AbstrakEvaluasi merupakan salah satu rangkaian kegiatan dalam metodologi pengembangan sistem atau aplikasi yang seharusnya dilakukan setelah sistem atau aplikasi yang dibangun digunakan oleh pengguna, salah satu kegiatan yang dapat dilakukan untuk melakukan evaluasi terhadap sistem yaitu dengan melakukan kegiatan audit. Audit dilakukan tidak hanya untuk melakukan evaluasi apakah sistem tersebut sudah sesuai dengan prosedur dan mampu menjaga keamanan informasi, tetapi yang tidak kalah penting yaitu mengevaluasi dampak atau value apa saja yang didapat oleh organisasi dari penggunaan sistem tersebut, apakah perusahaan akan mendapatkan value atau malah mendapatkan kerugian. Penelitian ini berfokus pada hasil dari audit yaitu laporan yang didalamnya memberikan rekomendasi untuk menentukan prioritas audit berdasarkan hasil dari Root cause analysis dengan menggunakan dua tools yaitu Pareto chart dan Fishbone diagram. Setelah menerapkan dua metode tersebut dalam penelitian ini, menurut peneliti gabungan pareto chart dan fishbone diagram lebih tepat diterapkan pada audit berbasis risiko yang mendapati temuan dengan tingkat risiko high atau diatsnya lebih dari lima temuan, sehingga akan memudahkan dalam menetukan prioritas audit yang nantinya bisa dijadikan dasar audit annual planning. Saran dari peneliti untuk penelitian selanjutnya bisa mencoba menggabungkan Pareto Chart dan Relation diagram.Kata kunci: audit sistem, teknologi informasi, Root cause analysis, pareto chart, Fishbone AbstractDetermining System Audit Priority And Information Technology Based On Root Cause Analysis Using Pareto Chart And Fishbone. Evaluation is one of a series of activities in the system or application development methodology that should be carried out after the system or application built is used by the user, one of the activities that can be carried out to evaluate the system is by conducting audit activities. An audit is carried out not only to evaluate whether the system is in accordance with procedures and is able to maintain information security, but what is no less important is to evaluate what impact or value the organization gets from using the system, whether the company will get value or even get a loss. This study focuses on the results of audits, namely reports which provide recommendations for determining audit priorities based on the results of root cause analysis using two tools, namely Pareto charts and Fishbone diagrams. After applying these two methods in this study, according to the combined researchers, the pareto chart and fishbone diagram are more appropriate for risk-based audits that find findings with a high risk level or above more than five findings, so that it will make it easier to determine audit priorities which can later be used as a basis annual planning audits. Abstract:
Construction projects are inseparable from their unique management. The unique management of construction projects makes it difficult to maintain their quality. One of the tools to measure quality in project management is the Pareto chart. Pareto chart is a statistical technique that is usually used for decision making based on the frequency of events compared to the causes of events. This research was conducted to analyze the causes of repair work on project X by using Pareto chart. The conceptual framework used in this study is based on two approaches, namely literature and field observations. The main literature for analysis is the Pareto chart then field data collection on each component first which is then categorized based on the type of work. After the data is categorized, a Pareto chart will be made from the data. The processing results obtained two dominant types of errors in the casting work, namely cracks and unevenness of the casting results. Cracks and unevenness of the casting results can be overcome by the chipping method then followed by grouting if necessary. The chipping method is used to repair porous concrete or flatten the casting if it swells, and the grouting method to fill cracks and chipping results with a mixture of cement and water until the surface becomes flat and the casting work results are better.

Abstract:
Professionals in the field of analysis of quality systems is widely known for the Pareto principle and the corresponding graphs showing the contribution of private reasons in the overall picture of the functioning of the system. Pareto chart and the associated cumulative Lorenz curve are used to illustrate the dominant alternatives in terms of their total number. The reason for the popularity of Pareto charts lies in the objectively obvious visibility of the apparatus when nagoricino the analysis of functioning of complex systems. The aim of this work was to determine the possibility of using Pareto analysis to identify the causes that affect the quality of the students in the learning process of knowledge. Students were asked to analyze their own learning with the use of Pareto charts the results of activities and causes. In the first stage, the students in the scoring books expected average score in all disciplines, included in the above blocks. The obtained information was used to build the Pareto chart of learning outcomes. In the second phase, the students formulated a list of the most probable reasons for low (or high enough) progress on the most problematic unit, previously identified and evaluated the significance of each reason on a 100-point system. Addressing the identified causes of failure the student can't and requires improvement of the curriculum, and improving logistical support of the educational process. Pareto analysis, successfully used industrialized countries in assessing the quality of products, efficient and at the analysis of quality performance as well as allows to reveal problem places of the educational process.

