Full textBakgrund: Tidigare forskning visade att ett kirurgiskt ingrepp är en stressfylld upplevelse både fysiskt och psykiskt eftersom kroppens integritet hotas. Preoperativ oro var ett vanligt problem vid elektiv kirurgi. Omvårdnad var ett stöd till medicinsk behandling t.ex. genom information som minskar patientens oro och ängslan samt ökar patientens tolerans för smärta. Syftet: Var att beskriva vad patienterna är oroliga för inför elektiv kirurgi samt ge kunskap i vad sjuksköterskan omvårdnadsmässigt kan göra för att minska oron inför elektiv kirurgi. Metod: Beskrivande litteraturstudie baserad på fyra kvalitativa och 16 kvantitativa artiklar från år 2001-2008. De sökord som användes var Anaesthesia, Anxiety, Nursing, Preoperative, Presurgery och Surgery. Resultat: Fyra centrala fynd framkom; orosmoment, information, musik och akupunktur/ akupressur/värme. Separation från familjen, funktionsoduglighet, att förlora sin självständighet, rädsla för kirurgi och död var alla faktorer som triggar symtom på preoperativ oro. Resultatet visade att musik kan minska oron signifikant. Den omvårdnadsåtgärden kunde tillämpas på flera olika sätt och har visat sig effektiv oavsett vilken musik patienten lyssnar på. Information var viktigt och det bästa var om den individanpassas.
Background: Previous research has shown that a surgical operation means a lot of stress both physically and mentally since the human integrity is threatened. Preoperative anxiety was a common problem at elective surgery. Caring was a support to medical treatment for example information that decreases the patient anxiety and increases the tolerance for pain. Aim: The aim of the research was to describe the preoperative anxiety in elective surgery and to give knowledge in what the nurse can do to decrease this anxiety. Method: Descriptive literature review based on four qualitative and 16 quantitative articles from year 2001-2008. The searching words that was used was Anaesthesia, Anxiety, Nursing, Preoperative, Presurgery and Surgery. Findings: Four central categories emerged; things patients worries about, information, music and acupuncture/acupressure/heat. Being separated from the family, incapacitation, loss of independence, fear of surgery and death were all things that contribute to preoperative anxiety. The findings showed that music can decrease the anxiety significantly. It can be carried out in different ways and has shown to be effective irrespective of what music the patient is listening to. Information was important and the best thing if it´s adapted to the individual.

Full textBackground: Malnutrition is common in patients with chronic obstructive pulmonary disease (COPD). Malnutrition in patients with COPD can cause impaired immune system, increased dyspnoea, decreased quality of life, and the COPD risk being increased in severity as well as having a faster disease course. In 2030, COPD is estimated to be the third most common disease in the world. Therefore, it is important as a nurse to have knowledge of which nurse care can be used in malnutrition in COPD patients. Aim: The aim of this literature review was to compile COPD patients' perceived problems with nutrition and what nurse care the nurse can take in malnutrition. Method: A literature review based on 15 scientific articles. Results: There were problems that occur for COPD patients that were caused according to their nutrition. These conditions could be both physical and psychological. The nurse could seek advice from a dietician to be informed of the best solution regarding nutritional needs of the patient. As a nurse it was important to pass on knowledge and give support to the patient whilst also advising on selfcare according to the patient's current condition and preferences. Conclusion: The variety of nutritional problems COPD patients face were highly individual and are therefore a complex subject as the effects are both physical and psychological. In general, the nurse should work closely with the patient as there can be nutrition related problems and in doing so, ensures appropriate and safe care.

Full textThe aim of this study was to investigate nurses' experiences of the possibilities and conditions required in order to practice nursing care according to local and general nursing guidelines, exemplified within postoperative care. Data collection was preformed through six semi- structured interviews with nurses working within a surgery speciality. The analysis of the interviews was made by using content analysis. Several barriers and required conditions to work on the basis of scientific knowledge could be identified. Barriers that were experienced included the lack of time, poorly updated guidelines and unstructured integration of new knowledge.This creates an understanding of how important these found scientific factors are for a work based on scientific knowledge.

