Abstract:
Collagen content and cross-linking are believed to be major determinants of tendon structural integrity and function. Gender and chronic resistance training have been shown to alter tendon function, and may also alter these key structural features of tendon. Patellar tendon biopsies were taken from untrained men (M, n=8, 25±1 y, 1RM: 53±3 kg), untrained women (W, n=8, 23±2 y, 1RM: 29±2 kg), and resistance-trained (10±1 y trained) men (RTM, n=8, 24±2 y, 1RM: 71 ±6 kg). Biopsies were analyzed for dry mass, collagen content, and collagen cross-linking (hydroxylysylpyridinoline, HP). Tendon dry mass was significantly lower in women than men (M: 376±8, W: 343±5 µg dry mass/mg tendon wet wt, P<0.01), and was not influenced by chronic resistance training (RTM: 364±20 µg dry mass/mg tendon wet wt, P>0.05). The lower tendon dry mass in women reduced (P=0.08) collagen content per tendon wet weight (M: 339±14, W: 306±11 µg collagen/mg tendon wet wt). Collagen content of tendon dry mass was not influenced by gender (P>0.05) or resistance training (P>0.05) (M: 903±38, W: 892±29, RTM: 881±43 !,g collagen/mg tendon dry mass). Similarly, cross-linking of collagen was not impacted by gender (P>0.05) or training (P>0.05) (M: 401±47, W: 418±35, RTM: 424±38 mmol HP/mol collagen). In women, the overall lower amount of collagen per tendon wet weight may explain gender differences in tendon function, while collagen content and cross-linking of the dry mass are remarkably consistent across gender and training status.<br>School of Physical Education, Sport, and Exercise Science

Introducción: Determinar la asociación entre el ángulo de inclinación del tendón patelar (AITP) y la presencia de dolor anterior de rodilla (DAR), así como conocer la prevalencia de DAR en deportistas universitarios. Material y métodos: Estudio de corte transversal en 143 deportistas universitarios. La presencia de DAR se detectó usando la Anterior Knee Pain Scale de Kujala y se calculó el AITP analizando una fotografía de ambas piernas con el programa Kinovea ®. Cada rodilla se consideró como una muestra. La razón de prevalencia entre el DAR y el AITP se calculó usando regresión de Poisson robusta. Resultados: Se evaluaron 143 sujetos, por tanto, 286 rodillas, donde 176 (61.5%) presentaban DAR. El AITP se clasificó en cuartiles. Para el análisis se unió el segundo y tercer cuartil, por lo que se obtuvo un cuartil medio (9°y 14°).Se encontró que un AITP menor a 9° tiene 1,34 (IC95%; 1,09-1,65) más probabilidad de tener DAR, y un AITP mayor a 14° tiene 1,35 (IC95%; 1,09-1,69) más probabilidades. Conclusiones: Los deportistas con un AIPT menor a 9° o mayor a 14° tienen mayor probabilidad de presentar DAR. Se recomienda que el profesional considere el ángulo de inclinación del tendón patelar en su evaluación.<br>Introduction: To determine the association between patellar tendon angle (AITP) and the presence of anterior knee pain (DAR) and the prevalence of DAR in college athletes. Materials and methods: Cross-sectional study of college athletes. The presence of DAR was detected using the Anterior Knee Pain Scale by Kujala and AITP was calculated by analyzing a photograph of both legs with Kinovea program ®. Each knee was considered as a sample. Conclusions: Athletes with AITP fewer than 9 ° or greater than 14 ° are more likely to have DAR. It is recommended that the professional considers the inclination patellar tendon angle in the evaluation

Após a lesão, um grupo de animais foi solto na gaiola; o segundo grupo foi realizado mobilização precoce e solto na gaiola sendo continuado a mobilização todos os dias ate o dia do seu sacrificio; e o terceiro grupo foi submetido a uma imobilização do segmento lesado com uma tala de gesso e resina acrílica. Os animais foram sacrificados após 24, 48 horas e 7 dias de lesão. Após o sacrifício, foi coletado o TP e emblocado em parafina. Cortes foram corados pela hematoxilina-eosina e picrossírius. Além disso, nas lâminas de picrossírius foram mensurados o tamanho de ondulações...