Full textOnychocryptosis is a common pathology in podiatry medical services and, in a considerable percentage, surgical procedures are required to achieve a solution. There are multiple surgical approaches for ingrown toenails, incisional procedures and non-incisional procedures, such as chemical matrixectomies and physical matrixectomies using CO2 laser. This study presents a surgical procedure for Onychocryptosis using a physical matrixectomy with 1064 nm laser applied with a 400 micron optical fiberand its posterior cauterized tissue surgical removal to achieve a healing by first intention. This technique was performed on 30 patients with Onychocryptosis affecting the big toe, in Mozena’s stadium I and IIa, and all of the patients were followed up postoperatively for 12 months. The results showed minimal postoperative pain, shorter surgical postoperative healing and a rapid return to activities of daily living, as well as a minor postoperative recurrence, compared with previous studies using incisional procedures and chemical matrixectomies.

Full textAbstractIntroduction: Onychocryptosis is an inflammatory disease of the lateral nail fold resulting from the embedding of the lateral border of the nail plate in the lateral sulcus associated with infections that culminate in formation of granulation tissue and hypertrophy of the lateral nail fold. Therapeutic options include conservative and surgical strategies, however, there is no consensus regarding the strategy to be adopted at each stage of severity.Materials and Methods: The bibliographic research for the elaboration of this paper was obtained through Pubmed and ClinicalKey platforms. There were selected review articles, scientific articles and descriptions of clinical cases. After careful and critical analysis of the selected articles, the present literature review was carried out.Results: It was verified that the choice of the therapy for each stage of onychocryptosis is complex and requires great judgment. Conservative treatment is safe, convenient, and inexpensive, yet it only has proven efficacy in stage I (Heifetz classification). It includes hygienic measures associated with strategies that prevent penetration of the nail plate in the nail fold. Among these are taping, packing, gutter treatment or nail braces. Phenolization after partial nail avulsion proves to be safe, inexpensive and effective and therefore should be the first line in the treatment of stages II or III. In cases where there is marked hypertrophy of the nail folds, matricectomy becomes insufficient and difficult to perform. In these situations, resection techniques are needed that prove to be quite effective, although they are more invasive and lead to a longer and painful recovery period. Since the experience of the surgeon is one of the main factors determining the efficacy of the treatment, it is essential to carry out new prospective comparative studies and randomized trials that allow us to conclude with confidence the best treatment to be applied for each stage, so this can be consistently implemented in clinical practice.Conclusion: When treating onychocryptosis, the conservative approach is indicated for stage I, while surgical treatment is indicated for stages II and III. The first line in stages II and III should be 88% phenol solution matricectomy. Other chemical agents or surgical matricectomy are presented as valid alternatives. In cases where there is considerable hypertrophy of the adjacent tissues it is recommendable to perform a resection of the periungual soft tissues, with Howard-Dubois, Vandenbos and Super U techniques being the most effective and chosen according to the location and severity of the hypertrophy of the nail folds. The Howard-Dubois technique is more suitable in the slight hypertrophies of the lateral and distal folds, while the Vandenbos and Super U techniques in severe hypertrophies. If severe hypertrophy occurs at the level of the lateral folds, Vandenbos technique is chosen, whereas if the severe hypertrophy includes both the lateral folds and the distal fold, Super U technique should be chosen. New studies with greater level of evidence to those existing in the literature are necessary in order to implement an algorithm for onychocryptosis treatment.Key-words:
ResumoIntrodução: A onicocriptose é uma doença inflamatória da prega ungueal lateral que resulta do encravamento do bordo lateral da placa ungueal no sulco lateral, associando-se a infeções secundárias que culminam na formação de tecido de granulação e hipertrofia da prega ungueal lateral. As opções terapêuticas englobam estratégias conservadoras e cirúrgicas, no entanto não existe consenso quanto à estratégia a adotar em cada estadio de gravidade.Materiais e Métodos: A pesquisa bibliográfica para a elaboração deste trabalho foi obtida através das plataformas Pubmed e ClinicalKey. Foram selecionados artigos de revisão, artigos científicos e descrições de casos clínicos. Após análise cuidada e crítica dos artigos selecionados, realizou-se a presente revisão da literatura.Resultados: Verificou-se que a escolha da terapêutica consoante o estadio da onicocriptose é complexa e exige grande ponderação. O tratamento conservador é seguro, cómodo e barato, contudo apenas apresenta eficácia comprovada no estadio I (classificação de Heifetz). Inclui medidas higiénicas associadas a estratégias que evitem a penetração do bordo ungueal na prega ungueal. Entre estas estão o taping, packing, gutter treatment ou aplicação de onico-órteses (nail braces). A fenolização após avulsão ungueal parcial revela-se segura, barata e eficaz, pelo que deve ser a primeira linha no tratamento de onicocriptose de estadio II ou III. Nos casos em que existe hipertrofia acentuada das pregas ungueais a matricectomia torna-se insuficiente e de difícil realização. Nestas situações são necessárias técnicas de resseção que se revelam bastante eficazes, embora sejam mais invasivas e levem a um longo e doloroso período de recuperação. Visto que a experiência do cirurgião é um dos principais fatores a determinar a eficácia do tratamento, é essencial a realização de novos estudos comparativos prospetivos e ensaios randomizados que permitam concluir com segurança qual o melhor tratamento a aplicar em cada estadio, de forma a que este possa ser implementado de forma consistente na prática clínica.Conclusão: No tratamento da onicocriptose, a abordagem conservadora está indicada para o estadio I, enquanto o tratamento cirúrgico está indicado para os estadios II e III. A primeira linha nos estadios II e III deve ser a matricectomia com solução de fenol a 88%. Outros agentes químicos ou matricectomia cirúrgica apresentam-se como alternativas válidas. Nos casos em que existe hipertrofia considerável dos tecidos adjacentes é aconselhável executar uma resseção dos tecidos moles periungueais, sendo as técnicas de Howard-Dubois, de Vandenbos e do Super U as que se apresentam mais eficazes e são escolhidas consoante a localização e severidade da hipertrofia das pregas ungueais. A técnica de Howard-Dubois é mais adequada nas hipertrofias ligeiras das pregas laterais e distal, enquanto as técnicas de Vandenbos e do Super U nas hipertrofias severas. Se a hipertrofia severa ocorre ao nível das pregas laterais opta-se pela técnica de Vandenbos, enquanto que se a hipertrofia severa englobar tanto as pregas laterais como a prega distal, deve optar-se pela técnica do Super U. Novos estudos, com níveis de evidência superiores aos existentes na literatura, são necessários para que se possa implementar um algoritmo para o tratamento da onicocriptose.