Full textKonfusion är mycket vanligt förekommande hos äldre och skapar stort lidande, ger höga samhällskostnader och orsakar hög mortalitet. Syftet med studien var att belysa sjuksköterskans icke farmakologiska omvårdnadsåtgärder vid konfusion hos äldre patienter. Metoden var en litteraturstudie, vilken innefattade tio artiklar där relevanta omvårdnadsåtgärder framkom, dessa presenterades i resultatet under VIPS - modellens åtgärdssökord. Resultatet av studien visade att patienter med konfusion mindes och uppmärksammade vad som skedde med och runt omkring dem. Viktigt var att få information om sitt konfusionstillstånd för att förstå vad som hände och varför. Samspelet mellan sjuksköterska och patient var av stor vikt. Återorientering av patienten var vanligt och ansågs oftast ha positiv effekt. Att bekräfta patientens upplevelse, och att visa omtanke och förståelse var väl fungerande stöd. Kontinuitet av personal, att eliminera bakomliggande orsaker och att främja god sömn var av stor betydelse. Vanligt förekommande var tvångsåtgärder för att skydda patienten från att skada sig själv och andra. Musik och ljusterapi togs upp som omvårdnadsåtgärder. Även närstående hade en stor roll vid omvårdnaden av patienten med konfusion. Relativt lite forskning finns inom området konfusion, i synnerhet gällande omvårdnadsåtgärder i samband med konfusionstillståndet. Behov av ytterligare forskning föreligger inom området. //
Confusion is quite common among elderly and creates great suffering, high cost for social services and high mortality. The purpose of this study was to highlight the non-pharmacological actions in care provided by nurses to elderly with confusion. The method was a literature study, based on ten articles containing relevant care measures where the result is presented in the VIPS-model’s keywords. The result of the study showed that patients with confusion remembered and registered what was happening around them. It was important for the patient to obtain proper information regarding the state of confusion to understand what is happening. The teamwork between nurse and patient is of great importance. Re-orientation of patient was common and mostly considered to have a positive effect. Important support by the nurse is to confirm the patient’s perception by showing concern and understanding. Other key concepts where continuity of staff, eliminate underlying causes, facilitate good sleep. It was common with supervision and constrainer to protect the patients from harming themselves and others. Music and light therapy were mentioned as care measures. Relatives also played an important part in treating a patient with confusion. There is relatively little research regarding confusion especially with focus on suitable care actions. There is a need of further studies in this matter.

Full textMental illness among adolescents aged 10-19 years increases and there are several reasons why they suffer from it. Causes such as the desire of body change, increased accessibility and internet usage, pressure to fit in and exploration of sexual identity can affect the mental health. Depression is one of the leading causes of mental illness progression among adolescents. The purpose was to investigate how adolescents with depression experience nursing interventions and treatments. To answer the purpose, a literature study was conducted with article searches in three different databases. The result led to three main categories in this study: The nursing intervention physical activity as exercise was perceived to improve self-esteem, The nursing intervention participation was perceived to create a sense of security and the third category Treatments with focus on changes in thoughts and emotions was perceived to give insight. It is important to understand the adolescents’ experience of nursing interventions and treatment for depression because it gives an awareness into how they receive it.

Full textIt is estimated that around 200 000 people in Sweden suffer from heart failure. It is of the utmost importance to gain an understanding of heart failure from an overall perspective as the illness affects each individual in many different ways. The purpose of the study was to examine how living with heart failure affects the daily life of the patients. The focus was also laid on how nurses performed their care based on these experiences. The research process was inspired by Goodman’s seven steps. The literature study is based on 14 scientific articles. We have used, as a theoretical frame of reference Carnevalis Daily life Functional health condition – model. The result shows that patients with heart failure experience physical limitations due to fatigue, lack of energy, dyspné and sleep deficiency. The illness also affects nutrition and the psychosocial state of the patients. The result of the study also shows a lack of knowledge amongst some patients with heart failure exists. An important task for nurses was to assess and observe physical and psychosocial factors amongst patients. Relieving symptoms and informing patients about heart failure was also included in nurses´ care.

