Abstract:
Introduction.Pachydermodactyly is a rare disease with asymptomatic swelling of proximal interphalangeal joints. This disorder should be considered in the differential diagnosis of juvenile idiopathic arthritis or rheumatoid arthritis. However, pachydermodactyly is not well recognized by many orthopaedic surgeons and rheumatologists.Case Presentation.We report herein a case of a 13-year-old girl with pachydermodactyly. She presented to our clinic with symmetrical swelling of digits II through V without functional loss for the last 4 years. X-ray examination demonstrated no bone or joint destruction and magnetic resonance images showed only thickened skin tissues. No inflammatory signs were seen with laboratory blood tests. We reached a diagnosis of pachydermodactyly by exclusion. We had administered tranilast to her for 6 months and her symptom slightly improved.Conclusion.It is important to recognize pachydermodactyly and be able to differentiate it from other causes of PIP joint swelling such as rheumatoid arthritis, although pachydermodactyly is rare and benign. Physicians including orthopaedists and rheumatologists should make a prompt diagnosis to avoid unnecessary investigations and prevent the patient from receiving inappropriate treatment with steroids or cytotoxic agents. On the other hand, tranilast might be an effective drug to pachydermodactyly.

Pachydermodactyly is characterized by asymptomatic, progressive swelling of the lateral aspects of the 2nd to 4th finger along the proximal interphalangeal (PIP) joint without involving the joint itself. We present 2 interesting cases of patients with periarticular swelling who were initially diagnosed and treated as juvenile idiopathic arthritis (JIA) with subsequent clinical and pathology confirmation of pachydermodactyly. These cases emphasize the importance of considering pachydermodactyly in young patients with development of periarticular swelling and no joint involvement.

Abstract:
Pachydermodactyly is a rare digital fibromatosis that can be misdiagnosed with inflammatory rheumatic diseases. So far in the literature only about 150 cases of PDD have been reported. A prompt clinical diagnosis of the disease would prevent inappropriate treatment and unnecessary expensive diagnostic procedures such as biopsy or magnetic resonance imaging. Gold standard therapy for pachydermodactyly is not established yet.A rare case of a 25 years-old male patient with pachydermodactyly is reported. He complained swelling and thhickening around joints in both hands since 7 months ago. He denied having any pain or morning stiffness of the joints and was also free other symptomatic symptomps. Family history was not significant. Dermatological state showed skin colouredhyperkeratotic patches. Laboratory showed no abnormalities. Radiology results shows no bone and joint abnormalities. Dermoscopy examination on lesion shows whitish scaling and cobblestone appearance. Diagnosis of pachydermodactyly is based on anamnesis, physical examination, laboratory findings, and plain radiograph. Early and correct diagnosis is important to prevent unnececessary, expensive, and invasive diagnostic procedure

We present the case of a 17-year-old Asian man diagnosed with pachydermodactyly, a rare digital fibromatosis. Although this is a non-inflammatory periarticular soft tissue disorder, the clinical appearance can mimic inflammatory arthritis. The patient had a 2-year history of fusiform swelling of multiple proximal interphalangeal joints. He was initially diagnosed with juvenile idiopathic arthritis and treated with methotrexate, but a lack of clinical response led to the diagnosis of pachydermodactyly. Recognising this rare condition can prevent unnecessary and potentially harmful treatment.
