Full textBackground: Diabetes mellitus is a chronic disease which affects more and more people around the world. Studies have shown a close relationship between lifestyle and diabetes type 2 and the importance of having a good self-care treatment to reduce the risk of complications. The diabetes nurse has an important responsibility to work with both health promotion and disease prevention through guidance in self-care. Purpose: The purpose of the study was to describe the diabetes nurse’s experiences of guiding people with diabetes in self-care. Method: 16 semi-structured interviews was performed during January – March 2016 with diabetes nurses who worked in primary care. The interviews were transcribed and analyzed using a qualitative content analysis. Result: The content analysis resulted in three categories; the ground for commitment, Knowledge transfer and Organizational conditions. The four main results in the study was: The diabetes nurse’s possibilities to create participation, knowledge that both the diabetes nurse and the person possessed, group educations health promoting impacts and the team’s impact on guidance. Conclusion: The result highlights that guidance in self-care should be built on knowledge and lead to a higher level of engagement. A well-functioning communication is crucial to achieve common goals and that the team around the person with diabetes is working together towards the same goal.

Full textBackground: Heart failure is a global health problem that is constantly increasing. Symptoms of heart failure affects the performance in activities of daily living and the ability to perform self-care. Proper self-care can decrease symptoms, increase self-esteem and well-being. Orem's theory Self-care deficit nursing theory was used as a theoretical framework. Aim: was to describe chronic heart failure patients experiences of daily self-care. Method: The literature review had scientific articles with a qualitative method which was analyzed by Friberg's (2017). Results: A total of 13 scientific articles were selected for the results and presented in three main categories with respective subcategories: Emotional challenges (changed emotions), Physical limitations (dyspnea & fatigue), Support through self-care (resources). Conclusion: Patients experienced that negative emotions affected their ability to perform self-care. Social support from the family had both a positive and negative impact. Conversations with various professions were valuable for patients as they gained increased knowledge about heart failure to manage their self-care. Inadequacy in healthcare staff resulted in incorrect information being communicated to patients with heart failure and causing inefficient self-care.

Full textBackground: Type 2 diabetes mellitus is a welfare disorder in which the number of people with the disease is increasing in the world. An important part of the treatment is self-care where the person with the diabetes disease has a great responsibility. The nurse has a responsibility to provide the person with information and knowledge about the illness for the person to be able to perform self-care. Aim: To describe the experience of self-care in people with diabetes mellitus type 2. Method: A qualitative literature review with a compilation of 13 scientific articles. The analysis followed Friberg's five-step analysis. Results: The two main themes developed in the literature review's results: "Means lifestyle changes" and "Significant with support". Difficulties that arose in self-care were changing dietary habits, physical activity and changing daily routines. The result showed that self-care has some challenges in which support from the environment and health care personnel can facilitate how self-care was experienced. Conclusion: Managing self-care meant changing living habits where adaptation to the new life went very fast. Support from the environment and health care played a major role in how self-care worked and was experienced.

Full textBackground: Diabetes mellitus type 2 is a growing health care concern, in Sweden and globally. The implementation of self-care is a significant part to avoid complications and live a long and healthy life. Aim: To describe people’s experiences of performing self-care in diabetes mellitus type 2. Methods: A qualitative literature review was conducted in which 16 articles were analysed and used for the result. An inductive approach has been used. The analysis was conducted in five steps according to Friberg. Results: The analysis resulted in two themes: “Changed lifestyle” and “Need for support and information”. Changes regarding diet and physical activity entailed readjustment in the everyday life. Dietary changes were found as a barrier to the social life. Support from both health care, family and friends was meaningful. The information that was provided affected the execution of self-care. Conclusion: The management of self-care involved various extent of lifestyle changes. The most difficult to implement in the daily life was dietary changes. Information and support from surroundings and health care facilitated the implementation of self-care. The literature review may be used as support for health care professionals to achieve more insight in people’s experiences.

