Abstract:
This study aims to examine the macroeconomic and bank specific predictors of Credit Risk (NPL) and their relevant degree of impact on banks in Pakistan. For bank variables a sample of big 10 banks has been taken from 2009 to 2018. For macro-economic variables sample of 2009 to 2018 has been taken from the world bank. As financial institutions play their role to support industries and alleviate poverty in a country, this study checks the effect of banking variables as well as the economic variables on the credit risk of banks by taking industrial sector growth as a moderator. The study found that NPLs are negatively associated with Lending interest rate, Bank investment, Capital adequacy ratio, Domestic credit to private sector, Financial depth and GDP growth while positively associated with Lending capability, Return on equity, Interest spread and Liquidity Ratio. The moderation effect of Industrial sector growth on the relationship of Lending Capability and NPLs is found to be strengthening the relationship.

Abstract:
Decentralized Finance (DeFi) takes the promise of blockchain a step further and aims to transform traditional financial products into trustless and transparent protocols that run without involving intermediaries. Similar to how 2017 was the year of ICOs, 2020 was the year of DeFi, with more than fifteen billion dollars of total investments. The decentralized platforms utilize oracles to retrieve asset data from the external world, but their choice and management criteria are often unknown to the end-users. If oracles are poorly selected or managed, the funds of a rising number of investors are inevitably in danger. The issue, known as “the oracle problem”, which makes real-world applications controversial and debated due to the loss of decentralization, had recently drawn attention to DeFi, given the crescent number of related hacks that caused the loss of millions of dollars held in DeFi projects. Through a multivocal approach that considers academic papers, whitepapers, preprints, and opinion posts, this study aims to shed light on the pattern that identifies the oracle problem in DeFi and outline the most promising ways to overcome the related weaknesses. This research supports the view that the oracle problem in decentralized finance bears specific characteristics which require standardization and appropriate economic incentives to be addressed.

Abstract:
Mainstream competitiveness and international development analyses pay little attention to the significance of a country’s resource security for its economic performance. This paper challenges this neglect, examining the economic implications of countries resource dynamics, particularly for low-income countries. It explores typologies of resource patterns in the context of those countries’ economic prospects. To begin, the paper explains why it uses Ecological Footprint and biocapacity accounting for its analysis. Data used for the analysis stem from Global Footprint Network’s 2018 edition of its National Footprint and Biocapacity Accounts. Ranging from 1961 to 2014, these accounts are computed from UN data sets. The accounts track, year by year, how much biologically productive space is occupied by people’s consumption and compare this with how much productive space is available. Both demand and availability are expressed in productivity-adjusted hectares, called global hectares. Using this biophysical accounting perspective, the paper predicts countries’ future socio-economic performance. This analysis is then contrasted with a financial assessment of those countries. The juxtaposition reveals a paradox: Financial assessments seem to contradict assessments based on biophysical trends. The paper offers a way to reconcile this paradox, which also elevates the significance of biophysical country assessments for shaping successful economic policies.

The information system transition from the local and client-server financial and management information systems to n-tier information system is a very painful process, especially for academic and NGOs with a limited budget for computer equipment and software modernization. If such organizations area of activities are financial and project management, integrity, consistency and security become the key factors for information system development and implementation. The article presents results of the analyses, development and implementation of n-tier Oracle-based information systems for a NGO. Organizational and management problems, current technological infrastructure, financial resources availability, software platforms and developers solutions, local and headquarter offices requirements were analyzed. The systems were developed within the following requirements: financial and management information confidentiality and appropriate availability improvement and systems integration; organizational information and procedure standards development; incomplete and inconsistent information elimination from the data flow; transparency of the projects activity for public evaluation and information technology resources utilization by the non-for-profit, academic and NGOs for the project activity within Internet components. The first system prototype was presented at the European Foundations Center (EFC) Information Technology Conference in Brussels, Belgium, 24-25 June 1999. During 2000-2001, the system was migrated into Oracle Portal environment and development process was completed.

