Background: Delayed sternal closure (DSC) has been an essential part of neonatal and infant heart surgery. Here, we report our single institution experience of DSC for eight years. Methods: The successive 188 patients were analyzed retrospectively. Sternum was closed at the end of the operation in 97 (51.6%) patients (primary sternal closure [PSC] group). Ni-Cu-P-W coating was deposited by electroless method on mild steel substrate to study the crystallization and tribological behavior at different annealing temperatures. Energy dispersive x-ray (EDX) analysis, scanning electron microscopy (SEM), x-ray diffraction (XRD), and differential scanning calorimeter (DSC) were used to study the composition, surface morphology, phase behavior, and thermal behavior of the coating, respectively. Tribological study was conducted using Pin-on-Disc tribotester. EDX analysis confirms the presence of Ni, Cu, P, and W in the deposit. SEM image shows the surface is dense, smooth, and without any observable nodule. Some of the samples were heat treated to 300°C, 500°C, and 700°C for 1 hour to observe the crystallographic change by XRD. One sharp crystalline peak of Ni (111) is present in all condition, but the intensity increases rapidly with the heat treatment temperature. The phase transition temperature of this quaternary coating analyzed by DSC was 431.8°C.

Phospholipids, which provide valuable model systems for lipid membranes, display a variety of polymorphic phases, depending on their molecular structure and on environmental conditions. High hydrostatic pressure has been used as a physical parameter to study the thermodynamic properties and phase behavior of these systems. High pressure is also a characteristic feature of certain natural membrane environments. In the first part of this article, we review our recent work on the temperature- and pressure-dependent phase behavior of phospholipid systems differing in lipid conformation and headgroup structure. In the second part, we report on the determination of the (T, x, p) phase diagrams of binary phospholipid mixtures. An additional section deals with effects of incorporating ions, small amphiphilic molecules, and steroids into the bilayer on the experimental temperature- and pressure-dependent phase behavior of lipid systems. Finally, we discuss lamellar to nonlamellar thermotropic and barotropic phase transformations, which occur for a number of lipids, such as phosphatidylethanolamines, monoacylglycerides, and lipid mixtures. It has been suggested that nonlamellar lipid structures might play an important role as transient and local intermediates in a number of biochemical processes. High-pressure smallangle x-ray (SAXS) and neutron (SANS) scattering, differential scanning calorimetry (DSC), high-pressure differential thermal analysis (DTA), and p, V, T measurements have been used as experimental methods for the investigation of these systems. Lipid bilayer dispersions, in particular the phosphatidylcholines and phosphatidylethanolamines, are the workhorses for the investigation of biophysical properties of membrane lipids because they constitute the basic structural component of biological membranes. They exhibit a rich lyotropic and thermotropic phase behavior (Cevc &amp; Marsh, 1987; Marsh, 1991; Yeagle, 1992). Most fully hydrated saturated phospholipid bilayers exhibit two principal thermotropic lamellar phase transitions, corresponding to a gel to gel (Lβ′–Pβ′) transition and a gel to liquid-crystalline (Pβ′–Lα) main transition at a temperature Tm. In the fluid-like La phase, the hydrocarbon chains of the lipid bilayers are conformationally disordered, whereas in the gel phases the hydrocarbon chains are more extended and relatively ordered.

In this chapter the author explores teacher preparation as the logical site for cultivating praxis for secondary inclusive education. The author describes a university course on inclusive education for preparing critical special education teachers. The author offers disability studies in education (DSE) as a theoretical framework for supporting critical thought and creating just and inclusive educational practices for students with disabilities in P-21 settings. The author then outlines two themes that ground the architectural design of the course: re/thinking students (person-first narratives of disability) and re/designing pedagogy (Universal Design for Learning). According to Smith (2009), DSE “sees oppression and prejudice in sociocultural contexts and seeks to address those concerns” (p. 215). In preparing critical special educators it is our job to help students not only “see” the oppression of exclusion, but to provide them with concrete ways to create change and remain resilient.

Internet Gaming Disorder (IGD) was introduced in the recent DSM-V as a condition needing more research. Therefore, this chapter contributes to this discussion by a summary of recent research findings and introduces an empirical study concerning differences between an engaged and a problematic use of games. We surveyed 577 participants (mean age 24.38 years; 77.1% male) from German speaking areas. We used a gaming addiction questionnaire and the Internet Addiction Scale (ISS-20). 93.7% are high-level player (level 85); 3.1% are addicted according to the ISS-20. We found 2 factors explaining “addiction” and “engagement”. Addicted players spend more time per week playing online with 31.31 hours/week compared to highly engaged players with 22.19 hours/week (p &lt; .001), have higher scores in the Internet addiction scale (p &lt; .001), and significantly lower scores in scales measuring the quality of life (p &lt; .001). Therefore we conclude that items tapping euphoria and cognitive salience are of limited use when it comes to a classification of IGD.

The manual’s editors wanted a symptom-based, descriptive classification and generally rejected any reference to causal theories about mental processes. PTSD was an exception to the rule of creating a classification that is ‘‘atheoretical with regard to etiology or pathophysiological process’’ (American Psychiatric Association, 1980 p. 7), but the exception was not noted anywhere in the manual. Not only did the PTSD definition include an etiological event, but it incorporated a theory, an underlying process, that connects the syndrome’s diagnostic features (McNally, 2003; Young, 1995). In 1994, the American Psychiatric Association published the fourth edition of the DSM. The definition of PTSD, which had already undergone some revisions in DSM-IIIR, maintained the syndrome’s description but changed materially the stressor criterion. The range of events was widened, and the emphasis shifted to the subjective experience of victims. The list of ‘‘typical’’ traumas in the DSM-IV left no doubt that the intent was to enlarge the variety of experiences that can be used to diagnose PTSD beyond the initial conception of directly experienced, life-threatening events such as combat, natural disaster, rape, and other assault. Persons who learned about a threat to the physical integrity of another person or about a traumatic event experienced by a friend could be considered victims. A novel form of PTSD took shape following the 9/11 terrorist attacks, when the entire population of the United States was considered to have been affected by a ‘‘distant’’ trauma, produced chiefly by viewing television coverage.

Accessibility refers to the relative ease by which the locations of activities, such as work, shopping and healthcare, can be reached from a given location. Access varies across space because of uneven distributions of supply and demand (spatial factors), and also varies among population groups because of their different socioeconomic and demographic characteristics (nonspatial factors). Taking healthcare access for example, spatial access emphasizes the importance of geographic barrier (distance or time) between consumer and provider, whereas nonspatial access stresses non-geographic barriers or facilitators such as social class, income, ethnicity, age, sex, and so forth. Since the 1960s, health policymakers in the United States have attempted to improve health care for the citizenry by considering aspects of both spatial and nonspatial factors. Such efforts are exemplified in designations of Health Professional Shortage Areas (HPSA) and Medically Underserved Areas or Populations (MUA/P) by the U.S. Department of Health and Human Services (DHHS), for the purpose of determining eligibility for certain federal health care resources.
