Teori self-care (perawatan diri) oleh Orem memiliki penjelasan yang lengkap tentang konsep perawatan diri, defisit perawatan diri, serta tindakan keperawatan untuk mengatasi masalah yang muncul. Dalam konteks rehabilitasi, teori Orem sangat sesuai dengan tujuan rehabilitasi yaitu membantu klien untuk memandirikan klien seoptimal mungkin. AbstractMental health nurses should professionally address the self-care deficit problem in schizophrenic patients, especially in the psychosocial rehabilitation phase. The nursing intervention of the self-care deficit patients can be delivered based on the nursing theoretical framework. The Orem’s self-care theory elucidates the concept of self-care, self-care deficit, and its nursing care. In the rehabilitation context, the Orem’s theory is relevant to the aim of rehabilitation – optimalizing the client’s own daily functional capacities.

The purposes of this study were to evaluate the psychometric properties of English and Spanish instruments that measure the nutrition behavior and practices of children and their parents. Orem’s self-care deficit nursing theory was used in this methodological study. A convenience sample of 333 children and 262 mothers participated from two schools in Washington, D.C. and two schools in Santiago, Chile. Principal component analysis indicated three component per instrument corresponding to Orem’s Theory of operations demonstrating construct validity of the instrument. The study findings showed evidence for validity and reliability of the English and Spanish versions and indicated that the instruments appropriately represented Orem’s operations. The results have implications for the development of health behavior measurement instruments that are valid, reliable, designed for children, culturally appropriate, and efficient. Measuring the nutrition behavior of children and parents is critical for determining the effectiveness of nutrition intervention programs. Furthermore, instruments are needed so that researchers can compare corresponding child and parent behaviors or compare behaviors across cultures.

ABSTRACT Objective: to identify self-care deficits in women with leg ulcers and sickle cell disease. Method: a qualitative, descriptive, exploratory approach with 14 women. The collection was performed from December 2015 to January 2016, while the analysis was supported by Orem’s Self-Care Deficit Theory. Results: self-care requirements are not fully met. The following were identified as universal: balance between loneliness and social interaction, rest and proper nutrition; development: inability to perform work, abrupt changes in the environment due to hospitalization, educational deprivation and dropout; health deviations: ulcer recurrences, pain, changes in skin appearance and gait changes. Conclusion: nursing care for women with leg ulcers and sickle cell disease requires planned actions for comprehensive care.

This article describes the desire for normalcy as experienced by 6 men and 13 women with serious mental illness based on secondary analysis of data from two previous qualitative studies. Original data were collected by structured and semi-structured interviews, and thematic analysis was done. Major categories of desire for normalcy are having normal things and experiences; doing meaningful activities; and being well, safe, free, and independent. Self-care actions to promote and maintain normalcy are identified, and findings are discussed in relation to Orem’s self-care deficit theory of nursing. Implications for nursing theory, research, and practice are addressed.

People with schizophrenia have increased morbidity and mortality rates associated with poor physical health that requires a holistic and comprehensive nursing approach. Dorothy Orem’s self-care deficit nursing theory (SCDNT) has made a substantive contribution to nursing by focusing on people as whole entities who engage with their environment to sustain health. We offer an expanded view of the SCDNT by showing how it is a useful framework for nurses to integrate interpretive, empirical, and critical theoretical perspectives for addressing both the physical and mental health needs for people with schizophrenia. Understanding patient needs through different theoretical lenses will help nurses integrate high-quality, evidence-based care with patients’ realities, needs, and values while considering the influence of the broader sociopolitical context. We demonstrate how the SCDNT allows nurses incorporate a critical perspective for critiquing the influence of societal barriers on individual’s health and advocate for initiatives to address the complex sociopolitical, economic, and contextual factors that affect the physical well-being of individuals with schizophrenia.

This research aimed at systematizing nursing care to HIV/aids patients in view of Orem's Self-care Deficit Nursing Theory, using the convergent-care method and the Self-Care Nursing Process. Subjects were thirteen HIV/AIDS patients attended at a non-governmental organization in Fortaleza/CE, Brazil. We used interview techniques, physical examination, observation and information records, with a structured instrument, addressing requisites related to universal self-care, development and health alterations. Self-care deficits corresponded to nineteen nursing diagnoses, named according to NANDA's Taxonomy II, ten of which were based on the requisites for universal self-care, five on the requisites for self-care related to development and four on the requisites for self-care related to health deviations. In care planning, goals were established and the system and health methods were selected, prioritizing support-education actions in order to engage HIV/aids patients in self-care.

