Abstract:
Thyroid-associated orbitopathy is the most common cause of unilateral or bilateral proptosis in adults. A mainstay of surgical treatment is orbital decompression utilizing osteotomies to increase the size of the affected bony orbit to accommodate the larger soft tissue volume. Over the past several decades, numerous approaches have been described for orbital decompression. However, given the intricate osseous and soft tissue anatomy within the orbit, orbital decompression is a potentially hazardous intervention. With advances in three-dimensional imaging and virtual planning, extensive orbital

PROBLEM: This study explores contribution of the orbital floor to mechanical outcomes of orbital decompressions. METHOD OF STUDY: Endoscopic medial wall orbital decompressions with and without extensive medial orbital floor removal (OFR) were performed on opposite sides of ten thawed fresh-frozen cadaver heads Bone removal was compared on pre- and post-dissection CT scans and after orbital exenteration. RESULTS: Bony removal in the anterior orbital apex was significantly better after OFR (117 vs 66, p < 0.0001). An average of 10.3% (range 0 - 45.5%) of the orbital floor directly under the g

Orbital decompression for Graves' disease has traditionally been performed with the patient under general anesthesia. Endoscopic instrumentation allows for removal of the medial orbital wall and floor through an intranasal approach with local anesthetic techniques. Twelve endoscopic orbital decompressions were performed on awake patients with uncovered eyes so that vision could be monitored throughout the procedure. Simultaneous lateral decompressions were performed in 11 cases. No intraoperative or postoperative complications occurred. Visual acuity remained stable or improved in all cases. P

AbstractSurgical treatment for patients with Graves’ orbitopathy consists of orbital decompression, strabismus surgery and eyelid surgery. In this chapter, we will limit ourselves to orbital decompression (refer for strabismus surgery: Chap. 6 and for eyelid surgery: Chap. 22).There are many different orbital decompression techniques, that all have the potency to improve visual functions and restore-in combination with strabismus and eyelid surgery-the premorbid appearance. In addition, they are relatively safe. This does not alter the fact that an orbital decompression, in the center of the f
