Abstract:
ABSTRACT Objective: To identify self-care demands of men with sickle cell disease and priapism and describe self-care measures in light of Orem's Self-Care Theory. Method: This is a descriptive exploratory study with qualitative approach conducted with nine men with a history of sickle cell disease and priapism. Data were analyzed using Orem's Self-Care Theory. Results: Some demands were identified: from universal self-care - difficulty in social interaction and solitude, changes in self-image, self-esteem and sexual activity; from development - the experience with priapism and little knowledge about the pathophysiology of the disease; regarding health deviations - pain crises. Conclusion: Orem's theory allowed to identify self-care demands, which are essential for the nursing care provided for men with priapism. Nursing has an essential role in the measures for the different demands presented.

ABSTRACT Objective: Classify the diagnoses in the conceptual framework of vulnerability of Ayres and in the Orem's self-care theory; Elaborate operational definitions of nursing diagnoses for elderly women vulnerable to HIV/AIDS. Method: A descriptive exploratory study, developed from March to December 2016 in the stages: 1. Classification of diagnoses in the conceptual framework of vulnerability of Ayres and in the Orem's self-care theory; 2. Operational definition of nursing diagnoses. Results: 70 nursing diagnoses were classified in the conceptual framework of vulnerability of Ayres and Orem's self-care theory, and their operational definitions were constructed, where 75.7% of these were validated. Final consideration: Diagnoses represent conditions that make older women vulnerable to HIV/AIDS and are linked to their self-care practices. Operational definitions contribute to a systematic approach to care and greater clarity in its implementation.

Abstract:
The objective was to implant the Nursing Consultation for hypertensive patients, using Orem's Self-Care Theory and to systematize nursing care. Thirty-six patients were interviewed. Of these, 58.9% were women, 75.0% of them ranging from 50 to 80 years of age, 76.4% were married, 42.9% were housewives, 47.2% were retired, and 67.3% completed elementary education. Patients were interviewed with a structured instrument addressing the requirements for universal, developmental, and health deviations self-care. Data analysis made it possible to assess the altered self-care requirements. In patient care planning, support-education actions were our top priority. Orem's self-care theory permitted identifying important aspects, which need to be developed by the nurses.

OBJECTIVE: To apply the "Orem's Self-care Theory" in ostomy patient care. METHODS: This is a clinical study with an intestinal ostomy patient, whose illness is a consequence of the "Chagas disease". Data collection was performed during the home visits, using a form with questions referred to the Orem's presuppositions. The data were analyzed and presented according to the determining factors in self-care development. RESULTS: Some self-care requirements were modified, such as: "Balance between loneliness and social interaction" and "Self-care in health disorders". The home care delivered based on the supporteducation system allowed for the promotion of health and the patient's perception regarding the importance of self-care. CONCLUSION: The self-care theory allowed for appropriate care and therapeutic communication adjusted to the patient's situation.

Abstract:
Self-care is an area that aims to develop habits, knowledge and attitudes in helping one self. Children of the typical population adopt these skills by observing the activities of their parents or caregivers. In children with a mild cognitive impairment, the situation is different. Within this population, there are several developmental obstacles that cause delays in the adoption of self-care activities. The delayed development of imitation skills, low quality of ability for receptive speech, and frequent fluctuations in attention make it difficult to perform tasks in this field. However, research shows that, with adequate support, children with a mild cognitive impairment mainly develop self-care skills at the operational level. This paper describes the developmental specifics of children with a mild cognitive impairment in self-care, as well as adequate techniques for supporting these skills. Relying on Dorothea Orem's Self-Care Theory (Orem, 1991), a system of educational support for self-care skills is proposed. This system is used when a child has abilities to master these skills but requires minimal support. The methodological approach in Orem's Self-Care Theory has three main components: assessment of self-care skills, process schemes as a learning plan for the skills of self-care and implementation of the implemented. Adequate techniques used in learning self-care activities in children with a mild cognitive impairment are described in the paper by definition, with the given operationalization. These are: Task analysis, Creating a visual guide, Verbal and gesture stimulation, “Links in the chain”, Social stories, Video modeling and self-modeling and using mobile applications.

