Abstract:
Aim: Hyperfunctioning intrathyroidal parathyroid glands are rare and often result in thyroid lobectomy. This study examines the utility of radioguided surgery to guide enucleation of intrathyroidal parathyroids. Methods: Between December 2002 and March 2018, 2291 patients underwent parathyroidectomy by one surgeon for primary hyperparathyroidism. A total of 74 (3%) patients had an ectopic intrathyroidal parathyroid gland and underwent radioguided. Results: All of intrathyroidal parathyroid glands were localized with the gamma probe. In vivo radionuclide counts were above 120% of the background

Background and Objectives Near-infrared (NIR) fluorescence photo imaging provides real time parathyroid anatomy enhancement. Moreover, autofluorescence enables intraoperative virtual reality parathyroid exploration of the optical characteristics of the parathyroid gland. Parathyroid glands are responsible for maintaining the calcium level in blood and usually a pair of superior and inferior parathyroid glands are located in the thyroid gland. Parathyroid glands are smaller and have variations in their size, shape, number and location. Thus, it is difficult to identify the parathyroid gland and leads to its accidental removal during thyroidectomy. This case report is an incidental finding of two superior parathyroid glands in the left lobe of thyroid gland in 68-year female cadaver. Both superior parathyroid glands were located at the first tracheal ring, 2 mm

In this article, we discuss difficulties in parathyroid localization modalities in diagnosis of primary hyperparathyroidism. Most often, superior parathyroid glands are located on the posteromedial surface of the right and left lobes of the thyroid gland, however, they also could be found at the carotid bifurcation, behind pharynx and esophagus, as well as inside thyroid gland. Location of the inferior parathyroid glands is more variable: on the side or back surface, or below the lower pole of the thyroid gland, as well as in thymus, posterior or anterior mediastinum, or inside thyroid.