Background: Nursing interventions for persons affected by long-term conditions should focus on providing support to enhance the ability to manage disease in everyday life. Many clinical nurses feel they have inadequate training or experience to provide self-management support in a beneficial and structured way. This study explores the process towards independent self-care and management of disease in persons affected by Parkinson’s disease and the support required from healthcare to achieve this. It presents a nursing model to guide nurses in providing self-management support in the clinical care encounter. Methods: The results from three previously published articles investigating a self-management support program for persons with Parkinson’s disease were combined to form a new data set, and analyzed using qualitative thematic analysis. Results: Three separate, but interrelated, themes were identified, which described the process towards self-management of disease as expressed by the participants of the self-management program. Themes describe the factors important for developing and improving self-management abilities and actions. The results were applied to Orem’s Self-care deficit theory to suggest a model of self-management support in the clinical nursing encounter. Conclusion: This study investigated factors important for self-management and highlighted the unique contribution and focus of nursing support to promote independent self-care.

Introduction Many researchers have commended the self-care deficit nursing theory (SCDNT) developed by Orem as a means of improving patients' health outcomes through nurses' contributions. However, experimental research has investigated specific aspects of SCDNT, such as self-care agency and self-care requisites, rather than how the construct is practiced and understood as a whole. The current research presents a case study in which an advanced practice nurse (APN) used SCDNT-led practice within a primary healthcare setting that illustrates how the theory is applied to case management. Methods A case study was conducted by observing an APN during her work in the asthma clinic of a public hospital in Hong Kong. A comparison was made between the case management of the APN under observation with the nursing processes stipulated by the SCDNT across four key operations: diagnostic, prescriptive, treatment or regulatory, and case management. Conclusion During the observed consultation, the APN applied the four key operations. In SCDNT, the role of the APN is to apply practical nursing knowledge by determining how a patient can best undertake self-care within the circumstances of their living arrangements and support facilities. The case study also demonstrated that SCDNT-based nursing practice has strengths and limitations in a primary healthcare setting. The study concluded that Orem's SCDNT serves as an appropriate theoretical framework for nursing practice within primary healthcare settings. One practical consequence of using SCDNT is that it enables APNs to use nurse-sensitive indicators when evaluating their clinical practice. This study offers a practice update to increase the accountability of nursing practice for nurse-led healthcare services.

Background and PurposeAccording to Orem's self-care deficit theory, when patients cannot meet their care needs, they need nursing systems for maintaining their health. Nursing care for elderly patients with rheumatoid arthritis (RA) should be based on maintaining self-care. This study aims to determine the effects of Orem's self-care model of nursing care given to geriatric patients with RA on hand symptoms, life activities, and hand pain.MethodsThe study sample comprised a total of 22 patients (intervention group, 11; control group, 11) who met the sample selection criteria at a rheumatology outpatient clinic of a university hospital between June 17, 2019 and September 20, 2019. All interviews with patients in the intervention group were conducted by daily phone calls and a face-to-face interview at the hospital every 4 weeks. Patients continued to receive routine prescription by a physician during the course of application.ResultsNo difference was observed between the groups in terms of descriptive patient characteristics (p > .05). Hand pain, hand symptoms, and life activities of patients in the intervention group were measured at study initiation, week 4, and week 8. Intragroup comparison revealed that hand pain, hand symptoms, and life activity scores were lowest at week 8 in the intervention group (p < .05).Implications for PracticeThis study indicates that nursing care given according to Orem's self-care model is effective in reducing pain, improving hand functions, and performing life activities.

Problem: A review of the literature indicated that human trafficking is a worldwide problem and not all nurses in the healthcare setting were adequately trained to identify and care for human trafficking victims.Purpose: The purpose of this study was to further explore nurses’ awareness regarding human trafficking.Method: This quantitative, cross-sectional, non-experimental study with a descriptive design, studied the issue by administering a survey to nurses actively working in the field. The theoretical framework of forced labor and Orem’s self-care deficit nursing theory were used to guide the study. The data were collected using SurveyMonkey®, an Internet-based confidential data-collection tool. The survey contained eight demographic questions and sixteen Likert-type statements. The researchers obtained personal email addresses and used Facebook contacts to send out the survey initially. In this study, a Snowball sampling was used to obtain nurse participants.Results: A total of 166 responses were received from nurses across the United States. The results confirmed that there is a lack of awareness among nurses about human trafficking. An increase in both training and education amongst nurses could increase the number of victims, which go unnoticed, being saved or provided with assistance. Nurses also must be more self-aware about the patients they are encountering and be more observant of signs that look suspicious. The most agreed upon statement by participants was that counseling should be available to people who are affected by human trafficking (M = 3.75, SD = 0.48).