<br>Tendons are important structures for the movement, being constituted of layes of alonged fibroblasts and fibrocytes separated by large and aligned type I collagen fibers, providing a great tensile strenght. Tendons lesions are frequent, mainly by excessive loading or induced by surgical proceeding for anterior cruciate ligament (ACL) reconstruction. The main source of graft for ACL reconstruction is the patellar tendon. Currently, there are several treatment to improve the tendon repair after an injury, among than we can destak the immobilization and early mobilization of the joint. During the reabilitation of ACL after reconstruction little attention has been given to the repair of tendon donor of the graft. In this study, were investigated the effects of the immobilization in comparison to the early mobilization of the joint on the process of pattelar tendon repair. Forthy eight adult male Wistar Rat were divided into 3 groups of 16 animals. All the animals were submitted to a surgical lesion, under anestesia, in the central part of the left patte1ar tendon. After the lesion, a group of animals were left free in the cavage; a second group had the lesioned joint immobilized; and a third group were submitted to early mobilization of the lesioned joint and left free in the cavage. The mobilized group animals were submitted to dayly series of mobilization. The animals were euthanased 24, 48, and 168 hours after the surgery. Abstract:
Tendon injuries have, for many years, frustrated clinicians and patients alike due to their longevity and resistance to therapy. In recent years there has been good response in the extensor tendons of the lower limb to an intense painful eccentric exercise protocol. As yet there is no established reason known why a tendon should develop degeneration within its structure or why it should respond to the eccentric exercises. We do however know that, like bone, tendons are biologically active and rapidly adapt to the mechanical environment to which they are exposed. Recent investigations have revealed that within a tendon such as the Achilles or the patellar tendon there may be regions that experience different strains to the rest of the tendon. Much of this work has been in vitro and an ultimate goal would be the development of a non-invasive method by which intra-tendinous strain might be measured. The basis of this thesis is the validation of an existing grey-scale speckle pattern matching software programme developed for tracking motion through serial ultrasound images. Through in vitro and in vivo work we have developed its use for tracking the unique type of speckle found in tendons. By verifying, in vitro, that the displacements tracked in phantoms and tendons alike are representative of reality we provide confidence in the use of an exciting tool for measuring tendon motion in vivo. Furthermore, we have established the method by which the tracking can be adapted to accurately represent tendon strain in vitro which again provides assurance for its reliability when applied to examine tendon strain in vivo. The methods of data collection and analysis developed in this study provide the foundations for an exciting avenue of research into tendon biomechanics.

Abstract:
Syfte: Sammanställa och granska studier gällande postoperativ behandling vid ruptur av ligament patella. Detta för att beskriva effekt och kvalitet på studier om postoperativ behandling. Metod: En beskrivande litteraturstudie valdes som design, med sökorden: patellar ligament, patellar tendon, rupture, knee injuries, surgery. Av totalt 1240 träffar valdes 13 relevanta artiklar publicerade mellan 1999-2013 efter sökningar i databaserna PubMed, PEDro, COCHRANE, SCOPUS, CINAHL, SPORTDiscus och AMED. Artiklarna kvalitetsgranskades enligt PEDro scale. Resultat: Två postoperativa behandlingar beskrevs, tidig och sen mobilisering. Antingen placeras knäleden i ett stabiliserande knäskydd som till en början låser knäleden i full extension eller så påbörjas tidig mobilisering av den opererade knäleden. Ingen signifikant skillnad påvisades mellan tidig och sen mobilisering vad gäller effekten på muskelstyrka, rörelseomfång och återgång till tidigare aktivitetsnivå. Vidare var kvaliteten på de granskade artiklarna genomgående låg (PEDro scale ≤ 5). Konklusion: Ytterligare forskning krävs för att fastställa vilken postoperativ behandling som ger bäst effekt.<br>Objective: Summarize and examine studies regarding postoperative treatment following rupture of the patellar ligament. Further the aim was to evaluate the effect of the postoperative treatment and the quality of the studies. Method: As design, a descriptive review was chosen, keywords: patellar ligament, patellar tendon, rupture, knee injuries, surgery. From a total of 1240 items, 13 relevant articles published 1999-2013 were selected after search in the databases PubMed, PEDro, COCHRANE, SCOPUS, CINAHL, SPORTDiscus and AMED. The articles were quality-graded according to PEDro scale. Results: Two postoperative treatments were described, early and delayed mobilization. There was no significant difference between them regarding the effect on muscle strength, range of motion and return to preinjury level of activity. The quality-graded articles had overall low quality (PEDro scale ≤ 5). Conclusion: Further researches are required to determine which postoperative treatment has the best effect.

