Ethics involves wrestling with difficult, conflicting courses of action. Medical ethics, and psychiatric ethics in particular, is especially challenging, given the need to negotiate a course of action with patients and family. Confidentiality, which is at the centre of psychiatric practice, and is essential if psychiatric treatment is to be successful, has been challenged and even somewhat eroded by legal developments and social change. Psychiatrists cannot promise absolute confidentiality to their patients. While it is tempting to look to ‘the law’ to determine when breaking confidentiality is the ethical decision, psychiatrists must decide whether acting in accordance with the law is the more ethical choice to make when grappling with a difficult clinical situation. The need for strict confidentiality also places constant demands on the psychiatrist to be mindful lest inadvertent lapses in vigilance results in the unethical sharing of patient information. This chapter explores ethical dilemmas in confidentiality in a range of clinical situations involving hospitals, clinics, and private outpatient settings. The unique ethical issues that arise in the treatment of children, adolescents, families, and groups are discussed in detail. The chapter also examines the ethical dilemmas that are often encountered in academic settings, speaking, and publishing. Considerable attention is devoted to the implications of the Tarasoff decision in California and to the Health Insurance Portability and Accountability Act of 1996, which has had a profound impact on the practice of psychiatry in the United States, as well as influencing the law in other countries.

Mental health problems are estimated to affect one in four people each year in the UK, making mental illness one of the commonest presentations to GP surgeries, outpatient clinics, and Emergency Departments. Yet many doctors and medical students feel uncertain about how to approach patients with a psychiatric disorder. The key to becoming a good psychiatrist lies in the clinical interview. There are few physical signs or investigations that allow doctors to diagnose psychiatric illness, so a detailed history and mental state examination are important. As a psychiatrist, you are in the privileged position of having patients tell you their personal stories, and the skill is in listening attentively and asking relevant questions to help to clarify parts of the story. The best way to practise these techniques is to watch experienced clinicians at work and to interview patients yourself. Obviously diagnosis is important, so you need to be aware of the types of symptoms that fit with each type of disorder, as well as the medical conditions that may mimic psychiatric illness. Investigations may be necessary to rule out other diseases, and you need to be able to request these appropriately. Psychiatrists have access to a range of treatments—medical (e.g. antidepressants), psychological (e.g. cognitive behavioural therapy), and physical (e.g. electroconvulsive therapy)—and you need to know which ones to recommend. Most of these treatments are delivered in conjunction with the multidisciplinary team, so you should be clear about the roles of each team member. Finally, there is overlap between psychiatry and the law, which can raise interesting ethical issues. It is sometimes necessary to treat a person against their will, for the safety of that person or others, so you need to know about mental health law. Psychiatrists are also often requested to provide a second opinion in difficult capacity assessments.

This chapter examines key innovations that began to project Saint Elizabeths’ psychiatric authority into the community—specifically, the outpatient clinic, psychiatric social work, and child guidance centers. Broadly defined as mental hygiene, these efforts were aimed at reducing the number of institutionalized patients and providing them follow-up care. They were also aimed at preserving the mental health of individuals and consequently preventing their institutionalization in the first place. The chapter is in part concerned with how black patients and their family members shaped the outpatient experience. It also addresses the role of Washington’s segregationist culture and politics in limiting these larger mental hygiene reforms among the District’s African American community.

No other known risk factor has such a high suicide prevention potential as a psychiatric disorder. However, to release this potential, psychiatric care needs to be made more available to suicidal individuals, and there is a necessity to establish a continuity of care between hospital and outpatient treatment for patients who have been admitted after a suicide attempt. In this chapter, important aspects of the psychiatric treatment of suicidal individuals are described. The importance of conducting systematic and repeated clinical risk assessments, providing treatments targeted at the patient’s needs, and instigating safety measures according to the actual risk level, is reinforced. Treatment standards should be kept high through continuous staff education measures, clinical supervision, adherence to written procedures, and should be improved through suicide review procedures whenever there has been a severe suicidal incident within the context of the clinical unit.

Abstract:
Chapter 3 presents a case example of role dispute-focused IPT and demonstrate supervision in an outpatient teaching clinic where IPT is offered to all patients with depression as part of an evidence-based treatment program. In this chapter, the therapist is a third-year psychiatry resident learning IPT as a required part of the residency training curriculum.

Abstract:
In 1996, when I was working with Channi as a psychiatry trainee, we were both struck by how often we were seeing women with psychotic disorders in the outpatient clinic who were seeking advice on whether or not they should try to conceive and have a family. Although there was a small amount of literature on the parenting difficulties some of these women would experience, there was a very limited evidence base on their risk of adverse obstetric outcomes. We felt we needed to know more about the outcome of pregnancy for women with pre-existing psychotic disorders, and this led to my PhD, funded by a Wellcome Trust Clinical Training Fellowship, with Channi as my primary supervisor. Channi became too ill to continue working on this project, but our work together led to my perinatal mental health research programme, for which I will always be in his debt. This chapter describes this early PhD research, and the research it has led on to, over the subsequent years. We initially thought about collecting detailed clinical outcome data from the women we were seeing locally, who came from a large catchment area. However, to obtain a large enough sample size would take many years and it became clear we needed large epidemiological datasets in order to optimize statistical power and minimize bias. We therefore started to look for such data, with the first dataset available coming from colleagues at the Institute of Psychiatry—the PriSM psychosis study (Thornicroft et al. 1998). The PRiSM psychosis study research team conducted the complete ascertainment of all prevalent cases of psychosis in the two study catchment areas in the index year (1991–1992) providing us with a representative population of mothers with psychotic disorders. We found that 63% of women with psychotic disorders (n = 155) were mothers, and that these women were more likely to be older and live in unsupported accommodation than women who had not had children, although they had similar levels of disability and health and social care needs (Howard et al. 2001).
