Full textThe management of craniofacial syndromes is classically ensured by a team of maxillofacial surgeons, specialized surgeons and orthodontists. The integration of an osteopath in a paediatric maxillofacial and plastic surgery department for almost twenty years has extended the field of application of paediatric osteopathy. The creation of the FACE analysis software capable of analysing the child’s face, anonymising it with thermal images, has permitted us to evaluate the facial growth of a cohort of 312 eumorphic children (106 girls, 206 boys) between 1 month and 7 years of age. Secondly, the software analysis enabled us to quantify the results obtained by osteopathic practice and to answer in part our initial question: is it possible to compensate very early for growth deficits in the major maxillofacial syndromes? The quantified reduction of growth deficits in children presenting a syndromic dysmorphosis, induced by osteopathic treatment, seems to us to be of a mechanotransductive character and that consequently this treatment seems to participate in the epigenetic modifications during development. The positive results of this early treatment of these young patients encourage us to continue this type of treatment. Further clinical studies and a new field of fundamental research are still to be conducted to shed light on the clinical elements of our paediatric osteopathic treatment approach

Full textAbstract: Osteopathy is growing in popularity in Quebec. Interprofessional collaboration (IPC) between physicians and osteopaths is a new phenomenon and information about it is scarce. This explanatory sequential mixed-methods study aimed to 1) describe current collaborative practices between physicians and osteopaths involved with pediatric patients and 2) explore enablers of and barriers to the development of IPC. Methods: Postal questionnaires about collaborative practices were first sent to all physicians and osteopaths working with pediatric patients in Quebec. Semi-structured individual interviews were then conducted with a subset of 21 participants. The data collected in the questionnaires were analyzed using descriptive statistics and regression models, while a thematic analysis was carried out for the content of the interviews. Results: A total of 274 physicians (14%) and 297 osteopaths (42%) completed the survey while 10 of those physicians and 11 osteopaths were purposely selected for interviews. Osteopathic referral was positively associated with having a professional relationship (odds ratio [OR] 4.10 (95% confidence interval [CI] 2.12; 7.95), p < 0.001), personal consultation (OR 2.58 (95% CI 1.35; 4.93), p = 0.004), community-based practice (OR 1.89 (95% CI 1.03; 3.47), p = 0.040), and knowledge about role of osteopathy (OR 1.22 (95% CI 1.01; 1.47), p = 0.042). Additional enablers identified by respondents include: positive experiences reported by parents; the osteopath having had pediatric experience or previous training as an allied health practitioner; mutual respect for professional boundaries and complementarity; perceived safety of osteopathy; and parents’ requests for interprofessional communication. The main barriers to IPC include: language; difficulties communicating findings of osteopathic palpation assessment; organizational and legal contexts; uncertainty regarding one another’s roles, lack of interprofessional interactions; and limited scientific evidence. While some collaboration currently exists between physicians and osteopaths, optimizing these relationships and interactions still poses several challenges. In the current context of the pending regulation and standardization of osteopathic training in Quebec, these results could be used to guide efforts to promote productive collaboration and safe patient-oriented care.

Full textConsidering on one hand that nonconventional medicines constitute a very disparate set of practices that can't be fully comprehended by one analysis, and, on the other hand, that manual medicines, massively invested by non-physicians, have ended up taking a very important place in several countries, this study examines the specific place of the etiopaths in the field of health. It intends to demonstrate that these practitioners reveal a vast therapeutic market left vacant by official medicine, composed of relatively benign disorders that are not considered as “public issues”, but which can nevertheless be very disabling from the users' point of view. The first part of this work provides a necessary sociohistorical illumination of the etiopaths population (chapter 1), and specifies the conditions of the investigation and the empirical materials used (chapter 2). The second part, dedicated to the study of the "demand" for an alternative treatment, deals with the concrete work of the practitioner (chapter 3), the recruitment of clients and their methods of consultation (chapter 4), as well as the genesis of a gap between the "needs" of patients and the offer of an official hospitalcentered medicine (chapter 5). The third part, devoted to the analysis of the constitution of the "supply", first describes the social trajectories leading to the profession of etiopath (chapter 6), then the rhetoric of legitimation of practitioners facing the medical profession and other manual therapists (chapter 7)

Full textThe purpose of this research is to study how caregivers in France have evolvedsince the 1950s in the way they touch a patient’s body, especially in the case ofinfants. It is based on a multi-centred field survey of young mothers andpractitioners: traditional healers, osteopaths and physicians.Our society is marked by the steady rise in the process of individuation whichincreasingly forbids us from touching one another, even in the medical environment.To analyse this process, we distinguish two forms of physical manipulation thatrespond to different expectations with regard to distinctly different issues. The first isan intrusive "manipulative" touch, whereby a relationship of domination between thecaregiver and his/her patient is established. The second is an "affective" touchmixing body contact and warmth, implying control of the drives within a civilizationwhich is increasingly governed by a strong libidinal self-control.In the case of baby care, these two facets of touch have evolved in Francefollowing a reverse trajectory: the manipulative touch, long considered essential, hasbeen progressively discredited since the 1970s leading to its condemnation in the1990s. At the same time, the contact touch, following an exactly opposite dynamic,has gone from stigma to valorization.In this context, osteopathy appears to be a way achieving a compromise betweencontradictory societal injunctions: touching without penetrating, being effectivewhile being gentle, acting quickly while taking time to listen to the patients’ claims,getting involved personally while maintaining a professional distance and, above all,adjusting to the individuality of the patient, while taking into account the overallfamily, economic and emotional contexts. The success of this approach highlights aseries of complex and sophisticated popular expectations and, beyond them, a set ofrepresentations regarding of the body and the patient’s identity