Abstract:
Financial time series is often assumed to be stationary and has a normal distribution in the literature. Both assumptions are however unrealistic. This paper proposes a new methodology with a focus on volatility estimation that is able to account for nonstationarity and heavy tails simultaneously. In particular, a local exponential smoothing (LES) approach is developed, in which weak estimates with different memory parameters are aggregated in a locally adaptive way. The procedure is fully automatic and the parameters are tuned by a new propagation approach. The extensive and practically oriented numerical results confirm the desired properties of the constructed estimate: it performs stable in a nearly time homogeneous situation and is sensitive to structural shifts. Our main theoretical “oracle” result claims that the aggregated estimate performs as good as the best estimate in the considered family. The results are stated under realistic and unrestrictive assumptions on the model.

Much of the excitement around blockchain is mainly due to promising applications in the financial sector. However, many also believe in the technology’s potential to disrupt non-financial sectors and applications, including supply chains, energy, e-voting, healthcare, and education. The application of blockchain within the education sector is expected to make improvements to academic transcripts, credentials, digital libraries, and student records. Research in this domain is rapidly increasing, and current reviews summarize the proposed improvements. On the other hand, the analysis undertaken has remained at a general level that lacks the depth required to cover diverging proposals that have emerged. This review focuses on the application of blockchain for academic transcripts. The aim is to find, among the proposed models, converging aspects that resolve common challenges and may lead to a universally accepted de-facto standard. Furthermore, since academic institutions will serve as oracles to the particular blockchain applications, a discussion on their trustworthiness will be outlined to explore if the proposed applications efficiently address the oracle problem. The outcome of this review highlights the need for a standardized approach built on a public blockchain to promote faster adoption and acceptance. Furthermore, oracles should be incentivized in order for the system to be sustainable, while their identities and activities should be known and identifiable.

Abstract:
The forecasting literature has come to mistrust the predictions made by experts who forecast political events in mass media. Distinguishing between judgements made by one or few individuals (‘oracles’) and assessments made by larger groups (‘crowds’), we contrast journalistic predictions with forecasts stemming from the financial industry. These two competing views were evaluated in a quantitative analysis of the ex ante success of 24 ceasefire agreements in various conflicts which took place in the Levant from 1993 to 2014. Our analysis compares the forecasts appearing in press commentaries ( Haaretz, Jerusalem Post and New York Times) with the expectations that the Tel Aviv Stock Exchange had about the stability of these cooperative efforts. To evaluate the predictions of these very dissimilar sources, the effectiveness of the ceasefires was analysed through the number of violent events following the official start of the truce. The analysis shows that the financial industry performs better than the media industry in the comparative evaluation of ceasefire forecasts, but that neither source provides sufficiently accurate predictions. The partial support for the crowd thesis is discussed in light of recent literature that resuscitates the usage of well-trained experts for forecasting purposes, but warns against the dramatizing predictions of media pundits.

Pura Barutama Inc. is a printing and packaging company based in the city of Kudus, where its financial information such as pre-order reports, accounts receivable balance reports, invoice information, and income tax article 23, must be inquired manually to the finance department. So there are issues with access to the financial information that is directly linked to consumers and vendors, namely a lack of flexibility and productivity in marketing, tax, and purchasing departments. Therefore, this study aims to provide solutions to the problems by designing and building a web-based financial information system. This information system was designed using object-oriented modeling methods, namely the Unified Modeling Language (UML) method, and was built with the PHP programming language using the CodeIgniter framework and Oracle database which was directly connected to Pura Barutama Inc. This information system is expected to increase flexibility and efficiency in the marketing, taxation, and purchasing departments of the company’s financial data management process and can obtain financial details without requesting the finance department.