Full textEste relatório surge no âmbito do Curso de Mestrado em enfermagem Médico-Cirúrgica (EMC) e tem como finalidade apresentar reflexivamente o percurso conducente ao desenvolvimento de competências de enfermeiro especialista (comuns e específicas) e de Mestre. Para isso, foi utilizada a metodologia de trabalho de projeto, integrando dois projetos distintos: projeto de intervenção em serviço (PIS) e projeto de aprendizagem clínica (PAC), desenvolvidos ao longo dos estágios realizados num serviço de cirurgia geral (SCG). O PIS, formulado sob a premissa de que uma eficaz gestão dos cuidados está numa das bases da qualidade dos mesmos, incidiu sobre a necessidade de uniformizar procedimentos relativamente à avaliação funcional das pessoas internadas no SCG. Este, veio permitir, através de formação e criação de documentos normativos e orientadores desta prática, promover a qualidade dos cuidados de enfermagem através da otimização da intervenção dos enfermeiros no domínio da avaliação funcional da pessoa internada no SCG. Foi nosso referencial teórico a Teoria do Défice do Auto-Cuidado de Orem, com enfoque na importância de direcionar os cuidados prestados às necessidades de cada pessoa, partindo de uma avaliação funcional, dotada de objetividade através do uso de escalas validadas, como a escala modificada de Barthel. Por sua vez, no PAC projetaram-se atividades específicas, das quais se destacam: participação num exercício de simulação de inundação numa unidade de cuidados intermédios e realização de respetivo poster de divulgação; realização de ação de sensibilização aos familiares das pessoas internadas no SCG, sobre importância da higienização das mãos na visita; e realização de um poster sobre hipodermoclise, classificado em 2º lugar num encontro de enfermagem. Neste documento analisamos crítica e reflexivamente ambos os projetos, e os contributos dos mesmos no desenvolvimento das competências supra-citadas.
This report is set within the context of the Medical Surgical Nursing (MSN) Master’s Degree, in order to reflexively present the route which led us to the development of specialist (either common or MSN specific) and Master nurse skills. Therefore, we used the project methodology, integrating two distinct projects: intervention project in service (IPS) and clinical learning project (CLP), both developed throughout the internship in a general surgery service (GSS). The IPS was formulated under the premise that effective care management is one of the bases of its quality, and focused on the need to standardize procedures for the functional evaluation of people admitted to the GSS. Through training and creation of normative and guideline documents, the IPS has promoted the quality of nursing care by optimizing the nurses’ involvement in the functional evaluation of the hospitalized patient in GSS. Our theoretical framework was the Orem’s Self-Care Deficit Theory, focusing on the importance of care directed to the needs of each person, and based on a functional evaluation, endowed with objectivity through the use of validated scales such as the modified Barthel index. In turn, on the CAP we designed specific activities, among which the following are highlighted: participation in an exercise of flood simulation in an intermediate care unit and the development and disclosure of the respective poster; performing of an awareness session to the relatives of those admitted to the GSS, about the importance of hand hygiene in the visit; and creating a poster on hypodermoclysis, that was ranked 2nd in a nursing meeting. In this document we analyze critically and reflexively both projects, and their contributions to the development of the above-mentioned skills.

The objective of this study is to apply one of the theories taught at training institutions to teenage families in the community health practice and to develop a strategy to make such application acceptable to nurses. Orem' s self-care deficiency theory was consequently applied to case studies at a community clinic of a Municipal Health Department. Personal interviews were mainly used as research technique to identify problems, determine needs and address self-care deficiencies. Data was evaluated according to UNICEF' s GOBI FFFF, the "PKK" 's five concepts, the nursing process and OREM's Constructs. Evaluation has proved that Orem' s theory offers an instrument to evaluate self-care abilities and guide the nurse in identifying deficiencies and planning nursing care in order to reduce multidisciplinary consultation, which results in professional nurses being employed appropriately, resources being utilised to the maximum and each individual developing into a complete self-care agent.