If Yoga practitioners who have MS share their experiences,a deeper understanding of their lives and how Yoga affects those lives may be gained. Study Design: Using case-study research methods,two women were interviewed to gain an understanding of their experiences of practicing Yoga and having MS. The theoretical framework for this study is based on Dorothea Orem's Self-Care Deficit Nursing Theory. Findings: The findings showed that participants had affirming life philosophies, strong motivators, an ongoing development of body awareness, and the ability to think and act outside the norm. Conclusions: This information could impact the development of meaningful client-driven information. Future research should include larger and more diverse populations, quantitative study, and in-depth qualitative evaluation.

O objetivo deste estudo foi implementar o Processo de Enfermagem de Orem e os Métodos de Ajuda para se evitar o déficit de autocuidado aos clientes portadores de colostomia. Trata-se de uma pesquisa bibliográfica, exploratória e descritiva em que foi realizada uma revisão da literatura sobre o tema proposto no período de 2000 a 2012. Pela análise de conteúdo, verificou-se que os clientes colostomizados necessitam de apoio psicossocial, promoção, educação em saúde, reabilitação e orientações para o autocuidado. O Processo de Enfermagem de Orem determina claramente os papéis do enfermeiro e do cliente para a obtenção das exigências do autocuidado terapêutico, e os Métodos de Ajuda propostos por Orem ajudam ao enfermeiro a fundamentar “o porquê ele se faz necessário”. Entende-se que o enfermeiro não deve só priorizar a prática curativa e assistencial, mas também procurar exercer atividades educativas e de pesquisa para melhor identificar os problemas advindos da colostomia, valorizando os colostomizados como seres humanos.Descritores: Teoria de Orem, Enfermagem, Paciente Colostomizado. It is a bibliographical, exploratory and descriptive research that was carried out a literature review on the topic proposed for the period 2000 to 2012. For the content analysis, it was found that the colostomy customers need psychosocial support, promotion, education health, rehabilitation and guidelines for self-care. The Orem's nursing process clearly states the roles of the nurse and the client to achieve the requirements of therapeutic self-care, and help methods proposed by Orem help the nurse to give reasons "why it is necessary." It is understood that the nurse should not only prioritize the curative and care practice, but also seek to exercise educational and research activities to better identify the problems arising from colostomy, valuing the colostomy as human beings. Objective: to apply the nursing process fundament on the Theory of Orem’ Self-care, through the conduct of a report of clinical case, with a patient submitted to bariatric surgery. Methods: this is about a descriptive study, from qualitative approach, report of clinical case type, performed in a patient with morbid obesity, included in Bariatric Surgery Group of a teaching hospital in João Pessoa-PB city. For data collection was used a script adapted to Theory of Orem’ Self-care, which led to the identification of deficits of self-care and therefore to nursing diagnoses. It was then developed the plan of nursing care, with the determination of goals, objectives, method of assistance, type of system and nursing interventions. This study has been approved by the Research Ethics Committee of the Hospital of the Federal University of Paraiba (054/07). Results: from the identification of nursing diagnoses was established and implemented the plan of nursing care with the aim of restoring the patient to prevent postoperative complications, promote recovery and prepare you for the self-care. Conclusion: it is considered that the application of the nursing process, based on Theory of Orem’ Self-care, enabled the provision of assistance and qualified individual, encouraging the patient to participate actively in their treatment, but also to increase their responsibility in the outcome of care. Middle-range nursing theories provide a bridge between the more abstract grand nursing theories, research, and clinical practice. As such, middle-range nursing theories derived from extant nursing theories are critical for developing and advancing both nursing science and practice. This paper describes the strategy used in developing a middle-range theory of heart failure self-care. After integrating theoretical and empirical literature, a hypothetico-deductive approach was used to develop the middle-range theory of heart failure self-care from Orem’s theory of self-care. Theoretical substruction was used to provide a graphic representation of the conceptual-theoretical-empirical structure demonstrating the congruence between the theoretical and operational systems.

