Abstract:
The following case studies describe the use of the Smith Medical implanted ports and Gripper huber needles. Smiths Medical produces a range of implanted ports that include the Port-A-Cath and P.A.S. Port Power P.A.C. systems. The ports are easy to implant, maintain and remove. They are lightweight and have features that are designed to reduce complication rates, including a highly compressed septum to increase needle retention, a titanium chamber with a gouge-resistant floor, a bevelled chamber for optimal rinsing, a round shape to avoid overturning and the Ultralock connection. They are available in various configurations, with single and dual lumens. There is also a needle for power-injection of contrast media for certain types of diagnostic imaging scans (Smith Medical 18). The Gripper Plus needle is designed with an emphasis on safety, effectiveness and patient comfort. These non-coring needles have a bevelled tip that sits flush with the back of the port without impeding the flow of fluid; this also prevents holes forming in the septum ( Barton et al, 2018 ). The needle is available in different gauges and lengths, which can be tailored to reflect individual patient needs and the amount of adipose tissue present. It is essential to select the correct size, which will reduce the risk of dislodgement. The Gripper needles are compatible with paclitaxel and lipid solutions, and are recommended by the National Institute for Occupational Safety and Health (NIOSH) (1999) . They are compatible with both the Port-a-Cath and P.A.S. Port Power P.A.C. systems, and can be ordered from the same manufacturer, which has the potential to make ordering, training and support more efficient.

Aims and methodThis audit examines the quality of ‘as required’ (p.r.n.) prescribing for in-patients before and after standard setting.ResultsThere were improvements in writing oral/intramuscular prescriptions and in the use of different drugs regularly and p.r.n. There was a trend for improvement in completion of guidelines for those on high doses but completion was not influenced by hidden p.r.n. prescribing. Writing p.r.n. prescriptions as a ranged dose was common and did not improve significantly.Clinical implicationsBy using some simple measures, improvements in some aspects of potentially problematic p.r.n. medication can be achieved.

Abstract:
Cellular proliferation in the prostatic ducts and acini presents a morphological continuum from benign proliferation without cytological atypia (hyperplasia) to proliferations with different degrees of dysplasia. The term prostatic intraepithelial neoplasm (P.I.N.) indicates the theoretical morphological extremity of that continuum. The criteria for differentiating P.I.N. into three grades are to be found in the cytological and architectural aspect as well as the integrity of the basal cell layer and the basal membrane. Clinically P.I.N. is strongly associated with prostatic carcinoma and diagnosis should be followed by further investigations for a co-existing carcinoma. All the elements (age, severity of P.I.N., P.S.A., D.R.E. and T.R.U.S. characteristics) which may be used for correct follow-up are discussed, as well as possible therapeutic options.

Abstract:
This article examines how P.E.N., an organisation born in imperial Britain, endeavoured in some cases and floundered in others to create conditions for collaboration between Indian and British writers. Drawing on the P.E.N. archives at the Harry Ransom Center (HRC), I examine communication among and between Indian and British writers in P.E.N.'s orbit during the World War II era and leading up to the Indian Independence Act of 1947. As a forum for collaboration among writers internationally not only to develop writing and editing projects together, but also to forge a unifying conception for the modern era of the relationship between literature and political freedom, P.E.N. aimed to create opportunities for exchange among Indian and British writers. Analysing Indian writers' articulation of the necessary conditions for cross-imperial collaboration, I consider how mutuality was compromised under political conditions of imperialism hinging on hierarchal notions of culture.

Aims and methodThe Royal College of Psychiatrists' Consensus Statement on “The use of high-dose antipsychotic medication’ suggests only fully qualified psychiatrists (MRCPsych) should recommend the prescribing of high-dose antipsychotic treatment. We observed changes in anti-psychotic prescribing in two surveys of psychiatric in-patients conducted eight and 32 months after publication of the Consensus Statement.ResultsOverall mean chlorpromazine equivalent doses of antipsychotic drugs reduced between the surveys. When p.r.n. (as required) prescribing (usually done by junior doctors) is included, mean potential doses and numbers of patients who might receive ‘high-doses' increases substantially, although the reduction between surveys in total mean dose and proportion of patients on high-dose antipsychotic medication is preserved, and the actual use of p.r.n. medication was low (4–5% of p.r.n. prescriptions).Clinical implicationsWe recommend the development of local guidelines for junior staff concerning antipsychotic drug prescribing, regular monitoring of p.r.n. medication by consultants, and pharmacists' involvement in reviews of patients prescribed high-dose antipsychotic medication.

