Depuis une vingtaine d’années, les parcs de Chine urbaine sont largement mobilisés par les personnes âgées comme espaces de loisirs. Partant d’une enquête ethnographique menée au parc Ditan, à Pékin, cet article revient sur la manière dont les personnes âgées investissent cet espace et lui attribuent aujourd’hui des significations spécifiques. Après une historicization des usages et publics des parcs de Chine urbaine depuis leurs créations au début du XXe siècle, l’article se penche sur les raisons expliquant leur remobilisation récente par un nouveau public de retraités et sur les significations que ces derniers leur attribuent, dans un contexte de transformations urbaines et de tensions intergénérationnelles.

- An important urban transformation was achieved in Paris with the redevelopment of the Bercy quarter. It was characterised by farsightedness and an ability to monitor and manage on the part of the public sector operators who guided the entire operation. While on the one hand the Bercy case presents a series of ‘extraordinary' elements deriving from the particular history of the site, the continuity with which the municipal administration moved forward with its strategic decisions, its capacity to frame those strategies in a broader and more complex context and the ways in which the entire process was implemented certainly constitute important factors on which to reflect in the framework of a more general discussion on the effectiveness and potentials of large urban projects. This paper looks at the main stages of the transformation starting with the framing of the operation within the provisions of the main urban planning instruments and it seeks to highlight the most significant aspects of the intervention with a particular focus on the outcomes of the project implemented.

Même si l’insertion de nouveaux logements luxueux dans le tissu existant peut paraître statistiquement marginale, les résidants traditionnels ont été et sont témoins de transformations de leur milieu de vie. Ces transformations ont un impact sur leur territoire quotidien. Les résultats de nos enquêtes basées sur une approche méthodologique mixte indiquent une (re)définition du territoire du chez-soi, excluant le parc du canal de Lachine, parmi les résidants traditionnels du secteur.

ABSTRACT We report a duplex real-time PCR assay for the simultaneous screening of mutations involved in fluoroquinolone resistance within gyrA and parC quninolone resistance-determining regions (QRDRs) in Neisseria gonorrhoeae. Our assay clearly detects all mutated QRDRs and allows the identification of common genotypes, whether the QRDRs contain single or double mutations, providing valuable epidemiological tools. When this method is used in conjunction with similar assays and in vitro analyses, essential antibiotic resistance surveillance can be performed for public health purposes.

ABSTRACT This study reports the prevalences of qnrVC genes in 74 ciprofloxacin-resistant Vibrio sp. isolates. Two novel functional qnrVC alleles, qnrVC8 and qnrVC9, sharing 98% and 99% nucleotide similarity with qnrVC6 and qnrVC7, respectively, were identified. Our findings suggested that carriage of qnrVC alleles, together with target mutations in gyrA and parC genes, may contribute to the development of fluoroquinolone resistance in Vibrio species, posing a serious threat to public health.

Decades of research support the idea that striving for lower sound levels is the cornerstone of protecting urban public health. Growing insight on urban soundscapes, however, highlights a more complex role of sound in public spaces, mediated by context, and the potential of soundscape interventions to contribute to the urban experience. We discuss Musikiosk, an unsupervised installation allowing users to play audio content from their own devices over publicly provided speakers. Deployed in the gazebo of a pocket park in Montreal (Parc du Portugal), in the summer of 2015, its effects over the quality of the public urban experience of park users were researched using a mixed methods approach, combining questionnaires, interviews, behavioral observations, and acoustic monitoring, as well as public outreach activities. An integrated analysis of results revealed positive outcomes both at the individual level (in terms of soundscape evaluations and mood benefits) and at the social level (in terms of increased interaction and lingering behaviors). The park was perceived as more pleasant and convivial for both users and non-users, and the perceived soundscape calmness and appropriateness were not affected. Musikiosk animated an underused section of the park without displacing existing users while promoting increased interaction and sharing, particularly of music. It also led to a strategy for interacting with both residents and city decision-makers on matters related to urban sound.