Background and PurposeAccording to Orem's self-care deficit theory, when patients cannot meet their care needs, they need nursing systems for maintaining their health. Nursing care for elderly patients with rheumatoid arthritis (RA) should be based on maintaining self-care. This study aims to determine the effects of Orem's self-care model of nursing care given to geriatric patients with RA on hand symptoms, life activities, and hand pain.MethodsThe study sample comprised a total of 22 patients (intervention group, 11; control group, 11) who met the sample selection criteria at a rheumatology outpatient clinic of a university hospital between June 17, 2019 and September 20, 2019. All interviews with patients in the intervention group were conducted by daily phone calls and a face-to-face interview at the hospital every 4 weeks. Patients continued to receive routine prescription by a physician during the course of application.ResultsNo difference was observed between the groups in terms of descriptive patient characteristics (p > .05). Hand pain, hand symptoms, and life activities of patients in the intervention group were measured at study initiation, week 4, and week 8. Intragroup comparison revealed that hand pain, hand symptoms, and life activity scores were lowest at week 8 in the intervention group (p < .05).Implications for PracticeThis study indicates that nursing care given according to Orem's self-care model is effective in reducing pain, improving hand functions, and performing life activities.

This research aimed at systematizing nursing care to HIV/aids patients in view of Orem's Self-care Deficit Nursing Theory, using the convergent-care method and the Self-Care Nursing Process. Subjects were thirteen HIV/AIDS patients attended at a non-governmental organization in Fortaleza/CE, Brazil. We used interview techniques, physical examination, observation and information records, with a structured instrument, addressing requisites related to universal self-care, development and health alterations. Self-care deficits corresponded to nineteen nursing diagnoses, named according to NANDA's Taxonomy II, ten of which were based on the requisites for universal self-care, five on the requisites for self-care related to development and four on the requisites for self-care related to health deviations. In care planning, goals were established and the system and health methods were selected, prioritizing support-education actions in order to engage HIV/aids patients in self-care.

Introduction Many researchers have commended the self-care deficit nursing theory (SCDNT) developed by Orem as a means of improving patients' health outcomes through nurses' contributions. However, experimental research has investigated specific aspects of SCDNT, such as self-care agency and self-care requisites, rather than how the construct is practiced and understood as a whole. The current research presents a case study in which an advanced practice nurse (APN) used SCDNT-led practice within a primary healthcare setting that illustrates how the theory is applied to case management. Methods A case study was conducted by observing an APN during her work in the asthma clinic of a public hospital in Hong Kong. A comparison was made between the case management of the APN under observation with the nursing processes stipulated by the SCDNT across four key operations: diagnostic, prescriptive, treatment or regulatory, and case management. Conclusion During the observed consultation, the APN applied the four key operations. In SCDNT, the role of the APN is to apply practical nursing knowledge by determining how a patient can best undertake self-care within the circumstances of their living arrangements and support facilities. The case study also demonstrated that SCDNT-based nursing practice has strengths and limitations in a primary healthcare setting. The study concluded that Orem's SCDNT serves as an appropriate theoretical framework for nursing practice within primary healthcare settings. One practical consequence of using SCDNT is that it enables APNs to use nurse-sensitive indicators when evaluating their clinical practice. This study offers a practice update to increase the accountability of nursing practice for nurse-led healthcare services.

Objetivos: describir la relación de la actividad física (pasos/día)/ejercicio con la obesidad (índice de masa corporal, circunferencia de cintura y grasa corporal) en adultos con diabetes tipo 2. Se planteó además: 1) determinar la relación de la educación/comprensión en diabetes con la actividad física/ejercicio, y 2) establecer diferencias de actividad física/ejercicio y obesidad/sobrepeso de acuerdo con variables sociodemográficas. Materiales y métodos: diseño descriptivo correlacional, muestra aleatoria de 124 pacientes de 17 centros comunitarios de Tampico, Tamaulipas, México. Las mediciones incluyeron: podómetro New-Life Style 2000, cuestionarios de autocuidado y Perfil de Cuidado en Diabetes, IMC, CC y grasa corporal. Los datos se analizaron con medidas de tendencia central, correlación de Spearman, prueba U de Mann-Whitney y Kruskal Wallis. Resultados: el promedio de pasos/día mostró relación inversa con el IMC (rs = - 0,282, p < 0,01) y la CC (rs = - 0,300, p < 0,01). La comprensión en diabetes tuvo relación positiva con el ejercicio (rs = 0,179, p = 0,04). Las mujeres fueron quienes tuvieron mayor porcentaje de grasa corporal (p < 0,05). Conclusiones: los hallazgos tienen implicaciones importantes para el ejercicio práctico de la profesión de enfermería que juega un papel esencial en el fomento de la actividad física y otros comportamientos básicos de autocuidado a través de la educación. La educación basada en teoría se hace necesaria para el avance en el conocimiento de la disciplina.