From its inception, hospitality was inscribed in the vision of the English P.E.N., one of its stipulated aims being to provide a ‘vehicle for friendliness and hospitality’. Yet several writers who took leading administrative and representational roles during the 1930s and 1940s – Storm Jameson, E. M. Forster and J. B. Priestley, for example – were uncomfortable with the kinds of hospitality the P.E.N. sometimes purported to offer. They were concerned about the wartime propriety of elaborate lunches, dinners and parties. At the same time, this was an organisation that also tirelessly advocated for more substantial forms of refuge for displaced writers. Drawing on extensive archival material, this essay will examine some of the ambiguities embedded in the P.E.N.’s conception of hospitality during this period: the perceived clash between a social etiquette of hospitality (expressed through P.E.N. social gatherings) and a wider politics of hospitality (expressed in its drive to facilitate refugee reception in Britain).

Within the setting of a regional secure unit, all doses of medication given p.r.n. over three months were ascertained and the details of each administration determined from prescription charts and a semistructured interview with the nursing staff involved. Thirty-two patients were resident for all or part of the study, all compulsorily detained. Only 15 were given medication p.r.n.) compared with the other patients they were significantly younger and more likely to be detained under civil orders than under the criminal provisions of the Mental Health Act 1983. Rarely was the medication the sole intervention in a crisis. In terms of effects on the overall treatment programme of the patient, medication given p.r.n. seemed to have an impact in only one small subgroup.

O P.E.N. Clube do Brasil é uma associação de escritores, fundada em 1936 por Cláudio Justiniano de Souza (1876-1954) nos moldes do P. E. N. Internacional e que desfruta de autonomia em termos de procedimentos administrativos e culturais. O acrônimo P.E.N., que tinha por significado “Poetas, Ensaístas e Novelistas”, posteriormente ampliado para “Poetas, Playwrights [dramaturgos], Editores, Ensaístas e Romancistas”, foi organizado em 1921 na cidade de Londres pela poetisa Catherine Amy Dawson-Scott (1865-1934) e tinha a ambição de representar a classe intelectual em âmbito mundial. A pesquisa tem por objetivo principal analisar, durante o período da presidência de seu fundador (1936-1954), os projetos e os ideais defendidos pelo P. E. N. Clube do Brasil que tinham como destinatários os “homens de letras”, tanto no que diz respeito às questões dos escritores nacionais, quanto aos assuntos supranacionais que circulavam pelo globo. Entre as fontes merece destaque os documentos produzidos pela própria instituição, que são confrontados com o material disponível sobre a agremiação em jornais e revistas da época.

The acronym PEN, which has as meaning “Poets, Essayists and Novelists”, later expanded to “Poets, Playwrights, Editors, Essayists and Novelists”, was organized in 1921, in London, by the poetess Catherine Amy Dawson-Scott (1865-1934) and had the ambition to represent the intellectual class worldwide. O sector agrícola português vai enfrentar novos desafios a partir do início do século. A nova Política Agrícola Comum, cuja proposta consta do documento "Agenda 2000", penaliza os agricultores portugueses. O aumento da pressão concorrencial nos mercados dos produtos agrícolas, devido à maior abertura ao mercado mundial, por força dos compromissos assumidos nos acordos assinados no quadro do "Uruguay Round" pela União Europeia, e pelo alargamento a Leste da União, promoverá a queda dos preços internos no produtor. A conjugação do cumprimento dos compromissos assumidos no "Uruguay Round" pela União Europeia e das medidas da nova Política Agrícola Comum levará a quebras no rendimento dos agricultores portugueses, com consequências negativas na ocupação do espaço rural, no emprego e no equilíbrio social e ambiental. A quebra do rendimento dos agricultores portugueses constitui o problema deste trabalho de investigação. A sua delimitação e identificação permitiu explicitar três objectivos principais. 0 primeiro objectivo pretende desenvolver um modelo de programação matemática para avaliar o impacte de algumas medidas da nova Política Agrícola Comum (PAC) e do aumento da pressão concorrencial no mercado interno dos produtos agrícolas, na produção, na utilização dos factores e no bem-estar, no sector agrícola, avaliado pelos excedentes do consumidor e do produtor. 0 segundo objectivo pretende quantificar os efeitos da extinção do subsídio cofinanciado, da eliminação do pousio obrigatório e do estabelecimento de uma ajuda específica de 66 ecus/tonelada, multiplicada pelos rendimentos regionais, na produção, no uso dos factores e nos excedentes do consumidor e do produtor. 0 terceiro objectivo pretende avaliar o impacte, em cada um dos cenários propostos no segundo objectivo, num regime de maior concorrência no mercado dos produtos agrícolas, naquelas variáveis e agregados. O estudo do problema deste trabalho de investigação é feito através de um modelo de programação matemática desenvolvido para o sector agrícola português. A função objectivo é uma função quadrática, inclui o risco e maximiza a soma dos excedentes do consumidor e do produtor. Este modelo é utilizado para simular cada um dos cenários propostos em cada um dos objectivos deste trabalho de investigação. Os resultados do modelo base foram comparados com as estatísticas oficiais e permitiram validar o modelo para o ano base de 1994. A análise dos resultados do modelo, para cada um dos cenários propostos em cada um dos objectivos, permite quantificar os efeitos na produção, na utilização dos factores e nos excedentes do consumidor e do produtor de cada uma das políticas alternativas e do aumento da pressão concorrencial no mercado interno dos produtos agrícolas. A principal conclusão é que, segundo este modelo, o sector agrícola vai ser penalizado com todas as medidas da nova Política Agrícola Comum estudadas neste trabalho. O aumento das trocas e da pressão concorrencial nos mercados dos produtos agrícolas, resultante da maior abertura destes mercados, provoca o aumento do excedente do consumidor, em todas as políticas estudadas, que se reflecte no bem-estar. Contudo, a redução da protecção aduaneira e o alargamento da União Europeia amplia as variações negativas no excedente do produtor, as quais absorvem os ganhos de bem-estar induzidos através do excedente do consumidor, excepto no caso do cenário 111, no qual este agregado aumenta 4,52%, em relação ao mesmo cenário antes da modificação das condições do mercado. Segundo os resultados do modelo, os produtores agrícolas são extremamente penalizados com o reforço da reforma da PAC de 1992 e com as novas condições de mercado. Os resultados do modelo, para cada um dos cenários propostos, no quadro do segundo e do terceiro objectivos, permitem concluir que a tendência decrescente do rendimento dos agricultores pode ser contrariada pela aposta em actividades alternativas. Esta conclusão fortalece a tese das ajudas aos produtos mediterrânicos, defendida pelos agricultores portugueses e pela Confederação dos Agricultores Portugueses (CAP), para diminuir as injustiças de que a agricultura portuguesa tem sido alvo. Os resultados do modelo sugerem ainda que as decisões das empresas e as medidas específicas necessárias ao desenvolvimento do sector agrícola devem privilegiar as actividades em que a agricultura portuguesa tem vantagem comparativa.

