Abstract:
Abstract The aim of this study was to identify the bacterial species circulating in various wards of the Emergency Clinical Hospital Constanta, involved in infections associated with hospitalization. Clinical specimens were obtained during 2016 year period. Identification of species and sensitivity tests were performed on VITEK 2 System. There were isolated 163 bacterial strains suspected to be responsible for infections associated with hospitalization. Surgery wards had the most important number of cases, 49% of all. The second most affected ward was neonatology (11%), as many as gynecology (5%) and pediatrics (6 %) together. According to the site of the infection, digestive are majoritary (42.94%), followed by surgical wounds (16.56%) and respiratory infections (15.33%). Septicemia cases (11.04%) are important due to the severity. Distribution on months varies between a minimum number of cases in January (5) and a maximum of cases in April (19).Gram-negative bacilli were the most frequently involved (49%). The most frequent species was Acinetobacter baumanii (28 cases), followed by Pseudomonas aeruginosa (15) and E.coli (7). Anaerobic bacteria (Clostridium difficile) are the second most important group (32% of the cases), followed by Gram positive cocci (15%) mostly staphylococci. Multidrug resistance was diagnosed in most Acinetobacter baumanii strains. A special attention must be given to associations of bacteria, that even though are in small number (2% of the cases), are more difficult to be treated.

Background: Association of various types of congenital lesions in a single patient is a very common finding in congenital heart disease cases. Many such lesions are amenable to nonsurgical interventions in single setting. Such interventions along with surgical intervention are also possible in hybrid laboratories. Here we are reporting on our experience of twelve cases of multiple interventions in the year 2015.Methods: This is a retrospective review of double or multiple interventions done on cases from January 2015 to December 2015 In Lab aid cardiac hospital and Combined Military Hospital Dhaka. Type of Congenital lesions were identified from chest x-ray, ECG, and colour Doppler Echocardiography and combination of interventions were planned accordingly.Results: Out of twelve cases, 66.66% were male and 33.34% were female. Majority of cases (50%) were in 4-8 years age group. Most common type of disease combination was Atrial Septal Defect (ASD) with Patient Ductus Arteriosus (PDA). Association of Ventricular Septal Defect (VSD) with PDA was found in 16.67% cases, combination of PDA with coarctation of aorta was found in 16.67% cases and combination of ASD and VSD was found in 16.67% cases. Combination of ASD, VSD, PDA was also found with aortic and pulmonary stenosis in some cases. Outcome of multiple interventions in all the cases were good.Conclusion: Existence of multiple simple congenital heart lesions is a commonly encountered problem in Pediatric cardiac Practice. Multiple Interventions can be done on these lesions safely. Multiple interventions can be done on complex cases also for life saving purpose. One years experience of multiple intervention reported in this study showed excellent outcome.Cardiovasc. j. 2016; 9(1): 36-42

Abstract Background The acceptable noise level (ANL) is a measure of willingness to listen to speech in the presence of background noise and is thought to be related to success with amplification. To date, ANLs have only been assessed over short periods of time, including within a session and over a 3-week and 3-month time period. ANL stability over longer periods of time has not been assessed. Purpose The purpose was to examine the stability of ANL over a 1-year time period. Research Design A repeated-measures, longitudinal study was completed. Study Sample Thirty young adults with normal hearing served as participants. The participants were tested at two different sites. Data Collection and Analysis Two trials of most comfortable listening levels (MCLs), background noise levels (BNLs), and ANLs were assessed for each participant during three experimental sessions: at 0 months, 6 months, and 1 year. Results Two-way repeated-measures analysis of variances revealed no significant change in MCLs, BNLs, or ANLs within a session or over a 1-year time period. These results indicate that ANLs remain stable for 1 year in listeners with normal hearing. Conclusions The finding that the ANL is stable over a longer period of time supports the theory that the ANL is an inherent trait of the listener and mediated at in the central auditory nervous system.

Abstract. Current testosterone substitution therapy either by injectable or oral testosterone esters suffers from markedly fluctuating serum testosterone levels often far above or below the physiological range. Recently, a transdermal therapeutic system (TTS) for the delivery of testosterone was developed which, when applied to the scrotum, provides smooth serum testosterone levels. Here we report results from seven hypogonadal men treated with the TTS for 14 months by applying a new patch every day. In all patients serum testosterone and dihydrotestosterone (DHT) determined 3–5 h after applying a new patch increased significantly and remained within the physiological range during the entire treatment period. The DHT/testosterone ratio remained constant. In 4 of these patients and 2 others under TTS treatment serum testosterone and DHT were also determined over a 24-h period at regular intervals. In these patients serum testosterone levels in the physiological range were seen during the entire observation period, whereas an increase in the DHT/testosterone ratio occurred towards the end of the one-day treatment phase. All patients in the 14-month treatment study were clinically well substituted and responded with good compliance. Clinical chemistry showed no abnormalities during treatment. Thus, the TTS appears to be an effective and safe new modality for the treatment of male hypogonadism.

The present study was conducted in Department of Forensic Medicine and Toxicology of a Government Medical College and Hospital in year 2017. Total 2781 postmortem examinations were conducted in 2017. Out of that total 2781 cases, 176 compression of neck cases came for postmortem and amongst these 105 cases were studied in detail as per inclusion criteria. Out of 105 cases of compression around neck, which were studied, there were 93 cases of hanging, 10 cases of ligature strangulation, 01 case of manual strangulation and 01 case of accidental strangulation studied. On gross in total 11 cases of strangulation hemorrhage in subcutaneous tissue, strap muscle and sternocleidomastoid muscle observed and in only 20 cases of hanging there was hemorrhage observed in subcutaneous tissue and no hemorrhage observed in strap and sternocleidomastoid muscle in hanging cases. In this present study on histopathology vascular congestion and extravasation of red blood cells observed in 28 cases of compression of neck. These findings indicate the importance of histopathology of underlying neck structure in neck compression cases in documenting antemortem nature of hanging and strangulation in doubtful cases.