The way to improve mother's independence is by the influence of a leader (health worker). Transformational leadership is a leadership style that can motivate subordinates to be able to do better, in this case can be applied in improving the independence of postpartum mothers. Objective: Provide an overview of how to improve self-care of postpartum mothers through transformational leadership. Method: The method used in the search process using the PubMeds website, Nursing Journal Bina Husada, Diponegoro Nursing Journal. Search keywords are independence, postpartum mothers, self care. Result: Idealized / Charismatic Influence: the influence of a leader is needed to create an ideal influence, the experience of childbirth supports the ability of postpartum mothers to perform postpartum care. Inspirational Motivation: motivation and inspiration that can provide encouragement, independent family development affects the level of mother's independence. Individualized Consideration: a nurse must be able to respect the patient's background and environment in order to create a harmonious relationship and increase the patient's confidence. Intellectual Stimulation: transformational leadership style that encourages subordinates to solve problems carefully, the higher the level of knowledge the easier it is to receive information. AbstractObjectivePain is a prevalent and distressing symptom in patients with cancer which has a significant impact on patients’ physical health, psychological well-being, and quality of life (QOL). Deficiency of pain care, inadequate pain communication, and reluctance toward pain medication contribute to the difficulties in improving the situation of patients with cancer pain. Fortunately, the integration of self-management and professional healthcare provides new opportunities for patients with cancer pain.MethodsA two-arm randomized controlled trial will be conducted to assess the effectiveness of self-management application compared with routine care among patients with cancer pain. Based on Orem's self-care theory and Naylor's transitional care theory, cancer pain self-management application “Pain-housekeeper” was developed with dual client: patient client and medical client. A total of 120 patients with cancer pain are recruited from two university-affiliated tertiary public hospitals in China. They are randomly assigned to the control group and the intervention group. The patients in the control group get routine care while the patients in the intervention group get routine care and Pain-housekeeper App. The intervention is delivered by registered doctors, nurses, and postgraduate nursing students specialized in pain care. Effect measurements for both groups are carried out with questionnaires at baseline (T0), after 4 weeks (T1) and after 12 weeks (T2). The primary outcomes include pain condition, self-efficacy, and QOL. The secondary outcomes include pain medication adherence, satisfaction, anxiety, and depression. Compliance of data submission, economic evaluation, usage, and clinical data are also considered in the test. Data for the evaluation are gathered continuously during the study period.DiscussionThis article provides insight into the effectiveness of a theory-based self-management mobile application intervention delivered to patients with uncontrolled cancer pain. There is no suitable instrument available for assessing self-management needs of adult patients with knee OA in Taiwan. This study developed an instrument that could assess the self-management needs of knee OA patients using Orem's self-care theory as a theoretical framework.AimsThis study developed and psychometrically tested a new instrument for measuring adult patients’ self-management needs of knee OA (SMNKOA).MethodsDevelopment of the instrument involved three phases: item generation and scale development; content and face validity of the initial instrument; and evaluation of validity and reliability of the new instrument. Participants (n = 372) were purposively sampled from orthopaedic clinics at medical centres in Taiwan.ResultsThe self-care theory guided the development of the 35-item SMNKOA scale. The content validity index was 0.83. Principal components analysis identified a 3-factor solution, accounting for 53.19% of the variance. The divergent validity was –0.67; convergent validity was –0.51. Cronbach's α was 0.95, Pearson correlation coefficient was 0.88, and the intraclass correlation coefficient was 0.95.ConclusionsThe SMNKOA scale can measure and identify the individual self-management needs of knee OA patients. Purpose: To examine the lives and experiences of people with Multiple Sclerosis (MS) who practice Yoga. If Yoga practitioners who have MS share their experiences,a deeper understanding of their lives and how Yoga affects those lives may be gained. Study Design: Using case-study research methods,two women were interviewed to gain an understanding of their experiences of practicing Yoga and having MS. The theoretical framework for this study is based on Dorothea Orem's Self-Care Deficit Nursing Theory. Findings: The findings showed that participants had affirming life philosophies, strong motivators, an ongoing development of body awareness, and the ability to think and act outside the norm. Conclusions: This information could impact the development of meaningful client-driven information. Future research should include larger and more diverse populations, quantitative study, and in-depth qualitative evaluation.