The objective of this study is to develop a high resolution melt curve (HRM) assay to rapidly screen for mutations in quinolone-resistant determining region (QRDR) of gyrase and topoisomerase IV genes. DNA sequencing was performed on 62Salmonellastrains to identify mutations in the QRDR ofgyrA,gyrB,parC, andparEgenes. Mutations were detected in QRDR ofgyrA(n=52; S83F, S83Y, S83I, D87G, D87Y, and D87N) andparE(n=1; M438I).Salmonellastrains with mutations within QRDR ofgyrAare generally more resistant to nalidixic acid (MIC16>256 μg/mL). Mutations were uncommon within the QRDR ofgyrB,parC, andparEgenes. In the HRM assay, mutants can be distinguished from the wild-type strains based on the transition of melt curves, which is more prominent when the profiles are displayed in difference plot. In conclusion, HRM analysis allows for rapid screening for mutations at the QRDRs of gyrase and topoisomerase IV genes inSalmonella. This assay markedly reduced the sequencing effort involved in mutational studies of quinolone-resistance genes.

Abstract:
Background: Appendicitis is the most common abdominal emergency in the pediatric population. The clinical features of appendicitis in children are usually atypical, leading to misdiagnosis in 19–57% of preschool age children, which can result in complications. Thus, scoring systems are used to estimate the risk of appendicitis based on symptomatology and laboratory results. Aims: The aims of this study were to analyze the conformance of the Pediatric Appendicitis Score (PAS), the Pediatric Appendicitis Risk Calculator (pARC), and the Alvarado score as screening tools for appendicitis in children attending a public hospital emergency department. Material and methods: The inclusion criteria were all children aged < 14 years who presented with symptoms of appendicitis and were confirmed to have appendicitis either by imaging or surgery. The exclusion criteria were incomplete medical records. Data was collected retrospectively from medical records. Data analysis included examination of the proportion and distribution of data, and statistical analysis in the form of the Kappa conformance test. Results: The result of the conformance test between the PAS and the Alvarado score was 75%, but the Kappa conformance value between them was only 46.8κ although statistically significant (p value = 0.013). The conformance test between the PAS and the pARC was 50%, but the Kappa conformance value was only 19.4κ and not statistically significant (p value = 0.143). The conformance test between the Alvarado score and the pARC was 55%, but the Kappa conformance value was 13.5κ and not statistically significant (p value = 0.492). Conclusions: The PAS has advantages over other appendicitis screening questionnaires with greatest conformance between PAS and Alvarado.

Introduction: Listeria species are susceptible to most antibiotics. However, over the last decade, increasing reports of multidrug-resistant Listeria spp. from various sources have prompted public health concerns. The objective of this study was to characterize the antibiotic susceptibility of Listeria spp. and the genetic mechanisms that confer resistance. Methodology: Forty-six Listeria spp. isolates were studied, and their minimal inhibitory concentrations of antibiotics were determined by microdilution using Sensititre standard susceptibility MIC plates. The isolates were screened for the presence of gyrA, parC, lde, lsa(A), lnu(A), and mprF by PCR, and the amplified genes were sequenced. Results: All isolates were susceptible to penicillin, ampicillin, tetracycline, erythromycin, and carbapenems. Resistance to clindamycin, daptomycin, and oxacillin was found among L. monocytogenes and L. innocua, and all species possessed at least intermediate resistance to fluoroquinolones. GyrA, parC, and mprF were detected in all isolates; however, mutations were found only in gyrA sequences. A high daptomycin MIC, as reported previously, was observed, suggesting an intrinsic resistance of Listeria spp. to daptomycin. Conclusions: These results are consistent with reports of emerging resistance in Listeria spp. and emphasize the need for further genotypic characterization of antibiotic resistance in this genus.

We report a case of Mycoplasma genitalium-positive non-gonococcal urethritis who failed to respond to multiple different courses of antibiotics including azithromycin, minocycline, moxifloxacin and spectinomycin. Extracted M. genitalium DNA from urethral swabs obtained from the patient harbored mutations of 23S rRNA, parC and gyrA. Microbiological cure was eventually achieved by a 14-day course of doxycycline.

