Abstract:
Objective: To explore the effect of nursing record sheet and operation registration form in daytime ophthalmic surgery. Methods: the nursing record sheet and operation registration form of daytime ophthalmic surgery were designed and used to evaluate the incidence of adverse events and the satisfaction of doctors in the operating room before and after use. Results: from January 2018 to June 2019, there were 15 adverse events caused by unclear handover, wrong record, wrong eye classification, wrong application of antibiotics and waiting for surgical consumables in ophthalmology due to nursing record, with an incidence of 3.71%;From July 2019 to December 2020, there were only 2 cases of ophthalmic defects caused by the record sheet, with an incidence of 0.42%;The difference was statistically significant(P=0.00095). When the daytime ophthalmic surgery nursing record sheet was not used from January 2018 to June 2019, the satisfaction of ophthalmic surgeons was 71%, and after using the daytime ophthalmic surgery nursing record sheet from July 2019 to December 2020, the satisfaction of ophthalmic surgeons increased to 96%. Conclusion the design of daytime ophthalmic surgery nursing record sheet is reasonable and convenient, which can effectively ensure the quality of daytime ophthalmic surgery nursing, improve work efficiency and improve job satisfaction of surgeons.

Artificial intelligence can effectively improve the efficiency and accuracy of medical diagnosis, clinical data analysis, medical image recognition, treatment plan decision-making, etc. It has broad application prospects in the ophthalmic diagnosis, treatment, and nursing industry. However, the application of artificial intelligence in the ophthalmic diagnosis and nursing industry in China started relatively late, and there are insufficient ophthalmic diagnosis and nursing personnel who are familiar with artificial intelligence technologies. In order to promote the modernization of ophthalmic medicine in China and accelerate the development of a high-quality and modern medical education system, it is necessary to train a new generation of compound ophthalmic medical talents who are skilled in artificial intelligence and develop an advanced talent training model that meets the needs of the ophthalmic profession and the society. Abstract:
This paper compares the results of studies of ophthalmic advanced practice in two similar but distinct health economies and integrates the effects of the setting, health policy and professional regulation on such roles. A mixed method questionnaire design was used, distributed at national ophthalmic nursing conferences in the UK and in New Zealand. Participants were nurses undertaking advanced practice who opted to return the questionnaire. Data were analysed separately, and are compared here, integrated with national health policy and role regulation to provide commentary on the findings. The findings suggest that health policy priorities stimulate the areas in which advanced practice roles in ophthalmic nursing emerge. The drivers of role development appear similar and include a lack of experienced doctors and an unmanageable rise in healthcare demand. Titles and remuneration are different in the two health economies, reflecting the organisation and regulation of nursing. In clinical terms, there are few differences between practice in the two settings and it appears that the distinct systems of regulation have minimal effect on role development. Ophthalmic nursing, as a reactive, needs based profession and in common with nursing in general, evolves in order that practice reflects what is needed by patients and services.

Abstract:
This thesis discusses the meaning of visual impairment from ophthalmic patients', nurses' and nurs1ng auxiliaries' perspectives, and analyses the roles of nursing staff in ophthalmic out-patient departments. Drawing on philosophical hermeneutics, I justify an important theme of this text - reflexivity. In doing so, I reject the notion of objective knowledge, claiming that, in particular, historical prejudices, or the collective and individual experiences of people and the process of dialogue, are the cornerstone to understanding. The research was carried out at two out-patient settings. First, reflexive ethnography was selected to enable the collection of in-depth data suitable for developing theory. Reflexive, flexible, largely informal interviews, consisting mainly of open-ended questions, were employed to further this aim, and concurrent comparative analysis of data enabled probing of emerging themes. A conceptual framework is detailed for understanding the meaning of visual impairment. within the framework, I highlight that the most influential factors in patients' emotional and social experiences of visual impairment are their perception of losses incurred through reduction in sight, and their prognosis of the eye condition. Nurses and nursing auxiliaries evaluated the prospect of visual impairment negatively, as do most people, and tended to overlook the complex nature of the experiences of ophthalmic patients. My sample of visually impaired patients conceived nurses' and nursing auxiliaries' work in out-patient departments as marginal and subsidiary to that of the doctors. Nurses' and nursing auxiliaries' perceptions of their current roles were largely cognisant with those of the patients. I discuss twelve categories which outline their roles, and draw on feminist and sociological theory to explain the present situation. I also argue that medically oriented care dominates at the expense of holistic care. It was planned that this initial analysis would lead to an action research project. Philosophical hermeneutics and reflexivity were integrated into other philosophical notions of action research to form a reflexive model of action research. A multi-method research strategy was employed to enable analysis of the process of change, in which I formally introduce the notion of dialectics, in order to link philosophical hermeneutics and reflexivity to action research. I identify eleven areas of achievement of the project, and I discuss thirteen factors which, depending on the local and national context, may stand in contradiction and, consequently, hinder change. I conclude that these hindrances and contradictions need to be resolved for advances to be made in nursing practice.

