Abstract:
Abstract Five randomized, double-masked, placebo-controlled trials were conducted in the US in adults with DED, an eye dryness score (EDS, visual analogue scale [VAS], 0–100) ≥40, and inferior corneal staining score ([ICSS], 0-4) ≥2.0 at study entry: four 84-day efficacy trials (phase 2, lifitegrast n=58, placebo, n=58; phase 3: OPUS-1, n=293, n=295; OPUS-2, n=358, n=360; OPUS-3, n=355, n=356) and a 1-year safety study (SONATA, lifitegrast n=220, placebo n=111). The pooled population had a mean age of 59.4 years, and 76% were females. Lifitegrast treatment, versus placebo, significantly improved EDS from baseline to day 84 in three of the four trials: OPUS-1 (treatment effect [TE] 4.7; P=0.0311), OPUS-2 (TE 12.3; P<0.0001), and OPUS-3 (TE 7.5; P=0.0003). Lifitegrast significantly improved ICSS in the phase 2 (TE 0.25; P=0.0498) and OPUS-1 (TE 0.23; P=0.0007) trials, and nominally in OPUS-3 (TE 0.17; nominal P=0.0135). The responder analyses from OPUS-2 and OPUS-3 assessed the proportion of subjects with an EDS reduction from baseline (≥10, ≥15, ≥20 points), and percentage change from baseline (≥30, ≥50, ≥70%), to days 14, 42 and 84. More subjects achieved ≥30% EDS reduction with lifitegrast versus placebo in Opus-2, and Opus 3 at days 14, 42, and 84 (all nominal P<0.0001). A similar trend was seen at other response thresholds. Pooled safety data (lifitegrast n=1287; placebo, n=1177) indicated Lifitegrast was well tolerated with no serious ocular adverse events. Lifitegrast significantly improved signs and symptoms of DED in adult subjects, with EDS improvements observed starting at day 14 after treatment.

BackgroundInformation about the cost-effectiveness of early intervention programmes for first-episode psychosis is limited.AimsTo evaluate the cost-effectiveness of an intensive early-intervention programme (called OPUS) (trial registration NCT00157313) consisting of enriched assertive community treatment, psychoeducational family treatment and social skills training for individuals with first-episode psychosis compared with standard treatment.MethodAn incremental cost-effectiveness analysis of a randomised controlled trial, adopting a public sector perspective was undertaken.ResultsThe mean total costs of OPUS over 5 years (€123683, s.e. = 8970) were not significantly different from that of standard treatment (€148751, s.e. = 13073). At 2-year follow-up the mean Global Assessment of Functioning (GAF) score in the OPUS group (55.16, s.d. = 15.15) was significantly higher than in standard treatment group (51.13, s.d. = 15.92). However, the mean GAF did not differ significantly between the groups at 5-year follow-up (55.35 (s.d. = 18.28) and 54.16 (s.d. = 18.41), respectively). Cost-effectiveness planes based on non-parametric bootstrapping showed that OPUS was less costly and more effective in 70% of the replications. For a willingness-to-pay up to €50000 the probability that OPUS was cost-effective was more than 80%.ConclusionsThe incremental cost-effectiveness analysis showed that there was a high probability of OPUS being cost-effective compared with standard treatment.

