Differentiated models of service delivery (DSD models) for HIV treatment in sub-Saharan Africa were conceived as a way to manage rapidly expanding populations of experienced patients who are clinically “stable” on antiretroviral therapy (ART). These models thus systematically exclude newly-initiated patients, who instead experience the conventional model of care, which requires frequent, multiple clinic visits that impose costs on both providers and patients. Abstract:
Thoracic endovascular aortic repair (TEVAR) is a less invasive alternative to open surgery for aortic arch pathologies, but extreme arch tortuosity or sharply angulated “gothic” arches can impede successful stent-graft delivery. Conventional adjunct techniques (such as brachiofemoral through-and-through guidewires or externalized transseptal wires) often provide inadequate support or carry substantial procedural risks. To address these challenges, we developed the Amber technique – a modified right brachiofemoral approach that forms a stable M-shaped guidewire loop (from femoral to right brach

Open-access publishing represents a transformative shift in the way knowledge is created, shared, and consumed. By removing barriers to access, this model democratizes information, enhances global collaboration, and accelerates innovation. This article explores the rise of open-access publishing, the challenges it faces, and the role of Pyrrhic Press in championing this movement. Through sustainable practices and a commitment to accessibility, Pyrrhic Press exemplifies how publishers can adapt to the changing landscape of modern scholarship.

Architectural firms can be regarded as creative professional service firms. As such, architects need to navigate creative, professional and commercial goals, while simultaneously attempting to fulfil client, user and societal needs. This complex process is becoming increasingly difficult, as the historically established role of architects has become more blurred, contested and heterogeneous. While attempting to reclaim their role or to take on new roles in collaborations with other actors, architectural firms are challenged to develop business models that are financially viable and professiona

Responsible Research and Innovation (RRI) has been a central pillar of the European H2020 research framework. A social lab organized within the European project NewHoRRIzon explored ways to stimulate further integration of RRI principles (public engagement, open access, gender, ethics, science education) in the nuclear research. Under the title “Nuclear Dating”, a pilot action was designed in this lab with the aim of creating awareness of RRI among Early Career Researchers, providing a venue for experimenting interdisciplinary collaborations and stimulating their reflexivity to the wider socia

Abstract:
AbstractThe attractive specifications of vertical axis wind turbine (VAWT) are its operating with low noise and wind in various directions. To achieve a higher performance of these turbines, modeling of a curved‐blade VAWT by the modified double multiple streamtube (DMST) method and optimizing this wind turbine are performed. The power coefficient and the total normal force acting on blades were selected as two objective functions. Minimizing the normal force acting on turbine blades as the second objective function was not observed in the open literature. The amount of normal force is crucial

Purpose The purpose of this paper is to investigate the literature on resilience factors applied to post-disaster reconstruction projects and to develop a guiding framework to assist in their strategic selection and application. Design/methodology/approach A systematic review was undertaken on the literature’s account in four major bibliographic databases to elicit resilience factors contributing to improving post-disaster reconstruction projects' robustness. Through summative content analysis and open-coding of research outputs over the past decade, the factors identified informed the develop

AbstractThe dynamic connectivity and functionality of sensors has revolutionized remote monitoring applications thanks to the combination of IoT and wireless sensor networks (WSNs). Wearable wireless medical sensor nodes allow continuous monitoring by amassing physiological data, which is very useful in healthcare applications. These text data are then sent to doctors via IoT devices so they can make an accurate diagnosis as soon as possible. However, the transmission of medical text data is extremely vulnerable to security and privacy assaults due to the open nature of the underlying communic

An objective of network neutrality is to design regulations for the Internet and ensure that it remains a public, open platform where innovations can thrive. While there is broad agreement that preserving the content quality of service falls under the purview of net neutrality, the role of differential pricing, especially the practice of zero-rating remains controversial. Zero-rating refers to the practice of providing free Internet access to some users under certain conditions, which usually concurs with differentiation among users or content providers.
