Abstract:

Abstract:

Abstract:

"LO72: Implementation of an educational program to improve the cardiac arrest diagnostic accuracy of ambulance communication officers: a concurrent control before-after study." CJEM 19, S1 (May 2017): S52—S53. http://dx.doi.org/10.1017/cem.2017.134.

Abstract:
Introduction: Most ambulance communication officers receive minimal education on agonal breathing, often leading to unrecognized out-of-hospital cardiac arrest (OHCA). We sought to evaluate the impact of an educational program on cardiac arrest recognition, and on bystander CPR and survival rates. Methods: Ambulance communication officers in Ottawa, Canada received additional training on agonal breathing, while the control site (Windsor, Canada) did not. Sites were compared to their pre-study performance (before-after design), and to each other (concurrent control). Trained investigators used a piloted-standardized data collection tool when reviewing the recordings for all potential OHCA cases submitted. OHCA was confirmed using our local OHCA registry, and we requested 9-1-1 recordings for OHCA cases not initially suspected. Two independent investigators reviewed medical records for non-OHCA cases receiving telephone-assisted CPR in Ottawa. We present descriptive and chi-square statistics. Results: There were 988 confirmed and suspected OHCA in the “before” (540 Ottawa; 448 Windsor), and 1,076 in the “after” group (689 Ottawa; 387 Windsor). Characteristics of “after” group OHCA patients were: mean age (68.1 Ottawa, 68.2 Windsor); Male (68.5% Ottawa, 64.8% Windsor); witnessed (45.0% Ottawa, 41.9% Windsor); and initial rhythm VF/VT (Ottawa 28.9, Windsor 22.5%). Before-after comparisons were: for cardiac arrest recognition (from 65.4% to 71.9% in Ottawa p=0.03; from 70.9% to 74.1% in Windsor p=0.37); for bystander CPR rates (from 23.0% to 35.9% in Ottawa p=0.0001; from 28.2% to 39.4% in Windsor p=0.001); and for survival to hospital discharge (from 4.1% to 12.5% in Ottawa p=0.001; from 3.9% to 6.9% in Windsor p=0.03). “After” group comparisons between Ottawa and Windsor (control) were not statistically different, except survival (p=0.02). Agonal breathing was common (25.6% Ottawa, 22.4% Windsor) and present in 18.5% of missed cases (15.8% Ottawa, 22.2% Windsor p=0.27). In Ottawa, 31 patients not in OHCA received chest compressions resulting from telephone-assisted CPR instructions. None suffered injury or adverse effects. Conclusion: While all OHCA outcomes improved over time, the educational intervention significantly improved OHCA recognition in Ottawa, and appeared to mitigate the impact of agonal breathing.

This article follows a previous article in "Bibliotheca Medica Canadiana" describing the original merger of this library. Library Services at The Ottawa Hospital (TOH) was created as the result of a series of hospital mergers. It supports a large teaching hospital serving the city of Ottawa, Ontario, and the surrounding area. TOH was formed in 1999 with the amalgamation of three hospitals: the Ottawa Civic, Ottawa General, and Riverside Hospitals. Before the first merger, there were three separate libraries. Library Services serves primarily staff from TOH and its affiliated research institute, the Ottawa Health Research Institute.

Abstract:

Abstract:
At the University of Ottawa we have a multidisciplinary undergraduate (concentration) program in Medieval Studies housed within the Faculty of Aits. Most of the students take it as a second concentration, most frequently with history, literature, or classics. When we took stock of the program in 1994, it was barely alive, with an enrolment that had been consistently under ten students and no attention given to the program in matters of curriculum or publicity for more than a decade. This situation was not surprising, since all of the university's resources and administrative attention flow through departments and our medieval studies program received only what came its way occasionally and incidentally.

Abstract:
Introduction/Innovation Concept: There is a paucity of peer-reviewed works investigating the History of Emergency Medicine (EM) in Canada, and none examining a single centre. This study analyzed the academic and clinical evolution of EM in the City of Ottawa from its origins to present. Methods: The study comprised primary and secondary historical research and an oral history methodology. A literature review was performed on the following databases: PubMed, Medline, EMBASE, JSTOR, Web of Science, Historical Abstracts; five medical history journals were also searched. Data were collected from City of Ottawa Archives, Archives of the Sisters of Charity of Ottawa, The Ottawa Hospital Libraries, University of Ottawa Libraries, RCPSC and CFPC Archives, Historical Society of Ottawa documents, Ottawa newspaper archives, and professional correspondences. The oral history component consisted of formal interviews with seven practicing and retired Emergency Physicians in Ottawa. Ethics approval was not required though consent was obtained from respondents. Curriculum, Tool, or Material: The literature review yielded the following: PubMed: 218 results, 180 excluded for non-relevance, 3 papers included in analysis. Historical Abstracts: 1 result, overlap with PubMed. Other databases and medical history journals yielded no papers. Along with extensive archival data, these results were used to construct a detailed timeline of EM history in Ottawa and Canada more broadly. Residency training in EM in Ottawa was initiated in 1972 at the impetus of the Board of the Ottawa Civic Hospital. Two main themes recurred in the interviews: resistance from existing specialties to EM becoming a specialty, and early Emergency Rooms staffed by the least trained people treating the least differentiated patients. Early EM physicians were not viewed positively by other specialists. Conclusion: Pioneering EM physicians were forced to validate the specialty as distinct, rigorous, and credible. In Ottawa this was achieved by developing strong core academics and research. Nationally, this has been instrumental in establishing EM as a viable standalone academic specialty. Modern consult pushback may have evolved from existing specialists fighting against the creation of EM combined with their negative perception of EM physicians. These data could be incorporated into learning modules for EM residency academic programs, and the methods applied to other centres.

