Full textBackground: Today’s advanced surgery demands the theatre nurse to be highly skilled and competent. The theatre nurse is working in a constant developing environment. Her specific knowledge of perioperative care makes the theatre nurse a key-player in the operating team. The newly graduated theatre nurse lacks deeper competence in both working experience and professional skills that could help her meet the highly set standards in surgery. Aim: The aim of this study was to examine which challenges newly graduated theatre nurses experience and which coping strategies they use. Method: Six newly graduated theatre nurses working in an operating setting, with six to twelve months of working experience, were interviewed. The narrative data was analyzed with qualitative content analysis. Results: Being a newly graduated theatre nurse with lack of previous working experience created mixed feelings about the profession and the role. The experiences were characterized a satisfaction in learning and developing skills but at the same time experiencing feelings of being overcome by work burden. The challenges they encountered were entering the role of an operation theatre nurse, allowing one self to gain competence and accepting the situation of being a novice. Adjusting to the situation by adopting different strategies became a way of coping with the experienced challenges. Conclusion: Gaining access to the role of becoming a competent theatre nurse can be supported by acknowledging the challenges experienced by newly graduated theatre nurses and by mentoring their progress to prevent negative feelings such as insecurity and stress.

Full textThe surgical safety checklist was developed by the World health organization (WHO) to reduce the risk of surgical complications and death. The communication in the operating room needs to be improved between all categories of the staff so that the safety measures will be consistently and correctly performed. The target with hospital care is to promote a high patient safety to minimize the share of care related injuries. In the spite of WHO´s checklist has shown positive result there are indications that compliance decreases and it is also met with resistance from the hospital staff. The purpose of this study would like to illustrate the theater nurses experience about the application of WHO´s checklist. Four theater nurses where interviewed and the result was analyzed by qualitative content analysis. The process of the analysis resulted in three categories: safety of the care process, development and compliance. The result showed how the application of WHO´s checklist promotes safety for the patient in the close contact and in the theatre nurse independent occupation. The need of development to meet the changing surgical field was another aspect. The importance of compliance where described by the theatre nurse to achieve the best outcome of WHO´s checklist for safe surgery. A consistent implementation of WHO´s surgical safety checklist lead to increased patient safety.

Full textIntroduction: Nursing is the main area for the operating room (OR) nurse. The OR nurse will prevent injuries and complications that can occur during surgery. One such complication can be compartment syndrome. Compartment syndrome is a rare complication but can lead to serious injuries. It is therefore important that the OR nurse take this in consideration in their work. Aim: This essay aims to highlight the OR nurse's experiences of working with the prevention of compartment syndrome in the perioperative nursing. Method: A qualitative approach was applied and the data were collected through interviews. Collected data were analyzed by qualitative content analysis. The essay enrolled 11 OR nurses, ten women and one man, who worked in four different surgical wards at two different hospitals. Result: After a qualitative content analysis of the data were conducted two categories emerged, Preventive work and To act on uncertain foundations. The first category described how the OR nurse undertook an individual risk assessment, was accurate in positioning and performed a change in position and massage of the patient’s leg to prevent compartment syndrome. The second category, To act on uncertain foundations, described an uncertainty in the perioperative care regarding compartment syndrome. It highlighted an uncertainty around the topic, nursing interventions were questioned and a lack of scientific evidence emerged. Conclusion: There were a lack of guidelines and recommendations regarding care about compartment syndrome. There was also a lack of implementing new knowledge in nursing to provide an evidence-based practice.