Salah satu penyebab kegagalan menyusui pada ibu antara lain kurangnya kepercayaan diri pada ibu. Relaksasi autogenic training (RAT) merupakan metode untuk meningkatkan rasa percaya diri dan membangun pikiran positif ibu. Hal ini sesuai dengan teori keperawatan self-care dari Orem yang bertujuan membantu ibu mencapai kemandirian untuk mempertahankan hidup, kesehatan, perkembangan, dan kesejahteraan. Tujuan penelitian: Penelitian ini bertujuan untuk mengetahui efek RAT terhadap keberhasilan menyusui pada ibu postpartum. Metode: Desain penelitian ini menggunakan quasi experiment dengan rancangan nonequivalent control group design. Sampel terdiri dari masing-masing 15 responden untuk kelompok intervensi dan kelompok kontrol. RAT diberikan melalui media audio voice selama 10-15 menit sebanyak 2-3 kali sehari selama 7 hari. Posttest dilakukan dengan cara observasi melalui kunjungan pada hari ketujuh. Instrumen Via Christi Breastfeeding Assessment Tool yang telah valid dan reliabel digunakan untuk menilai keberhasilan menyusui. Analisis data menggunakan t-test. Hasil: ibu yang melakukan RAT bisa menyusui lebih efektif dibanding kelompok kontrol (p=0,000). Diskusi: RAT memberikan ketenangan dan kemudahan kepada responden dalam membantu meningkatkan keberhasilan menyusui karena memiliki efek relaksasi. Kesimpulan: RAT efektif terhadap keberhasilan menyusui. Oleh karena itu RAT dapat digunakan oleh perawat sebagai sebuah asuhan keperawatan dalam program dukungan ibu menyusui di rumah sakit atau fasilitas kesehatan.Kata Kunci: autogenic training, menyusui, relaksasi Relaxation Effects of Autogenic Training on Lactation Success in Post Partum Mothers ABSTRACTOne of the causes of failure to breastfeed, among others, is a lack of confidence in mothers. Autogenic relaxation training (RAT) is a method to increase self-confidence and build positive thoughts for mothers. This is in accordance with Orem's self-care nursing theory, which aims to help mothers achieve independence to maintain life, health, development, and well-being. Objective: This research aims to reveal the effect of RAT on breastfeeding success in postpartum mothers. Methods: This research employed a quasi-experimental design with a nonequivalent control group design. The samples consisted of an intervention group and a control group, with 15 respondents for each. RAT was given through audio voice media for 10-15 minutes 2-3 times a day for 7 days. The posttest was conducted using observation through visits on the seventh day. A valid and reliable instrument of the Via Christi Breastfeeding Assessment Tool was used to assess breastfeeding success. Data were analyzed using a t-test. Results: Mothers who did RAT could breastfeed more effectively than the control group (p=0.000). Discussion: RAT provides comfort and convenience to respondents in helping to increase breastfeeding success because it has a relaxing effect. Conclusion: RAT is effective for breastfeeding success. Therefore, nurses can use RAT as nursing care in breastfeeding mother support programs in hospitals or health facilities.Keywords: autogenic training, breastfeeding, relaxation

ABSTRACTObjective: to understand the meanings of ostomized patients regarding stoma, as well as the factors involved in self-care based on the assumptions of the Orem Theory. Method: it is a qualitative, descriptive study, with phenomenological analysis. The collection was done through the semi-structured interview, with the participation of five people who have a colostomy bag. Results: it is revealed that, from the analysis of the interviews, the following categories emerged: Embarrassment/Social Isolation; Faith and Resignification. It is observed that the stoma carriers present difficulties even though they have adapted, mainly in the aesthetic aspects and the insecurity caused in some by the fear of leaks, flatulence and of causing annoyances in the people around them. It was found that most of the ostomized patients did not meet three of Orem's assumptions: human functioning, the dangers of life and well-being, and potential developments. Conclusion: participants demonstrated a still repressed demand for information, listening and individualized and holistic care. AbstractNursing theories have been developed to provide guidance in clinical practice, so their knowledge by nurses is mandatory in order to provide advanced nursing care. This paper presents the relationships between the concepts and major assumptions of Henderson’s model and Orem’s theory and then their comparative analysis. Both, Henderson’s model and Orem’s theory of self-care deficit have a major impact, internationally, in education and research, but also in nursing practice, as a result of which their knowledge can open a new perspective on care, especially in countries where they are not well enough known.

Abstract:
The purposes of this study were to evaluate the psychometric properties of English and Spanish instruments that measure the nutrition behavior and practices of children and their parents. Orem’s self-care deficit nursing theory was used in this methodological study. A convenience sample of 333 children and 262 mothers participated from two schools in Washington, D.C. and two schools in Santiago, Chile. Principal component analysis indicated three component per instrument corresponding to Orem’s Theory of operations demonstrating construct validity of the instrument. The study findings showed evidence for validity and reliability of the English and Spanish versions and indicated that the instruments appropriately represented Orem’s operations. The results have implications for the development of health behavior measurement instruments that are valid, reliable, designed for children, culturally appropriate, and efficient. Measuring the nutrition behavior of children and parents is critical for determining the effectiveness of nutrition intervention programs. Furthermore, instruments are needed so that researchers can compare corresponding child and parent behaviors or compare behaviors across cultures.
