem formação gerontológica para fazê-lo
Abstract: Intending to improve the long-term care institutions, this study aims to analyze how three distinguished groups - dwellers, volunteers and managers - affect na institution's history and the new significance of their own lives concerning their relationship with it. The research was developed in Vila São Vicente de Paulo, a long-term care institution in Atibaia, SP, founded in 1924. The methodology used was action-search using photographs from the institution?s collection, and oral history throughout the history reconstruction. Data analysis involved three steps: A) overview of the individual narratives, recorded from the chosen photographs, b) discussion of the themes considered the most important ones for each group na linking them with concepts of the literature: affect in the old age in an institution (for the elderly), bodily decay and finitude (dwellers), valorization of activities and social relationship (volunteers) and perceptions of caring for older persons (managers); c) interpretation of the photographs which were not chosen. Affection and sexuality in the institution are dealt with in symbolic ways, with daily strategies like parties and events. The theme finitude is frequently mentioned somehow. The volunteers intend to link institution and extramural community, and provide activities considered (by them) fundamental. the managers concern about caring for the institution and offering more than only proper alimentation, health caring and homes, but many consider themselves insufficiently prepared concerned knowledge

This study uses a qualitative in-depth research design to explore the attitudes that community living people over 85 years of age (n=lO) hold towards relocating to an aged care facility. Aged care facilities are supported accommodation options for the elderly that were previously known as nursing homes or hostels (Commonwealth Department of Health and Family Services, 1997). This type of accommodation currently houses 31 % of the total population of people over 85 years of age (Australian Bureau of Statistics, 2004-2005). Attitudes towards new environments are associated with relocation adjustment, and a review of the literature reveals negative perceptions by younger elderly people towards aged care facilities. Demographic trends indicate a rapid increase in the number of very-old people who are in the high-risk group for admission to aged care facilities; however there is a paucity of research regarding their attitudes towards such relocation. There are many implications for aged care service providers as a result of these demographic trends, but particularly in relation to accommodation options for the elderly. This research examines the attitudes that the very-old hold about going into residential care. It also explores the emotions underpinning that attitude. The main areas that emerged included concerns over media representation, perceived lack of control and fear of a loss of independence. There were very negative attitudes toward the loss of the home itself followed by concerns over loss of personal possessions. The very-old have such rich histories embedded in their home and possessions that these things become a part of their identity and culture (Moore, 2000) rather that a separate entity, and therefore the loss of these possessions could ultimately lead to the loss of self. The results have proven to be generally negative toward relocation to an aged care facility and combined with an assumption by the participants that there is no opportunity for future planning once in care, some expressed they would choose death rather than relocation. The implications of allowing these negative attitudes to continue without interventions based on further research and community consultation, will only add to the relocation stress syndrome already being experienced by many of very senior members of society (Capezuti, Boltz, & Renz, 2004).

Le vieillissement de la population est un défi en termes d’accompagnement de la dépendance physique et/ou psychique du sujet âgé.Une étude épidémiologique dont l’approche est pluridisciplinaire, et qui utilise les outils de l’anthropologie en insistant sur l’approche qualitative menée sur dix EHPAD (Limousin et Ile de La Réunion) a eu pour objectif de contribuer à l’amélioration des connaissances sur les expériences vécues par les personnes âgées et les familles de résidents Alzheimer, les étudiants et les professionnels de santé. Pour cela, les représentations socio-culturelles/expérience vécue de la population de l’étude ont été explorées ainsi que la décision d’entrée, la vie quotidienne et la prise en soins prescrite, perçue et réelle en EHPAD.Ainsi, les résultats montrent que l’entrée en EHPAD est multifactorielle et influence les stratégies d’adaptation des résidents. Le contexte de vie contraint, encadré par des règles de fonctionnement renvoyant à la vie en collectivité, collectivité de « vieux », impacte le processus de socialisation de la personne âgée. Vivre en maison de retraite, c’est un changement socio-culturel radical. D’ailleurs, exercer en maison de retraite pour les professionnels de santé relève d’un travail sur la posture et l’identité professionnelles en raison du conflit entre les représentations de la vieillesse et les expériences vécues. Cela implique le développement de compétences liées aux savoirs relationnels. En outre, pour les étudiants en santé, faire son stage en EHPAD, c’est découvrir une institution qui est un véritable lieu de vie et de travail envisageable. Pour finir, l’EHPAD bouleverse les représentations de la vieillesse, ce qui questionne l’accompagnement de l’entrée en EHPAD discuté dans ce travail
The ageing of the population is a challenge in terms of accompanying physical and / or psychic dependence of the elderly subject.An epidemiological study whose approach is multidisciplinary, and which uses the tools of anthropology by emphasizing the qualitative approach, conducted on ten retirement homes in France (Limousin and Reunion Island) was intended to improve knowledge about experiences of seniors and families of Alzheimer’s residents, students and healthcare professionals.To achieve this goal, socio-cultural representations / lived experiences of the study population were explored as well as the entry decision, daily life and care taken prescribed, perceived and actual in EHPAD (acronym for nursing home in French).Thus, the results show that the entry into accommodations structures is multifactorial and influences the adaptation strategies of residents. The forced living context, framed by operating rules referring to community life, the “old” community, has an impact on the process of socializing the elderly person. Living in a retirement home is a radical socio-cultural change. Besides, practicing in a retirement home for health professionals is a work on professional posture and identity because of the conflict between representations of old age and lived experiences. This involves development of skills related to relational knowledge. In addition, for health students, doing an internship in EHPAD means discovering an institution that is a real place to live and potentially work.Finally, the EHPAD upsets the representations of old age, which questions the support of the entry into EHPAD discussed in this work

