An abscess of the deep parotid lobe is an uncommon complication of acute parotitis. Characterized by warm erythematous facial skin and ipsilateral cheek swelling, parotid abscesses have often been associated with decreased saliva production and immunodeficiency. We offer a case of a large deep parotid lobe abscess presenting similarly to a peritonsillar mass, causing significant odynophagia and difficulty swallowing. Temporal lobe abscess as a complication of parotid abscess is not described in the English literature. In this case report a 66-year-old gentleman is described who presented with a left-sided parotid abscess, which extended to other deep neck spaces, and advanced to develop a temporal lobe abscess and subdural parietal empyema. Treatment included intravenous antibiotics, incision and drainage of parotid abscess, and burr hole aspiration of the temporal lobe abscess. Lumbar puncture confirmed cryptococcal meningitis. Multiple splenic lesions with radiographic features consistent with cryptococcal splenic abscesses were also seen. We report a case of an abscess from an ’ectopic’ accessory parotid gland in the cheek demonstrated by sialography and computed tomography (CT). The accessory parotid gland was ectopically located anterolateral to the masseter muscle and isolated from the main parotid gland. The orifice and ductal system of the ectopic accessory parotid gland were separated from those of Stensen’s duct. The abscess developed from this ectopic accessory gland, and the main parotid gland was free of inflammation. Using sialography and CT, we confirmed the presence of this ectopic accessory gland.

Conventional surgical treatment involves incising the parotid parenchyma in the direction of the facial nerve until the abscess is located and evacuated. Abstract:

Even in countries like India where the general dictum is to consider Tuberculosis as one of the differential diagnosis to any condition that is still under evaluation. Parotid Tuberculosis is a very rare entity. There are fewer than 200 cases reported in literature worldwide of Parotid gland Tuberculosis, a form of extrapulmonary Tuberculosis. We present an interesting case of Parotid Tuberculosis which presented as unilateral swelling in the parotid region of a four-year-old male child with an underlying abscess.

Inflammation of salivary gland is rare during neonatal period.