Abstract Background Since the first OPUS trial 20 years ago, structured clinical assessments have been collected from a cohort of first episode psychosis patients at 2, 5 and 10 years follow-up. They found that the symptomatology of patients clustered in distinct groups, and they were able to determine stable long-term trajectories of positive and negative symptoms. The Suffolk County Medical health project has followed patients after a first episode psychosis for 20 years. They also found a stable course of trajectories but with an overall significant worsening of symptom severity over time. The 20 year OPUS follow-up will give us the first opportunity to assess the long term outcome in a large representative cohort treated within modern mental health services with treatment available for all. Methods From 1998 to 2000 578 participants were randomized to OPUS or TAU. Baseline characteristics of the cohort were as follows: mean age 26.6 years, 59% were males, 66% had a diagnosis of schizophrenia and 27% had a secondary diagnosis of alcohol or substance abuse At the 20 year follow-up the investigators will be blinded to the original treatment allocation. The patients who wish to participate will be assessed using SAPS, SANS, SCAN, PSP and GAF. Socio-demographic factors and suicidal ideation will be register via self report. Cognitive function will be tested using BACS and all participants will be asked to fill out a number of self-rating questioners including WHO quality of life-BREF, self-perceived health, strengths and difficulties, the parenting scale and self-perceived negative symptoms. Using national Danish registers we can collect information on all former participants regarding the use of psychiatric and general healthcare services, medication, supported housing or homelessness, employment status, substance abuse and mortality. Results The OPUS 20 study started collecting data in Jan 2018. We are attempting to contact as many patients as possible from the 578 participants in the original OPUS cohort. At the time of writing we had included data and attempted contact to 322 participants. Overall 104 people (31,7%) have agreed to participate in the interviews. In the follow-up 10 years ago, the participation-rate was 60% so this is a big drop in participation rate. 41 (14%) have died, 31 (9,5%) were lost due to emigration, homelessness or hidden identity and/or disempowerment. 70 (21,3%) didn’t wish to participate and 76 (23,2%) never responded. Discussion Psychotic disorders and schizophrenia in particular are associated with progressive worsening of symptoms and profound social impairment, and as such are still very stigmatized. Results from the 10 year OPUS follow-up found stable trajectories of positive and negative symptoms over time, with a tendency of reduction and stabilization of positive symptoms but less variation of negative symptoms. They found poor but stable social functioning with a mean GAF score of 55 after 10 years. The Suffolk County mental health project also found stable trajectories of psychopathology measured with SAPS and SANS. They however found progressive worsening of GAF scores declining form 49 points at the beginning to 36 after 20 years. So far we have seen stable GAF scores and SAPS and SANS scores compared to OPUS 10. This gives rise to some optimism about the prognosis for schizophrenia compared to the findings of the Suffolk study. In our study the extensive interviews combined with the data collected form Danish registers give us a unique opportunity to look at the long term course of illness after FEP. The ability to test if previous findings are robust over time will be essential to the development of targeted interventions, differentiated to the needs of different patient groups.

Abstract Background: Lacking assay standardization, different myoglobin methods may produce results that differ significantly. Methods: A multicenter study was carried out to compare the analytical performance of five commercially available assays for myoglobin measurement. Linearity, imprecision, interferences, and method comparison were studied according to NCCLS guidelines, whereas reference values were determined following IFCC recommendations. Results: The BNA and Opus showed relatively high imprecision (all but one total CV >7.4%). Other assays showed lower CVs, but they varied among laboratories, particularly at a normal myoglobin concentration (Access, 6.0–11%; Hitachi, 3.8–5.8%; Stratus, 3.4–6.5%). Results were lower in anticoagulated samples on the Access, in heparin and citrate samples on the Stratus, and in citrate samples on the BNA and Opus, and increased in heparin and EDTA samples on the Hitachi. Use of separator gel produced results significantly lower (P <0.001) on the Hitachi and higher (P = 0.016) on the Opus. Bilirubin, turbidity, and hemoglobin had no effect on evaluated methods, but rheumatoid factor affected the Access. In method comparisons, high correlation coefficients (≥0.98) were obtained. The Stratus gave higher results; however, the Access and BNA gave the lowest. The following upper reference limits (μg/L) for men and women, respectively, were obtained: Access, 70 and 52; BNA, 51 and 49; Hitachi, 67 and 58; Opus, 80 and 50; and Stratus, 86 and 63. Conclusion: The possibility of high imprecision and marked disagreement among commercial myoglobin assays should be carefully considered in clinical practice.

Abstract Objectives Most laboratories have difficulties to determine their own reference intervals for the diagnostic evaluation of patient results by direct methods. Therefore, data is often just taken from the literature or package inserts of the analytical tests. Methods The section on Reference Limits of the German Society for Clinical Chemistry and Laboratory Medicine (DGKL) first uploaded the Reference Limit Estimator (RLE) as an R-program with MS Excel-interface on the DGKL home page and now this tool is implemented in the commercial Laboratory Information System OPUS::L (OSM AG Essen, Germany). We used this OPUS::L “Population specific Reference Limits” tool online with our laboratory database. First calculations were done using the example of lipase. Results The manufacturer’s original reference interval for lipase 12–53 U/L (adults) was changed to age dependent upper reference limits of <41 U/L (<20 years), <60 U/L (20–80 years) and <70 U/L (>80 years). Conclusions By means of the OPUS::L “Population specific Reference Limits” tool we were able to establish our laborarotry specific reference interval for plasma lipase activity. The new reference limits helped to solve an old problem of implausible low elevated lipase values.