Understanding the origins of the multiaxial material properties of soft tissues is crucial for quantifying the tissue material properties that govern the material behavior for acurate predictive modeling of the biological systems. Unlike many engineering structural materials, biological materials exhibit complex material behavior due to structural anisotropy and various interactions between each of the microstructrual units of the tissue. Therefore, this study aims to quantify the shear material behavior of rabbit patellar tendon under simple shear loading along the fiber direction and perpendicular to the fiber direction and to understand the role of tissue shearing and its contribution to the overall mechanical behavior of the tendon. It was hypothesized that tendon demonstrates anisotropic material response under simple shear. Results suggests that tendon exhibits direction-dependent viscoelastic shear properties, reflecting structural anisotropy. The data obtained from our present study could be used for development of constituent models using internal state variable theory.

INTRODUÇÃO: A ultrassonografia (US) e a ressonância magnética (RM) representam os principais métodos de diagnósticos por imagem na avaliação do mecanismo extensor do joelho. Há carência de trabalhos que avaliem por US e RM a presença de alterações no mecanismo extensor de jogadores de futebol assintomáticos. OBJETIVO: Avaliar, por meio de RM e US, o tendão quadríceps e o ligamento patelar de jogadores de futebol assintomáticos e dos participantes do grupo-controle. Identificar se há alterações de imagens ou lesões. Determinar se essas sofrem influências pelos seguintes critérios: canhoto ou destro, quanto à dominância ao chute, posição em campo, idade do atleta e tempo de prática esportiva. MÉTODO: Foram avaliados um total de 248 joelhos: 112 eram de 56 de jogadores de futebol assintomáticos e 136 eram do grupo-controle. O grupo composto por jogadores de futebol apresentava idades entre 14 e 34 anos. Já o grupo-controle era composto por indivíduos com idade também entre 14 e 34 anos, principalmente, universitários e funcionários do hospital, com gênero igual aos dos jogadores selecionados, mas sem atividades físicas de impacto, como vôlei, futebol e salto, por exemplo. RESULTADO: Constatou-se a presença de alterações de imagens ou de lesões em nove ligamentos patelares e apenas duas nos tendões quadríceps de jogadores de futebol assintomáticos, tanto pela US quanto pela RM, e nenhum caso alterado no grupo-controle. Houve concordância significativa entre RM e US quanto aos resultados obtidos. Além disso, os jogadores de futebol apresentaram anormalidades (alterações de imagens ou lesões) no tendão quadríceps ou no ligamento patelar significativamente (p<0,05) maior do que no grupo-controle; a idade média dos jogadores com alguma anormalidade é significativamente menor do que a dos jogadores sem lesão (p<0,003); o tempo médio de prática desportiva dos jogadores que apresentam anormalidade é menor do que o tempo médio dos jogadores que não têm anormalidade (p<0,001); os jogadores destros apresentaram anormalidades, enquanto que os jogadores canhotos não (p<0,05). Dentre os jogadores, há correlação entre mais alterações de imagem ou lesão de joelho e menor tempo como jogador e menor idade (R² de Nagelkerke = 0,700). Destros têm risco aumentado de ter anormalidade nas referidas estruturas (Razão de Chances = 15,204) quanto menor for o tempo como jogador de futebol. Os jogadores da defesa têm 4,76 vezes mais chance de desenvolver anormalidade do que atacantes e goleiros (p<0,04). CONCLUSÃO: Os jogadores de futebol assintomáticos apresentam anormalidades no tendão do quadríceps e ligamento patelar detectadas pela RM e US. As alterações estão relacionadas a menor idade, menor tempo de prática desportiva, ser destro