The demands of palliative care are increasingas the world's population grows older and more chronic diseases are emerging.Palliative care is nowadays considered to be a part of the basic healthcare, despitethis, it is not sufficiently accessible from a global perspective. Maintaining andpromoting the patient´s quality of life is an essential component of the palliativestage. In order to achieve a good quality of life and to meet the patient´s needs inthe palliative stage, the nurse needs to have knowledge and clarity when it comesto nursing responsibility. Purpose: The aim of this literature study was toinvestigate the nurse´s perspective on nursing responsibility for palliative stagepatients. Method: A literature study based on eleven empiric studies consisting ofqualitative approach. Cinahl, Psycinfo and PubMed was used as databases. SBU:s(2014) quality assessment template and SBU:s criteria for quality assessment(2017) was used in assessing the quality of the articles and the data was analyzedwith help from Friberg (2017). Results: Out of eleven articles emerged one maintheme; To create a dignified dying and three sub-themes; To have competence, Tosee the unique person in each patient and To see relatives. Conclusion: Nurse´sresponsibility for palliative stage patients considered to be important in promotingpatient´s quality of life. The nurse is viewed to be one of the pillars of palliativecare for the patient. A context of; To have competence, See the unique person ineach patient and To see relatives were reviewed as essential perspectives ofnursing responsibilities. Lack of time was observed as a component that couldnegatively affect the nurse´s care of responsibility.

Full textBackground: There is knowledge up to date on which nursing interventions are used in a variety of conditions, including nursing interventions for treating type 2 diabetes and obesity. There is, however, no knowledge up to date in form of a review about the nursing interventions used in treating a person suffering from both type 2 and obesity. Aim: The purpose of this study was to describe nursing interventions that the nurse carries out aiming at overweight people with type 2 diabetes and to describe the effects of these interventions. Method: This study involved 13 original works analyzed with inspiration of a deductive content analysis. The effects of nursing measures were listed along with the categories and subcategories. Result: A total of four categories regarding nursing interventions were identified; Good advice, Meaningful feedback, Cultural strategies and Coordination of care. The effects of nursing interventions often led to weight loss or diabetes values improving Conclusion: Data from 13 scientific studies, analyzed with content analysis, resulted in four distinct categories. When explained with the help of Fawcett’s meta concepts related to nursing interventions, these categories led to new knowledge that when combined nursing interventions focus on obesity and type 2 diabetes, the positive effect is clearly noticeable. Clinical significance: It is important that the nurse focuses on nursing care around the fragile state of health such as obesity coupled with type 2 diabetes. Since the nurse's commitment is of great importance for the care of patients with this type of medical condition, it is important that teaching on these conditions should be emphasized in the nursing education.

Full textBackground: Pressure ulcers occur when the skin's oxygen and nutrient supply is insufficientdue to local poor blood circulation. This causes pain and suffering to the patient. Previous research shows that pressure ulcer prevalence remains high today despite awareness of the risk factors that contribute to the development of pressure ulcers. Nurses in general rely ontheir previous knowledge and experience in the assessment and decision making regarding pressure ulcers. Aim: The aim of this study was to describe nursing interventions to prevent and treat pressure ulcers. Also to describe the articles included data collection methods.Method: Descriptive literature review. Articles published between 2010.01.01-2015.01.30 were searched in the databases Cinahl and PubMed. The result is based on 15 articles. Main result: The result showed that the most effective treatment of pressure ulcers arepreventive methods, including pressure-relieving mattresses and skin care. Nurses can also use risk assessment tools and decision support regarding pressure ulcer problems. Of the included articles in this study the data collection method is clearly described in nine of 15 items. Four of the articles describe both the validity and reliability of their data collection methods. One article only describes the validity and two of the articles describe only there liability of their data collection methods. One article had a multi-method approach and there was no description of how the interviews were performed. Conclusion: According to this study the main nursing interventions to prevent and treat pressure ulcers is to use risk assessment tools, provide skin care, pressure relieve exposed skin areas and to use decision support regarding pressure ulcer care.

Full textVenösa bensår är ett stort hälsoproblem och medför en stor kostnad för
samhället. Trots att mycket forskning har gjorts inom området
behandlingar av venösa bensår, appliceras inte detta tillräckligt i vården.
Patienter som lever med venösa bensår upplever oftast en försämrad
livskvalitet. Syftet med litteraturstudien var att ur ett
omvårdnadsperspektiv belysa effekterna av kompressionsbehandlingar
och hudtransplantationer vid venösa bensår. Resultatet var en bearbetning
av 18 vetenskapliga artiklar som sammanställdes utifrån olika
problemområden. Resultatet visade att patientens följsamhet påverkar
graden av sårläkning. Vid långvarig sårläkning upplevde patienten en
försämrad livskvalitet men om behandlingen var bekväm och enkel att
använda ökade följsamheten och därmed sårläkningen avsevärt. Patienten
upplevde smärtan som den svåraste faktorn, vid behandling med
hudtransplantation reducerades smärtan avsevärt och patienten fick en
större frihet. Majoriteten av kompressionsbehandlingarna visade goda
resultat vid sårläkning men behandlingarna var långdragna vilket
minskade följsamheten. Det är viktigt att sjuksköterskan har ett väl
utvecklat holistiskt synsätt och goda, aktuella kunskaper inom området
för att kunna ge patienten rätt vård. Många sjuksköterskor kände sig dock
osäkra och deras kunskaper var inte tillräckligt evidensbaserade. Fortsatt
forskning rekommenderas över sjuksköterskans kompetens och dess
inverkan på sårläkning vid venösa bensår samt utveckling av
sjuksköterskeutbildningen inom ämnet sårvård rekommenderas så de
nyutexaminerade sjuksköterskorna har en god grund i ämnet
sårbehandling.