With the growth of technology, the question of their appropriate use has arisen. The state is the key factor of influence and the main body that regulates the activities of innovation and information. The development of the IT sector is the key to sustainable financial development of the country, which contributes to the inflow of finance to the country. When informatizing society, the main attention is paid to a set of measures aimed at ensuring the full use of reliable, comprehensive and timely knowledge in all human activities. Information technology in the financial sector is a kind of mechanism that provides the whole process of efficient and operational work. After all, the process of economic formation acquires a new meaning. It becomes more productive and more coordinated. In modern conditions, information technologies that provide optimization of activities, integration and communication with the external environment are becoming more widespread and used in today's dynamic market. Over the past 3 years, the IT sector has become one of the important sectors for the export of services in Ukraine. In 2019, it brought in more than 5 billion USD in exports per year. An increase in the share of exports leads to an increase in the share of GDP, so the development of the IT sector should be a priority for sustainable financial development of the country. Namely, facilitating means the introduction of small and medium-sized IT enterprises. Creating competitive software for finance and accounting, namely EPR and CRM systems, facilitates the management of human resources, assets, supplies, and production of the final product. The world market leaders are such representatives as: Oracle, Salesforce, SAP, QuickBooks, 1C etc. Important areas of using IT technologies are also facilitating the exchange of information in the process of working on projects, accelerating document flow, improving planning efficiency, implementation of general business and marketing strategy, finance, HR-strategy, simplification of brand promotion, interaction with target audience , strengthening control over financial costs, etc. In addition, these systems help to set tasks, deadlines, results. The paper covers the assessment of the general state of development of e-commerce in Ukraine. The main criteria for the development of information technology are: information services, computer and telecommunications services. The dynamics of foreign trade in these types is highlighted and the most important consumer countries that use the services of Ukrainian specialists in the labor market are studied.

The paper discusses issues related to life cycle costs in construction. Life cycle cost is a key element in the assessment of environmental sustainability in construction and it provides a tool for the economic evaluation of alternative sustainability options exhibiting different capital, operating costs or resource usage. The authors reviewed selected models of estimating life cycle costs in construction, drew attention to the complex mathematical models developed so far, namely those which take into account only financial risks and those which involve the possibility of the influence of other risk factors and described the main assumptions accompanying the original model for estimating the whole life costs of buildings, including: reasons for choosing theory of possibility, division and parametrization of input data. The aim of this paper is to confirm the validity of the model structure assumptions adopted by the authors by comparing the originally selected fuzzy approach to calculating life cycle costs taking into account the risk with the probabilistic approach, as well as indicating the domain in which the probabilistic approach will complement the fuzzy approach chosen by the authors. The paper presents a comparison analysis of two approaches used in the authors’ model, a fuzzy and a probabilistic approach, recommended by the ISO standard 15686-5:2008. The authors used the Oracle Crystall Ball software in their simulations.

Abstract:
Projects for the regulation of torrent basins carry various unforeseen adverse effects that may result in breached deadlines, increased costs, a reduction of quality etc. The paper presents the basic characteristics and most frequent risks associated with erosion control. Furthermore, it provides an overview of risk management through its basic stages - starting from risk identification and risk analysis to risk responses, including the methods used for risk analysis. As a part of quantitative methods for risk analysis, the Monte Carlo method is presented as the one most frequently used in simulations. The Monte Carlo method is a stochastic simulation method consisting of the following stages: the identification of criterion and relevant variables, the allocation of probability for relevant variables, the determination of correlation coefficient among relevant variables, simulation execution and result analysis. This method was applied in the analysis of the total cost of the project for the basin regulation of the Dumaca River in order to determine the funding that would be used as a backup in case of unforeseen events with a negative impact. The project for the regulation of the Dumaca River includes basin regulation in the form of complex flow profile and the lining of zones where necessary in terms of stability. The total cost is presented as a sum of costs of all works (preliminary works, earthworks, masonry works, concrete works and finishing works). The Monte Carlo simulation for cost analysis is carried out using the Oracle Crystal Ball software with its basic steps described in the paper. A sum of funding needed as a financial backup in case of unforeseen events with negative effects is obtained as the simulated total cost of the project.

Abstract:
The article describes the selection of cloud-oriented learning tools for educational process of the students of higher education establishments by the expert appraisal method. The criteria and appropriate indicators for learning tool selection have been defined. Three learning tools to study databases were selected out of the list of offered tools, such as: Amazon RDS, Google Cloud SQL, Heroku PostgreSQL, Microsoft SQL Azure, Oracle Database Cloud Service, Rackspace Cloud Databases, SQLite Viewer. The expert appraisal method has been used with involvement of teachers, lecturers, departments’ heads and deans that have experience in teaching databases at the Ukrainian higher education establishments. The comparing analysis of the cloud-oriented database learning tools, such as Google Cloud SQL, Microsoft SQL Azure, SQLite Viewer with Google Drive, has been conducted. The functionally-didactical and organizational criteria were defined. The possibility to use such cloud-oriented learning tools in the higher education establishments were examined. Appropriate indicators for each criterion were defined. The functionally-didactical criterion has the next indicators: possibility to create, to edit and drop tables in the databases; finding the primary and external keys for the tables; creation the connections for the tables in the databases; data modification in the tables of databases; functionality to analyze the results and errors of the queries execution in the databases. The organizational criterion has the following indicators: affordability (presence of the free of charge version; validity term for the free of charge version); need in the financial ability of the customer prove; customer interface usability. As the result of the expert appraisal selection method, has been revealed that SQLite Viewer with Google Drive is the most reasonable learning tool, by the manifestation of all the criteria, for the students of higher education establishments.

