Abstract:
Die individualisierte Dosierung ist wesentlicher Teil der «personalisierten Onkologie». Von den klassischen Zytostatika ist bekannt, dass die Dosierung nach BSA keine Verbesserung der stark variablen, individuellen Plasmakonzentrationen bringt. Die neuen oralen Kinaseinhibitoren wie auch die endokrinen Substanzen weisen eine erhebliche Variabilität der individuellen Plasmakonzentrationen aus, bedingt durch variable Absorption, hepatische Metabolisierung, sowie eine Vielfalt möglicher Interaktionen. Therapeutisches Drug Monitoring und die Genotypisierung sind wichtige Werkzeuge, die in Zukunft eine individualisierte Dosierung in der Onkologie erlauben könnten. Die grosse Anzahl neu zugelassener und in Entwicklung sich befindender Onkologika bedeutet aber eine grosse Herausforderung an die Entwicklung entsprechender Dosisalgorithmen. Nur wenn individualisierte Dosisstrategien bereits in die frühe klinische Entwicklung einbezogen werden, können Substanz-spezifische Empfehlungen rechtzeitig in die klinische Praxis einfliessen.

Šie antikoaguliantai skiriami trombozinių komplikacijų profilaktikai. Naujausi tyrimai rodo, kad mažos molekulinės masės heparinai tromboembolizmą sergant vėžiu veikia efektyviau ir gali pailginti išgyvenamumą. Metodai Straipsnyje apžvelgiama naujausia literatūra apie mažos molekulinės masės heparinų poveikį tromboembolizmui. Pateikiamos daugiacentrių tyrimų apžvalgos ir išvados. Rezultatai Mažos molekulinės masės heparinai, slopindami koaguliacijos procesus, veikia naviko augimą, invaziją, metastazavimą ir angiogenezę, kartu pagerina išgyvenamumą. Išvada Po didelių onkologinių operacijų rekomenduojama ilgalaikė 2–4 savaičių antitrombozinė terapija mažos molekulinės masės heparinais. These anticoagulants are prescribed for the prophylaxis of thromboembolic complications. Recent trials have shown that low molecular mass heparins are effective more in cancer patients with thromboembolism and may lower their mortality. Methods The newest literature on the effectiveness of low molecular mass heparins in the management of thromboembolic disease is reviewed. Multicenter clinical trials are presented. Results By inhibiting the coagulation processes, the LMMH exert a positive effect on tumor growth, invasion, metastasing, and angiogenesis, thus improving the survival. Conclusion Long-lasting 2–4 week antithrombolic LMMH therapy is recommended after major oncologic operations. Key words: low molecular mass heparin; thromboembolic disease; antithrombolic therapy

ZusammenfassungMistelpräparate finden seit über 100 Jahren Anwendung in der Tumortherapie und sind im deutschsprachigen Raum nach wie vor die am häufigsten eingesetzten Arzneimittel in der integrativen Onkologie. Sie werden sowohl adjuvant als auch palliativ, meist in Kombination mit konventionellen Therapieverfahren angewandt und dienen in erster Linie zur Verminderung der durch diese Therapien bedingten Nebenwirkungen, der Verbesserung der Lebensqualität sowie der Reduktion tumorbedingter Symptome. In manchen Fällen kann auch eine direkte Tumorhemmung oder Tumorreduktion beobachtet werden. Einige Studien weisen auch auf eine Verlängerung des Gesamtüberlebens durch die Misteltherapie hin.Zur Misteltherapie liegt eine Vielzahl an präklinischen und klinischen Studien vor. In dieser Übersicht wird die klinische Wirksamkeit anhand der aktuellen Studienlage und systematischen Reviews bzw. Metaanalysen besprochen. Außerdem werden Möglichkeiten einer Kombination mit gängigen Onkologika aufgezeigt.