Nursing in hypertension care comprises counselling about lifestyle changes, blood pressure measurement, and being a translator for the physician. For the patient, changing lifestyle means performing self-care. As not much in the form of research and guidelines for nurses is available, a middle-range theory of nursing in hypertension care was developed to guide nurses in their practice, in order to improve the nursing of patients and design studies for investigating nursing in hypertension care. Concepts are presented related to the patient (attitude and beliefs regarding health and sickness, autonomy, personality and traits, level of perceived vulnerability, hardiness, sense of coherence, locus of control, self-efficacy, and access to social support and network) and the nursing (applying theories and models for behavioural change in the consultation and using counselling skills, patient advocacy, empowerment, professional knowledge and health education, and supporting the patient). Then the concepts related to the consultation (communication, shared decision-making, concordance, coping, adherence, and self-care) are integrated with Orem’s theory of nursing. Clinical and research implications of the theory are discussed.

Chronically ill persons with a condition requiring self-care activities can benefit from learning from reliable internet sources. Orem’s theory of self-care was used to answer the question: Does increasing technical knowledge about reliable internet sites for health information increase self-care agency of low-income persons living with HIV/AIDS ( N = 100)? Self-care agency, as measured by the Self-As-Carer Inventory, increased but not significantly, after two educational interventions. We hoped to gain insight into the measurement of self-care agency in our African American and Hispanic population. The only significantly different basic conditioning factor was ethnicity in that non-Hispanic/Latinos scored higher on self-care agency than Hispanic/Latinos; the only significantly different subscale was the Judgment and Decisions Affecting Production of Self-care subscale. Self-care agency remained stable over the 1-week period, pointing to two possible conclusions: Scores accurately reflect self-care agency, and a more intensive, longer intervention may be needed to change self-care agency.

A middle-range theory of heart failure self-care, derived from the self-care deficit theory of nursing, was tested among 175 Arab American older adults with heart failure. The middle-range theory achieved good statistical fit, but not all hypothesized relationships were supported. Specifically, conceptualizing basic conditioning factors as a single latent variable was not supported. However, individual factors of depression, social support, and time living with heart failure had a direct effect on both self-care agency and quality of life. Understanding predictors and outcomes of self-care within a theoretical framework is essential in caring for patients with heart failure.

Objective: to assess the effect of the preparation plan for hospital discharge on the self-care agency capacity and adherence to treatment in the patients with Diabetes Mellitus treated in the Erasmo Meoz University Hospital (Hospital Universitario Erasmo Meoz, HUEM) during 2019. Materials and methods: a quasi-experimental study, with pre-/post-test assessment in the study group, guided by Dorothea Orem’s theory, with a non-probabilistic sampling of 88 individuals, by convenience. It was conducted by implementing the following instruments: therapeutic conduct, disease or lesion, and scale to assess the self-care agency capacity (ASA); data analysis was performed in Microsoft Excel 2016 and SPSS, version 22.0. Parametric (Student’s t-test for paired samples) and non-parametric (Wilcoxon) statistical tests were also used, considering p-values < 0.05 as statistically significant. Results: the initial self-care agency capacity was 61.48 (low) and the final was 80.43 (fair); therapeutic adherence compliance in the initial assessment was occasionally shown (2.82) and frequently shown at the end (4.38), with both variables showing a statistically significant difference with p-values < 0.05. Conclusions: it was determined that the intervention performed had a positive effect on therapeutic adherence and the self-care agency capacity of the diabetic patients subjected to the intervention.

Objective: to verify the opinion of judges about a nursing consultation tool for people with co-infection by the human immunodeficiency virus and tuberculosis. Methods: methodological study, involving the construction of a tool based on Orem’s self-care theory and its submission for evaluation by judges experts in the area, which gave opinions about organization, clarity and relevance. Results: the instrument gained a positive evaluation for all assessment criteria, with agreement greater than 75% and Content Validity Index greater than 0.80 in most items. Following judges opinion, three items were excluded and two modified. Conclusion: the adequacy of the nursing consultation tool for people with HIV/tuberculosis co-infection was confirmed regarding organization into subdivisions and items. Minimal disagreement among judges in the analysis of the issues was noted. Although most items were evaluated positively, further validation studies will be needed.