La transférabilité des interventions en santé est un enjeu essentiel à la réduction des inégalités sociales de santé. Mais ces interventions sont complexes et leur efficacité n’est pas garantie lors d’un transfert d’un contexte à l’autre. La description des fonctions-clés des interventions concourt à une meilleure transférabilité. Toutefois, celle-ci repose rarement sur une méthode permettant de concevoir l’intervention en situation de transfert. Cet article explicite le processus méthodologique permettant l’identification des fonctions-clés de l’intervention en situation de transfert, et leurs déclinaisons nommées : « éléments de passation » (EP). Nous nous sommes appuyés sur une recherche collaborative orientée par la conception (RoC) visant à concevoir une intervention d’éducation pour les locataires âgés du parc social, leur permettant de décider et d’agir pour adapter leur logement à leurs besoins. Deux cycles de recherche sont menés au sein d’entreprises sociales pour l’habitat (ESH) avec des comités de recherche réunissant chercheurs, locataires et professionnels. Plusieurs étapes collaboratives ont été nécessaires : l’étude du contexte de l’ESH1 et la création d’une culture commune ; la formation des professionnels impliqués dans l’intervention d’éducation ; la déclinaison du modèle selon les critères de transférabilité d’ASTAIRE (outil pour l’analyse de la transferabilité des interventions en promotion de la santé) ; l’identification des variations et des éléments stables du modèle au moment du transfert vers deux autres ESH ; l’identification des fonctions-clés du modèle et de ses « éléments de passation ». À l’épreuve du transfert, l’intervention est décrite en quatre catégories de fonctions-clés, déclinées en EP. Les EP sont des résultats issus de la RoC jugés signifiants par les acteurs de la recherche. Ce sont des phénomènes explicatifs révélant des situations à prévoir pour réussir le transfert et rendre l’intervention efficace. Ils permettent de donner sens aux fonctions-clés pour que les acteurs souhaitant transférer l’intervention dans leur contexte puissent mieux s’y préparer.

ABSTRACT Fluoroquinolones commonly are used to treat adult Salmonella infections. Fluoroquinolone treatment has failed for persons infected with nalidixic acid-resistant Salmonella. From 1996 to 2003, state public health laboratories forwarded 12,252 non-Typhi Salmonella enterica isolates to the Centers for Disease Control and Prevention for antimicrobial susceptibility testing; 203 (1.6%) of the isolates were nalidixic acid resistant, and 14 (7%) of those were ciprofloxacin resistant. Resistance to nalidixic acid significantly increased from 0.4% in 1996 to 2.3% in 2003. All ciprofloxacin-resistant isolates had at least one point mutation in the quinolone resistance determining region (QRDR) of gyrA and did not harbor qnr or have point mutations in the QRDR of gyrB, parC, or parE. Continued surveillance of antimicrobial resistance among non-Typhi S. enterica isolates is needed to mitigate the increasing prevalence of nalidixic acid resistance.