Introduction/Main Objectives: This study determines the quality control of CV. Mawaheejra, determine the factors causing product defects and the application of Statistical Quality Control (SQC) methods. Background Problems: CV. Mawaheejra has carried out control but the quality control is not optimal because of poor quality and errors often occur in the production process which causes product defects that not comply with the quality standards. Research Methods: This study uses a quantitative method. It carried data collection techniques through observation, interviews, documentation, and literature studies. Data analysis using Statistical Quality Control (SQC) methods of flowcharts, check sheets, Pareto charts, control charts, and fishbone diagrams. Finding/Results: Based on the results of research with check sheet aids found four types of defects. In the Pareto chart found the highest number of product disability percentage of 38.47%. From the control chart result, it found deviations because of the U control chart there are three points that exceed UCL and two points that exceed LCL. From the fishbone diagram, there are five factors that are most prominently caused by human and machine factors. Conclusion: Based on the results of the analysis, the company must conduct supervision of employees and machine maintenance to reduce product defects.

To solve the drawbacks that the existing Pareto chart do not consider the fuzzy attribute of quality and imbalance of the improvement opportunities for quality defects, the nonconformity membership analysis method was introduced to lead a classification analysis on quality defects data, which increased the flexibility of data analysis, and Pareto priority index was introduced to assess improvement opportunities for quality defects, which increased the rationality during decision-making for quality defective items to improve. The two ways were both integrated to the analysis instance of equipment maintenance quality data based on fuzzy weighted Pareto chart, then weights and dynamic threshold were set to get different results of the Pareto analysis. The presented method can provide effective method support for organizations to scientifically analyze equipment maintenance quality data and accurately determine the direction of quality improvement.

Abstract:
Statistical Process Control (SPC) is a process used to monitor standards, make measurements, and take corrective action on products or services being produced. This research aims to determine the factors that cause damage to the packaging of ground coffee products at CV. Dewi Starindo and applying the SPC method to reduce the number of packaging rejects along with improvements. This research method uses Statistical Process Control (SPC) to determine whether the packaging quality is within control limits or not. Data analysis uses p control charts (p-chart), Pareto diagram, fishbone chart. The data used is the number of rejected products and packaging from July to September 2023. The research results show that the types of defects found in product packaging are slanted labels, open seals, inappropriate weight and holes. 5 data are outside the control limits and 3 data are within the control limits. The Pareto diagram shows that the largest reject rate is from unsuitable heavy reject types, 55.56%, and the smallest for perforated packaging, 1.94%. As for fishbone chart, the main factors causing rejects come from humans, materials, and machines. After improvements were made with suggestions in the form of providing direction and training, installing written SOPs, carrying out calibration and maintenance on machines, and checking the materials used, the Pareto diagram showed that there was a decrease in the number of rejects, from 55.56% to 47.76%. It shows that there is no more data that is outside the control limits.

 Statistical Process Control (SPC) merupakan proses yang digunakan untuk mengawasi standar, membuat pengukuran dan mengambil tindakan perbaikan pada produk atau jasa yang sedang diproduksi. Penelitian ini bertujuan untuk mengetahui faktor-faktor penyebab kerusakan kemasan produk kopi bubuk di CV. Dewi Starindo dan menerapkan metode SPC dalam mengurangi jumlah reject kemasan beserta usulan perbaikannya. Metode penelitian ini menggunakan Statistical Process Control (SPC) guna mengetahui kualitas kemasan dalam batas kendali atau tidak. Analisis data menggunakan peta kendali p (p-chart), diagram pareto, fishbone chart. Data yang digunakan, yaitu jumlah produk dan kemasan reject pada bulan Juli hingga September 2023. Hasil penelitian menunjukkan bahwa jenis cacat yang terdapat pada kemasan produk kopi bubuk Nusantara adalah label yang miring, segel terbuka, berat yang tidak sesuai dan berlubang. 5 data yang berada di luar batas kendali dan 3 data di dalam batas kendali. Diagram pareto menunjukkan bahwa tingkat reject terbesar adalah pada jenis reject berat yang tidak sesuai, yaitu sebesar 55,56% dan terkecil pada kemasan berlubang, yaitu 1,94%. Adapun pada fishbone chart diketahui faktor utama penyebab reject berasal dari manusia dan mesin. Setelah dilakukan perbaikan dengan saran berupa memberikan arahan dan pelatihan, memasang SOP tertulis, melakukan kalibrasi dan pemeliharaan pada mesin, dan memeriksa bahan yang digunakan, diagram pareto menunjukkan bahwa terjadi penurunan jumlah reject, dari 55,56% menjadi 47.76%. Setelah dihitung kembali menunjukkan bahwa tidak ada lagi data yang berada di luar batas kendali.