Introduction. Municipal eldercare is increasingly multifaceted. First line managers in nursing homes and home help services are responsible for personnel, budget and processes. The resources and work conditions of these managers have changed over time. The first line manager is often responsible not only for economic but also for quality development of their unit. Research focusing on first line management within municipal eldercare is lacking in specific contexts, for example, research on leadership in nursing homes and home help services. Aims. The aim of this thesis is to study and analyze perceptions of leadership in eldercare. The main objective is to analyze differences in leaders’ and nursing assistants’ perceptions of leadership in nursing homes and home help services, and to study leadership factors and their associations with psychosocial work environment among nursing assistants who are engaged in eldercare. Design and methods. This thesis is based on two studies, and uses cross-sectional data from two different surveys: the Developmental Leadership Questionnaire, DLQ (Larsson, 2006a) and a Questionnaire for Psychological and Social factors at Work, QPS, (Dallner, et al.,2000). Assessments from 21 leaders and 95 nursing assistants in nursing homes and home help services were analyzed in study I. Assessments from 1132 nursing assistants in nursing homes and in home help services (n=844 nursing homes and n=288, home help service units) were analyzed in article II. The data were analyzed with descriptive and analytic statistics. Results. Nursing assistants in nursing homes assessed their leaders to be more controlling and observant than nursing assistants in home help services. According the leaders self assessment, leaders in home help services were less successful at being supportive, being a good example, and promoting creativity than leaders in nursing homes. Psychosocial work environment was related to leadership factors, but stronger associations occurred more frequently in nursing homes than in home help services. The leadership factors empowering leadership, support from superiors, focus on human resources and control over decisions were associated with better results on all the variables that were related to the psychosocial work environment in both nursing homes and in home help services. Conclusions and implications for practice. Organisational differences in conducting leadership in old age care must be considered. Some leadership characteristics are better conditions for creating and maintaining a positive psychosocial work environment for nursing assistants in nursing homes and home help services. Due to the differences in organisational settings, it is important to consider the differences in conditions for conducting leadership. To influence nursing assistants’ performance and to increase quality in eldercare in the long term, appropriate leadership is necessary. Therefore, it is important for the leaders to provide support in work, help to develop skills, encourage employees to participate in important decisions, and show interest in nursing assistants’ well-being. Also, the leaders might benefit if they reduce the specialisation of work, which counteracts the possibility to have variation in work in nursing homes and home help services. In particular, nursing homes should benefit if they reduce the amount of role conflicts, and if nursing assistants receive reasonable quantitative work demands. The municipality eldercare should take into account the organisational differences in conducting leadership in nursing homes in relation to home help services. Recruiting leaders with characteristics that are desirable in the specific context will create better conditions for a good psychosocial work environment in nursing homes and home help services.

La tendencia de crecimiento del segmento de la tercera edad tanto a nivel mundial como en el Perú y la menor disponibilidad de tiempo por parte de los hijos para encargarse del cuidado de los adultos mayores , se percibió como solución a dicho problema, la posibilidad de satisfacer las necesidades de atención de las personas de la tercera edad a través de medidas como las residencias para adultos mayores. Sin embargo, existe la posibilidad de dos escenarios por parte de los consumidores respecto al servicio: aceptación o rechazo. El objetivo general de la investigación ha consistido en determinar los elementos que principalmente influyen en el comportamiento de consumo del servicio de residencias geriátricas, por parte de aquellos adultos mayores con capacidad de decisión propia. Tal objetivo ha implicado identificar las principales necesidades que inducen a las personas de la tercera edad a contratar el servicio , determinar los factores del comportamiento del consumidor que influyen con mayor relevancia e identificar los actores que intervienen e la decisión de consumo. Para la solución del problema de investigación se ha empleado una metodología de carácter cualitativa dado que permite profundizar en los puntos de vista de interpretaciones de los individuos. La unidad de análisis ha sido enfocada en personas de la tercera edad, especificamente de 89 años a más, de los niveles socioeconómicos A, B y C de Lima metropolitana; cuyas facultades mentales y capacidades para tomar decisiones propias se encuentran con lucidez. La técnica selccionada ha sido la de entrevistas en profundidad dado que permiten un intercambio libre de información, sin ningún tipo de presión grupal, además de brindar mayor eficacia para tratar temas sensibles y preguntas que pueden invadir la privacidad de las personas. Por último, el muestreo empleado ha sido el de bola de nieve.