Atualmente tem crescido a demanda por espaços públicos de recreação e lazer pela população urbana, já que além de todos os benefícios físicos, mentais e psicológicos, esses espaços aliados ao discurso do desenvolvimento sustentável são divulgados como símbolos do progresso de suas cidades. Entretanto, na realidade muitas vezes observa-se que não há nenhuma relevância ambiental ou socioespacial e sim a predominância dos fatores políticos, o que acaba por causar contradições visíveis e danos nesses espaços, mesmo quando aparentemente adequados e agradáveis. A partir dessa reflexão, foi produzido esse estudo de caso sobre o Parque Ambiental Ipiranga, esse se encontra no alto curso do córrego Ipiranga, ainda próximo a nascente que está localizada no Clube Ipiranga, propriedade vizinha ao do estacionamento do parque, na cidade de Anápolis, Goiás. A partir da pesquisa bibliográfica sobre o conceito de parque urbano utilizou-se da análise crítica para verificar a compatibilidade do discurso promovido com o espaço implantado. Por fim, busca-se discutir como que os espaços públicos urbanos já estabelecidos podem ser apropriados de maneira a combater os danos causados pela negligência em relação aos condicionantes ambientais e socioespaciais, que resultam principalmente no descaso, falta de manutenção e abandono desses espaços. ABSTRACT Nowadays, the demand for recreational and leisure public spaces by the urban population has increased, since besides all the physical, mental and psychological benefits, these spaces allied to the discourse of sustainable development are divulged as symbols of the progress of their cities. However, in reality it is often observed that there is no environmental or socio-spatial relevance, but rather the predominance of political factors, which causes visible contradictions and damages in these spaces, even when apparently appropriate and pleasant. Based on this reflection, this case study was produced on the Ipiranga Environmental Park, which is located in the upper course of the Ipiranga creek, still near the source that is located in the Ipiranga Club, a property near the parking lot of the park, in Anápolis city, Goiás state. Based on the bibliographical research on the concept of urban park, critical analysis was used to verify the compatibility of the promoted discourse with the implanted space. Finally, it is sought to discuss how established urban public spaces may be appropriate in order to combat the damage caused by neglect of environmental and socio-spatial constraints, which result mainly in neglect, lack of maintenance and abandonment of these spaces. Sur la base de cette réflexion, cette étude de cas sur le parc environnemental d'Ipiranga a été réalisée. Il est située sur le cours supérieur du ruisseau Ipiranga, toujours à proximité de la source qui se trouve au Club Ipiranga, une propriété à côté du parking du parc, dans la ville d'Anápolis, Goiás. Antimicrobial-resistant Neisseria gonorrhoeae (NG) is a global public health issue that threatens effectiveness of current treatments of NG. Increased use of nucleic acid amplification tests (NAATs) in lieu of cultures makes obtaining clinical isolates for susceptibility testing difficult and samples collected in commercial transport buffer for NAATs do not preserve viable organism, while molecular methods of assessing antibiotic susceptibility do not require viable organism. We evaluated 243 NG-positive samples in Aptima transport media including urine, oral, and rectal swabs from Nigerian men who have sex with men for markers to penicillinase-producing NG, ciprofloxacin ( GyrA and ParC mutations), and extended spectrum cephalosporins (ESCs, PenA mosaic [allele X], PonA, mtrR, PorB mutations) by real-time PCR. NG DNA was recovered in 75% (183/243) of samples. Of these, 93% (171/183) were positive for at least one resistance marker. We observed a prevalence of dual resistance markers to penicillin and ciprofloxacin at 46.2% (79/171). Six percent of samples (10/171) tested positive for the PenA mosaic (allele X) ESC marker. These data indicate that antibiotic-resistant NG is common in Nigeria. Laboratory and clinical capacity building in Nigeria should include development of methods to culture NG and determine antimicrobial susceptibility.