Full textBackground: Overweight and obesity among children is a growing problem both globally and in Sweden. The main reasons being an increased intake of energy dense foods and reduced physical activity with an increased risk of complications such as diabetes and cardiovascular disease. Aim: To describe the nurses nursing interventions in overweight and obesity among children and to describe the data collecting methods of the included literature. Method: A descriptive literature study that includes ten articles, five with a qualitative approach and five with a quantitative approach. Main result: Counseling sessions between nurses and overweight or obese children and/or their parents are presented under two headings. Under the first heading counseling regarding diet and meal patterns are described. The nurses believe that diet counseling is an important part of their work with overweight and obesity among children. The second heading describes counseling sessions where the nurses encourage increased physical activity and reduced sedentary behavior among the children. The nurses interaction with the children's parents is described as one of the most important parts of the nursing interventions. To support the nursing interventions aids, instruments and resources in the form of other professions are used by the nurses. Conclusion: In collaboration with children and their parents the nurses primary mission is to inform, motivate and implement nursing interventions. Ultimately this gives the families the ability to implement nursing actions on their own. The focus should primarily be on the interaction with the parents as no child should be held responsible for these behavioral changes.

Full textStenosed coronary arteries cause ischaemic heart disease (IHD). The use of catheters during interventions in the coronary arteries has become more pro-nounced in treating patients with IHD. The most common intervention method using a catheter is percutaneous transluminal coronary angioplasty, PTCA and for the patient it is a major procedure, both physical and psychical. The purpose of this degree project was to examine the patient´s experiences before, during and after PTCA and which nursing interventions that should be performed be-fore and after PTCA. The purpose was also to examine whether there are any differences or similarities between men and women regarding experiences and nursing interventions. The theoretical framework used was Doris Carnevali´s model of nursing, where health is viewed as a balance between the individual´s health related demands in the daily life and the functional health status. The most important results that emerged in the study was the importance of infor-mation to the patient in order to decrease their worries and fears related to PTCA and the importance of the patient´s positioning in bed.

Full textBackground: Lymphedema is a feared complication after breast cancer surgery, causing swelling, pain and restriction of mobility in the affected arm. This condition is chronic and difficult to treat. Aim: To compile scientific literatur describing nursing activities and methods for self care performed by patients for prevention and treatment of lymphedema after breast cancer surgery. Methods: The PubMed database was used for data collection during february 2016. Four combinations of the search terms “blood draws”, “lymphedema”, “self-management”, “nurse”, “breast cancer” and “knowledge” were used. A quality review and an analysis of the results were conducted. Results: Ten relevant scientific original articles were identified; seven using a quantitative design and three using a qualitative design. Blood sampling, infusions, blood pressure measurements, trauma and air travel did not increase the risk of lymphedema despite current recommendations that this should be avoided. Patienteducation by nurses and self care were important to prevent and treat lymphedema. However, the education was perceived as inadequate by several patients, suggesting that nurses must expand their knowledge in this area. Although most patients were aware of the importance of self care, it was not performed by everyone. Lack of knowledge and support were two difficulties identified. Regular exercise decreased the risk of lymphedema. Conclusions: Some recommendations concerning nursing care related to lymphedema should be reconsidered. Patienteducation seems to be a condition for self care and should be given greater emphasis. Self care has been proven effective, and strategies for dealing with difficulties are needed. The nurse may be considered as a resource in nursing care for this group of patients and has an important function as responsible for blood sampling, injections and blood pressure readings as well as patienteducation and self care support.