235 Background: The MD Anderson Clinical and Translational Research Center (CTRC) focuses on Phase I and early Phase II clinical trials and is managing approximately 156 studies annually. This 18-room combined treatment and laboratory unit manages an average of 12,500 research pharmacologic time points and over 3,000 individual shipments yearly. In FY12, 74% of shipments were done on the day of collection (DOC), whereas 26% of the shipments were shipped periodically as required by the corresponding protocol. All studies are monitored at each step for errors - minor (simple DOC label corrections) or major (improper or missed specimen). Activity is analyzed and reported monthly. Methods: In 2010, to ensure accuracy of timing, collection method, proper storage, specimen condition, and destination accuracy, we developed a robust proprietary Oracle-based laboratory tracking system (LabTracker). In addition, we designed an Advanced Research Management and Data Analysis interface (ARMADA) which provides daily step by step protocol guidance using an innovative color-icon touch-screen display of all required protocol activities for patients in the CTRC. LabTracker contains all auditable items required in the clinical trial and markedly reduces work duplication and handwritten data transcription. We hypothesized that this input system would help decrease errors and improve research quality. Results: In FY10 there were 2,506 DOC shipments with an overall error rate of 3.07%. In FY12 errors decreased to 1.31% out of 3,128 DOC shipments. In FY10, there were 14,507 timepoints with 28 major errors (0.19%). In 2012, with 13,490 timepoints, major errors decreased to 19 (0.076%). LabTracker database also supports billing and has catalogued over 275,000 samples. This system provides real-time accounting and has revolutionized the quality control system of the CTRC as well as the research finance component with 98% charge capture efficiency within 30 days. Conclusions: The LabTracker/ARMADA systems are high volume phase I protocol management tools which have improved quality, decreased redundant work, and support a real time financial management system for complex phase I clinical trials.

The purpose of this paper is to investigate the relationship between factors affecting the decision to purchase and the need for innovative pharmaceutical products by the Thai pharmaceutical industry. An orally disintegrating tablet (ODT) is an example of an incremental pharmaceutical innovative product which is still a new concept in Thailand. The results show that financial factors, as well as government policy and drug regulations, have a positive effect on the purchasing decision of pharmaceutical excipients for ODT from the Thai pharmaceutical industry.

The unexplained and inconsistent behavior of financial markets provides the motivation to engage interdisciplinary approaches to understand its intricacies better. A proven approach is to consider investors as heterogeneous interacting agents who form information networks to inform their investment decisions. The rationale is that the topology of these networks has contributed to a better understanding of the erratic behavior of financial markets. Introducing investor heterogeneity also allows researchers to identify the characteristics of higher performing investors and the implications of investors exhibiting short-termism, a feature recognized by some as detrimental to the performance of the economy. To address these topics, an agent-based artificial stock market is implemented, where investors utilize various information sources, including advice from investors in their network, to inform their investment decisions. Over time investors update their trust in their information sources and evolve their network by connecting to outperforming investors—Oracles—and discarding poor advisers, thereby simulating the evolution of an investor network. The model’s most significant finding is uncovering how the market’s behavior is materially affected by the time-horizon of investors, with short-term behavior resulting in greater volatility in the market. Another finding is the reason why short-term investors generally outperform their long-term counterparts, particularly in more volatile environments. By providing significant insights into the formation of an investor network and its ramifications for market volatility and wealth creation (destruction), this paper provides crucial clues regarding the empirical data that needs to be collected, assessed, and tracked to ensure policymakers and investors better understand the dynamics of financial markets.