Heart failure (HF) is a growing public health problem affecting approximately 23 million people worldwide. Treatment options for advanced HF patients have moved beyond pharmacologic therapy to include left-ventricular assist devices (LVADs). Patients with an LVAD must manage a complex regimen of care. This article proposes the use of self-care deficit nursing theory (SCDNT) as a framework to identify and organize the care needs of patients with an LVAD from a nursing perspective. Within SCDNT, self-care refers to actions designed to meet self-care requisites to achieve regulatory goals. When formalized, self-care requisites have two components: the general action to be taken and a “factor” to be controlled. The reformulation of health-deviation self-care requisites common to LVAD patients are presented and may serve as an exemplar for other technology-assisted living situations. The strengths and limitations of using the SCDNT for patients with such complex needs, as well as implications for clinical practice, research, and advancement of nursing science, are examined.

Introduction: Discharge planning is one of nursing intervention that aim to promote the independence of patient’s self care-activities after discharge from hospital. This study was aimed to examine the effect of discharge planning on wound care independence of post sectio caesarea patient at Melati Room, Dr. Soegiri Hospital, Lamongan. Methodes: Design of this study was Quasy Experiment. The population were post sectio caesarea patient at Melati Room Dr. Soegiri Hospital Lamongan, since May – June 2012. The samples were taken by purposive sampling technique. Fourteen respondents matched with the inclusion criteria and divided into treatment and control groups. The datas were collected by using questionnaire and observation sheet. Then datas were analyzed by using Wilcoxon Signed Rank Test and Mann Whitney U Test with significance level of α ≤ 0.05. Result and Analysis: The result of this study showed that post sectio caesarea patient’s knowledge had significance level of p=0.027 in treatment group, and in control group was p=0.034, for post sectio caesarea patient’s skill p=0,015 in treatment group and in control group was p=0,017. The result of Mann Whitney U test was p=0,001, it means there was different in knowledge, and skill of wound care on post sectio caesarea patient between treatment and control groups. Discussion and Conclusion: It can be concluded that there are significant effect of wound care independence of post sectio caesarea patient with discharge planning approach based on orem’s self care theory to improve patient’s independence. Hospital need to develop discharge planning procedure to meet the needs of patient with post sectio caesarea wound and decrease the number of surgical wound infection.

This study aimed to investigate the experience of patients’ success in facing a post-stroke UI. Methods A qualitative study using the Rapid Assessment Procedure (RAP) approach was used in this study. Informants were selected using purposive sampling. In-depth interviews with as many as 8 patients who had recovered from post-stroke UI and living in the greater area of Southeast Sulawesi (Indonesia) were conducted. In-depth interviews were also conducted with 8 caregivers and 2 nurses. Data were analyzed using a thematic analysis approach and interpretation of data was based on Humanbecoming theory and Self-care deficit theory of nursing. Results Five successful things the patients experienced during post-stroke UI were identified. The five successes were as follows: they provided information to get to know and understand post-stroke UI, followed the procedures to overcome post-stroke UI, conducted self-control exercises and stayed motivated, performed daily activities independently according to ability, and made use of family support and peers’ attention. Conclusions These findings indicated that persistence, belief, independence, and social support (family and peer) made patients to successfully face their post-stroke UI and improved their quality of life. These findings also became the basis for developing a post-stroke UI management model based on Humanbecoming theory and Self-care deficit theory of nursing.

IntroductionLiver cirrhosis affects health-related quality of life (HRQoL) even in its early stages. Morbidity is especially high when the disease decompensates and self-care actions become essential. Nurse involvement in secondary prevention in other chronic diseases has contributed to better symptom control, less need of inpatient care and improved HRQoL. In order to evaluate the impact of nurse involvement in the follow-up of patients with liver cirrhosis, we decided to compare structured nurse-led clinics, inspired by Dorothea Orem’s nursing theory and motivational strategies, with a group of patients receiving standard care. The primary outcome is HRQoL and the secondary outcomes are quality of care, visits to outpatient clinics or hospitals, disease progress and health literacy.Methods and analysisThis is a pragmatic, multicentre randomised controlled study conducted at six Swedish hepatology departments. Eligible patients are adults with diagnosed cirrhosis of the liver (n=500). Participants are randomised into either an intervention with nurse-led follow-up group or into a standard of care group. Recruitment started in November 2016 and is expected to proceed until 2020. Primary outcomes are physical and mental HRQoL measured by RAND-36 at enrolment, after 1 and 2 years.Ethics and disseminationThe study is ethically approved by the Regional Ethical Review Board in Uppsala.