Full textBakgrund Diabetes mellitus är en kronisk sjukdom som drabbar många personer runt om i världen. Diabetes mellitus delas in i två typer; typ-1 och typ-2. Diabetes mellitus typ-2 har större risk att utveckla komplikationer då dessa ökar vid dåliga levnadsvanor, rökning, övervikt och dålig nutrition. Tidigare forskning har visat att patienter inte alltid får tydlig information om sin diagnos. T ex hur den skall hanteras och vilka livsstilsförändringar som behöver göras. Sjuksköterskans omvårdnadsåtgärder är att främja eller bibehålla hälsa, förebygga ohälsa och stärka välbefinnandet. Syfte Syftet med denna studie har varit beskriva sjuksköterskans omvårdnadsåtgärder vid diabetes mellitus typ 2. Metod En beskrivande litteraturstudie som har identifierat 10 artiklar genom databasen Medline via PubMed. Dessa artiklar har analyserats och tematiserats till denna litteraturstudie. Resultat Det visade att sjuksköterskan har en viktig roll att stödja patienten till att följa behandlingsprogrammet som är ordinerat av andra vårdgivare, t ex av läkare eller dietist. Sjuksköterskan upplevde att de utförde omvårdnadsåtgärder som främjar hälsan hos patienterna på olika sätt. Resultatet har sammanställts under en huvudrubrik, Stimulera till bättre hälsa och fyra underrubriker; Motivera till bättre kost, Motivera till ökad fysisk aktivitet, Diabetesutbildning - stödja och informera och Stöd till egenvård. Slutsats Sjuksköterskans omvårdnadsåtgärder vid diabetes mellitus typ 2 är att motivera till bättre kost, motivera till fysisk aktivitet, stimulera till bättre hälsa och ge diabetesinformation. Dessutom skall sjuksköterskan stödja och granska den information som patienterna får av andra vårdgivare. Fortbildning och träning inom rådgivningsteknik, skulle förbättra motivationsarbetet som ingår i sjuksköterskans omvårdnad av diabetes mellitus typ-2.

Full textI Sverige är det omkring 200 000 personer som fått diagnosen demens. Läkemedelskostnaderna för behandling av demenssymtomen anses uppgå till 160 miljoner kronor/år i Sverige. Att använda sig utav icke-farmakologiska omvårdnadsåtgärder kan ses som ett alternativ, både ur etisk synpunkt såväl som ekonomisk. Det är därför viktigt att sjuksköterskan känner till vilka omvårdnadsåtgärder som finns tillgängliga. Syftet med litteraturstudien var att belysa sjuksköterskans icke-farmakologiska omvårdnadsåtgärder i omvårdnaden av oroliga, agiterade och aggressiva personer med demens. Studien genomfördes som en litteraturstudie där 14 vetenskapliga artiklar och två avhandlingar granskades och analyserades. I resultatet skapades fyra kategorier, aktivitet relaterat till social samvaro, musik, kommunikation och dans. Resultatet visade att för att lindra lidande hos personer med demenssymtom, är insikten om livshistorian och intressen grundläggande för det goda vårdandet. Detta kan ske genom att föreslå en meningsfull aktivitet. Exempel på sådan aktivitet kan vara dans, musik, små gruppträffar eller handarbete. Det framgår att det existerar alternativa icke-farmakologiska omvårdnadsåtgärder som kan lindra oro, agitation och aggression hos personer med demens. Dessa anses inte vila på lika hög vetenskaplig grund som farmakologisk behandling då kunskapen inom det området inte är lika evidensbaserad. Ytterligare forskning inom området är därför önskvärt i form av kliniska framtida pilotprojekt för att bevisa dess effektivitet
The number of people with dementia in Sweden is around 200 000. The costs of medication for those people are 160 million Swedish crones/year. Using non-pharmacological nursing measures can be seen as an alternative, at an ethical point of view as well as an economic. It’s therefore very important that the nurse knows about the different treatments and is capable to delegate this further. The aim of this study was to illuminate the nurse non-pharmacological nursing measures in purpose to reduce the anxiety, agitation and aggression in people with dementia. This study was a literature study where 14 articles and two dissertations were used to form the result. In the result 4 categories were created, activity related to socializing, music, communication and dance. The result shows that to relief suffering in persons with dementia it's important to know the individual and this person’s life-history, for the purpose to relief anxiety but also to divert aggressive behaviors in different situations. To know the persons interests and life-history is important to be able to give the person a meaningful activity. An example of this can be dance, music, small group meetings and handcraft. There exist non-pharmacological approach to reduce anxiety, agitation and aggression in persons with dementia. Those strategies are not as scientific as the pharmacological nursing measures because is not that evidence-based. More research in this area is therefore requested such as a clinical pilot study to prove its effectiveness.