Katarakt är en grumling i ögats lins som leder till nedsatt syn. Detta tillstånd är fortfarande en av de främsta orsakerna till nedsatt syn i hela världen och kataraktoperationer är därför en av de vanligaste operationerna inom ögonkirurgi. Patienter upplever att den information de får om det kirurgiska ingreppet ofta är bristfällig. Det är komplext att ge information inför en ögonoperation. Syftet med denna integrativa litteraturstudie var att belysa faktorer som bidrar till patientnöjdhet i samband med kataraktoperationer. Datainsamling gjordes i specifika databaser som CINAHL, PubMed och SienceDirect, vilka innehåller flest tidskriftsartiklar från området medicin och omvårdnad. En analys av tio artiklar genomfördes. Litteraturstudien resulterade i tre kategorier av patientnöjdhet: Information, Hälso-och sjukvårdspersonalens bemötande och Trygghet. Många olika faktorer bidrar till patientnöjdhet. De viktigaste faktorerna är att få en individuell och patientanpassad information om kataraktoperationen, att hälso-och sjukvårdpersonalen har ett respektfullt och vänligt bemötande mot patienten samt att kunna känna trygghet. Resultatet visade också att ögonsjuksköterskan har en viktig roll när det gäller att stödja och informera patienter i samband med kataraktoperationer. Genom att förstå patienters upplevelser kan ögonsjuksköterskan hjälpa och bemöta dem på ett bättre sätt. I utbildningen till ögonsjuksköterskor bör resultatet i denna litteraturstudie kunna användas men det rekommenderas att större studier görs om patientupplevelser vid kataraktoperationer i Sverige.<br>A cataract is a clouding oft he lens of the eye that leads to impaired vision. This condition remains one of the main causes of vision impairment worldwide. Cataract surgery is amongthe most common eye interventions all over the world. Patients often consider that the information they receive about the surgical procedure is inadequate. It is difficult to provide information about eye surgery. The aim of this integrative literature review was to highlight factors that contribute to patient satisfaction in cataract surgery. Data collection took place in databases such as CINAHL, PubMed and SienceDirect, which mainly contain scientific articles from the field of medicine and nursing. The study includes an analysis of ten articles. The literature review resulted in three categories of patient satisfaction: Information, Healthcare professionals`response and Safety. Many different factors that contribute to patient satisfaction. The most important factors are to get individual and patient information about cataract surgery, respectful and friendly encountering by the healthcare professionals and being able to feel safe. The result revealed even that the ophthalmic nurse has an important role in supporting and informing the patient in the context of cataract surgery. Ophthalmology nurses can encounter patients in a more professional manner if they understandpatients` experiences. In the education of ophthalmology nurses, the result of this  literature study should be able to be used, but it is recommended that greater studies be done on patient experience in cataract surgery in Sweden.