Full textBackground:During the past years, there have been investments to make the elective healthcare more effective (Fast-track). The hospital stay after a knee replacement (TKA) is today just a few days. Studies indicate that there is a concern among patients to be discharged too early. There are also studies that shows that the theatre nurse through perioperative care can lay the foundation for an effective rehabilitation of the patient after surgery. This study focuses on the patients experience of a TKA surgery and throughout the care process based on the short hospital stay.Purpose:The purpose with this study was to describe the patient`s experience of undergoing a TKA surgery and the short hospitalization.Method:An interview study was made with ten patients who have undergone a TKA-operation from a strategic selection. The patient`s pain was evaluated with VAS scale. The interviews were recorded, transcribedverbatim and analysed using qualitative content analysis.Outcome:Six subcategories which in turn created three categories emerged; information, treatment and symptom relief. All the patients were very satisfied with the experience of the care process. The treatment and information given from the medical staff was part of what made the patient satisfied. VAS estimation showed that patients scored high pain. Conclusion:The patient ́s experience of undergoing a TKA surgery and the short hospitalization was seen as positive. Despite the positive result further research from a larger number of patients to get a more generalizable result of the patient ́s experiences from TKA and the short hospitalization is advocated. Theatre nurses ́ participation in theperioperative healthcare can advantageously be discussed at the current hospital.

Full textNational Board of Health and Welfare describes an upcoming shortage of perioperative nurses in Sweden, and statistics show that the number of nurses applying to specialized training in the perioperative field is declining. In the literature, the perioperative nursing profession is described as non-visible to people in general, and that may be a reason for the lack of interest in choosing the profession. The purpose of this pilot study is to describe nursesʹ perceptions of the perioperative nursing profession. The study was conducted using qualitative inductive approach as an interview with open entry question. Data was analyzed by manifest content analysis. The results revealed three categories; Invisible and low valued, Member of the team and Practical work. The result is consistent with prior research conducted. A larger study is of value to obtain a deeper understanding of differences in perceptions. Knowledge of these perceptions may facilitate the recruitment of perioperative nurses. Specialized competence at the surgical departments increases the chances for patients to receive a safe perioperative care.

Full textBackground: Trauma is a worldwide problem. In Sweden 4500 people die annually of trauma-related injuries. Lennquist (2007) argue/mean that optimized organization and improved competence in trauma care could decrease the number of deaths due to trauma. Aim: To investigate operating nurses' own view of their competence in trauma care in relation to Landstinget Västmanland's model for competence, based on Benner's stages of clinical competence. Method: Descriptive analysis of quantitative data. Operating nurses working in a operative department were given questionnaires to respond to. The questions were based on Landstinget Västmanlands stages of competence and The Swedish Nurses Association's six core competencies. Results: Thirty-six operating nurses worked at the department during the week the questionnaires were distributed and fifteen chose to participate. Nine operating nurses (n=15) were classified as "experts", but performed duties at a lower competency level. The majority of the operating nurses responded "agree completely" to four of the six core competencies and "partly true" on the remaining two. All participants had participated in some kind of trauma education. Conclusion: There is a need for greater structure and better guidelines regarding operating nurses' competence development since experts preformed their duties at a lower competency level. Operating nurses had good knowledge in four of the six core competencies but needed to improve their knowledge in two areas.

Full textBackground: Operating nurses have great responsibility for safe care and play a significant role in the operating room to maintain patient safety. Their profession also includes supervising prospective operating nurses. The role as a supervisor requires competence and experience. Aim: The purpose of the study was to investigate operating nurses´ experience of supervising students. Method: The study has a qualitative method. Data collection was conducted through semi-structured interviews with eight operating nurses who have experience of mentoring students. The data material was analyzed using qualitative latent content analysis. Result: The participants highlighted three key areas on which the result was built. The first area was where a lack of time was seen to reduce their chances of being the best kind of supervisor in their own opinion. The second area was the supervisor role itself and how they perceived it. The third area concerned guiding and how they received and what support they wanted while mentoring/supervising students. Conclusion: Through increased understanding of how the operating nurses perceive the mentor role, support can be provided for better quality supervision for students and this can in turn contribute to patient safety.