12086 Background: Patients with bone metastases may experience pain and decreased quality of life. Standard of care therapies such as radiation therapy could take weeks for pain relief and carry a risk of radiation induced fracture. Minimally invasive percutaneous radiofrequency ablation (RFA) have been shown in small observational studies to be an alternative treatment for bone metastases. We report the results of the OPuS One trial evaluating RFA for the palliative treatment of patients with painful bone metastases. Methods: OPuS One (NCT03249584) was a multicenter prospective trial. 218 subjects with painful bone metastases (≥ 4/10 worst pain scores, Brief Pain Inventory [BPI], at target treated site) were enrolled from 15 sites. RFA was performed under image guidance at one or two locations. Vertebral augmentation was followed based on physician’s discretion. Subjects’ pain (BPI) and quality of life (EQ-5D) scores were calculated in subjects at three days, one week, and one, three, six, and 12 months post RFA. Rate of complete (0 pain score at treated site with no concomitant analgesic increase) and partial responders (≥ 2 pain score reduction without analgesic increase or analgesic reduction of ≥ 25% from baseline) were calculated. Device-, procedure-, and/or therapy-related adverse events (AEs) were collected. Results: 206 subjects, 113 (55%) female and 93 (45%) males (mean age was 63.7 years) were treated with RFA. Most common primary cancers were breast (23%), lung (23%), and kidney (10%). 184 (89%) subjects were treated for metastatic lesions involving the thoracolumbar spine and 22 (11%) subjects were treated for iliac crest, periacetabulum, sacrum or mixed vertebral and pelvic location. 99% (262/264) of RFA procedures were technically successful and 97% were followed by vertebral augmentation. Subjects reported significant improvement in worst pain from baseline at 7.8 to 5.5, 4.7, 3.6, 3.2, 2.4, and 2.6 at three days, one week and one, three, six, and 12 months post RFA, respectively (p < 0.0001 for all visits). Significant improvements were also seen in average pain (p < 0.0001 for all visits), pain interference (p < 0.0001 for all visits), and quality of life scores (p < 0.0001 for all visits). Overall response rates were 53%, 58%, 61%, 63%, 70%, and 75% at three days, one week, one month, three months, six months, and twelve months post RFA, respectively. Six AEs were reported with three as serious: intra-abdominal fluid collection, pneumonia and respiratory failure. 82 deaths were reported during the study, none were related to the device, therapy, and/or procedure. No skeletal related events were reported. Conclusions: In a large prospective multicenter trial, OPuS One, RFA provided rapid, significant, and durable improvements in pain relief and quality of life up to 12 months. We used near-infrared reflectance spectroscopy (NIRS) to assess biochemical parameters in whole oil flax seeds, regardless of differences in seed coat color of the samples. At the first stage of work, the set the task to develop calibration models for the MATRIX-I IR analyzer to determine the oil and moisture content in flax seeds. The carried out the research in the laboratory of biochemistry on brown and yellow seed samples of oil flax, grown in 2015-2020 in various agro-ecological conditions of the Russian Federation. We determined the oil content on an AMV 1006M NMR analyzer in accordance with the GOST 8.597- 2010 measurement procedure; we assessed the moisture content by the standard method of GOST 10856- 96. We used the results of determination of the oil and moisture content of the seeds of test lot in accordance with the accuracy indicator of the calibration of GOST 32749-2014 to verify the reliability of the developed models. We received the best indicators of the quality of calibration models (root-mean-square prediction error, coefficient of determination and the value of the residual deviation of prediction for the rank displayed on the graph) by determining the oil content (RMSEP = 0.27 %, R2 = 99.2 and RPD = 11.2) and moisture content (RMSEP = 0.06 %, R2 = 99.9 and RPD = 39). In the OPUS LAB program we developed the “Flax 51” method for mass analysis based on the developed calibration models for the determination of oil and moisture content in whole oil flax seeds (9-20 g) in a sample cell with a diameter of 51 mm. It enables the quick carrying out a preliminary assessment of the breeding material at a high speed – more than 120 samples in 7 hours without seed destruction.