Under climate change conditions smallholders’ farmers are compelled to change their livelihood strategy as existing are not suitable for new changing situations. So, alternative income sources and diversification of farm enterprises extensively help to get income. In this situation, it is important to know the personal-economic factors which influence small and marginal farmers in choosing the livelihood option for the future. The present study was conducted to identify major determinants influencing farmers’ livelihood options for the future in climate-vulnerable districts of Assam. Cross-sectional survey method was adopted for the study. The study was conducted in Assam state of India. The total sample size was 300 small and marginal farmers. Farmers’ choice for alternate livelihood options was collected through open-ended questions. Farmers’ six characteristics were selected by categorising them into different sub-categories to find out their dominant factors. Pareto chart analysis was done to find out the dominant factors. The findings show that farmers chose 18 livelihood strategies though all were not dominant. Pady, piggery, fishery and vegetable cultivations were major enterprises chosen by the respondents. The Pareto chart shows that among the chosen enterprises their different characteristics played a major role. The Pareto analysis also indicates those four factors along with the determinants identified where the focus should be given to get good results for livelihood strategies. The study recommended that Pareto chart analysis can be used as a tool in analysing farmers’ choices of livelihood strategies.

Abstract:
This study is a monitoring analysis of COVID-19 in Africa. The data used for the study is sourced from the Africa Centre for disease Control (Africa CDC) as t 10:00PM on the 24th of April, 2020 which comprises number of Africa countries with laboratory confirmed cases, number of death and number of discharged/recovered cases. The quality pandemic monitoring/control tools used in this study is fish-bone diagram, Pareto analysis, control chart, bar chart and pie chart. The fish bone diagram depicts the likely symptoms to check out for in a patent infected by COVID-19; the Pareto analysis shows that 14 countries to the left of this line (South Africa, Egypt, Morocco, Algeria, Cameroon, Ghana, Ivory Coast, Djibouti, Tunisia, Nigeria, Guinea, Niger and Burkina Faso) constitute 80% of all the infected countries; the trend analysis shows that the spread of the pandemic is still on an increase rate; and lastly, from the performance assessment, it is seen that the pandemic is still under control from the pie chart while the death rate is already out of control.

Abstract:
Statistical process control (SPC) which includes very powerful techniques used in other mature engineering disciplines for providing process control is not used by many software organizations. In software engineering domain, SPC is currently utilized only by organizations which have high maturity levels according to the process improvement models like CMM, ISO/IEC 15504 and CMMI. Guidelines and software tools to implement SPC techniques should be developed for effective use and dissemination of SPC especially for low maturity organizations. In this thesis, a software tool (SPC-AAT) which we developed to assess the suitability of software processes and metrics for SPC and use of SPC tools is presented. With SPC-AAT, we aim to ease and enhance application of SPC especially for emergent and low maturity organizations. Control charts, histograms, bar charts and pareto charts are the supported SPC tools for this purpose. We also explained the validation of the tool over two processes of a software organization in three case studies.