Centralised electronic health record (EHR) systems have high potential to provide suicide attempt surveillance, to inform public health action aimed at reducing risk for suicide attempt in the population, and to provide data-driven clinical decision support for suicide risk assessment across healthcare settings. To exploit this potential, we designed the Catalonia Suicide Risk Code Epidemiology (CSRC-Epi) study. Using centralised EHR data from the entire public healthcare system of Catalonia, Spain, the CSRC-Epi study aims to estimate reliable suicide attempt incidence rates, identify suicide attempt risk factors and develop validated suicide attempt risk prediction tools.Methods and analysisThe CSRC-Epi study is registry-based study, specifically, a two-stage exposure-enriched nested case–control study of suicide attempts during the period 2014–2019 in Catalonia, Spain. The primary study outcome consists of first and repeat attempts during the observation period. Cases will come from a case register linked to a suicide attempt surveillance programme, which offers in-depth psychiatric evaluations to all Catalan residents who present to clinical care with any suspected risk for suicide. Predictor variables will come from centralised EHR systems representing all relevant healthcare settings. The study’s sampling frame will be constructed using population-representative administrative lists of Catalan residents. Inverse probability weights will restore representativeness of the original population. Analysis will include the calculation of age-standardised and sex-standardised suicide attempt incidence rates. Logistic regression will identify suicide attempt risk factors on the individual level (ie, relative risk) and the population level (ie, population attributable risk proportions). Machine learning techniques will be used to develop suicide attempt risk prediction tools.Ethics and disseminationThis protocol is approved by the Parc de Salut Mar Clinical Research Ethics Committee (2017/7431/I). Content-Centric Networking (CCN) is a new networking paradigm for the future Internet, which shifts the communication paradigm from host-centric to data-centric. In CCN, contents are routed by their unique names and they are stored in network nodes by units of segment during transmission for future usage. Since contents are stored in network nodes in a distributed manner, security is built into CCN data packets by embedding a public key signature to enable any content requesters to verify authenticity and integrity of contents. However, the use of public key signatures for authenticating CCN data packets incurs significant overhead regarding computation and communication, which limits universal utilization of CCN. Furthermore, this can lead to a new kind of DDoS attacks. Even though CCN adopts an aggregate signature method based on Merkle Hash Tree (MHT) in its reference implementation, it still incurs large amount of overhead. This paper presents TLDA, an efficient Two-Layered Data Authentication mechanism, which can considerably reduce overhead of computation and communication for authenticating data segments in CCN. For efficiency of computation and communication, TLDA newly introduces the concept of authentication Meta part consisting of data segments’ hash values. To a great extent TLDA not only reduces the computation and communication overhead compared with CCN’s basic authentication method, but also provides robustness against transmission loss and out-of-order transmission. We have implemented TLDA and demonstrated that it provides 74.3% improved throughput and 36.557% reduced communication overhead compared to those of the original CCNx library developed by PARC when transmitting a 128Mbyte content in units of 1Kbyte segment with RSA-2048 and SHA-256 as its signature algorithm and hash algorithm, respectively.

ABSTRACT The objective of this study was to determine antimicrobial resistance and elucidate the resistance mechanism in nontyphoidal Salmonella enterica serovars isolated from food products imported into the United States from 2011 to 2013. Food products contaminated with antimicrobial-resistant nontyphoidal S. enterica were mainly imported from Taiwan, Indonesia, Vietnam, and China. PCR, DNA sequencing, and plasmid analyses were used to characterize antimicrobial resistance determinants. Twenty-three of 110 S. enterica isolates were resistant to various antimicrobial classes, including β-lactam, aminoglycoside, phenicol, glycopeptide, sulfonamide, trimethoprim, and/or fluoroquinolone antimicrobial agents. Twelve of the isolates were multidrug resistant strains. Antimicrobial resistance determinants blaTEM-1, blaCTX-M-9, blaOXA-1, tetA, tetB, tetD, dfrA1, dfrV, dhfrI, dhfrXII, drf17, aadA1, aadA2, aadA5, orfC, qnrS, and mutations of gyrA and parC were detected in one or more antimicrobial-resistant nontyphoidal S. enterica strains. Plasmid profiles revealed that 12 of the 23 antimicrobial-resistant strains harbored plasmids with incompatibility groups IncFIB, IncHI1, IncI1, IncN, IncW, and IncX. Epidemiologic and antimicrobial resistance monitoring data combined with molecular characterization of antimicrobial resistance determinants in Salmonella strains isolated from imported food products may provide information that can be used to establish or implement food safety programs to improve public health.

Com isso, numa perspectiva macro, gerar informações sobre a evidenciação da NPG poderá contribuir para o desenvolvimento de estratégias que atendam às demandas que os cidadãos impõem à gestão, algo até então pouco efetivado pelos modelos existentes de administração pública. Dessa forma, este trabalho objetiva levantar dimensões para a análise da NPG, ainda carente na literatura. Conforme demonstrado, existem fundamentos para isso, entre eles: democracia deliberativa, coprodução e valor público. Por meio desses pressupostos, são analisadas dimensões sobre dois focos principais: estrutura e processos. Verificou-se que a NPG utiliza ferramentas dos modelos administrativos anteriores, mas busca sua ênfase dentro dos princípios democráticos e de legitimidade. Por fim, ressalta-se a necessidade de estudos teóricos e empíricos que possibilitem maior compreensão dessas ferramentas, permitindo uma administração pública mais inclusiva e com ações mais legítimas.