Abstract Background: The European Society of Cardiology/American College of Cardiology Committee for the Redefinition of Myocardial Infarction (MI) has recommended that an increased cardiac troponin should be defined as a measurement above the 99th percentile value of the reference group. A total imprecision (CV) at the decision limit of ≤10% is recommended. However, peer-reviewed data on assay imprecision are lacking. The purpose of this study was to construct the clinically relevant imprecision profiles for each of the commercially available cardiac troponin assays. Pools of human sera containing cardiac troponin concentrations around the MI decision limit were assessed to identify the lowest concentration associated with a 10% CV. Methods: Eight serum pools targeting different concentrations of cardiac troponin (I and T) were prepared and stored at −70 °C until usage. The cardiac troponin measurement protocol consisted of two replicates per specimen per run, and one run per day for 20 days, using two reagent lots and three calibrations. Manufacturers of each cardiac troponin assay directly performed the measurements. Data analysis for each assay was centralized and performed according to the NCCLS EP5-A guideline. Results: The lowest concentrations (μg/L) providing a 10% CV were as follows: AxSYM, 1.22; ACS:180, 0.37; Centaur, 0.33; Immuno 1, 0.34; Access, 0.06; Vidas, 0.36; Liaison, 0.065; Dimension, 0.26; Opus, 0.90; Stratus CS, 0.10; Immulite, 0.32; Vitros ECi, 0.44; Elecsys, 0.04; AIA 21, 0.09. Conclusion: No cardiac troponin assay was able to achieve the 10% CV recommendation at the 99th percentile reference limit defined by the manufacturer.

The leafminer Liriomyza trifolii is nowadays the main pest of melon orchards in the region of Chapada do Apodi, Northeast of Brazil. In order of the high infestation level of this insect, the control based only in pesticides has not been plenty satisfactory. However, considerable parasitism levels occasioned by Opius sp. have been found in field, mainly in the beginning of the production. Because of this fact, the first studies with this parasitoid are been developed in the Entomology Laboratory of UFERSA. The present study had the objective of adapting a rearing technique to the parasitoid Opius sp. in laboratory conditions, and study the aspects of it s parasitism, aiming the future utilization of this parasitoid in the leafminer integrated management programs, in the Semi-Arid conditions of Rio Grande do Norte State. Initially, infested melon leaves with L. trifolii larvae were collected in field to obtain the parasitoids. From the first parasitoids obtained from field, a technique to multiply them in L. trifolii on melon plants was developed. In the observations of the parasitism aspects of Opius sp., was perceived that it prefer to oviposite in 2nd and 3rd instar larvae. In addition, was observed that females present the highest levels of parasitism in the first two days of life
A mosca-minadora Liriomyza trifolii (Diptera: Agromyzidae), é atualmente a principal praga do meloeiro Cucumis melo L., na região da Chapada do Apodi, nordeste brasileiro. Devido às altas infestações deste inseto, o controle baseado exclusivamente no uso de inseticidas não tem sido plenamente satisfatório. Por outro lado, índices consideráveis de parasitismo, ocasionados pelo parasitóide Opius sp. (Hymenoptera: Braconidae), têm sido constatado em campo, principalmente no início da safra. Em função deste fato, estudos básicos com este parasitóide estão sendo realizados no Laboratório de Entomologia da UFERSA, Mossoró, RN. Dessa forma, o presente trabalho objetivou descrever uma técnica para criação de Opius sp., em condições de laboratório, e estudar aspectos de seu parasitismo, visando sua utilização em futuros programas de manejo integrado da mosca-minadora do meloeiro, nas condições do semi-árido do Rio Grande do Norte. Inicialmente, folhas de meloeiro infestadas com larvas da mosca-minadora foram coletadas em campo para obtenção dos parasitóides. A partir dos primeiros parasitóides obtidos foi adaptada uma técnica para multiplicação destes sobre L. trifolii em plantas de meloeiro. Nas observações sobre os aspectos do parasitismo do Opius sp., constatou-se que estes preferem ovipositar em larvas de 2º e 3º ínstar. Além disso, foi observado que as fêmeas do Opius sp. apresentam maiores índices de parasitismo nos primeiros dois dias de vida

Abstract:
Hanne Darboven’s (1941-2009) Opus 17a is a composition for solo double bass that is rarely performed due to the physical and mental challenges involved in its performance. It is one of four opuses from the composers monumental 1008 page Wünschkonzert (1984), and was composed during her period of making “mathematical music” based on mathematical systems where numbers were assigned to certain notes and translated to musical scores. It can be described as large-scale minimalism and it is highly repetitive, but even though the same notes and intervals keep repeating, the patterns slightly change throughout the piece. This is an attempt to unfold the many challenges of both interpreting, preparing and performing this 70 minute long solo piece for double bass consisting of a continuous stream of eight notes. It is largely based on my own experiences of preparing, rehearsing and performing Opus 17a, but also on interviews I have conducted with fellow bass players Robert Black and Tom Peters, who have both made recordings of this piece as well as having performed it live. One is met with few instrumental technical challenges such as fingering, string crossing and bowing when performing Opus 17a, but because of its long duration what one normally would take for granted could possibly prove to be challenging.