El presente trabajo se enfoca en la mejora de la calidad concerniente a los elementos estructurales de un centro comercial, a efectos de reducir y minimizar las No Conformidades más relevantes que se presentaron en obra. El proyecto se basó en la construcción y ampliación de locatarios de un centro comercial que tendrá como fin su alquiler. Mediante la aplicación del Análisis Causa Raíz y con las herramientas de diagrama de Ishikawa y diagrama de Pareto, se pudo encontrar las posibles causas del incumplimiento de la calidad en los elementos estructurales, las que subsecuente se validaron a fin de determinar acciones correctivas. En el primer capítulo se señala el planteamiento del problema, problemas principales, secundarios, justificación del estudio, limitación y los objetivos generales y específicos. En el Segundo Capitulo se señala el marco teórico, donde menciona la calidad en el Perú, la gestión de la calidad total, los costos de la calidad en la construcción, ingeniería de la calidad y definiciones. En el tercer capítulo se indica la utilización del Análisis Causa Raíz, las herramientas Diagrama Causa Efecto y Diagrama de Pareto. En el cuarto capítulo, se presenta el desarrollo del análisis de causa raíz mediante una secuencia de pasos. En el quinto capítulo, La Evaluación Económica, Presupuesto de obra, Costo de Reparación y Análisis del Gasto Incurrido. Finalmente, en el capítulo seis, se presentará las conclusiones y recomendaciones del presente trabajo.<br>This work focuses on quality improvement concerning the structural elements of a shopping center, in order to reduce and minimize the most relevant Non-Conformities that occurred on site. The project was based on the construction and expansion of tenants of a shopping center that will be rented as its purpose. By applying the Root Cause Analysis and using the Ishikawa diagram and Pareto diagram tools, it was possible to find the possible causes of quality noncompliance in the structural elements, which were subsequently validated in order to determine corrective actions. In the first chapter the problem statement, main and secondary problems, justification for the study, limitation and general and specific objectives are indicated. In the Second Chapter the theoretical framework is pointed out, where it mentions the quality in Peru, the total quality management, the costs of quality in construction, quality engineering and definitions. The third chapter indicates the use of Root Cause Analysis, the Cause Effect Diagram and Pareto Diagram tools. In the fourth chapter, the development of root cause analysis is presented using a sequence of steps. In the fifth chapter, The Economic Evaluation, Construction Budget, Repair Cost and Incurred Expense Analysis. As result, the low quality and high instability of processes create a lot of waste in the sector. This is intensified when it happens during the harvesting, one of the activities that most contributes to the production costs of a forestry business. This study aimed to evaluate the predictability of timber harvesting process based on critical points identified in the thinning and harvest operations. For this, research was divided in three stages. The first one was mapping the harvesting process and the elaboration of fluxograms. The second stage was the identification, through interviews, and assessment of critical points using the Failure Mode and Effect Analysis (FMEA) and Pareto chart. The third stage was the evaluation of the process using Statistical Process Control (SPC) through attributes and variable in the main failures. In the interviews conducted with workers, seven critical points were identified: damage to the remaining trees, sorting, stump height, dirt in load, knot on the second log, tree left in the field and safety. The evaluation with Pareto chart showed that 80% of the failures identified during harvest are attributed to three causes: damage to the remaining trees, sorting and stump height. The FMEA analysis showed that the failure with highest risk index was knots on the second log, followed by safety. For evaluation with the SPC, damage to remaining trees, sorting and stump height were selected. In general, control charts showed that the forest harvest process was considered unstable and unpredictable, even though it is within the limits defined by the company<br>No setor florestal brasileiro, algumas organizações não acompanham o ritmo do desenvolvimento de outros setores e da adoção de ferramentas e metodologias de gestão da qualidade. Em função deste atraso, a baixa qualidade e alta instabilidade dos processos, geram grandes desperdícios nas organizações. O fato se agrava quando ocorre na operação de colheita da madeira, uma das atividades que mais contribui nos custos de produção da empresa florestal. Este trabalho objetivou avaliar o processo de colheita florestal de uma empresa quanto a sua previsibilidade com base nos pontos críticos identificados nas operações de desbaste e corte raso. Para isso, a pesquisa foi dividida em três etapas, a primeira foi o mapeamento do processo de colheita e elaboração de fluxogramas. A segunda etapa foi a identificação, por meio de entrevistas, e avaliação dos pontos críticos utilizando a metodologia Failure Mode and Effect Analysis – FMEA e gráfico de Pareto. A terceira fase foi a avaliação do processo empregando o Controle Estatístico do Processo (CEP) por atributos e variáveis nas principais falhas. Nas entrevistas realizadas com os colaboradores, sete pontos críticos foram apontados: danos às árvores remanescentes, sortimento, altura de toco, sujeira na carga, nó na 2ª tora, árvores deixadas no talhão e segurança. Na avaliação com o gráfico de Pareto, observou-se que cerca de 80% dos problemas identificados na colheita da empresa são atribuídos às três primeiras causas supracitadas. A análise com o FMEA indicou que a falha com maior índice de risco foi o nó na 2ª tora, seguido da segurança. Para a avaliação com o CEP, selecionou-se os pontos críticos: danos às árvores remanescentes, sortimento e altura de toco. De maneira geral, os gráficos de controle indicaram que o processo da colheita florestal, apesar de estar dentro dos limites especificados pela empresa, foi considerado instável e não previsível

Magister Commercii - MCom<br>This research project focuses on the application of process improvement techniques in a clothing manufacturer to address delay problems in workflow in the factory. The objective of the research is threefold; investigate delays at the beginning of production and make suggestions, show the usefulness of continuous improvement techniques in improving activities in a clothing manufacturer and demonstrate how action research can be used in doing research in production and operation management. Using tools such as flow charts, check sheets, pareto analysis, fishbone diagrams, interviews and the "ask why five times" tool, an investigation into delays led to a second investigation into sewing defects. This established that these sewing defects are caused mainly by time constraints, the malfunctioning of machines, the wrong handling of garments, and previous operations. After an investigation using the above tools it was established that these defects could be addressed by setting realistic targets, doing regular maintenance on machines, cautioning operators to be more careful during their operations, and encouraging regular checks on garments before the next operation. The action learning methodology led to the following lessons; selecting a correct measuring tool is important, that not all tools need to be used, and that it takes time to do a research project using this method.

У сфері міжнародного туристичного обміну річний оборот складає понад 2 трлн. дол. США. На туризм припадає 12% загальносвітового національного продукту, 7% світових інвестицій. За валютними надходженнями туризм обійшов експорт нафтопродуктів, телекомунікаційного устаткування, автомобілебудування і всіх інших видів послуг. Дані 2014-2016 рр. свідчать про падіння попиту на послуги туризму з таких причин: економічна та політична нестабільність країни, високий рівень інфляції національної валюти, падіння платоспроможності населення, військові дії (ООС) на сході країни, анексія Криму. Ці загрози та ризики існують і тепер.<br>The paper considers theoretical aspects: tourism is one of the most dynamic branches of the world economy. According to the World Tourism Organization (WTO), tourism ranks first among the sectors of the world economic complex in terms of exports of goods and services. In the field of international tourist exchange, the annual turnover is over 2 trillion. dollars USA. Tourism accounts for 12% of the world's national product, 7% of world investment. By currency income tourism has bypassed the export of petroleum products, telecommunication equipment, automotive and all other types of services. The data for 2014-2016 indicate a fall in demand for tourism services for the following reasons: economic and political instability of the country, high inflation of the national currency, declining solvency of the population, military actions in the east, annexation of the Crimea. These threats and risks exist now.