A 57-year-old woman presented with a 1-year history of major depressive disorder. She was started on venlafaxine XR 75 mg orally daily and a few days later developed severe dysosmia to foods she used to enjoy. She never had previous problems with smell or taste. At her 1-month follow-up, her depressive symptoms had improved, but she reported persistent dysosmia and was found to have associated weight loss due to decreased oral intake. She was advised to switch medications, but due to financial constraints she continued taking the same dose. At follow-up 48 days later, she reported complete resolution of her dysosmia and was eating normally again, but she had persistence of some depressive symptoms so her dose was gradually increased to venlafaxine XR 225 mg orally daily until her depressive symptoms and postmenopausal hot flashes were well controlled. There were no changes with continued use over the following 8 years.

These difficulties include inability to pay for basic necessities, presence of medical debt, and high out of pocket burdens relative to income. The primary objective of this study was to estimate the proportion of patients with multiple myeloma (MM) who experience financial difficulties in the past 12 months.Methods: Data collection entailed a comprehensive, theoretically grounded telephone survey and companion medical chart abstraction. Subjects included individuals with a current diagnosis of MM whose current or recent treatment included pharmaceutical-based care and who were not enrolled in a treatment-based trial. Practices eligible to recruit respondents included 44 NCORP affiliates of the Alliance. 14 geographically diverse NCORP affiliates participated between 11/2019 and 6/2020. The primary endpoint of the study was the proportion of subjects who reported financial difficulty in the past 12 months, as measured by the EORTC QLQ-C30 item #28. This proportion and 95% Wilson score confidence interval were estimated for all MM patients who responded to the financial difficulty question ((# reported financial difficulty)/(total # in that category who answered the question)). NCI Central IRB approved this study. Results: 393 subjects were recruited. 304 subjects completed the survey (77.4% response rate). Mean age was 67.5 years (SD 9.8), 143 (46.4%) were female, 24 (7.8%) self-reported race as ‘Black or African American’, 82 (26.6%) reported insured by government insurance Medicare only, 116 (38.2%) reported highest education as high school or below, and 94 (30.5%) reported high income. Mean time from diagnosis to survey enrollment was 3.6 years (SD 4.5). 292 (95.1%) were currently receiving treatment and 192 (62.5%) reported currently receiving a pre-defined ‘expensive’ oral pharmaceutical-based cancer treatment. 20.2% (95% CI:16.1%, 25.0%) reported financial difficulties. Conclusions: This is the first national study to systematically assess the prevalence of financial difficulties and its correlates among MM patients. Approximately 1 in 5 surveyed patients reported financial difficulties. Results of this study aim to inform efforts to improve financial navigation and resources for cancer patients.

Objective: This systematic review aims to evaluate nasoalveolar molding (NAM) in the context of burden of care defined as physical, psychosocial, or financial burden on caregivers. Search Methods: Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, 5 databases were searched from inception through December 24, 2019, for keywords and subject headings pertaining to cleft lip and/or palate and NAM. Eligibility Criteria: Clinical studies on NAM with reference to physical (access to care, number of visits, distance traveled), psychosocial (caregiver perceptions, family interactions, breast milk feeding), and financial (direct and indirect costs) burden were included. Data Collection and Analysis: Study selection was performed by 2 independent reviewers. Results: The search identified 1107 articles and 114 articles remained for qualitative synthesis. Burden of care domains were discussed but not measured in 43% of articles and only 25% assessed burden of care through a primary outcome. Of these, 20 articles reported on physical, 8 articles on psychosocial, and 12 articles on financial burden. Quality of evidence is limited by study design and risk of bias. Conclusion: Nasoalveolar molding has been indiscriminately associated with burden of care in the literature. Although NAM may not be the ideal treatment option for all patients and families, the physical considerations are limited when accounting for the observed psychosocial advantages. Financial burden appears to be offset, but further research is required. Teams should directly assess the impact of this early intervention on the well-being of caregivers and advance strategies that improve access to care.

