Abstract:
Finding a good way to organize activities in a public sector organization can be crucial for the organization’s possibility to reach defined goals. The procurement function in the public sector can be challenging to organize, based on the fact that it is both important for the organization in question, but also an important tool for the government. Overall goals are defined nationally, in the form of national law, regulations, and policy. Each organization normally has a procurement policy document, guiding procurement decisions. In this chapter, we take a closer look at the organization of the procurement function, based on data from the 2018-procurement maturity assessment survey by Difi, as well as international research on the topic. We do not have data that provides a basis for drawing conclusions about the reasons for choosing the specific form of organizing the procurement function throughout the Norwegian public sector, therefore we have chosen to set up a discussion that can facilitate an increased degree of reflection where the structure of the procurement function is up for debate.

Brukermedvirkning er i dag et sentralt prinsipp i helse- og velferdssektoren. Noen ganger er tjenestene imidlertid organisert slik at dette er vanskelig å ivareta. I dette kapitlet ses det nærmere på hvordan organisering av beslutningsprosessene kan skape utfordringer for brukermedvirkningen. Det diskuteres hvordan brukermedvirkning kan forstås og analyseres. Ved hjelp av blant annet beslutningsteori og eksempler fra forskning trekkes det frem en del organisatoriske utfordringer for brukermedvirkning og hvordan disse kan løses. Kapitlet avsluttes derfor med en oppfordring til å være bevisst på at brukermedvirkningen må sikres ved at tjenesteyterne aktivt engasjerer seg for å legge til rette for at brukerne kan delta i beslutningsprosessene og fremme sine interesser.

Helsevesenet har alltid hatt pasienten som mål. Idealene og mulighetene for organisering av helsearbeid har imidlertid endret seg gjennom historien. I vår samtid er «pasienten i sentrum» og «digitalisering» to fremtredende prinsipper for organisering av tjenesten. Begrepene knyttes til kvalitetsforbedringsarbeid innen helsesektoren. For å kunne gi mer god helsehjelp til flest mulig pasienter jobber både fagmiljøene, politikere, byråkrater og forskere for å realisere endringer knyttet til pasientsentrering og bruk av IKT. På et diskursivt nivå er alle enige om at både pasientsentrering og digitalisering er ønskede endringer. Men drar disse trendene alltid i samme retning når de skal oversettes til praktisk sykepleierarbeid? Dette kapitlet handler om spenningen mellom ideen om å sette den individuelle pasienten i sentrum på den ene siden, og idealet om objektive standarder for digital dokumentasjon på den andre. Hvilken fortelling om pasienten velger vi å legge til grunn når vi setter han eller henne i sentrum, og hvordan gjør vi det best mulig? Med utgangspunkt i gjennomgang av litteratur på feltet åpner kapitlet for en diskusjon og refleksjon rundt de faglige og praktiske utfordringene sykepleiere står overfor når de på en og samme tid forventes å sentrere sin aktivitet rundt pasienten, dokumentere digitalt, og bygge egen profesjonskunnskap.

Koordinatorordninger i helsetjenesten er temaet for dette kapittelet, der vi undersøker både bakgrunnen for og spesielle utfordringer ved denne typen samordningstiltak. Det teoretiske utgangspunktet er Luhmanns systemteori og en forståelse av helsetjenesten som funksjonelt differensiert. Samordning handler her om å kunne få til kobling mellom ulike delsystemer. Vi ser samhandlingsreformen som et tidsskille, og har gjennomført en dokument­analyse av nasjonale styringsdokumenter i perioden 2008–2018. Gjennomgangen avdekket 14 ulike koordinatorordninger, hvorav åtte var spesifikt rettet mot pasienter med sammensatte behov. Problembeskrivelsene i styringsdokumentene og trekk ved utformingen av koordinatorordningene danner grunnlag for å identifisere tre mulige paradokser. Det første handler om at koordinatorordningene – gjennom å være opptatt av å håndtere grenser mellom delsystemer – kan bidra til å bevare den funksjonelle differensieringen heller enn å redusere den. Det andre viser til at selve organiseringen av koordinatorordningene kan forsterke den funksjonelle differensieringen, mens det tredje synliggjør at ordningene i seg selv bidrar til økt kompleksitet, ved at det oppstår behov for å koordinere koordinatorene.