This book offers a comprehensive linguistic evaluation of the 376 personal names attested in the roughly 600 Aramaic inscriptions of Hatra, the famous Northern Mesopotamian city that flourished in the Parthian age, between the 1st century BC and the 3rd century AD. This study benefits from the publication of many Hatran inscriptions during recent decades, which have yielded rich onomastic data, and some fresh readings of these epigraphic sources. This work is subdivided into three main parts: an “Onomastic Catalogue”, a “Linguistic Analysis”, and a “Concordances Section”. The “Catalogue” is organized as a list of entries, in which every name is transliterated, translated (whenever possible), discussed from an etymological perspective, provided with onomastic parallels, and accompanied by its attestations in the Hatran Aramaic corpus. The “Catalogue” is followed by a “Linguistic Analysis” which describes, firstly, the principal orthographic, phonological, morphological, and syntactical features of Hatran names. The linguistic discussion proper is followed by a semantic taxonomy of the names which make up the corpus and an overview of the religious significance of the theophoric names. “Charts of Concordances” end the book.

Sense and Sadness is a story of the living practice of Syriac chant in Aleppo, Syria. To understand and explain this oral tradition, the book puts forward the concept of the emotional economy of music aesthetics, an economy in which the emotional and the aesthetic interrelate in mutually indicative ways. The book is based on observing chant practice in the Syrian Orthodox Church in contemporary contexts in the Middle East and beyond, while keeping as its nexus of analysis the Edessan chant of St. George’s Church of Hayy al-Suryan and focusing on Passion Week. It examines written sources on the music of Syriac chant in light of ethnographic analysis, thus combining various modes of knowledge on this problematic subject. This historically informed reading of an early Christian liturgical tradition reveals contemporary modes of significance in the dynamic social and political surroundings of a community that endures exile after exile. The book thus places the music, and its subject(s), in a global context the only stable element of which is uncertainty. The first of the book’s four parts addresses issues of contextuality, such as geographic and temporal situationality, along with musical complexity in conceptions of modality. The second and third parts address overlapping modes of knowledge and value, respectively, in the musical ecclesiastical enterprise. The final part brings together the book’s subthemes. This chapter explores literary influence and the idea that literature can and perhaps even must be read as an evolving system with certain inherent rules. Attempts to demonstrate literary imitation, intertextuality, and influence have relied almost entirely upon close reading. To chart influence empirically, we need to go beyond the individual cases and look to the aggregate. Information cascades theory provides an attractive framework for modeling literary influence and intertextuality at scale. This chapter discusses the results of the author's thematic-stylistic analyses of nineteenth-century novels using Gephi software to identify signs of historical change from one book to the next. The data reveal that the corpus appears to behave in an evolutionary manner. At the macro scale, we see evidence that theme and style are influenced by time and author gender. The findings suggest that a writer's creativity is tempered and influenced by the past and the present, by literary “parents,” and by a larger literary ecosystem.

This book charts the social historical development of the English public house from the period of the Restoration to the twenty-first century, culminating in the contemporary gastropub. Continuities and differences between taverns, inns, and (gastro)pubs are highlighted, with a focus on issues around food, drink, and sociality. The analysis of food and eating out encompasses their material, as well as their symbolic properties, both historically and at the present time. One recurring theme is the constant contest between English and French cuisine for diners’ allegiance. The book studies the gastropub in the context of large-scale pub closing since the 1990s and views it both as reaction to the end of the traditional drinking pub and as a promising alternative to it. The subordinate relation of the pub to both breweries/pub companies and to the regulatory and taxing state is presented as contributory to pubs’ decline. To address this challenge, this study develops a maintenance framework that power-generating industries in South Africa can implement. Primary data was collected from company X that is a major subsidiary of the power generating industry in South Africa to probe the root causes of power outages in recent times. The analysis of the data obtained was carried out using Pareto chart. The results obtained identified delay in maintenance operations as one of the possible causes of power outages. The rework job was observed to be tardy by 50 days. This length of days is critical enough to cause power outage. To address this, a framework was developed to assist in effective scheduling of maintenance activities. Hence, this work provides an insight into the possible causes of power outages in South Africa and how they can be mitigated to avoid load shedding.