Accessible and affordable eye care is only a dream for much of the population of developing countries. Strategies for improving the visual welfare of these people need to be appropriate to the local situation. In 1992 a proposal was devised to address the lack of eye care in Papua New Guinea. This thesis examines the outcome of this proposal and reports on the ophthalmic data collected by these trained eye nurses.Method: In 1994, 11 National nurses were trained in a 3 month intensive course to become ‘eye nurses’. A basic set of equipment was provided to each eye nurse. Appropriate follow-up and annual conferences supported this initial training. A second group of 14 eye nurses were trained in 1997. Monthly eye clinic reports from the eye nurses provide significant data on eye conditions and visual welfare in PNGResults: After 6 years 80% of the eye nurses were still actively working in eye care. An analysis was made of the eye conditions of the 30,000 patients examined by the eye nurses over this 6 year period. The data is generally consistent with previous ophthalmic data from Papua New Guinea. The eye nurses were able to provide appropriate eye care for 80% of the presenting patients without Optometric or Ophthalmic assistance.Conclusions: Training nurses to become ‘eye nurses’ functioning as basic optometrists is an effective strategy in improving eye care in developing countries. The eye nurses were able to deliver sustainable, accessible, affordable and appropriate eye care, independently treating and managing the most common eye conditions in Papua New Guinea.

Bakgrund: Telefonrådgivning har använts som kommunikationssätt sedan 30-talet inom sjukvården. I nutid är telefonrådgivning ett väl etablerat och beprövat kommunikationsalternativ som brukas runt om i Sverige. Dock belyser forskning telefonrådgivning som en komplex arbetsuppgift. Inom oftalmologi finns det vidare sparsam forskning kring just ögonsjuksköterskans uppfattning av telefonrådgivning.  Syfte: Syftet var att undersöka ögonsjuksköterskans uppfattning av telefonrådgivning på ögonmottagning.  Metod: Metoden som användes var av kvalitativ design med en fenomenografisk ansats. Åtta ögonsjuksköterskor från södra Sverige som arbetade på ögonmottagning intervjuades enskilt. En fenomenografisk analysmetod valdes att användas för analysering av data.  Resultat: Analysen resulterade i tre beskrivningskategorier: (1) Relationens betydelse mellan ögonsjuksköterska och patient, (2) Aspekter som begränsar och underlättar bedömningen mellan ögonsjuksköterska och patient samt (3) Betydelsen av kunskap, stöd och erfarenhet för ögonsjuksköterskan. Slutsats: Det framkom olika begränsande och underlättande aspekter som påverkar bedömningen och mötet med patienten. Ögonsjuksköterskan uppfattade relationen med patienten som en central del i arbetet med telefonrådgivning. Likaså uppfattades stöd i olika former samt kunskapsutveckling och erfarenhet kring området som betydelsefullt. Resultatet visar, likt tidigare forskning, att telefonrådgivning är en mycket komplex arbetsuppgift.<br>Background: Telephone counseling has been used as a means of communication since the 1930s in healthcare. Today, telephone counseling is a well-established and proven communication alternative that is used around Sweden. However, research highlights telephone counseling as a complex task. In ophthalmology, there is also sparse research on the ophthalmic nurse´s perception of telephone counseling. Aim: The purpose was to investigate the ophthalmic nurse conception of telephone counseling at an eye clinic.Method: The method used was of qualitative design with a phenomenographic approach. Eight ophthalmic nurse´s from southern Sweden who worked at the eye clinic were interviewed individually. A phenomenographic analysis method was chosen to be used for data analysis. Results: The analysis resulted in three descriptive categories: (1) The importance of the relationship between the ophthalmic nurse and the patient, (2) Aspects that limit and facilitate the assessment between the ophthalmic nurse and the patient and (3) The importance of knowledge, support and experience for the ophthalmic nurse. Conclusion: Various limiting and facilitating aspects emerged that affect the assessment and the meeting with the patient. The ophthalmic nurse perceived the relationship with the patient as a central part of the work with telephone counseling. Likewise, support in various forms as well as knowledge development and experience in the area were perceived as significant. The results show, like previous research, that telephone counseling is a very complex task.