Full textBackground: Every year, 25,000 - 30,000 people in Sweden suffer from stroke. Two-thirds of patients in acute stage of stroke suffer from dysphagia with greater risk of complications. National guidelines recommend patient with stroke are cared for in stroke unit with competent multidisciplinary stroke team, which increases patients’ chance of recovery and reduced incidence of complications. Nurses have a central role in providing support and work preventatively based on the best available evidence in order to ensure good nursing and safe care. Despite this, patients with dysphagia experience a shortage of support from nurses. Aim: The study aims to describe the nurse's knowledge of dysphagia as a result of stroke. Method: The study is a literature review based on 14 scientific articles with quantitative (n= 10) and qualitative (n= 2) approaches and mixed method studies (n=2). Article search was conducted via CINAHL and PubMed databases. Results: Two themes and three subthemes were identified and presented in the results that describe the nurse's knowledge of dysphagia as a result of stroke. The two themes of the results are: Knowledge of symptoms and signs and Knowledge of nursing measures with subthemes - assessment of swallowing ability, nutrition and oral care. Conclusion: Nurses need education as an intervention to increase their knowledge of caring about dysphagia as a result to stroke. This creates the conditions for the nurse to be able to provide good and safe care based on evidence and thus prevent complications as well as suffering for people with dysphagia as a result of stroke.

Full textBackground: The Competence Description for Swedish Certified Nurse describes the nurse's responsibility to give the patient a good and safe care from an ethical perspective and through proven experience. The patient focused work method is an approach that places the patient and his problems and needs in the centre. Nursing care quality increases and gives a quicker rehabilitation time for the patient. Objective: To examine what the direct patient time means to the nurse and see what it contains. How much time, total, spent on direct patient time in a day's work? Method: Empirical, structured non-participant observational study with quantitative and qualitative approach. The quantitative result have been processed and is presented in the form of diagrams and text. The qualitative results were analyzed with latent content analysis. Results: The study showed that the direct patient time was low at the observed ward. The measured average direct patient time was 15,5 % of a day between 7.00-13.00. Most of the direct patient time was devoted for care measures.Conclusion: The result shows that a need for change in the nurses work and its organization is eminent.

Smärta, immobilitet samt social isolering relaterat till venösa bensår, är några av de faktorer som påverkar patientens livskvalitet. Syfte: Syftet med litteraturstudien var att beskriva hur sjuksköterskan på bästa sätt kan främja läkningen av venösa bensår. Metod: Beskrivande litteraturstudie sammanställd utifrån 14 kvantitativa samt sju kvalitativa studier publicerade mellan åren 2001 till 2009. Databaserna Cinahl och PubMed användes i sökningen av vetenskapliga artiklar. Sökorden som användes var Leg Ulcer, Nursing, Activity, Pain, Venous leg ulcer, Treatment, Management, Exercise, Bandages samt Psychological. Resultat: De omvårdnadsåtgärder som visade sig ha stor betydelse för sårläkning var kompression/sårvård, fysisk aktivitet, psykologiskt stöd samt eftervård. Sjuksköterskans kunskap och förmåga att kunna se patienten som en helhet var av stor vikt för god omvårdnad och förbättrad sårläkning. Slutsats: Sjuksköterskor behöver förbättra sina kunskaper angående sårvårdsbehandling. Sjuksköterskan bör ha en holistisk syn på patienten.