Full textAbstract: Background: National and international policies recommend breastfeeding for its benefits on mothers’ and infants’ health. In Quebec, almost nine out of ten mothers initiate breastfeeding with their baby but half of these stop during the first month, due to biomechanical sucking difficulties, and this, despite increasing support from lactation consultants (LC). Osteopaths worldwide work with these babies, but there is little empirical evidence for this type of treatment. The primary objective of this study was to determine the efficacy of an osteopathic treatment coupled with usual lactation consultations on infants’ identified as having biomechanical sucking difficulties. Methods: A single blind randomized controlled trial including 97 mother-infant dyads referred by LC. The dyads were randomized into two groups: osteopathic intervention or usual care. Four measurement times over a 10-day period (pre and post intervention) allowed for the determination of the babies’ ability to latch, the intensity of pain felt by mothers as well as any perceived side effects. Osteopathic lesions were documented in an effort to establish a profile T-tests and Chi-square tests as well as regression models were used to compare groups. Results: Babies having received the osteopathic intervention showed a clinically and statistically significant increase (p<0.001) in their ability to latch compared to the usual care group. Analyses did not show any statistically significant difference between groups with regard to the pain felt by mothers at the different measurement times. However, 3 days post intervention, the mothers in the intervention group perceived a significant decrease in their level of pain (p=0.001). A profile of osteopathic dysfunctions in these babies was generated with 84,5% of them found to have a posterior skull dysfunction. No serious or severe side effects were reported. Conclusion: Osteopathic treatment coupled with lactation consultations seem to be a promising avenue for mother-infant dyads with biomechanical sucking dysfunctions. Further research is needed to optimize osteopathic treatment in terms of targeting babies, timing and duration of interventions and also to better document and standardize areas with osteopathic lesions.

Full textThis thesis focuses on osteopathy teachings in a private specialized institution in France. Following the founding principles of visual anthropology, according to which the filming experience constitutes the heart of the anthropological research, this work is divided in two complementary parts : firstly, four ethnographic films describe different ways of learning osteopathy (theory and practice lessons, semi-professional practice and internships in a maternity ward) ; secondly, the written dissertation, after describing the field study methodology, challenges the common views on how to teach osteopathy. If “educating the apprentice’s hand” is important, teaching him the correct positioning is essential. Learning osteopathy involves the whole body, as far as the execution of the techniques or the osteopathic “savoir-être” (social way of acting) are concerned. Being the one on whom techniques are performed becomes as important as being the one actually performing them. The dual aspect of feeling is acknowledged: skin reception is as important as kinaesthetic perception. Finally, the film study shows how, thanks to new filming strategies discovered throughout the survey, the osteopathy teaching enables a better description of therapeutic touching and a better understanding of osteopathy itself

Full textLa première partie de ce travail a comme objectif fondamental de déterminer la cinématique 3D inter-segmentaire de la colonne cervicale, lors de la rotation axiale maximale de la tête. Les valeurs déterminées pour tous les paramètres cinématiques 3D vont nous servir comme base de données de référence.
Dans la deuxième partie, nous souhaitons contribuer à une meilleure compréhension du rôle des ligaments alaires lors de la rotation axiale. Pour cela, nous avons déterminé la cinématique 3D de l’os occipital par rapport à l’axis. Même s’il est inhabituel d’ignorer l’atlas, il nous a semblé plus juste de considérer les segments osseux qui livrent les principales insertions des ligaments alaires. La biomécanique de ces ligaments dépend des mouvements de l’os occipital par rapport à l’axis.
La troisième partie est orientée vers l’objectif principal de la thèse qui vise à déterminer les amplitudes articulaires inter-segmentaires lors de la position pré-manipulative de la colonne cervicale. Cette étude va nous permettre de comparer les amplitudes atteintes lors de la mise en position pré-manipulative avec celles des mouvements physiologiques.
La dernière partie a pour objectif principal de déterminer la longueur du trajet de l’artère vertébrale en position neutre, et ensuite de comparer la variation de longueur du trajet de l’artère vertébrale entre la position en rotation axiale maximale de la tête d’une part, et la position pré-manipulative d’autre part, donc lors de l’application d’une technique manipulative à composantes multiples telle qu’utilisée régulièrement en ostéopathie.
En comparant la position pré-manipulative à la rotation physiologique, les résultats les plus importants de ce travail sont :(1) les amplitudes segmentaires sont inférieures pour la position pré-manipulative et (2) l’artère vertébrale est moins allongée lors de cette mise en position que lors de la rotation physiologique. Ces deux faits, jusque-là inédits, sont importants en clinique ostéopathique courante et montrent l’importance d’un choix judicieux de la technique manipulative et de son exécution.