Telefonrådgivning är en av arbetsuppgifterna för ögonsjuksköterskor, men studier om telefonrådgivning inom ögonsjukvård är begränsat. Syftet var att beskriva ögonsjuksköterskors upplevelser av användandet av beslutsstöd vid telefonrådgivning. Metoden var kvalitativ beskrivande. Sex ögonsjuksköterskor vid tre olika ögonkliniker intervjuades. Data analyserades enligt kvalitativ innehållsanalys. Resultatet gav huvudkategorierna kunskapskälla vid behov, stöd i yrkesrollen, brister i beslutsstödet och god vårdkvalitet ur ett patientperspektiv. Ögonsjuksköterskorna var positiva till användandet av beslutsstöd vid oerfarenhet av arbetsuppgiften samt vid sällan förekommande och komplexa situationer. Användandet av beslutsstöd upplevdes underlätta prioritering och samordning av vårdinsatser vilket upplevdes leda till god vårdkvalitet ur ett patientperspektiv och en känsla av trygghet. Beslutsstödet underlättade kommunikationen med andra vårdenheter och vårdsökande. Det upplevdes inte täcka alla situationer utan uttrycktes behövdes utvecklas och uppdateras kontinuerligt. Aktivt lyssnande och tänkande i kombination av erfarenhet och kunskap ansågs vara viktiga komponenter vid telefonrådgivning. Vidareutveckling och förbättring av de beslutsstöd som används idag är av betydelse.<br>Telephone counseling is one of the tasks in ophthalmic nursing and research within this area was found inadequate. The aim was to describe the ophthalmic nurses’ experiences of using a decision making system in when counseling. The method was a descriptive qualitative de sign. Six ophthalmic nurses at three different eye clinics were interviewed. The data were analyzed according to qualitative content analysis. The result was four categories: a source of knowledge source if needed, supportive to the professional role, deficiencies in decision system and good quality of care from a patient perspective. Ophthalmic nurses were positive using a decision making system if inexperienced and in infrequent and complex situations. The use of decision making system facilitated the prioritization and coordination of the care taken, high quality of care from a patient perspective and a sense of security. Decision making system facilitated communication with other health care providers and patients. The system though was not useful in every situation and there was a need of a continuous development and update. Active listening and thinking in the combination of experience and knowledge were considered to be important components in the telephone counseling. Further development of the system in use today is needed.

Barn är en stor patientgrupp inom ögonsjukvård, vilka har behov av att känna trygghet exempelvis genom att få förberedande information. För att möjliggöra detta krävs att barnet får möjlighet att kommunicera på sin nivå. Alternativ kompletterande kommunikation som till exempel bildstöd kan vara en möjlighet att ge barnet förutsättningar för att få sin röst hörd. Syftet med studien var att undersöka sjuksköterskors uppfattningar om att använda bildstöd i omvårdnaden av barn inom ögonsjukvård. En kvalitativ intervjustudie med induktiv ansats genomfördes där sex ögonsjuksköterskor och två allmänsjuksköterskor intervjuades med hjälp av semistrukturerade frågor. Data analyserades med hjälp av kvalitativ innehållsanalys, vilket resulterade i tre huvudkategorier: Barnet i centrum, Bildstödets betydelse, och Viljan att använda bildstöd. I resultatet framkom att ögonsjuksköterskan använder bildstöd i låg grad. Det fanns ett intresse för att börja använda sig av och implementera bildstöd. Genom att använda bildstöd kan barnet förberedas inför sitt besök och på så sätt skapas trygghet samt främjas kommunikation. Genom att göra barnets röst hörd skapas möjlighet att arbeta utifrån ett barncentrerat perspektiv. Då tidigare erfarenhet av bildstöd inom ögonsjukvård är bristfällig var det av stor vikt att utforska ämnet vidare.<br>Children are a large patient group in ophthalmic care. Children have a need to feel secure and have access to introductory information. To enable this, the child must be able to communicate at his or her level. Alternative augmentative communication such as image support may be an opportunity for the child to have their voice heard. The purpose of the study was to investigate nurses' perceptions of using image support in the care of children in ophthalmic care. A qualitative study was conducted with an inductive approach where six ophthalmic nurses and two registered nurses were interviewed with the help of a semi-structured questionnaire. Data was analyzed using content analysis, which resulted in three main categories: The child in focus, The meaning of image support and The willingness to use image support. The result showed that the ophthalmic nurse uses image support to a low degree. They showed an interest in using image support and implementing it in their routines. The use of image support provided security for the child by making them more prepared for the visit as well as promoted communication with the child. The thesis showed that by making the child's voice heard, the opportunity to work from a child-centered perspective was increased. As previous experience of image support in ophthalmic care is inadequate, it is of great importance to explore this subject further.