e ser jogador de defesa<br>INTRODUCTION: Ultrasonography (US) and magnetic resonance imaging (MRI) are the main diagnostic imaging methods used in evaluation of the extensor mechanism of the knee. Theres a scarcity of works assessing alterations on the extensor mechanism of the knee of asymptomatic soccer players using US and MRI. OBJECTIVE: To assess, by means of MRI and US imaging, the quadriceps tendon and patellar ligament of asymptomatic soccer players and healthy control group; identify abnormal imaging findings or injuries; ascertain whether these are affected by leg dominance (left or right), field position, age, and time spent practicing the sport. METHOD: A total of 248 knees were evaluated: 112 of 56 asymptomatic soccer players and 136 of healthy controls. The players ages ranged from 14 to 34 years. The control group was composed mostly of college students and hospital staff members, also with ages between 14 and 34, matched for gender to the selected soccer players, and who did not engage in high-impact activities such as volleyball, soccer, or jumping. RESULTS: Abnormal imaging findings or injuries were detected, by US and MRI alike, in nine patellar ligaments and two quadriceps tendons of asymptomatic soccer players. No abnormal findings were detected in the control group. CONCLUSION: There was significant consistency between MRI and US findings. Soccer players were much more likely (p<0.05) to have abnormal imaging findings or injuries of the quadriceps tendon and patellar ligament as compared with the control group; the average age of players presenting some abnormality is significantly lower than of those without injuries (p<0.003); the average time spent practicing the sport for players presenting abnormalities is lower than those who dont present injuries (p<0.001); a correlation between the dominant leg and the affected knee was observed, with the players with a dominant right leg presenting injuries while the ones with a dominant left leg not showing abnormalities (p<0.05). Amongst the players there is a correlation between more abnormal imaging findings or injuries on the knee and less professional practicing time and age (Nagelkerke R² = 0.700).Players with a dominant right leg have an increased chance of presenting abnormalities on the quadriceps tendon and patellar ligament (Odds Ratio = 15.204) with a lower average time spent practicing the sport. Defense players have 4.76 times more chance to develop abnormalities than forward players and goalkeepers (p<0.04). CONCLUSION: The asymptomatic soccer players present abnormalities on the quadriceps tendon and patellar ligament identified using ultrasonography (US) and magnetic resonance imaging (MRI). The alterations are related with less age, less time of professional practice, right leg dominance and defense field position

The principal aim of the present thesis was to prospectively follow (clinical status and ultrasound + Doppler findings) the patellar tendons in the young elite volleyball players at the Swedish National Centre for high school volleyball in Falköping. In an Olympic weightlifter with chronic painful jumper´s knee, the effects of treatment with sclerosing injections followed by early instituted very heavy weightlifting training, was also evaluated. First, in a prevalence study, we demonstrated that the clinical diagnosis patellar tendinopathy-jumper’s knee, together with structural tendon changes and vascularisation in the painful area of the tendon, was demonstrated in 12/114 tendons in Swedish junior elite volleyball players, but not in any tendons of individually matched (age, height and weight) not regularly sports active controls. Structural tendon changes alone was demonstrated among the volleyball players but also among the controls. In a 7 months prospective study of a total of 120 