Abstract:
© 2017. Abstract:
Abstract Purpose The aim of this study is to investigate the effect of an edible saliva substitute, oral moisturizing jelly (OMJ), and a topical saliva gel (GC) on dry mouth, swallowing ability, and nutritional status in post-radiotherapy head and neck cancer patients. Methods Sixty-two post-radiation head and neck cancer patients with xerostomia completed a blinded randomized controlled trial. They were advised to swallow OMJ (n = 31) or apply GC orally (n = 31) for 2 months. Outcome measures were assessed at baseline, 1, and 2 months, including subjective and objective dry mouth (Challcombe) scores, subjective swallowing problem scores (EAT-10), water swallowing time, clinical nutritional status (PG-SGA), body weight, and dietary intake. Results After 1 and 2 months of interventions, subjective and objective dry mouth scores, subjective swallowing problem scores, swallowing times, and clinical nutritional status in both groups were significantly improved (p < 0.0001). Compared to GC, OMJ group had higher percent improvement in all outcome measures (p < 0.001) except swallowing time and clinical nutritional status. Interestingly, subjective dry mouth scores were significantly correlated with subjective swallowing problem scores (r = 0.5321, p < 0.0001). Conclusions Continuous uses of saliva substitutes (OMJ or GC) for at least a month improved signs and symptoms of dry mouth and enhanced swallowing ability. An edible saliva substitute was superior to a topical saliva gel for alleviating dry mouth and swallow problems. These lead to improved clinical nutritional status. Thus, palliation of dry mouth may be critical to support nutrition of post-radiotherapy head and neck cancer patients. Clinical trial registry Clinicaltrials.gov NCT03035825

Ponder, Felix. "Nine-Year Response of Hardwood Understory to Organic Matter Removal and Soil Compaction." Northern Journal of Applied Forestry 25, no. 1 (March 1, 2008): 25–31. http://dx.doi.org/10.1093/njaf/25.1.25.

Abstract:
Abstract The effects of three levels of organic matter removal (OMR) and three levels of soil compaction (SC) on the development of understory vegetation in a central hardwood forest were evaluated 9 years after treatments were applied as part of a national program of long-term soil productivityresearch. The three levels of biomass removal (OMR) were removal of merchantable boles only (OM0), removal of the whole tree (OM1), and removal of the whole tree plus forest floor (OM2). The three levels of soil compaction (SC) were none (C0), medium (C2), and severe (C2). Weeds were controlledin all plots for the first 2 years. Understory vegetation within 81 7.9-m2 subplots was inventoried by species and quantified into plant groups of woody (trees, shrubs, and woody vines) and herbaceous (annuals, perennials, and grasses) at year 5 (after 3 years of no weed control)and year 9 (after 7 years of no weed control). Vegetation was analyzed for nitrogen (N), phosphorus (P), potassium (K), calcium (Ca), and magnesium (Mg). OMR did not significantly affect the overall number of plants over the 5-year measurement period, but there were differences for both woodyvines and grasses, which were highest in the OM0 treatment in 1999, but by 2003, they were not different. There were no differences in plant numbers among plant groups for SC in the 1999 measurement period between treatments for any plant group, but there were significantly fewer trees andwoody vines in the C2 treatments than in the C0 or C1 treatments in 2003; the opposite was true for herbaceous annuals, which were highest in C1 and C2 treatments. Over the 5-year measurement period, only the height of woody vines was significantly affected by OMR, but SC significantly affectedthe height of all plant groups over the 5-year measurement period. Annually, however, trees were tallest in the OM0 and C0 treatment than in OM2 and C2 treatments. The annual height of other plants, excluding trees, was affected only 1 year of 5 by OMR. Fewer trees and shorter trees in thesevere compaction treatment suggest that, in the short term, soil productivity has been affected on the site.

Abstract:
Abstract Context The Touro University California College of Osteopathic Medicine (TUCOM-CA) is one of many colleges of osteopathic medicine with osteopathic manipulative medicine (OMM) predoctoral teaching fellowship programs. OMM fellows serve as near-peer teachers for preclinical osteopathic medical students (OMS) at TUCOM-CA, with the objectives of increasing student satisfaction with and understanding of the OMM curriculum. Our aim was to assess whether the TUCOM-CA fellowship program has achieved these objectives. Methods All osteopathic medical students at TUCOM-CA were sent an electronic survey. The survey items queried: frequency of and type of interaction; impact on satisfaction with and understanding of the OMM curriculum; impact on confidence in using osteopathic manipulative treatment (OMT); valuation of OMT and intention to use OMT in future clinical practice. Frequencies, means and standard deviations were calculated, omitting “no basis for evaluation” responses. Two-tailed Z-tests of proportions were utilized for analysis of statistical significance, with significance set at 95% (P<.05 ). Results In total, 156 of 538 (29.0%) responses were received, and 150 had sufficient data to analyze. Respondents reported varied rates and modes of interaction with OMM fellows (OMM lab table-training [97.3%] to early clinical experiences [30.9%]). Response means for items regarding satisfaction with and understanding of the OMM curriculum fell between “strongly agree” and “agree” for all activities. Many respondents (82.5%–83.8%) reported that interaction with OMM fellows increased their confidence in using OMT. Additionally, respondents who were treated with OMT by OMM fellows reported significantly higher agreement with statements about clinical utility of OMT (Z=2.6, P<.05) and intention to use OMT in future practice (Z=2.3, P<.05). Conclusions The majority of osteopathic medical students at TUCOM-CA reported significant agreement with the positive impact of interaction with OMM fellows on satisfaction with and understanding of the OMM curriculum. This supports the conclusion that the OMM predoctoral teaching fellowship program achieves its objectives to increase student satisfaction with and understanding of the OMM curriculum. The survey data also showed significantly stronger agreement with statements supporting valuation of OMT in clinical practice and intention to use OMT in the future, among respondents treated with OMT by OMM fellows.