Abstract:
By studying how primary care providers organize their services for people with concurrent substance abuse and mental health problems (dual diagnosis), this study aims to investigate formal and informal integration mechanisms. The study has an explorative approach based on interviews with people with dual diagnosis, managers, and front-line professionals in a Norwegian municipality of medium size. We found poor formal structures for internal coordination between services for mental health care and services for substance abuse, although they were organised in the same unit. Further, there were challenges in cooperation between this unit and the other healthcare units. Front-line professionals seemed to compensate for poor formal organisational structures with individual, informal coordination arrangements. Drawing on organisational theory, in particular the dilemmas of street-level bureaucrats and the role they play in policy implementation, this chapter discusses the importance and limitations of informal coordination measures for people with dual diagnosis.

Abstract:
The chapter studies one municipality in the process of implementing the Mental Health Action Plan over a 12-year period. The research questions are whether implementation compelled the municipality to change its service delivery organisation, whether management and management roles changed, and if so, how? The data is 21 individual interviews with managers and professionals in the municipality. The chapter shows how the municipality established a mental health service integrated into the unit for home care services for elderly people, yet management at different levels did not include tasks aimed at enabling the services to cooperate, and this created conflict in the relations between them. Implementation of NPM made the conflict worse. More management positions were established in order to facilitate more tasks. The new positions, however, focussed on internal organisational processes, and the conflict was not resolved until the mental health unit was separated from the home care service unit, thus creating two independent organisations that could cooperate in a quasi-market – the operative tool being money.

Abstract:
This chapter explores ways in which work and other activities contribute meaning in the everyday lives of people with co-occurring challenges. There is still insufficient knowledge about the importance of work and activity among people with co-occurring challenges. This is evident in the Norwegian context, where people with co-occurring challenges are marginally represented in the workforce and other realms of activity. The conceptual framework in this chapter is based on symbolic interactionism, which implies meaning as a formative process. The methodological design is a bottom-up perspective, and the empirical sample consists of qualitative interviews with 13 women and men with challenges and/or co-occurring challenges. The findings indicate that work and activity have a significant impact, providing structure in their everyday lives and benefiting others. Work and activity contribute to affiliation in society and the local community. The conclusion indicates that emphasis and attention on work and activity as basic needs for people with co-occurring challenges is required: work and activity must become a central part of comprehensive services aimed at people with co-occurring challenges.

Abstract:
The purpose of this book has been to show how the challenges associated with services for people with complex needs, exemplified here by addiction and mental health disorders, are handled at the local level. The starting point was three characteristics of trends in national policy and organization of services for people with complex needs: decentralization, user participation and coordination. We argue that these characteristics represent conditions within the services for such disorders and also provide a framework for development within them. The characteristics are understood as entangled processes that constitute elements in the emergence of a “new paradigm” in the services, what is described as a focus on “locally based services”. The processes for change at the local level which we have studied show that institutional characteristics, based on different histories and traditions in the work with addiction and mental health disorders, are important for understanding how these change processes are organized. The three municipalities we studied all recognize that addiction and mental health disorders and illness are important societal problems that must be addressed locally. We can also state, however, that local strategies for achieving the goal of better coordination and integrated services evolve in very different ways.

In this chapter, we take a closer look at experiences using individual plans and the role of the coordinator responsible for those participating in such plans. The starting point is that only a small number of service recipients with dual diagnoses have individual plans, while more than half of them have an appointed coordinator. This raises several questions. One is whether individual plans are a tool for service recipients or for service providers; another question is how coordinators can best assist service recipients with their daily challenges. The chapter concludes that the best help for service recipients with dual diagnoses is to have a coordinator who works as a “boundary spanner”.

Collaboration has been emphasized as a solution for many complex problems, in both political agendas and research literature. Public reforms aim at coordination and integration of services for people with complex health and social challenges, such as addiction and mental health problems. How to organize services in a coordinated and integrated manner is for each local authority to determine. In this chapter, we examine an innovation process initiated by a large city in order to develop integrated services. By using the process approach as a method, this chapter provides in-depth insight into how different actors meet and collaborate on reorganizing the services and the challenges they encounter in their collaboration.