The provision of trauma care is a financial burden, continually associated with low reimbursement, and shifts the economic burden to major trauma centers and providers. Meanwhile, the volume of craniomaxillofacial (CMF) trauma and the number of surgically managed facial fractures are unchanged. Past financial analyses of cost and reimbursement for facial trauma are limited to mandibular and midface injuries, consistently revealing low reimbursement. The incurred financial burden also coincides with the changing landscape of health insurance. The goal of this study is to determine the opportunity cost of operative management of facial trauma at our institution. From our CMF database of greater than 3,000 facial fractures, the physician charges, collections, and relative value units (RVUs) for CMF trauma per year from 2007 to 2013 were compared with a general plastic surgery and otolaryngology population undergoing operative management during this same period. Collection rates were analyzed to assess if a significant difference exists between reimbursement for CMF and non-CMF cases. Results revealed a significant difference between the professional collection rate for operative CMF trauma and that for other operative procedures (17.25 vs. 29.61%, respectively; p < 0.0001). The average number of RVUs billed per provider for CMF trauma declines significantly, from greater than 700 RVUs to 300 over the study period, despite a stable volume. Surgical management of CMF trauma generates an unfavorable financial environment. The large opportunity cost associated with offering this service is a potential threat to the sustainability of providing care for this population.

Objective This study examined malpractice claims related to cleft lip and cleft palate surgery to identify common allegations and injuries and reviewed financial outcomes. Design The WestlawNext legal database was analyzed for all malpractice lawsuits and settlements related to the surgical repair of cleft lip and palate. Main Outcomes Measures Inclusion criteria included patients undergoing surgical repair of a primary cleft lip or palate or revision for complications of previous surgery. Data evaluated included patient demographics, type of operation performed, plaintiff allegation, nature of injury, and litigation outcomes. Results A total of 36 cases were identified, with 12 unique cases from 1981 to 2006 meeting the inclusion criteria. Six cases (50%) were decided by a jury and six by settlement. Five cases involved complications related to the specific surgery, and the other seven were associated with any surgery and perioperative care of children and adults. Cleft palate repair (50%) was the most frequently litigated surgery. Postoperative negligent supervision was the most common allegation (42%) and resulted in a payout in each case (mean = $3,126,032). Death (42%) and brain injury (25%) were the most frequent injuries reported. Financial awards were made in nine cases (after adjusting for inflation, mean - $2,470,552, range - $0 to $7,704,585). The awards were significantly larger for brain injury than other outcomes ($4,675,395 versus $1,368,131 after adjusting for inflation, P - .0101). Conclusion Malpractice litigation regarding cleft lip and palate surgery is uncommon. However, significant financial awards involving perioperative brain injury have been reported.

8 Background: The rising cost of cancer drugs may make treatment unaffordable for some patients. Patients often rely on drug manufacturer-administered Pharmaceutical Assistance Programs (PAPs) to obtain drugs at reduced or no cost. The overall impact of PAPs on cancer care delivery is unknown. Methods: We identified all patients obtaining cancer drugs across an academically affiliated, integrated health system in the state of North Carolina during 2014. The proportion of patients receiving PAP assistance, and the retail value of the assistance, were quantified for each oncology drug. Cancer drugs were analyzed with respect to route of administration. Results: 215 unique patients submitted a total of 478 successful PAP requests for cancer drugs. The majority of the retail value of drugs obtained was for oral cancer drugs, particularly targeted therapies including tyrosine kinase inhibitors. Among all cancer patients who received medical treatment, 5.5% required PAP assistance, whereas 10.6% receiving an oral agent required PAP assistance (Table). The proportion receiving PAP assistance varied substantially by drug, ranging from <1% of patients (e.g., carboplatin, methotrexate) to 50% of patients (e.g., ponatinib, temsirolimus). Patients obtained a total of $1,556,575 of imatinib and $1,449,633 of dasatinib, which were the two drugs with the highest aggregate retail value. 40% of PAP-utilizing patients were uninsured, 26% had Medicaid coverage, 20% had Medicare coverage, and 14% were commercially insured. Conclusions: A substantial proportion of patients with cancer receive private charitable assistance through PAPs in order to obtain standard-of-care treatments. In particular, a disproportionate share of patients treated with orally-available cancer drugs require PAP assistance. This includes patients with federal and private insurance, suggesting an inability of patients to meet cost-sharing requirements. [Table: see text]