AbstractThe productivity of a manufacturing organisation is limited by myriads of process wastes generated in this organisation. In light of this, the aim of this study is to prioritise various process wastes generated in a manufacturing organisation. In order to achieve this, on the one hand, a Hybrid Structural Interaction Matrix (HSIM), which is premised on the theory of subordination via systems thinking was deployed to carry out the process wastes pairwise ranking and weighting analysis. On the other hand, the Pareto Chart, was thereafter deployed to ascertain the vital few process wastes contributing to productivity loss experienced in a manufacturing organisation. A case study of the process wastes generated in an Electronic-Product Manufacturing organisation was used to validate the process wastes prioritisation model developed in this study. The result of the HSIM prioritisation analysis revealed that the intensity rating scores of the process wastes; overproduction, excess inventory, defect, motion, transport, waiting and over-processing limiting the productivity of an organisation are 7.53, 4.59, 6.06, 1.65, 3.12, 0.18 and 9 respectively. The result of the validation exercise revealed that transport, excess inventory and defects are the core process wastes that limit the productivity of an Electronic-Product Manufacturing organisation considered in this study. With this approach, operations managers of a manufacturing organisation would obviously reduce errors in the rating of process wastes, which is vital towards achieving continuous productivity improvement and sustainable manufacturing.

Abstract This chapter provides a real-life example of a quality improvement event from its inception to completion. It reviews how to identify a quality gap, assemble an interprofessional team, create a SMART aim statement, perform process analysis, perform cause analysis, select metrics, and navigate organizational change. The chapter provides examples of quality improvement methodologies such as Lean, Six Sigma, the Institute for Healthcare Improvement (IHI) Model for Improvement, and the Plan-do-Study-Act (PDSA) cycle. The chapter describes Lean Management System concepts such as Type 1 and Type 2 waste, value-added and non–value-added processes, mistake-proofing, and Taiichi Ohno’s seven wastes. It reviews examples of cause analysis, including Pareto charts, Five Why’s, and the fishbone diagram. It discusses three types of metrics used to understand the success of quality improvement projects: process metrics, outcome metrics, and balancing metrics. Finally, this chapter discusses how to transform a quality improvement project into organizational change and provides insight on how to sustain these changes in your organization.

Absrract This paper provides a real case study in regard to controlling the welding repair rate and NDT backlog in a company mega-project assigned to a new contractor. This case study shows the improvements in both KPIs, through the utilization of various quality and improvement tools, such as root cause analysis (Fishbone and Pareto chart), brainstorming, and PDCA cycle. The collaborative efforts between the client and contractor, including the welding and NDT subcontractors, resulted in reducing the WRR from 11.04% to below 5% (the set KPI). The NDT backlog was also controlled and reduced from 2500 joints to below 600 joints (less than one week production).

Bakpia is made from green beans and sugar wrapped in flour. Bakpia Latief Kediri is a bakpia industry in the City of Kediri. The company needs help in the production process activities. There are still products that need to be fixed. The types of it defects in the company are burnt and cracked. This study aimed to determine the performance of its quality control, determine the most dominant types of defects in its products, and provide suggestions for improving its production process. The research method uses the Statistical Quality Control method. Data is taken in the form of primary data and secondary data. Primary data was collected by interview and observation. Interviews were conducted using in-depth interviews. Observations were made for 30 days. Data analysis uses statistical tools, which include check sheets, histograms, fishbone diagrams, Pareto diagrams, and control charts p. Bakpia quality control at Bakpia Latief is under control because the p chart control chart shows that only one point is out of the control limits. The most dominant defect in its products is burnt, with an average percentage of 0.617%. Corrective actions to reduce the existence of defective products, namely repairing and maintaining production machines, improving the stages of the production process, developing human resources, and repairing production facilities. Numerous authors have demonstrated the effectiveness of the risk-based approach in the mining sector. However, the majority of earlier research has ignored the role that auxiliary machines, in total risk management play in favor of heavy machinery. This study aims to analyze backhoe loader maintenance data in order to offer a risk assessment approach for auxiliary machinery in open-pit mines. The severity, occurrence, and detection of failures were the three component indicators that together determined the overall risk, as per the FMEA method. The purpose of the Pareto chart was to grade the detection indicator by illustrating the failure type distribution and distinction. The downtime data statistical testing results enabled analytical calculation of the system's reliability and mean downtime, which in turn allowed for the evaluation of the frequency and severity of failures. As a result, a framework for evaluation was put out, supported by data that was gathered, and included a three-dimensional risk assessment matrix. The focus of further research efforts should be on expanding the present sample data in sense of quantity and also to analysis of other machines in the whole mining system.