Bakgrund: Ögonsjuksköterskan möter såväl vuxna som barn inom ögonsjukvården, dock saknas studier som behandlar ögonsjuksköterskans erfarenhet av att vårda barn. Vikten av att anpassa bemötande och kommunikation till barnets ålder och mognadsnivå samt göra dem delaktiga i vården poängteras såväl i svensk lagstiftning som i studier. Detta kan innebära en utmaning för sjuksköterskor som saknar specialistutbildning inom barnsjukvård och sällan möter barn i sin verksamhet. Syfte: Att belysa ögonsjuksköterskans upplevelse av att bemöta och vårda barn vid en ögonakutmottagning. Metod: En kvalitativ intervjustudie där sju ögonsjuksköterskor intervjuades med hjälp av en semistrukturerad intervjuguide. Data analyserades med kvalitativ innehållsanalys. Resultat: Analysen resulterade i fyra kategorier och tio underkategorier, där de fyra kategorierna omfattar: Möte med barn - en positiv utmaning, Skapa en trygg relation, Att bemöta barn är också att bemöta föräldrar samt Vägar till ny kunskap. Konklusion: Ett bättre utnyttjande av ögonsjuksköterskans kompetens på ögonakutmottagningen, där ögonsjuksköterskan kan ta ett större ansvar när det gäller mottagningsbesök med barn, samt ett utökat teamsamarbete rekommenderas. För att kunna bedriva en patientsäker vård bör evidensbaserade riktlinjer utarbetas när det gäller vård och bemötande av barn. Barns och föräldrars upplevelser i samband med besök på ögonakutmottagningen kan närmare beforskas i syfte att utveckla omvårdnaden.<br>Background: The ophthalmic nurse meets both adults and children in the ophthalmic care, there is a lack of studies regarding the ophthalmic nurse’s experience when it comes to meeting children. The importance of the health care staff adapting the treatment and communication to the child's age and level and involving them in care is emphasized in Swedish legislation, as well as in studies. This can pose a challenge to nurses who lack specialist training in child care and rarely encounter children in their work. Aim: The aim of this study was to illustrate the ophthalmic nurse’s experience of meeting and caring for children in the eye emergency room. Method: The study has a qualitative research approach based on semi-structured interviews with seven ophthalmic nurses. The data was analyzed using qualitative content analysis. Result: The analysis resulted in four categories and ten subcategories, where the four categories are: Meeting the child - a positive challenge, Creating a safe relation, Meeting children is also meeting parents and Ways to new knowledge. Conclusion: A better use of the ophthalmic nurse’s competence in the eye emergency room and improvement of team collaboration are recommended. To be able to carry out a safe patient care there is a need for evidence-based guidelines regarding care and treatment of children. Finally, suggestion of further research on children's and parents' experience of nursing in the eye emergency room is recommended.