Ett hälsofrämjande och sjukdomsförebyggande perspektiv ska vara en självklar deli all vård och behandling. Syfte: Syftet med studien var att beskriva hur sjuksköterskor kanförebygga och minska risken för fallskador hos äldre på särskilt boende. Metod: Arbetetvar en litteraturstudie på tidigare forskningsmaterial. Resultat: Den viktigaste åtgärden äratt få fram vilka äldre som tillhör riskgruppen. Detta innebär att det är viktigt attuppmärksamma äldre med kognitiva försämringar, tidigare fall, urininkontinens,nedstämdhet, lågt BMI och äldre med behov av hjälp i sin ADL. Andra åtgärder är blandannat att ha regelbundna läkemedelsgenomgångar, att skapa förutsättningar för äldrepersoner att bära höftskydd, att informera och handleda omvårdnadspersonalen, attinformera de äldre och deras anhöriga om risker och åtgärder. Slutsats: Ytterligareforskning behövs vad det gäller sjuksköterskans omvårdnadsåtgärder för att förebygga ochminska fallskador på särskilt boende för att nå ett tillförlitligt resultat inom dettaämnesområde. Förbättrade bedömningar och åtgärder, mer stöd och förståelse från cheferskulle kunna ge tydligare rutiner, minskade kostnader för samhället och inte minst minskadet lidande som det kan innebära för äldre med fallskador.
Background: Falls are common and can lead to hip fractures and earlier need ofinstitutionalization. A health promoting and preventive perspective is an important concernin public health. Aim: The aim of the study was to examine how the nurse can prevent andreduce the risk of fall injures at older people living in residential care. Method: This workwas a literature study on earlier research materials. Results: The most important measureis to identify older people with risk factors for falls. This means that it is important toattentive older people with cognitive impairment, history of falls, urinary incontinence,depressed mood, low BMI and older persons with needs of help in daily physical activities.Other measures are for example to have regular medicine reviews, to create conditions forthe older to carry hip protections, to inform and teach the care employers, to inform theolder and his relative about risks and measures. Conclusion: Further research is neededregarding nurses care measures to prevent and decrease fall injuries in residential care inorder to reach a good result within the subject. Improved assessment and measures, moresupport and understanding of the managers would give more clear routines, the costs forsociety would decrease and most important it would decrease the suffering that fall injuriescauses elderly residents.

Full textConfusion can be a life threatening condition that is often missed by the nurses, thus it occurs in the same range as pain, fever or infections. Aim: The aim of this study was to describe the phenomenon of confusion among elderly patients, in order to establish a wider understanding of the patient's nursing needs. Method: Literature study based on scientific material. Results: The result showed the patient's ability to recollect and reconstruct episodes of confusion. During the confusion the stories contained a wide variety of experiences, such as threat, fear, suspicion, chaos and order, unreal experiences, a need to escape and a bad sleep. Afterwards the patient's told that they experienced a sense of shame and guilt, abasement, fear, relief and a will to understand what had happened to them. They also felt a need to talk about what they had gone through. These experiences of confusion among elderly patients showed the importance of education and awareness among the nursing staff. The nursing strategies depended in which ground the nurses´ folkway by the elder patient was settled in. When the nurse establishes relation, brings and puts across enough knowledge, beholds, observes and evaluates, informs, explains, communicates and creates a encouraging inner and outer environment, she gives an opportunity for the patients recovery.

Full textBackground: Some patients can experience pain and discomfort for moments that include cannula and needles which can be mentioned as needle fear. Some patients can as a sequence of the fear refuse vaccinations programs, operations and other medicinal help which can have severe consequences for the health. Nurses works daily with administering injections, infusions and taking blood samples, therefore it is desirable that the nurse have the knowledge and skill regarding significant nursing actions with needle fearing patients. Aim: The purpose was to describe which nursing actions a nurse can use with adult patients with needle fear to reduce pain and discomfort. Method: This is a literature review based on 15 scientific articles from the databases PubMed, Chinal and psycINFO. Results: To reduce pain and discomfort the nurse should opt to use the thinner needles, and use a slower injection technique, offer local pain relief/ or premedication with sedative before the needle procedure. The nurse should let the patient have control over the situation and respect the individual needs and request. As well should the nurse listen and be accommodating for adapting the needle procedure in collaboration with the patient. The nurse should apply fictitious visualization methods and distraction to reduce fear and discomfort during the moment. In cases when there is a recorded needle phobia the nurse should coordinate a nursing contact to a therapist or a psychologist for the patient. Conclusion: The result in this literature review shows nursing interventions that the nurse can use to reduce pain and discomfort at adult patients with needle fear. For the patients who suffers from needle fear should the nurse plan the needle procedure in conjunction with the patient based on a person-centered care in consideration to the individual needs and wishes. Because the nurse takes the time to listen to the patient’s story, show empathy and gives the patient participation and self-determination regarding the needle procedure, pain and discomfort can be reduced.