Abstract:
The research presented here addresses agricultural production in Brazil over four decades, highlighting the importance of the sector for the country's economy and the need for advanced techniques for analysis and forecasting. Using data from the Food and Agriculture Organization of the United Nations (FAO), the study assesses long-term trends and the productive efficiency of different crops between 1980 and 2019. The primary objectives include analyzing the total production of agricultural items, assessing the harvested area and the ratio of production to harvested area, in addition to identifying significant changes in the most produced items using the Pareto Chart and testing the normality of the data with the Shapiro-Wilk test. The methodology applied involves statistical and Machine Learning techniques, such as Random Forest and Support Vector Machines (SVM), to forecast agricultural production and identify complex patterns in historical data. The main results indicate robust growth in the production of items such as sugarcane and oilseeds, reflecting the expansion of agribusiness and the adoption of modern technologies. In contrast, cereal production showed significant fluctuations. The analysis of productive efficiency showed an increase in the ratio of production per harvested area, especially after 2000, attributable to improvements in agricultural practices and the use of advanced technologies. However, Machine Learning models presented limited predictive performance, with high MSE and negative R² values, highlighting the need for more robust approaches and the integration of additional data, such as climate information. These results highlight the importance of sustainable agricultural policies, continued investments in technology, and risk management strategies to promote sustainability and efficiency in the Brazilian agricultural sector in the long term.

Abstract Motor twist-off in the Bottom Hole Assembly (BHA) is a major challenge in the oil and gas industry, particularly during the drilling of highly deviated sections. This issue frequently occurred in Field N, Oman, while the drilling/tripping 8.5" highly deviated Hole Section. Fishing operation was required to retrieve the detached motor part, which is costly, time consuming, and has no guaranteed success. In some cases, this led to the need for cement plugs or sidetracks to meet well objective. To address this, an initiative was launched to study motor twist-offs using Practical Problem-Solving (PPS) Tool and recommended effective solutions. The PPS study collected data including drilling parameters, daily drilling operation reports, MWD vibrations data, and bending moment stresses analysis. Statistical analysis was performed to map the issue and identify occurrence pattern. Pareto Chart and IS / IS NOT Analysis were utilized to compare the wells that have motor twist-off incidents to those in the same field without incidents. Fishbone and 5 whys techniques were employed to identify and prioritize the root causes. Potential countermeasures were assessed based on benefits versus efforts and an action plan was developed to implement these countermeasures in 30 horizontal wells. The processes were evaluated &amp; the new practices were standardized at PDO. The PPS Tool identified several root causes of motor twist-off, including incompatible BHA design, high stress on the saver sub, and insufficient BHA stabilization, inadequate hole cleaning practice at the critical angles led to high vibrations, as indicated by MWD data showing hole enlargement and BHA swinging. Traceability issue was also found, such as improper inspection of the drilling motor and Saver sub, as life hours exceeded at the time of twist-off. The recommended countermeasures included adding a string stabilizer above the motor to reduce bending stress, standardizing motor bend settings, and establishing a new hole cleaning standard. These measures were piloted in multiple wells in Field N with promising results. The successful implementation of these countermeasures eliminated motor twist-off events, resulting in significant cost savings. This study not only optimized the drilling and tripping operations of the 8.5" horizontal Section but also delivered wells safely and without nonproductive time. The practices developed serve as a valuable guideline and can be replicated in other horizontal wells both within and outside Oman, to reduce well construction costs.

Lentil is one of the important pulse crops in the world with a high proportion of easily digestible protein. However, there are several pests and pathogens which cause losses during crop growth, harvesting and post-harvest storage. Microwave processing has shown great potential to eradicate pests and pathogens from grains; however, there is still concerns about its heat uniformity, which is of crucial importance in pest and pathogen control. Fluidization using forced hot air is one of the potential solutions for having uniform temperature during microwave processing. In this study, a single mode microwave cavity, with a 2.45 GHz microwave source, was modified to have a microwave fluidized bed and used to evaluate its potential to eliminate the Botrytis grey mold (BGM) pathogen, which is one of the important seed-borne pathogens of lentil crops in Australia. Air speed was maintained to be constant during the experiment and was just enough to fluidize 100g of red lentils in the sample holder. Two wet based (w.b.) seed moisture contents (m.c.) of 10.5% and 18.5% were prepared and the process parameters were selected as: air temperature at 50 and 60°C; microwave power at 0, 300, 400 W for 18.5% m.c. and 0, 400, 500 W for 10.5% m.c.; and exposure times of 5 and 10 min. These parameters were chosen to reach the final temperature of below 70°C. The effect of process parameters on seed moisture loss, seed germination, electrical conductivity of seed soaking water and percentage of infected seeds (IS%) were analyzed using general factorial regression and analysis of variance. The results showed that the most effective factors on moisture loss, after seed moisture content, was exposure time, followed by microwave power and air temperature. While final bed temperature was affected mostly by air temperature, and then by microwave power. Furthermore, based on general full factorial regression and pareto chart of standardized effects, moisture content had by far the most influence on the reduction of IS%. Seed pathogen inoculum reduction, without significant seed viability loss, was obtained by applying microwave power of 300W and set air temperature of 60°C (actual inlet air temperature of 57±1°C) on seeds with m.c. of 18.5% for 10 min. This gave a 27% reduction in IS% (from 82% to 55%).