Abstract Background Enteric fever is common in southeast Asia. However, there is little information on the circulating Salmonella enterica strains causing enteric fever in Myanmar. Methods We performed antimicrobial susceptibility testing and whole genome sequencing on S. enterica bloodstream isolates from febrile patients aged ≥12 y attending two hospitals in Yangon, Myanmar, from 5 October 2015 through 4 October 2016. We identified the serovar of S. enterica, determined antimicrobial susceptibility and the molecular mechanisms of resistance. We analysed phylogenetic relationships among Myanmar S. enterica isolates and those with isolates from neighbouring countries. Results Of 73 S. enterica isolated, 39 (53%) were serovar Typhi and 34 (47%) were Paratyphi A. All isolates were susceptible to ampicillin, chloramphenicol and trimethoprim-sulfamethoxazole but resistant to ciprofloxacin. We identified mutations in chromosomal genes gyrA, gyrB and parC as responsible for fluoroquinolone resistance. All S. enterica Typhi isolates were of 4.3.1 subclade (formerly known as H58) and formed two closely related genotypic clusters; both clusters were most closely related to isolates from India from 2012. All S. enterica Paratyphi A were lineage C, clade C4 and were closely related. Conclusion Our study describes currently circulating S. enterica serovars in Myanmar, the genetic basis of their antimicrobial resistance and provides a genotypic framework for epidemiologic study.

Ciprofloxacin-resistant Neisseria gonorrhoeae had been rarely detected on Merseyside and when found was associated with β-lactamase producing strains, imported from abroad. However, in August 2000, two cases of infection with ciprofloxacin-resistant β-lactamase-negative strains occurred in sexually unrelated patients with no history of foreign travel. Over the next 18 months a total of 120 patients presented with ciprofloxacin-resistant gonococci, from which 99 patient strains were available for study. Gonococcal DNA was subjected to molecular fingerprinting by polymerase chain reaction amplification followed by Taq1 digestion of their opa genes. Twelve differing opa-types were found, but 79 patients were infected with a single genotype, opa-type 1. The sexual histories of the majority of this group indicated acquisition in Merseyside. This endemic strain was further characterized by having the same amino acid substitutions on gyrA and parC genes. An endemic clone of ciprofloxacin-resistant N. gonorrhoeae has been established on Merseyside necessitating the introduction of ceftriaxone as first-line treatment. Despite the presence of 11 other clones in the city, opa type-1 strains have not yet been displaced, raising the possibility that this strain is endowed with added virulence/endemicity traits or that a number of source patients have not yet been found.

Abstract:
ABSTRACT The preferential use of older antimicrobial agents is, in general, sound public health policy and is meant to maintain susceptibility to newer agents. In the case of fluoroquinolones, however, this strategy is flawed and may actually hasten the spread of Streptococcus pneumoniae strains resistant to newer members of the class. In a mouse thigh infection model, we were unable to isolate clones of pneumococci resistant to the newer fluoroquinolone levofloxacin at 2 × or 4 × the baseline MIC. An initial exposure in vivo to the older agent, ciprofloxacin, allowed straightforward selection of clones resistant to levofloxacin in a subsequent experiment. The original ciprofloxacin exposure generated clones without changes in the parC/E and gyrA/B quinolone target sites almost exclusively but did allow overexpression of a reserpine-responsive pump. While this caused only minimal change in the levofloxacin MIC (0.6 mg/liter to 0.8 mg/liter), it allowed a major change in the mutational frequency to resistance for levofloxacin (<1/108.5 to approximately 1/104.5), which allowed levofloxacin-resistant clones to be isolated in a subsequent in vivo experiment. The reason underlying ciprofloxacin's propensity to select for pump-overexpressed clones is likely related to its hydrophilicity. To preserve the susceptibility of Streptococcus pneumoniae to newer members of the class of quinolones, use of ciprofloxacin for community-acquired respiratory infections should be minimized.