Full textBackground: According to research, the majority of people with dementia have some form of untreated pain. Lack of communication between nurse and patient is described as a reason for the difficulty for nurses to examine, assess and treat this pain. A functioning communication and understanding of the patient's perceptions in combination with knowledge of pain assessment, knowledge of how individuals with dementia experience pain and appropriate care measures are necessary to relieve the patient's pain. Aim: The purpose of the study was to describe the nurse's nursing measures to assess and alleviate pain in people with dementia. Method: A literature review based on 15 scientific articles, 11 of which have a qualitative approach and four with a quantitative approach. The data collection was carried out in the databases CINAHL and PubMed. Results: It is a major challenge for the nurse to make a pain assessment for a patient with moderate and severe cognitive failure. Assessment instruments are the basis for the nurse to be able to perform the pain assessment in an adequate, safe, systematic and time-efficient manner in this patient group. There are observational instruments and self-reported assessment scales that nurses can use in pain assessment based on the nurse's observation and interpretation and the patient's own pain assessment. The nursing measures that the nurse can take in order to alleviate the pain of the patient include music, massage, heat treatment and use of robotic animals. Conclusion: Pain assessment and care of people with dementia is a complex process due to the patient's impaired communication skills. The nurse therefore needs indepth knowledge of appropriate assessment instruments to successfully assess pain. From a person-centered holistic perspective, the nurse also needs to understand the patient's world of life viewpoints and communicate with relatives to successfully provide pain-reducing care.

Full textSyftet med denna litteraturstudie var att undersöka i vilka situationer patienten med hjärtinfarkt kunde känna ångest. Vi ville även undersöka vad sjuksköterskan kunde göra för att lindra denna ångest. Resultatet baserades på åtta vetenskapliga artiklar samt en litteraturstudie. Resultatet visade att patienten kunde känna ångest inför undersökningar, förflyttningar, brist på information från sjuksköterskan och vid överföring av ångest från anhöriga. Information, stöd, undervisning i olika avslappningsövningar och musikterapi är användbara omvårdnadsåtgärder, som sjuksköterskan kunde använda för att lindra ångest hos patienten med hjärtinfarkt.

Full textMS is a chronic disease that affects every aspect of human life; the daily life, family relations as well as working life. The aim of the study is to describe what effect MS has on the daily life and its influence on relations with other people and what nursing care is demanded. Chosen theoretical frame of reference for the study was Carnevali theories of nursing. The method in use is a systematic literature review that is based on nine articles evaluated according to Polit et al. (2001). The result revealed that MS effects the daily life and the nurse has an important role in the nursing of MS patient. The nurse should make an individual nursing plan to help each patient to keep the function of the daily life.

Full textBackground: Diabetes mellitus type 2 (DMT2) increases in the world and can lead to serious foot complications. It has been demonstrated deficiencies in preventive care to avoid foot complications. The nurse is responsible for supporting people with DMT2 and the goal is to get them to feel independent and involved in their self-care. Aim: The aim of the study was to highlight nursing care and self-care to prevent foot complications for people with DMT2 Method: A literature review based on 20 scientific articles by quantitative structure. Articles are searched in the databases Cinahl and Pubmed. All items are quality controlled. Results: Risk identification is a key factor in the prevention of foot complications. Important self-care measures as to inspect and examine their feet daily are substances as the nurse should provide individual instruction in. The nurse shall endeavor to support and motivate people with DMT2 until good self-care. Conclusion: The nurse should teach and give people with DMT2 sufficient knowledge, to independently carry out preventive self-care.

Full textThe purpose of this literature review was to compile research evidence on patients’ quality of life and nurse’s interventions with regard to oral complications which are problems seen frequently in patients within palliative care. Our questions were: How do oral complications influence quality of life in patients within palliative care? What kind of interventions can a nurse do on the one hand to help patients maintain good oral status, and on the other hand to help patients strengthen their resources to maintain the balance between demands in daily life and resources, internal as well as external? Method: A literature review based on eleven research articles. The authors chose as theoretical reference Carnevali’s balance model, Daily life - Functional state of health. The results showed that physical labour and psychosocial suffering in consequence of oral complications affected patients’ quality of life to a great extent. This resulted in imbalance between the daily life demand and the patients’ resources. The interventions that nurses can use are prevention, patient education, documentation, reports and treating of oral complications. Results from this review can be used in care of patients within palliative care who experience oral complications.
