Amr, Bassem Ismail Metwaly Ismail. "Aspects of the preoperative pathway in pancreatic head malignancy." Thesis, University of Plymouth, 2018. http://hdl.handle.net/10026.1/12167.

Full text
Abstract:
Malignancy within the pancreatic head can arise from pancreatic duct, distal bile duct, ampulla or duodenum. Since September 2000, surgery for all pancreatic head malignancy (PHM) has been centralised into regional pancreatic centres where assessment of preoperative imaging and subsequent surgery is undertaken. As part of this guidance, surgery must be performed within 62-days of referral. This project will assess four aspects of the pre-operative pathway in PHM: 1) Potential variation in outcome of patients referred from different sites within a Cancer Network 2) Potential variation in outcom

Abstract:
Background: Screening is offered to individuals who have an identified increased risk of pancreatic cancer. Such risks may be increased because of a family history or a known genetic mutation which has been shown to confer increased risk. Amongst those being screened intraductal papillary mucinous neoplasm (IPMN) of the pancreas are increasingly common entities being detected incidentally; their risk of malignancy can be as high as 85%. Consensus guidelines exist for the management of these lesions – the morbidity associated with pancreas resection is as high as 50%. We set out to identify a m

Abstract:
Introduction Endoscopic ultrasound (EUS) elastography is a recent ultrasound method used for the real-time visualization and evaluation of tissue elasticity. Qualitative and quantitative methods have been used, in particular in evaluation of pancreatic diseases and malignant lymph nodes, with interesting results regarding the accuracy and the differential diagnosis between malignant and benign masses. No consensus has been reached with regard to the superiority of different quantitative methods, but strain ratio and strain histogram (SH) remain the most used. SH corresponds to a graphical re

The development of animal models for pancreatic cancer has magnified the controversy regarding the cell of origin of the tumor in animals and man. In rats, most pancreatic tumors have an acinar histology. By contrast, tumors in hamsters have a ductular morphology, resembling the predominant type of human exocrine pancreatic cancer. While some workers advocate a ductular origin for hamster tumors, others insist that the progenitor is an acinar cell.<br>With the use of normal adult rat pancreatic ductular cell lines, this work shows that four representative pancreatic carcinogens all display dos

Abstract:
Biliary and pancreatic cancers are associated with poor prognosis and high mortality. A variety of genetic and environmental factors have been implicated as possible causes of these cancers. New epidemiological studies are looking into the possible role of heavy metals such as lead, chromium, nickel and cadmium as carcinogens. Studies from areas of wide geographical differences such as Egypt, USA and India have reported a possible association of heavy metals such as lead and cadmium with pancreato-biliary malignancy. These studies were from areas of significant environmental pollution with the

Abstract Although the critical role of the small GTPases Ras and Ral in oncogenesis has been well documented, much remains to be investigated about the molecular mechanism by which these GTPases regulate malignant transformation. The work under this thesis made two major contributions to this field. The first is the discovery that K-Ras, RalA and/or RalB are required for the maintenance of the high levels of the anti-apoptotic protein survivin in some human cancer cells, and the second is the demonstration that down regulation of K-Ras, RalA and/or RalB, but not Raf-1 or Akt1/2, stabilizes the

Abstract:
Background: Pancreatic mucinous cystic neoplasms (MCN) are rare mucin-producing cystic tumors. They are predominantly found, incidentally, in middle-aged women and usually located in the pancreatic body or tail. They are differentiated from other mucin producing neoplasms by the presence of ovarian-type stroma. The current management of MCN is defined by the consensus European, International Association of Pancreatology (IAP) and the American Association of Gastroenterology guidelines. However, the malignant potential of these lesions remains uncertain, with differing rates of malignant potent

Pancreatic stellate cells (PaSCs) are the major contributors of the desmoplastic stromal microenvironment of pancreatic adenocarcinoma and play a crucial role in malignant progression through their heterotypic interactions with tumor cells. Recent evidence suggests that cells may develop a senescent-like growth arrest program when they are exposed to sub-lethal injuries such as ionizing radiation and/or cytotoxic agents. Intriguingly, c