tendons, we demonstrated that the clinical diagnosis patellar tendinopathy-jumper’s knee was associated with neovessels/vascularity in the area with structural tendon changes in 17/19 tendons. Seventy tendons that at start were clinically normal, and had normal ultrasound + Doppler findings, remained clinically normal after 7 months with intensive training and playing volleyball. In a 3-year prospective study it was demonstrated that normal clinical tests and normal ultrasound + Doppler findings at school start, indicated a low risk (8%) for these players to sustain patellar tendinopathy-jumper’s knee during the 3 school years with intensive training and playing. In a case study, involving an Olympic elite weightlifter with chronic painful patellar tendinopathy-jumper’s knee, successful treatment with ultrasound and Doppler-guided injection of the sclerosing agent polidocanol, allowed for pain-free very heavy weight training two weeks after treatment. Further heavy weightlifting training on a daily basis, preparing for European Championships, was done without causing tendon rupture and/or pain. Introduction: Exercise-induced muscle damage (EIMD) is an accepted consequence of eccentric exercise. Sex differences in EIMD are attributed to tendon properties, fascicle lengthening and direct hormonal influences, however these remain unreported in vivo. Furthermore, the classical definition of eccentric contractions omits the role of the elastic tendon in determining eccentric fascicle lengthening and subsequent EIMD. Aim: The overall aims of the current thesis were, to investigate the role of the patella tendon during eccentric contractions, to investigate whether muscle and tendon properties are determinants of EIMD and investigate group differences (sex and oral contraceptive (OCP)use) in EIMD. Materials and method: In brief, vastus lateralis (VL) and patella tendon properties were measured in males, females and female OCP users, using a combination of ultrasonography, electromyography and dynamometry. During maximal voluntary eccentric knee extensions ((MVEKE) 12 reps × 6 sets), VL fascicle lengthening and MVEKE torque was recorded every 10 of knee joint angle (20 - 90). Maximal isometric voluntary knee extensor (MVCKE) torque loss, creatine kinase (CK) and muscle soreness were measured pre, post, 48, 96 and 168 hours post damage as markers of EIMD. Main findings: Patella tendon properties appear to act as a mechanical buffer on VL fascicle lengthening during MVEKE in vivo. Furthermore, due to significantly higher patella tendon stiffness, VL fascicle lengthening was significantly greater in males compared to females. Despite evidencing an attenuating role of the patella tendon on VL fascicle lengthening, patella tendon properties did not correlate with any indirect markers of EIMD, nor did they explain group differences in EIMD. Furthermore, MVEKE torque, MVEKE torque made relative to estimated total quadriceps anatomical cross-sectional area and VL fascicle lengthening did not correlate with any functional indirect marker of EIMD, nor did they explain group differences in EIMD. Within the current thesis CK was the only indirect marker of EIMD to be significantly different between the groups (males > females < OCP users). Creatine kinase was consistently lower in the groups with lower circulating oestrogen levels. Therefore, it was concluded that the antioxidant and membrane stabilising role of oestrogen might explain the group differences in CK reported in the current thesis. Conclusion: In agreement with the historical definition, VL fascicles lengthen during MVEKE. Furthermore, it is evident from the current thesis that patella tendon properties determine the magnitude of VL fascicle lengthening during MVEKE, but do not appear to explain the variability or group differences in EIMD.