Full textBackground: Theatre nursing is performed in a unique environment and can be demanding. Job satisfaction can affect the theatre nurse´s performance and quality of care given. The theatre nurse´s competence is required to ensure patient safety of the surgical patient. Aim: The aim was to describe the theatre nurse´s experiences of job satisfaction Method: Semi-structured interviews with 12 theatre nurses at two hospitals and one private clinic in southern Sweden. Content analysis with inductive approach. Results: The theatre nurse's job satisfaction is founded on having a significant role in an interesting and varied profession with development opportunities. Job satisfaction increases through experience and perceived competence as well as good relationships with colleagues and a well-functioning collaboration with them. Staffing problems, high workloads and lack of opportunities to control their working hours pose threats to job satisfaction. The result can be understood and discussed in relation to Antonovskys Sense Of Coherence (SOC). Conclusion: In a resource-constrained organization, the work management needs to be particularly aware of what is important for the theatre nurse's job satisfaction in order to act in a long-term sustainable way in terms of personnel supply and patient safety. For example, mentoring for newly graduated theatre nurses is recommended and that the manager, in the composition of working teams and new recruits, is well acquainted and aware of the work group's dynamics.

Full textBackground The number of surgical procedures is expected to increase, and thus also the total number of complications might increase. World Health Organization, WHO, has developed a Checklist for use in connection with surgical procedures to minimize the risk of errors and to enhance patient safety. The aim of this study was to investigate the theatre nurses' experiences of this Checklist. Method A pilot study was performed and five theatre nurses were interviewed. The interviews were transcribed verbatim, and a qualitative manifest content analysis was conducted with an inductive approach. The result shows that there were obstacles in the use of the Checklist, the staff was unaccustomed to the new procedures the Checklist had led to, the surgical team’s attitudes affected the use of the Checklist, and some in the surgical team showed lack of understanding of the purpose of the Checklist. Uncertainty prevailed among theatre nurses when to perform the different phases of the Checklist, and who should be initiating the Checklist. The use of the Checklist led to increased safety for the patients as well as for the staff, and also led to an increased preparedness for theatre nurses. The presentation of the staff in the theatre room was considered to be positive for new staff, and negative since this took time and involved repetition. Conclusion The WHO's Checklist is a useful tool to increase communication and thereby increase patient safety. There was a need to adapt the Checklist to the own clinics.

Full textBackground Operating room personnel are at particularly high risk of injury and blood borne diseases due to handling of sharp items and frequent blood exposure. Operating room nurses aim to perform at a high level of security to reduce the risk of transmission of blood borne diseases. To prevent sharps injuries there are regulations, methods and safety devices but few written guidelines. Aim The aim of this study is to identify how the operating room nurse handles sharps within the operating room. Method A quantitative cross-sectional study was conducted at Södersjukhuset in Stockholm, Sweden. 18 operating room nurses participated in this structured observation study during 50 operations. Results The result indicates shortcomings in obtaining a safe working environment in all observed areas. Safety devices and other methods to prevent sharps injuries were seldom used. Despite this the work with sharps was mainly conducted in a safe way. At times the work was performed in a hazardous manner. Conclusions The safety devices and methods described in the study exist but are seldom used which leads to a subjective work method regarding safety in the operating room. The safety devices need to be made more accessible and explicit guidelines are needed. Clinical significance This study may be part of a quality improvement project at Södersjukhuset regarding handling and routines concerning sharps. The quality improvement effort can result in alterations of the contents in the procedure pack and the development of written guidelines.

Full textBackground The profession of operating room nurse is now the oldest nursing specialty, but remains primarily a hidden profession because it is exercised behind closed doors. The professional role were described as complex and revolved around the patient, cooperation and the technical equipment. Patricia Benners theories on the nurses’ professional development were used as a theoretical basis in the study. Aim The study aimed to elucidate how the operating room nurse perceives the professional role. Method The study was conducted with a qualitative approach and ten semi-structured interviews were carried out with operating room nurses from two hospitals in Sweden in spring 2011. All interviews were recorded and transcribed verbatim. To analyze the collected data content analysis with an inductive approach was used. Result In the text analysis twelve subcategories and four categories were identified: the role as being responsible of nursing, the experience of what makes the operating room nurse safe in the professional role, the meaning of being a co-worker during the work on the operating room and the view on the own profession Quotes from the informants were presented in the text to clarify the result. Conclusion The study showed that the operating room nurse has a central role for nursing in the surgical ward and the patient. The operating room nurse presence and expertise in operations was irreplaceable, but always dependent of the team around him/her.