Abstract SRAM bitmapping and failure analysis have been used as a driver for continuous yield improvement during pre-qualification manufacturing of a microcontroller. The combination of the embedded SRAM electrical failure data and failure analysis results was used to generate a pareto of failure modes and failure mechanisms and establish a correlation between the two. Bitmap trend charts can be used as a manufacturing line monitoring tool to supplement traditional in-line inspection. Identification of manufacturing issues can be obtained from bit failure information and compared with in-line inspection results to quickly identify which specific process module is responsible for a significant yield loss.

Isentropic analysis in this country originated with a particular purpose in view, namely as a means of using moisture distribution to determine flow patterns in the atmosphere It revealed, very successfully, certain theoretically anticipated patterns. Subsequently it has come into general use in connection with upper-air analysis but for the most part its application is dominated by the original particular purpose. A rather different approach is to use isentropic analysis in a more purely descriptive fashion as the principal tool for upper-air analysis. This demands that an isentropic chart represent synoptically as much useful information as possible and that all phases of its preparation receive due care.

Electric and natural gas utilities often find it advantageous to share rights-of-way. Available methods for evaluating electrical effects on gas pipelines have been difficult to use at best, and at worst, incorrect. A generalized approach that addresses inductive and conductive interferences has not been available. Initiated to fill that need, this work is part of a research effort cosponsored by EPRI and the Pipe Line Research Council International, Inc. (PRCI) �A generalized approach to the analysis of the effects of transmission line faults on natural gas transmission pipelines has been developed and is presented in this report. A state of the art user-friendly computational tool has been developed and verified for the analysis of interference between electrical power lines and nearby buried or aboveground pipelines. This computer program, ECCAPP, is distinguished by its ability to model and analyze accurately complex, realistic interactions between pipelines and power lines, using easily obtained input data. The final report consists of three volumes. An independent fourth volume was also developed to simplify the installation of the ECCAPP software.Volume 1 contains the theory upon which the ECCAPP computer program is based. A parametric analysis and graphical charts have been formulated using ECCAPP to permit estimates to be made in the field or during preliminary analyses for situations that are not too complex. A discussion of various useful mitigation methods is included. The discussion is based on previous research work and on the results of the parametric analysis.Volume 2 is a detailed user's manual which describes not only how to use the program itself, but also which engineering data must be sought during an analysis and how to assimilate it into a computer model. A detailed sample problem is included. A detailed \Glossary of Terms\" used by ECCAPP as well as suitable input data forms to be filled by power line and pipeline engineers are provided in the appendices.Volume 3 discusses the modeling and performance of pipeline insulation or coating.

The flexibility of polybenzoxazine (BA) can substantially be improved by alloying with IPDI-based urethane prepolymers (PU) or with flexible epoxy (EPO732). The experimental results, i.e. flexural testing and dynamic mechanical analysis, reveal that flexibility of the rigid polybenzoxazine and the PU or th EPO732 alloys systematically increases with the amount of both tougheners due to the added flexible molecular segments in the polymer hybrids. The curing temperature of the benzoxazine resin at about 225 degree celcius shifts to a higher value when the fraction of BA in both resin mixtures decreases. Interestingly, the synergism in glass transition temperature (Tg) obtained from the peak of loss modulus in the dynamic mechanical analysis of BA/PU alloys is clearly observed, i.e. Tg of the BA/PU alloys are significantly greater (Tg beyond 200 degree celcius) than those of the parent resins, i.e. BA (Tg = 165 degree celcius) and PU (Tg = -70 degree celcius). However, this synergistic characteristic is not observed in the BA/EPO732 alloy systems. The synergistic Tg of BA/PU alloy at 70/30 mass ratio is found to be 221 degree celcius while that of BA/EPO732 at the same mass ratio is reported to be of only 95 degree celcius and further decreases with the amount of epoxy fraction. The phenomenon is explained by the ability of the PU fraction to substantially enhance the crosslinked density of the polymer alloys thus create the more connected network structure of the resulting alloys whereas the EPO732 shows almost negligible effects on the alloys crosslinked density or even reduce the alloy’s crosslinked density when the amount of this flexible epoxy is greater than 50wt%. Furthermore, the degradation temperature based on the 5% weight loss of the TGA thermograms of BA/PU alloys is found to be improved with the presence of the PU while the opposite trend is observed in BA/EPO732 systems. Finally, the char yield of both alloy systems is steadily enhanced with increasing the amount of the benzoxazine fraction due to the inherent high char yield of the benzoxazine resin comparing with the other two tougheners. The toughened polybenzoxazine using IPDI-based urethane prepolymer, e.g. 70/30 BA/PU, is thus the promising system for a tough and high thermal stability polymeric network for bulk or composite matrix applications.