Abstract:
Telehealth has the potential to be a valuable tool for technical and clinical support of computer controlled prosthetic devices. This pilot study examined the use of Internet-based, desktop video conferencing for remote configuration of the Otto Bock C-Leg. Laboratory tests involved connecting two computers running Microsoft NetMeeting over a local area network (IP protocol). Over 56Kbs−1, DSL/Cable, and 10Mbs−1 LAN speeds, a prosthetist remotely configured a user's C-Leg by using Application Sharing, Live Video, and Live Audio. A similar test between sites in Ottawa and Toronto, Canada was limited by the notebook computer's 28Kbs−1 modem. At the 28Kbs-1 Internet-connection speed, NetMeeting's application sharing feature was not able to update the remote Sliders window fast enough to display peak toe loads and peak knee angles. These results support the use of NetMeeting as an accessible and cost-effective tool for remote C-Leg configuration, provided that sufficient Internet data transfer speed is available.

Abstract:
The Boreal Chorus Frog (Pseudacris maculata) was once common in the eastern Ottawa area. To assess its current status, we conducted auditory surveys at 184 wetlands in 2011 and 2012. Boreal Chorus Frogs were heard at only five (2.7%) of the surveyed sites. These five sites were spatially aggregated, with only 0.5–7.5 km between any two sites. Sites occupied by Boreal Chorus Frogs in eastern Ottawa were surrounded by significantly greater agricultural cover (at 1.0-, 1.5-, and 2.0-km radii), less forest cover (1.0- and 2.0-km radii), and less wetland cover (1.5- and 2.0-km radii) than occupied sites in western Ottawa. Sites in eastern Ottawa that were apparently unoccupied were surrounded by significantly greater agricultural cover (only at the 2.0-km radius), similar forest cover (all radii), and less wetland cover (all radii) compared with occupied sites in western Ottawa. Boreal Chorus Frog populations are commonly subject to extirpation resulting from stochastic events. The reduced wetland cover in eastern Ottawa may be accompanied by reduced wetland connectivity, making recolonization of wetlands difficult or impossible. Our data do not show whether wetland connectivity has been reduced, but future research should address this important topic.

Abstract:
Our aim was to study the role of the Ottawa and Pittsburgh rules to reduce the unnecessary use of radiographs following knee injury. We prospectively reviewed 106 patients who were referred to our clinic over a 3-month period. The Ottawa and Pittsburgh rules were applied to individual patients to evaluate the need for radiography. One hundred and one patients (95%) had radiography of their knee. Five patients (5%) had a fracture of their knee and in all cases, the Ottawa and Pittsburgh knee rules were fulfilled. Using the Ottawa rules, 27 radiographs (25%) could have been avoided without missing a fracture. Using the Pittsburgh rules, 32 radiographs (30%) could have been avoided. The Ottawa and Pittsburgh rules have a high sensitivity for the detection of knee fractures. Their use can aid efficient clinical evaluation without adverse clinical outcome and may reduce healthcare costs.

Abstract:
This paper describes various aspects of the design and construction of a new building for the National Aviation Museum, currently nearing completion on a site on the Rockliffe Airport, Ottawa. The museum will house the aeronautical collection now displayed in World War II hangars. The new museum building is shaped like an isosceles right-angled triangle with a short side 161 m long and a clear height of 13.2 m from the floor to the underside of the roof framing. A two-storey wing on the west side contains the public entrances and the administrative offices. The soil at the site consists of a deep layer of preconsolidated sensitive clay underlain by dense glacial till containing boulders, then a layer of dense sand with gravel and boulders. The main columns of the building are supported by 55 m deep piles driven to suitable resistance in the dense sand and boulder layer, while the perimeter columns rest on spread footings, supported on the surface clay layer. The roof structure, which is one of the largest of its kind, consists of a space truss with top and bottom chords staggered with respect to each other and laid on a square grid 3.3 m by 3.3 m. The depth of the roof framing is also 3.3 m.The paper presents details of subsurface exploration and the types of foundations used. The structural framing for the roof as well as the steps involved in the analysis and design of the roof are described. Also presented are details of the fabrication methods, weld testing, and erection procedures. Key words: National Aviation Museum (Ottawa), pile foundations, dynamic testing, structural steel, space truss, welded joint, ultrasonic weld inspection.