Full textIntroduction: Theater nursing role has over the years change as scientific advances in surgery. Theater nurse has to various degrees with a larger role in perioperative caring which include pre-, intra- and postoperatively. Purpose: The purpose of this study was to describe theater nurses perception of the responsibility of the perioperative nursing care. Method: Six interviews were conducted with theater nurses at a hospital in Norrland. The collected data were analyzed using the content analysis. Results: In text analysis identified four categories: Continuous evaluation of their own work, Competence gives greater responsibility, Experiencing that increased contact with patients could improve care and their talents and Theater nurses have their own idea of how her responsibility to follow policies and procedures look like. Theater nurses felt that new procedures were developed when errors are detected. They described a diverse work that was more than just instrumentation, and increased patient contact was believed to contribute to gaining knowledge, both technical and caring manner. What new information and new policies and procedures used were in some degree dependent on the theater nurse as an individual. Discussion: Theater nurses felt that new procedures were developed when errors were detected. They described a diverse work that was more than just instrumentation, and increased patient contact was believed to contribute to gaining knowledge, both technical and caring. What new information and new policies and procedures used were in some degree dependent on the theater nurse as an individual. The disadvantage of this study is that the participants have no pre- or postoperative dialogues, had it been so, the study would possibly have resulted in more or different categories. Conclusion: Theater nurses felt a great responsibility to develop, to work safely and treat the patient with a holistic approach. The authors believe that it would be worthwhile to conduct further research and study how theater nursing skills and knowledge could be used more within the perioperative care.

Full textBackground: The WHO checklist is intended for operating clinics around the world to reduce the number of complications combined with surgery. Studies have shown that the checklist not only does improve patient safety but also contributes to better teamwork and communication between staff. Little is known how the checklist affects non sedated patients undergoing surgery. Aim: The aim of this study was to illustrate how a couple of theatre nurses experience the use of checklist in presents of non sedated patient. Method: The study hade a qualitative approach and semistructured interviews were made involving three theatre nurses. The collected material was analyzed by content analysis. Result: The result was presented six categories describing how the theatre nurses experienced using the checklist in presents of non sedated patient; adjustment, information, communication, anxiety, discomfort, safety. Conclusions: The theatre nurses experienced safety as the most important issue in using the checklist. At the same time they were doubtful to use it in presents of non sedated patients. They described it as a feeling of discomfort and an unpleasant feeling that they felt. The theatre nurses suggested that more preoperative information conserning the checklist and other adjustments could make it easier to use the checklist in front of non sedated patients. However, they gave no specific suggestions how to adjust the checklist to non sedated patients.

Full textIntroduction: The operating theatre nurse (OTN) is responsible for the patient’s care during the perioperative process and it should be conducted on the basis of person-centered care (PCC). In the high-tech environment the OTN work as a link between technology and the patient. Only a few studies illustrate the OTN’s perioperative care and the need for a care-oriented work in the operating room is questionable. Aim: The aim of the study was to describe OTNs' experiences of the characteristics of perioperative nursing. Method: The study was a qualitative interview study with an inductive approach. The data was collected through reflexive interviews of ten OTNs’ from two surgical clinics. The interviews were based on a perceived situation of perioperative care. Data analysis was carried out with content analysis. Result: The OTNs' experiences of perioperative care can be summarized in one key category: Patient safe care. Three generic categories emerged: Preserving the patient's dignity, Coordination and control of the patient’s care and Protect the patient's body. The OTNs’ described how they worked according to individual patient needs and had specific operating and technical knowledge which prevented the emergence of health damage to the patient. Conclusion: The perioperative nursing characterized by the OTNs’ through their specific knowledge coordinates and controls the patient's care, protect the patient's body and contributes to the preservation of the patient’s dignity, which contributes to a patient safe care. According to the guidelines, the perioperative care should be person-centered which it partly is but further evidence regarding perioperative PCC would be significant.