Métodos: Foram utilizados 36 ratos (com peso 300 aproximado ± 10g) divididos em 6 grupos experimentais, (n = 6/ cada): controle (C), osteoartrite (OA), sham (S), exercício (E), osteoartrite e exercício (OAE), sham e exercício (SE). O treinamento de força foi realizado durante oito semanas no qual os animais subiram uma escada vertical de 1,1 m com pesos conectados a suas caudas. Os animais realizaram 10 sessões com intervalo de 30 segundos entre cada subida, 3 vezes por semana, em dias alternados durante um período total de 8 semanas. A atividade da MMP-2 foi analisada por zimografia. Resultados: Houve diminuição significativa nas bandas pró, intermediária e ativada MMP-2 no grupo de osteoartrite exercício (OAE) quando comparada com os grupos osteoartrite (OA) e sham com exercício (SE) em ambos, tendão quadríceps e ligamento patelar (p<0,05). Além disso, quando comparado o conteúdo da MMP-2 entre os dois tecidos (tendão e ligamento), um maior conteúdo de MMP-2 foi observado no ligamento do que no tendão patelar (p<0,05). Conclusão: O exercício resistido realizado durante oito semanas modulou positivamente a atividade de MMP-2 no tendão quadríceps e no ligamento patelar com OA. Dessa forma, a regulação negativa da ativação de MMP-2 representa uma resposta positiva promovida pelo treinamento para o tendão quadríceps e ligamento patelar em animais com OA. An eight week strength training period, during which the animals climbed a 1.1-m vertical ladder with weights attached to their tails. The animals performed 10 sessions climbingwith 30 seconds interval between each climb, 3 sessions per week on alternate days for a total period of 8 weeks. The MMP-2 activity was analyzed by zymography. Results: There wassignificant decreased in active-, intermediate- and pro-MMP-2in the osteoarthritis with exercise (OE) group when compared to the osteoarthritis (OA) and sham with exercise (SE) group in both tendon and patellar ligament (P< 0.05). In addition, a novel finding was that active, intermediate and pro-MMP-2 activity differed between tissues and a higher activity in the patellar ligament when compared to patellar tendon was observed respectively (P< 0.05). Conclusion: The strength exercise carried out for eight weeks changed the MMP-2 activity in the tendon and patellar ligament with injury. Thus, down-regulation of MMP-2 activation seems be a positive response for the patellar tendon and patellar ligament in animals with OA subjected to strength exercise program. It is highly possible that strength training exercise intervention for osteoarthritis will target the decrease of MMP-2 expression.

Projeto de Graduação apresentado à Universidade Fernando Pessoa como parte dos requisitos para obtenção do grau de Licenciado em Fisioterapia<br>Objectivo: analisar um conjunto de planos de intervenção com exercício excêntrico, no tratamento da tendinopatia do rotuliano, e definir as características que parecem evidenciar um melhor resultado. Métodos: foi efectuada uma pesquisa bibliográfica em diversas bases de dados utilizando os seguintes termos de pesquisa: “patellar”, “tendin*”, e “eccentric”. Resultados: foram seleccionados 10 ensaios clínicos randomizados controlados que analizaram o efeito do EE em pacientes com TR, com idade entre os 15 e os 50 anos, maioritariamente homens e desportistas. Conclusão: à luz da evidência actual, o tratamento com um programa de reabilitação com exercícios excêntricos, parece ser o tratamento de escolha para os pacientes com tendinopatia do tendão rotuliano. A melhor opção de tratamento com EE parece consistir em exercícios de flexão do joelho, efectuados lentamente, com aumento progressivo da carga, de acordo com a dor tolerada pelos pacientes, efectuados sob um plano inclinado e com a flexão joelho acima dos 60º.<br>Objective: analyze a set of treatment plans with eccentric exercise, in the treatment of patellar tendinopathy and define the characteristics that seem to evidence a better result. Methods: a literature search on various databases was performed using the following search terms: "patellar", "tendin*", and "eccentric". Results: 10 randomized controlled trials were selected to examine the effect of eccentric exercise in patients with patellar tendinopathy, the sample comprised individuals with a diagnosis of JP, aged between 15 and 50 years, mostly men and athletes. Conclusion: In light of current evidence, treatment with a rehabilitation program with eccentric exercise seems to be the treatment of choice for patients with patellar tendinopathy. The best treatment option with eccentric exercise consisted of knee flexion exercises, performed slowly, progressively increasing the load, according to the pain tolerated by patients, carried out under an inclined plane and with the knee flexed to 60°.