Welche Möglichkeiten stehen Ärzten bei der Integrativen Therapie des Mammakarzinoms zur Verfügung, und was macht diese überhaupt aus? Die Beantwortung dieser Fragen scheint selbst für viele Experten der Integrativen Onkologie eine Herausforderung darzustellen, wie sich bei einem Fachkongress anthroposophischer Ärzteverbände zum Thema Mammakarzinom zeigte. So konnten die spezifischen Besonderheiten oder medizinischen Vorteile der Integrativen Onkologie für die „schulmedizinische Onkologie“ – als vorgeblich notwendiger Ergänzung des onkologischen Leistungsspektrums – nur unbefriedigend dargestellt werden. Bei zahlreichen ärztlichen Kongress- und Workshop-Teilnehmern blieb angesichts dessen oft nur Hilf- und Ratlosigkeit. Zudem entspricht die von einigen angeführte einfache Formel „Integrative Onkologie = Onkologie + Komplementärmedizin“ nicht den, vor allem aus den USA stammenden, teilweise seit Jahrzehnten erprobten und wissenschaftlich fundierten Konzepten.

Socialinis darbas vaikų onkologijos srityje yra pakankamai dar jauna profesinė sritis, kuri nėra išplėtota ir išvystyta. Onkologiniai ligoniai šiuo atveju yra vaikai – jiems ir jų tėvams atvykus į ligoninę reikalinga kompleksinė pagalba, kuri padėtų greičiau adaptuotis prie ligoninės sąlygų, išgyventi pirmųjų dienų krizės laikotarpį, susitaikyti su netikėta vaiko liga ir rasti būdų kaip visiems kartu kovoti su ja. Socialiniai darbuotojai vaikų onkologijos srityje gali prisidėti prie socialinės pagalbos teikimo tiek ligoninėje, tiek už jos ribų. Tačiau nėra aišku, kokius vaidmenis socialinis darbuotojas turėtų kurti vaikų onkologijos srityje. Šio tyrimo tikslas – išnagrinėti kokius vaidmenis socialiniam darbuotojui priskiria socialinio darbo teorijos, LR teisės aktai, ligoninės pacientų ir jų tėvų esantys poreikiai. Darbe buvo taikomi mokslinės literatūros analizės ir ekspertų kokybinio tyrimo metodai. Socialinio darbo vaidmenys buvo analizuojami remiantis humanistine-egzistencine, bihevioristine, radikaliojo marksizmo ir įgalinimo ir atstovavimo perspektyvomis. Empiriniame tyrime dalyvavo šeši respondentai, tarp kurių penki buvo ligoninės personalo darbuotojai ir paciento tėvas. Paaiškėjo, kad socialiniam darbuotojui priskiriamas administratoriaus vaidmuo neatitinka socialinio darbo teorijos pagrindimo ir, remiantis LR teisės aktais, yra per siauras ir neišplėtotas. The social work in pediatric oncology is quite a young professional area which is not yet fully developed. The oncology patients are children who together with their parents arrive to the hospital for a long stay and need help to adapt to new life situation. Especially parents need help in the first days and get some time to accept the diagnosis of their child and find ways how to fight with it. The social workers in pediatric oncology can provide help in the hospital and outside the hospital. However, it is not clear what role the social worker should have while helping the patients and their families in pediatric oncology area. The aim of the research is to analize what role does the social work theory and the Lithuanian jurisdiction delegate and what are the needs of oncology patients and their parents. The research was conducted according to the scientific literature analysis and the methods of the grounded theory. The social work roles were analysed according to such social work theory perspectives: the humanism-existencialim, behaviorsim, radical-marxist and empowerment and advocacy. There were six respondeds that participated in the research. Five of those were children oncology staff and the sixth respondend was a patient‘s father. According to the research results the administrator‘s role of the social worker in children‘s oncology area does not comply with social work theory. Abstract:
Darbo tikslas: Nustatyti specialistų komandos formavimo reikalingumą gydytojų ir slaugytojų, dirbančių LSMUL VšĮ Kauno klinikų filialo onkologijos ligoninėje, požiūriu. Uždaviniai: 1. Ištirti onkologijos ligoninėje dirbančių gydytojų ir slaugytojų požiūrį į multidisciplinines specialistų komandas; 2. Nustatyti gydytojų ir slaugytojų požiūrį į tai, kokių specialistų pagalbos reikėtų onkologine liga sergantiesiems; 3. Palyginti gydytojų ir slaugytojų nuomones apie specialistų komandos reikalingumą. Tyrimo metodika: Tyrimas atliktas 2012 m. liepos – rugpjūčio mėnesiais, naudojant anoniminę anketinę apklausą. Apklausos metu anketos buvo išdalintos keturiuose onkologijos ligoninės skyriuose (konservatyviosios onkologijos, paliatyviosios onkologijos, chirurgijos, anesteziologijos ir intensyvios terapijos) dirbantiems gydytojams ir slaugytojams. Iš viso išdalintos 127 anketos, iš kurių grįžo 114 (atsako dažnis 89,8 proc.). Statistinė duomenų analizė atlikta SPSS 17.0 ir MS Excel programomis. Statistinis duomenų reikšmingumas tikrintas pagal statistinį reikšmingumą (p < 0,05), chi kvadrato (χ2) kriterijų, laisvės laipsnių skaičių ir z kriterijų. Rezultatai: Atlikus tyrimo duomenų analizę nustatyta, kad tyrimo dalyviai žino apie komandinio darbo svarbą ir privalumus gydymo įstaigoje. 48,2 proc. teigė, kad ligoninėje vyrauja individualus ir komandinis darbas. 48,6 proc. apklaustųjų nurodė, kad konfliktai daugiausiai kyla dėl bendravimo problemų ir hierarchinių santykių, o sprendimai... The aim of the study: To assess the need of multidisciplinary team formation from a perspective of doctors and nurses working in oncological hospital of Lithuanian University of Health Sciences Kaunas Clinics. Objectives: 1) To find out the attitude of doctors and nurses towards the need of multidisciplinary team; 2) To set the view on what kind of specialists are required for oncological patients according to doctors and nurses; 3) To compare opinions among doctors and nurses about the necessity of the multidisciplinary team. Research methodology: The research was carried out during July and August 2012 using an anonymous questionnaire method. Questionnaires were distributed among doctors and nurses working in four units of the oncological hospital. They were the following therapy units: conservative oncology, palliative oncology, surgery, anaesthetic and intensive therapy units. In total were distributed 127 questionnaires from which 114 questionnaires were returned (with a response rate of 89.8%). Statistical analysis of the data was done using SPSS 17.0 and MS Excel software. Statistical significance of the data was verified using statistical significance p value (p<0,05), chi-squared test, degrees of freedom and z value. Abstract:
Many humanistic physiologist are writing about the importance of forgiveness in one or another way. Forgiveness is associated not only with physical health but also with psychical health. Nuisance, soreness/hurt, reluctance to forgive might stir up stress, depression, hypertension, headache, back and neck pains, worryness, sleeplessness, spontaneous lurk and sorrow. By looking into other studies it showed that forgiveness therapy is usable in oncological clinic, like one of methods contributory to treat cancer. The aim of this study was to research the peculiarity of an individual, the forgiveness conception and its influence to the physical and psychical health; to analyze the self value, an attitude to their health and valuables also the forgiveness and empathy, receiving of God forgiveness, hurt and anger levels of people who has oncological diseases and to compare with healthy people. In our research participitated 280 people, both mean and women in the average age of 40,11±12,65, and from those 99 (35,4%) had oncological disese and 181 (64,6%) were healthy. Abstract:
Darbo tikslas. Įvertinti onkologinėmis ligomis sergančių pacientų nuomonę apie informuoto sutikimo doktrinos taikymą N sveikatos priežiūros įstaigoje. Uždaviniai. Įvertinti pacientų informuotumą apie rašytinio sutikimo doktrinos paskirtį sveikatos priežiūros įstaigoje. Nustatyti ryšį tarp onkologinėmis ligomis sergančių pacientų požiūrio į informuoto sutikimo doktrinos taikymą bei pacientų lyties ir išsimokslinimo. Nustatyti ryšį tarp pacientų pasitenkinimo gydytojų suteikiama informacija apie ligą ir jų nuomonės apie informuoto sutikimo doktrinos reikalingumą. Tyrimo metodika. N sveikatos priežiūros įstaigoje atlikta anoniminė anketinė 250 pacientų apklausa (atsako dažnis – 75,7 proc.). Duomenų analizei naudotas SPSS 13.0 statistinės analizės paketas. Diagramos ir lentelės parengtos Microsoft Excel 2003 programa. Rezultatai. Beveik pusė visų apklaustųjų (44,8 proc.) manė, kad sveikatos priežiūros įstaigoje pacientams pasirašyti pateikiamos asmens informuoto sutikimo formos paskirtis - apsaugoti sveikatos priežiūros specialistus nuo galimų pacientų pretenzijų, 35,6 proc. respondentų nuomone, informuoto sutikimo forma padeda pacientams išreikšti su jų gydymu susijusį apsisprendimą, 18,8 proc. apklaustųjų manė, kad tai formali procedūra. Didesnė dalis vyrų (34,5 proc.) nei moterų (18,4 proc.) visai neskaitė ligoninėje jiems pateiktų asmens informuoto sutikimo formų (p<0,01). Aim of the study. To evaluate the oncology patients` opinion about application of the doctrine of informed consent in the health care institution. Objectives. To evaluate patients` knowledge about the purpose of the doctrine of informed consent at the health care institution. To identify associations between oncology patients` opinion about application of the doctrine of informed consent, patients` gender and education. To identify associations between patients` satisfaction with given information about their disease and patients` opinion about the necessity of the doctrine of informed consent. Methods. Anonymous survey of 250 oncology patients (response rate – 75.7%) in health care institution was carried out. SPSS 13.0 version of statistic analysis program was used to process the data. Diagrams and tables were prepared using Microsoft Excel 2003 program. Results. Nearly half of the survey participants (44.8%) evaluated written informed consent procedure as protection instrument to the health care specialists. More than the one third (35.6%) of the participants thought that the doctrine of informed consent helped them to express their decision, to 18.8% of the survey participants the doctrine was formal procedure. Pagrindinės sąvokos: Onkologijos slaugytoja, stresas darbe, stresą darbe įtakojantys veiksniai. Tyrimo tikslas: ištirti onkologijos slaugytojų stresą darbe įtakojančius veiksnius. Tyrimo uždaviniai: 1. Ištirti darbo aplinkos veiksnių poveikį onkologijos slaugytojų streso darbe formavimuisi. 2. Nustatyti onkologijos slaugytojų dalyvavimo darbinėje veikloje įtaka streso formavimuisi. 3. Įvertinti socialinio klimato įtaką onkologijos slaugytojų stresui darbe. 4. Iš gautų tyrimo rezultatų pateikti pasiūlymus, kaip išvengti streso darbe Tyrimo populiacija: Anketuotos Vilniaus Universiteto Onkologijos Institute dirbančios slaugytojos. Tyrimo objektas: onkologijos slaugytojų stresą darbe skatinantys veiksniai. Hipotezė: onkologijos slaugytojų patiriamas stresas yra įtakojamas darbinės aplinkos. Tyrimo metodika: Tyrimas atliktas 2007 m. spalio mėn. – 2009 m. gegužės mėn. Tyrime dalyvavo 112 slaugytojų, dirbančių Vilniaus Universiteto Onkologijos Institute (toliau VUOI). Tiriamųjų apklausai panaudota standartizuota 10 klausimų anketa, kuri reprezentavo slaugytojų socialinę aplinką darbo vietoje.