Full textAbstract Background: The operating room is a high-tech and risky environment, where the patient is often in a vulnerable position. The operation nurse’s profession includes providing safe care to the patient which means, among other things, doing risk analyzes and working actively to prevent patient injuries. Although there are tools to ensure patient safety, such as the WHO Safe Surgery Checklist, unexpected events can occur. Unexpected events can involve arised errors and complications which can jeopardize the patient's safety. Aim: To investigate the operation theater nurse's experience of unexpected events in the operating room. Method: The study was a qualitative interview study conducted with semi-structured questions. A total of eight operating theater nurse’s were interviewed at two hospitals in southern Sweden. The interviews were analyzed with qualitative content analysis Result: Following the analysis three themes emerged that described how operating theatre nurses’ experienced unexpected events in the operating room; Feeling confident in oneself and others. The experience of preserving and losing control and Emotional aspects of unexpected events. How well-functioning the team is and the degree of professional experience are perceived to be key factors the influence how to handle an unexpected event. Mixed feelings such as inadequacy and stimulation in the profession emerged. Conclusion: Operating theatre nurses generally feel secure when unexpected events occur. The study shows both positive and negative experiences in connection with an unexpected event.

Full textIntroduction: The prevalence of musculoskeletal disorders in operating room nurses is high with as many as 77% reporting these conditions. Ill health is a distinct consequence where prior research shows impaired quality of life, reduced work ability and comorbidity. The Swedish Operating Room Nurses Association highlights the responsibility to identify and prevent risks in the operating room. To identify risks for musculoskeletal disorders in operating room nurses is health promoting through which risks can be brought to light and prevented. The aim was to identify and describe risk factors for musculoskeletal disorders in operating room nurses. Method: A systematic literature review was conducted in accordance with SBU and Rosén. The material was analyzed using an integrated analysis. The identified risk factors were divided into three main categories and seven subcategories. The result is based on 10 articles. The three main categories which emerged was ergonomic, demographic and psychosocial risk factors, including the subcategories: movement patterns and biomechanics, moving equipment/instruments and patients, gender, age and working experience, working conditions, surgical specialty and body constitution. Conclusion: Several risk factors are represented in health care, even if prolonged standing and the repetitiveness of certain tasks distinguish the work of operation room nurses from other nursing jobs. Working in the operating room is associated with risk and the hope therefor is that the literature review will help implementation of and compliance to preventative measures regarding musculoskeletal disorders.

Full textIntroduction: The operating room nurse is responsible for a good and safe perioperative care. Preoperative person-centered information is a prerequisite to meet the patient´s unique needs and promote continuity of care. The patient´s individual risk factors are essential for the operating room nurse to know in order to prevent hospital acquired injuries and to accomplish safe perioperative care. Aim: The aim was to describe operating room nurses´ experiences of obtaining information to create knowledge of the patient in perioperative care. Method: A qualitative study with an inductive approach was conducted. Ten operating room nurses at a hospital in Sweden were interviewed. The interviews were recorded and transcribed. Content analysis was chosen as the method of data analysis. Result: Two generic categories emerged: Read written information about the patient and The preoperative meeting with the patient. These generic categories together form the main category Prerequisite for person-centered and safe perioperative care. The generic category Read written information about the patient includes the subcategories To acquire basic knowledge and To prioritize and to seize opportunities. The generic category The preoperative meeting with the patient includes the subcategories To meet and talk with the patient and To create conditions for conversation. Conclusion: The findings contribute to a deeper knowledge of what is essential to know about the patient in perioperative care and why this is important information, from the operating room nurse´s perspective. Both written information and a preoperative meeting with the patient are required to create prerequisite for person-centered and safe perioperative care. The results indicate a need of change in the way of working to enable good knowledge of each patient, and thereby increase patient safety in perioperative care.

Full textBackground: Yearly in Sweden, patients are reported suffering from postoperative pressure ulcer and nerve damage due to incorrect positioning and/or excluded repositioning. These complications cause patient unnecessary suffering and extended time in care in the hospitals as well causes high costs for healthcare. Despite previous research which describes how these complications can be prevented, there is limited research on how the operating theatre nurse´s knowledge is on the subject. The study is conducted to investigate and describe today´s operating theatre nurses knowledge based on a sample of some randomly selected hospitals in Sweden. Aim: The aim of the study is to highlight the operating theatre nurse´s knowledge of positioning and repositioning to prevent postoperative complications, specifically pressure ulcers and nerve damage. Method: The study was conducted as an empirical cross-sectional study with descriptive design and quantitative approach where data was collected by a self-designed web questionnaire. In the study, seven hospitals in Sweden were participating, which resulted in 137 participants. Results: The results showed that today's theatre nurses are interested in gaining more knowledge about preventive work against postoperative complications, specific pressure ulcers and nerve injuries. It also showed a varied perception about who has the main responsibility for repositioning during surgery. Conclusion: Participants informed that they don´t receive regular information within the subject from their workplace. This may be one of the reasons why repositioning is not performed as recommended by previous research. It should be decided who should be in charge of the repositioning to prevent patients from unnecessary suffering.