Abstract:
Conjunctivitis is the most common cause of red eye and one of the most frequent causes of visits to clinicians. There is significant controversy on the diagnosis and treatment of the disease and the use of ophthalmic antibiotics. The inconvenience of the untimely diagnostic tests led to the trend of treating conjunctivitis empirically with ophthalmic antibiotics. Subsequently, the diagnosis of bacterial conjunctivitis is higher than it should be resulting in overuse of antibiotics. Consequences of inappropriate diagnosis and treatment include increased virulence, increased health care costs, and incorrect use of antibiotics leading to possible adverse reaction and antibiotic resistance. Appropriate diagnosis and treatment will assist to reduce the diagnosis of bacterial conjunctivitis and use of ophthalmic antibiotics resulting in reduced virulence and reduced health care costs. The AdenoPlus test has been shown to be effective in appropriately diagnosing viral conjunctivitis and subsequently decreasing the use of ophthalmic antibiotics however this evidence in limited. A national retail clinic has adopted the use of this tool to assist with proper diagnosis and treatment of viral conjunctivitis. Data has not yet been analyzed to determine if the implementation of this test changes the practice of clinicians at the national retail clinic.

Det ingår i ögonsjuksköterskans arbetsuppgifter att mäta synfunktioner och att informera om undersökningsresultat. Det kan kännas svårt att lämna information om att patientens syn inte uppfyller gällande synkrav för körkort, då informationen kan påverka patientens livskvalité negativt. Syftet med studien var att undersöka ögonsjuksköterskans erfarenheter av att informera patienten om att synkraven för körkort inte uppnås. En intervjustudie genomfördes där nio ögonsjuksköterskor medverkade. En kvalitativ innehållsanalys utfördes sedan på insamlad data. Tre kategorier framkom: personcentrering vid informationstillfället, patientreaktioner påverkar informationstillfället samt omständigheter påverkar informationstillfället. Resultatet visade att ögonsjuksköterskorna var djupt involverade i körkortsärenden och de flesta kände ansvar att informera patienten när synkraven för körkort inte uppnåddes. Genom ett personcentrerat förhållningsätt försökte ögonsjuksköterskorna skapa de bästa förutsättningarna för att göra patienten delaktig. Informationstillfället påverkades av patientens reaktioner samt andra omständigheter så som vilken diagnos patienten hade och vilka konsekvenser informationen skulle kunna innebära för patienten. Det visade sig vara av betydelse att ögonsjuksköterskorna hade goda kunskaper i handhavande och bedömningar i körkortsfrågor varför sådan utbildning rekommenderas. Oftalmologiska verksamheter bör avsätta tillräckligt med tid till dessa patientbesök då även detta visade sig vara viktigt. För att ytterligare förbättra den oftalmologiska omvårdnaden är det angeläget att i kommande studier belysa patientens upplevelse av informationstillfället.<br>It is part of the ophthalmic nurse’s duties to measure functions relating to sight and to report outcomes of such examinations. It can feel difficult to deliver information about sight no longer being sufficient to fulfil the requirements for a drivers’ licence, as such information may negatively impact quality of life. The purpose of this study was to examine the experiences of ophthalmic nurses in informing the patients that their vision no longer fulfils the requirements for holding a drivers’ license. The study comprised interviews in which nine nurses participated. A qualitative content analysis of the collected data was conducted. Three categories emerged: person-centering the delivery of information, reactions of patient affect the delivery of information and circumstances affect the delivery of information. The results show that ophthalmic nurses were deeply involved in drivers’ licence cases, and most of them felt a responsibility to inform patients when their sight fell short of requirements for drivers’ license. By means of a person-centred approach, nurses attempted to create the best conditions in which to involve patients. The time of information delivery was impacted by the reactions of patients, as well as other circumstances such as the type of patient diagnosis and the consequences this information might have for the patient. It was deemed significant that ophthalmic nurses had good knowledge of process and judgment in matters of drivers’ licenses, why such education is recommended. Sufficient time for those meetings was shown to be important. In order to further improve ophthalmic caring, it is important to also highlight the patients’ experience at the time of information.

To determine the best practice in pre-operative and post-operative care in patients who have undergone cataract and/or retinal detachment surgery regarding: health education offered by nurses, counselling to prevent psychological effects, and positioning to prevent physical complications. 2.