Purpose The low-income subsidy (LIS) substantially lowers out-of-pocket costs for qualifying Medicare Part D beneficiaries who receive orally administered chemotherapy. We examined the association of LIS with the use of novel oral immunomodulatory drugs (IMiDs; lenalidomide and thalidomide) among beneficiaries with myeloma, who can receive either orally administered or parenteral (bortezomib-based) therapy. Methods Using SEER-Medicare data, we identified Part D beneficiaries diagnosed with myeloma in 2007 to 2011. In multivariable models adjusted for sociodemographic and clinical characteristics, we analyzed associations between the LIS and use of IMiD-based therapy, delays between IMiD refills, and select health outcomes during the first year of therapy. Results Among 3,038 beneficiaries, 41% received first-line IMiDs. Median out-of-pocket cost for the first IMiD prescription was $3,178 for LIS nonrecipients and $3 for LIS recipients, whereas the respective median costs for the first year of therapy were $5,623 and $6, respectively. Receipt of the LIS was associated with a 32% higher (95% CI, 16% to 47%) probability of receiving IMiDs among beneficiaries age 75 to 84 years and a significantly lower risk of delays between refills in all age groups (adjusted relative risk, 0.54; 95% CI, 0.32 to 0.92). Duration of therapy did not significantly differ between LIS recipients and nonrecipients (median, 7.6 months). Patients treated with IMiDs had significantly fewer emergency department visits and hospitalizations compared with patients receiving bortezomib (without IMiDs), but 1-year overall survival and cumulative Medicare costs were similar. Conclusion Medicare beneficiaries with myeloma who do not receive LISs face a substantial financial barrier to accessing orally administered anticancer therapy, warranting urgent attention from policymakers. Limiting out-of-pocket costs for expensive anticancer drugs like the IMiDs may improve access to oral therapy for patients with myeloma.

3594 Background: Fibroblast activation protein (FAP) is selectively expressed by carcinoma stromal fibroblasts, but not by epithelial carcinoma cells, normal fibroblasts, or other normal tissues. FAP potentiates tumor growth in a murine model of human epithelial cancers. Val-boro-Pro, a boronic acid inhibitor of FAP protease activity, attenuates tumor growth in a mouse colorectal cancer model characterized by FAP expression by tumor-associated fibroblasts. We therefore initiated this first clinical trial of Val-boro-Pro in patients with colorectal cancer. Methods: Patients (pts) with previously treated, measurable metastatic colorectal cancer (CRC) were initially treated with 400 μg of Val-boro-Pro orally twice daily continuously. Due to unacceptable toxicities in the first 3 patients (including 1 grade 5 renal failure), the starting dose was reduced to 200 μg orally twice daily, with intrapatient dose escalation to 300 μg bid permitted after 4 weeks. The primary endpoint is response rate, with secondary endpoints including time to progression, and in vivo laboratory correlates of FAP inhibitory activity. Results: 20 patients (12 women, 8 men) with ECOG performance status 0–1 have been treated at 200 μg orally twice daily. The median age is 58 (range 41–80). At 200 μg bid, grade 1–2 adverse events include fatigue (12 pts), anorexia (7 pts), edema (7 pts), and fever (5 pts). The most common laboratory abnormalities are grade 1–2 anemia (7 pts), elevated alkaline phosphatase (4 pts), and hypoalbuminemia (3 pts). Grade 3–4 events are elevated alkaline phosphatase (1 pt, grade 3) and headache/syncope (1 pt, grade 3). Analyses of the first 9 patient plasma samples demonstrate a 92% (S.D. 2%) inhibition of dipeptidyl peptidase activity, and 22% increase in the FAP substrate α2-antiplasmin. Conclusions: Val-boro-Pro 200 μg orally bid is biologically active and well-tolerated, with toxicities likely related to cytokine upregulation. A planned interim analysis for clinical response will be conducted after treatment of 23 evaluable patients. The selectivity of FAP expression for tumor fibroblasts provides a unique opportunity to investigate and manipulate stromal contributions to tumor growth and invasion. No significant financial relationships to disclose.