Full textBackground: Surgical site infections increase suffering for the patient and entails great cost for society. The theatre nurse should work to make sure patient safety and to avoid patient injuries correlated to the surgery. The theatre nurse competence in hygiene and adherence to conduct the sterile field is needed to prevent surgical site infections. Aim: To describe the theatre nurse experiences of situations where the sterile field is jeopardized intraoperative and the actions to prevent it. Method: Qualitative design, semi structured interviews with 12 theatre nurses from a hospital in southern Sweden. Inductive content analysis by Elo and Kyngäs (2008) was used to analyze the data. Results: The study shows that communication and teamwork is necessary for the theatre nurse to maintain the sterile field. For the same reason the theatre nurse needs to be observant of everything going on in the operating room. When the patient's life is in danger, sterility comes in second place, and makes it necessary to prioritize. Unpredicted situations demands creativity to restore the sterile field but it gets easier with work experience Conclusion: The theatre nurse has a lot of experiences regarding situations where the sterile field is jeopardized. Several factors play a role in maintaining the sterile field. Research that facilitates the theatre nurse tasks in maintaining the sterile field should be further studied.

Full textIntroduction: Perioperative care in which the operating room nurse takes part includes the phases before, during and after operation and can be seen as a way of working in parallel with the care process. This approach has however not been applied to any great extent in pratice despite several studies having shown the positive effects for both patients and staff. The common view of the operating room nurse’s role is still a traditional one with the focus on the time when the patient is anaesthetized and during the actual surgery. Unit managers and section leaders perceptions of perioperative care are important since it affects how the approach is applied in practice. Aim: To describe the unit managers and section leaders perceptions of the operating nurse’s perioperative care work together with how the process is applied in practice. Method: Data were collected by semi-structured interviews with four unit managers and four section leaders within the operating room unit of two provincial hospitals and two university hospitals in Sweden. Data have been analysed with a qualitative content analysis. Main results: Perioperative care as an approach has, according to the unit managers and section leaders in the operating room unit, a value as it revitalises the operating room nurse’s method of working which in turn is thought to improve patient care. It is maintained that in order to introduce perioperative care certain requirements need to be fulfilled, such as clarification of the benefit to patients, support in order to create the right conditions, as well as the need for motivated operating room nurses. Conclusion: Perioperative nursing care is, despite its value, not possible to implement without farreaching changes to how operating room nursing is approached as well as organisational changes.

Full textBackground Postoperative wound infections cause major costs to society and causes patients great suffering. To prevent the occurrence of infections surgeon adherence to hygiene routines, in addition to the operating room nurse overall responsibility for hygiene and sterility during surgery, is needed. Aim To investigate surgical nurses experiences of surgeons adherence regarding sterility and hygiene during surgical procedures. And identify factors that according to surgical nurses can affect surgeons' adherence and proposed actions. Method Qualitative design, with semi-structured interviews with eight surgical nurses at two hospitals in northern Sweden. Inductive content analysis of Lundman & Hällgren Graneheim (2012) was used for analysis of data. Result All surgical nurses experienced surgeons to fail in adherence and the surgical nurses opinion was that this is partly due to knowledge gaps among surgeons, but that it could also be attributed to poor team cooperation and lack of leadership. The surgical nurses felt that strengthening of the surgeon's skills, strengthened cooperation and a more distinct leadership could improve adherence among surgeons. Conclusion Not only by addressing the surgeon's knowledge gaps can adherence to hygiene be improved but it is also of significant importance to improve team collaboration.