Existe evidencia sobre la influencia de algunos factores en el rendimiento escolar de los centros y entre ellos de manera consistente aparecen las características del profesorado y su formación. Este estudio analiza la formación del profesorado con los resultados escolares de los centros de educación secundaria en Asturias. De las variables de formación, es únicamente la formación en la asignatura y su didáctica la que se asocia a los resultados en la asignatura. Se realizan análisis en dos niveles de agregación, departamento y centro, observándose que es en los niveles de departamento donde existen las correlaciones más significativas. Se aporta evidencia para la organización de una formación del profesorado que asigne recursos de formación en los diferentes niveles en base a sus desigualdades de origen (contexto, variables del profesorado y de formación) y que garantice una efectiva igualdad de oportunidades para el conjunto de la población escolar de la Comunidad Autónoma.

Abstract:
This chapter examines the creation of the Congress of Industrial Organizations’ (CIO’s) political action committee or P.A.C. in 1943, following the collapse of Labor’s Non-Partisan League and passage of a new law restricting union money in elections. This was a critical point in the CIO’s embrace of a “dynamic partisan” electoral strategy. Through interventions in primary elections and the targeted provision of general election support to sympathetic Democratic candidates, P.A.C. sought to reshape the Democratic Party along more pro-labor and liberal lines. As this chapter reveals, P.A.C. leaders hoped to elect supportive lawmakers in the 1944 and 1946 elections, seeking out candidates who were strongly committed to labor’s goals. Despite public pronouncements of nonpartisanship, however, they chose not to look for allies on both sides of the aisle, instead favoring liberal Democrats over liberal Republicans—hoping to impress Franklin Roosevelt’s New Deal vision onto the Democratic Party as a whole.

Abstract:
The Interactive Robotics Unit of CEA LIST has developed a very challenging robotic carrier (called P.A.C.) which is able to perform high range intervention tasks inside blind hot cells. This long reach multi-link carrier has 11 degrees of freedom (DOF), an operational range over 6 meters of full extension and weighs less than 30 kg. The gravity effect in the manipulator is largely compensated by a special mechanical structure (the parallelogram) that helps to reduce the size of the actuators used to operate the robot. Due to its size and weight, this large robot manipulator holds lots of elastic and geometric deformations. Hence, it presents very low position accuracy. A flexible model is developed to take into account most of the structure deformations. A calibration method of the robot flexible parameters is used to reduce the positioning error of the end effector and the intermediate joints. Then, a second calibration method of the robot using generalized error matrices is applied to further reduce the residual positioning error of the system. These matrices are a polynomial function of the system geometry and joint variables. This method is first tested by simulation to ensure its viability on large manipulators. After encouraging simulation results, an experimental field is made for the calibration of the PAC manipulator. Results show that the adopted flexible model, with the new calibrated parameters, followed by the polynomial model is a good combination to correct and reduce the system errors.