Abstract:
15024 Background: S-1 is an effective oral anti-cancer drug which is formulation of 5-FU, and it is widely used for gastric cancers in Japan. However, the compliance of S-1 treatment is not satisfactory due to its side effects. We hypothesized that the plasma concentration of 5-FU could be verified individually, because the drug is administered orally. Therefore, we investigated the pharmacokinetics of 5-FU when it was administered orally in the form of S-1. Methods: The plasma concentration of 5-FU after S-1 oral administration for adjuvant chemotherapy was measured in 22 advanced gastric cancer patients (Stage II or III). Total gastrectomy was performed in 12 patients and distal gastrectomy was performed in 10 patients. All patients were given S-1 (80mg/m2) orally at 8:00 and 20:00, daily. Blood samples were collected at 10:00, 12:00, 14:00 and 16:00 on the 7th day after S-1 administration and we measured 5-FU concentration of plasma. Results: Pharmacokinetic parameters of plasma concentration of 5-FU were as follows: Cmax, 157.92 ± 42.9 ng/ml; Tmax, 2.36 ± 0.79 h; T1/2, 2.90 ± 2.36 h; and AUC 0–8 ,731.8 ± 236.4 ng ·h/ml in all patients given the usual dosage. The plasma concentration of 5-FU was quite different between patients. The difference of Cmax and AUC was 3∼ 4 times. The AUC0- 8 in the total gastrectomy patients (752.6± 273.3 ng·h/ml) was higher than that in distal gastrectomy patients (714.5±211.7 ng·h/ml), however this difference did not reach significance. Adverse events of more than grade 3 were observed in 7 patients. AUC0–8 of 4 of 7 patients were more than 800 ng·h/ml and those of 2 other patients were more than 750 ng·h/ml. The patients with high AUC0–8 levels had a tendency to show more severe adverse events. Conclusions: We conclude that physicians should pay close attention to individual differences in the plasma concentration of 5-FU in postoperative gastric cancer patients when this drug is given orally as S- 1. 263 Background: Met over-expression has been found in bladder cancer (CaB). However, to date, the effects of Met inhibition in CaB have not been reported. We used a small, orally available, highly specific Met-inhibitor currently on phase 2 clinical trials, to assess its effects in an orthotopic xenograft model of bladder transitional cell carcinoma (TCC). Methods: An orthotopic xenograft murine model of TCC of the bladder was developed with T24-Luciferase positive bladder cancer cells. NIH 3T3 cells producing human hepatocyte growth factor (hHGF) were implanted subcutaneously in mice to provide a source of hHGF. Animals were treated with the selective Met inhibitor (PF-2341066) intra-peritoneally after positive detection of bladder tumor primary and/or metatastatic disease. Fluorescence imaging (Xenogen IVIS) of mice was performed weekly. Mice were euthanized 4 weeks after the start of treatment and their tissues studied histologically. Results: PF-2341066 was found to reduce tumor burden to below detectable levels in both primary and metastatic sites in all mice treated. No noticeable side effects were detected in treated mice secondary to drug administration. Conclusions: This study is the first to test a small orally available Met-selective inhibitor in an orthotopic, HGF-driven model of human CaB. Our results demonstrate that Met-inhibition by PF-2341066 reduces TCC tumor burden supporting its potential use in patients with bladder cancer. No significant financial relationships to disclose.

5042 Background: Most entrometrial adenocarcinomas of the endometrioid type express estrogen receptors and are potentially hormone sensitive. The aim of this phase II study was to evaluate the responsiveness of these tumors to treatment with Exemestane, an aromatase inhibitor. Methods: So far, 31 patients have been recruited to this ongoing study. Patients had to have a measurable tumor. Evaluation of response by CT was performed every 3 months. Evaluation of estrogen receptors (ER) was performed by immunohistochemistry and was positive in 17 patients, negative in 5 and not performed in 6 of the 28 patients evaluable for response. Treatment was given with Exemestane 25 mg orally daily. Results: Radiotherapy had been given as part of the initial therapy to 16 patients. Before inclusion into the study, 22 patients had received chemotherapy and 7 of these had also received hormonal treatment with gestagens for relapse. Response to Exemestane treatment was evaluable in 28 patients. In 17 patients with ER positive tumors, 1 showed a complete response, 1 partial response and 6 have had stable disease for 3+ months (3 patients), 6+ months (2) and 9+ months (1). The treatment was well tolerated with no major toxicity. Updated response rates will be presented. Conclusions: Treatment with Exemestane orally showed effect in patients with ER positive tumors in this heavily pre-treated group of patients with endometrioid adenocarcinoma of the endometrium. No significant financial relationships to disclose.