Full textBalance disorders, as the fall, remain frequently in the elderly, with adverse consequences on the autonomy. Three randomized trials have been conducted to identify the parameters distinguishing the “fallers autonomous elderly” and to analyze the impact of an osteopathic treatment. The 1st study concerns 33 elderly patients: 15 fallers (68,3 ± 2,7 years) and 18 no-fallers (67,7 ± 2,5years). Three stabilometric parameters and three clinical tests distinguish the two populations (p <0,05). Moreover, the osteopathic treatment improves the evaluated characteristics of fallers elderly (no significant difference). The 2nd study concerns 40 fallers and lombalgic elderly: 20 receiving an osteopathic treatment (69,5 ±3,9 years) and 20 without the osteopathic treatment (69,9 ± 3,4 years). This study identifies that the lumbar mobility is restricted for every fallers elderly and the osteopathic treatment improves the lumbar motion just after the treatment, as well as seven days after it (p ≤ 0,01). The 3rd study concerns 34 elderly patients: 17 fallers (71,3 ± 3,5 years) and 17 no-fallers (71,5 ± 4,2years). Four cinematic parameters distinguish the two populations (p ≤ 0,04). The coefficient ofdetermination R2 and the Gait Variability Index confirm that distinction. The osteopathic treatment improves the evaluated characteristics of fallers elderly (no significant difference)

Full textOsteopathy is no longer considered as an illegal practice of medicine after the Act of the 4th of March 2002. However, following the 2007 decrees, the legislature created a shared title be-tween physicians and non-physician, without creating a health profession. Its controversial nature and its ambiguous regime have put the osteopathy in unique position in the French health landscape. The texts define a title, not a profession. The recommendations of the World Health Organization encourage the recognition of complementary medicine and Europe via its guidelines incites to recognize a pro-regulated profession. In France, the rule is often born of practice and the reality must be faced by the rule. Without forgetting the fact that the patient must remain primarily the engine, the main axis and the benchmark that should animate law-yers, professionals and legislators

Full textThis research theme was the contribution of osteopathy in the Global Development Children with Hyperactivity Disorder and Attention Deficit held in Master in Educational Sciences, specialization in Supervision. The work was divided into three distinct and complementary parts, the first, the Theoretical Framework, we conducted a literature review related to the topic under study which was the basis of the research. In the second part, the Methodology was applied to the data collection technique with questionnaire to all the elements that constituted the study group, observation diagnostic and Osteopathic of Body Adjustement Treatment (TOAC) and an interview the parents of a student and a teacher to a student. In the third part of the Discussion, we made a final assessment of the results and leave notes for the future. We aim to achieve the main objective of this work was to verify changes in cognitive and psychosocial development based on the perspective of the School, Family and Community, in order to contribute to the effective global development of the studied students and demonstrate the added value potential of osteopathy in the treatment of students with Attention Deficit and Hyperactivity Disorder (ADHD). The results showed that the TOAC contributed to the overall development of this four students, with controlled ADHD. We conclude that the intervention TOAC for Students with controlled ADHD, contributed significantly to improving the lives of these children in terms of autonomy, concentration, cognitive abilities, sleep quality, social integration and school, an asset should be considered in the multidisciplinary groups of monitoring and intervention accompanying students with ADHD.
Esta investigação teve como tema “O Contributo da Osteopatia no Desenvolvimento Global de Crianças com Perturbação de Hiperatividade e Défice de Atenção”, realizada no Curso de Mestrado em Ciências da Educação, Especialização em Supervisão Pedagógica. O trabalho foi estruturado em três partes distintas e complementares, na primeira, o Enquadramento Teórico, realizámos uma revisão da literatura referente à temática em estudo que constituiu a fundamentação da Investigação. Na segunda parte, a Metodologia, aplicou-se a técnica de recolha de dados com questionário para todos os elementos que constituíam o grupo de estudo, observação diagnóstico e Tratamento Osteopático de Ajuste do Corpo (TOAC) e uma entrevista aos pais de um aluno e a um professor de um aluno. Na terceira parte, a Discussão, realizámos um balanço conclusivo dos resultados e deixamos notas para o futuro. Visámos alcançar o principal objetivo do presente trabalho, verificar alterações do desenvolvimento cognitivo e psicossociais com base na perspetiva da Escola, da Família e da Comunidade, com o intuito de contribuir para o efetivo desenvolvimento global dos alunos estudados e demonstrar a potencial mais-valia da osteopatia no tratamento de alunos com Perturbação de Hiperativide e Défice de Atenção (PHDA). Os resultados revelaram que o TOAC contribuiu para o desenvolvimento global destes quatro Alunos, com PHDA controlada. Concluímos que a intervenção com TOAC em Alunos com PHDA controlada, contribuiu, significativamente, para a melhoria de vida destas crianças a nível de autonomia, concentração, capacidades cognitivas, qualidade do sono, integração social e escolar, devendo ser considerada uma mais-valia nos grupos multidisciplinares de acompanhamento e intervenção que acompanham os alunos com PHDA.