No Brasil, a porcentagem de pessoas idosas e sua expectativa de vida aumentam gradualmente. O envelhecimento é considerado um fenômeno natural, social, cultural e econômico. Ademais, a redução da taxa de fecundidade, diminuição de integrantes das famílias, as mudanças nos padrões de nupcialidade e a crescente inserção da mulher no mercado de trabalho reduzem a disponibilidade de cuidadores familiares; tudo isso torna o idoso mais suscetível ao stress. Assim, pensar em questões referentes ao envelhecimento e ao dia a dia dos idosos passa a ser uma necessidade dentro da realidade brasileira. Entre os serviços especializados voltados para a população idosa em nosso país, estão as ILPIs (Instituição de Longa Permanência para Idosos), responsáveis por auxiliar os idosos em suas tarefas diárias e/ou dificuldades relacionadas ao envelhecimento físico, psicológico, cognitivo e social. O aumento paradoxal do nível de stress na vida cotidiana e da longevidade dos seres humanos convida a refletir sobre as estratégias desenvolvidas para minimizar o efeito deletério do agente stressor. Assim, este estudo pretende compreender e descrever o stress, a partir da perspectiva ainda pouco estudada, de idosos em situação de dependência física e/ou financeira, com autonomia cognitiva-funcional, residentes em ILPIs. Investigaremos a relação entre a história pessoal e o nível de stress nessa população, buscando compreender e descrever o impacto de eventos estressores em sua qualidade de vida. Foram entrevistados apenas idosos em condição de autonomia cognitivo-funcional, com idade igual ou superior a 70 anos, que residem em ILPI, na cidade de Santos SP. Para tanto, foram aplicados um Quesionário de Informações Gerais, O Mini Exame do Estado Mental, o Life Events Units (LEU/VAS) e algumas Questões Complementares. Entre os eventos de vida com nível de stress mais relevantes, estão Morte do Cônjuge, Acidente ou Doença, Casamentos, Aposentadoria, Gravidez, Nascimento de Crianças na Família, Morte de Alguém na Família, Mudança na Condição Financeira, Férias e Cirurgia. Foi possível observar que, os eventos citados atingiram nível de stress altíssimo no período em que foram vividos e nível alto ainda nos dias de hoje. Apesar dos altos níveis de stress encontrados, o grupo em que houve diminuição do nível de stress corresponde a 100% da amostra
The percentage of senior citizens and their life expectancy have been increasing gradually in Brazil. Aging is considered to be a natural, social, cultural and economic phenomenon. Therefore, questions concerning this phenomenon and the daily life of the elderly people have become a necessity in the Brazilian society. The ILPIs (long stay old age homes) are the institutions responsible for assisting the senior citizens in need of special care such as carrying out daily chores and/or difficulties related to physical, psychological, cognitive and social aging. Furthermore, factors like the decrease of the fertility rate, the decrease of members in families, changes in marriage patterns and the increase of the participation of women at the work market , have affected the availability of relatives to become care takers. Therefore, the elderly have become more susceptible to stress as before. The absurd rise of stress levels in our daily lives and the increase of our longevity, invite us to reflect upon the strategies which should be developed in order to minimize the harmful effect of the stress then.That being the case, this study intends to cast light on the comprehension and description of stress in the elderly people who find themselves in an economical and/or physical dependency but are still cognitively and functionally fit and living in the ILPIs, mentioned above. We will investigate the relationship between their life history and the stress level while living in the old age homes. Only cognitively and functionally fit elderly people have taken part in our research, at the age of 70 and older. The following tests and questionnaires have been applied: Firstly, a general questions questionnaire, then The mental health status examination, The Life Events Units(LEU/VAS) and other complementary questions. Relevant events which are linked to the rise of the stress levels are: the death of a spouse, accident or illness, marriage, retirement, pregnancy, the birth of children in the family, death in the family, financial difficulties, holidays, and surgery. It is important to mention that the stress levels have reached their highest during the occurrence of these life events mentioned before, and they still have an impact on the stress levels presently. Despite of the high levels of stress, we could observe that the people in the group which had a stress reduction corresponds to 100% of the samples taken