Os animais foram divididos em dois grupos de acordo com o tempo de seguimento (12 e 24 semanas) e de acordo com a forma de aplicação do laser (contínua e em dois pontos). Utilizou-se aparelho de Laser de Hóhnio (pulsá til, estado sólido, com ondas de 2,1 microns, com potência de 40 Watts, ponteira OmniTip de 30<]. Após o sacrificio, foram comparadas, por meio de testes não paramétricos (considerando p = 0,05), as medidas do comprimento e da largura (proximal e dista!) dos tendões do lado operado e do lado não operado. A medida do comprimento do lado operado foi significativamente maior nos dois grupos de seguimento, quando comparada com o lado não operado, porém, não houve diferença significativa dessas medidas em função do tipo de aplicação do laser. Do mesmo modo, a medida da largura, tanto na região proximal quanto distal, foram significativamente maiores no lado operado nos dois grupos de seguimento, sem apresentar diferença significativa em função do tipo de aplicação. Quando se compararam as medidas nos dois grupos de seguimento, o comprimento e a largura dista tenderam a ser maiores após 24 semanas, enquanto que a largura, na região proximal, foi significativamente maior nesse grupo. Quanto à avaliação microscópica subjetiva, tanto nos cortes longitudinais como transversais, pôde-se verificar aumento do número de fibroblastos, principalmente no tendões não operados. Na região entre os fascículos, o tecido conjuntivo era exuberante no grupo com 12 semanas, com neoformação vascular evidente. The animaIs have been divided into two groups, according to the follow-up time (12 and 24 weeks) and pursuant to the for form of laser application (continuous or two-point). A Holmiurn laser (pulsed, solid state, 2.1 micron waves, 40 watts, OmniTip 30° tip) apparatus was used. Afier the animaIs were sacrificed, the proximal and distallength and width ofthe operated size of such rats were compared to those ofthe non-operated size by means ofnon-parametric testing (considering p = 0,05). The length in the operated size was significant1y bigger for both follow-up groups, when compared to the length of the non-operated size, however there was no significant difference in such measures in function of the type of laser application. In the same manner, the width, both in the proximal and distal regions, was significant1y bigger in the operated size in both follow-up groups, without showing any significant difference whatsoever in function of the type of application. When the measures in both follow-up groups were compared, distallength and width showed a trend to become bigger afier 24 weeks, while width in the proximal region was significant1y bigger in this group. As for the subjective microscopic evaluation, both in longitudinal and cross sections, it was possible to observe an increase in the number of fibroblasts, mainly in the 12-week group. The average fibroblast concentration in the tendon with 24-week follow-up was deemed to be intermediate between the 12-week group and non-operated tendon. The conjunctive tissue was exuberant in the region amidst the fasciculi for the 12-week group, with evident vascular neoformation. This thesis examined the change in skeletal muscle architecture with contractile force production, the relationship of architecture with muscle strength parameters and if muscle tendinous tissue stiffness determines in vivo explosive strength (i.e. rate of torque development, RTD). Muscle and tendinous tissue adaptations to contrasting strength training regimes, and the potential capacity of these tissues to adapt following chronic strength training were also explored. Quadriceps femoris fascicle length (FL) decreased, while the pennation angle (PA) increased in a curvi-linearly manner from rest to maximal voluntary contraction (MVC) torque. Consequently, effective physiological cross-sectional area (effPCSA) during MVC was 27% greater than at rest, although effPCSA measured at rest and during MVC had similar correlations to maximal strength. In the earliest phase of contraction, FL, but not PA, was negatively related (R2=0.187) to voluntary RTD. Neither FL nor PA was related to maximal isometric or dynamic strength. Muscle-tendon unit (MTU) and patellar tendon (PT) stiffness were unrelated to voluntary and evoked RTD. Relative PT stiffness was also unrelated to relative RTD, although relative MTU stiffness was related to voluntary RTD (25-55%MVT, R2≤0.188) and evoked RTD (5-50%MVT, R2≤0.194). MTU stiffness increased after sustained-contraction (SCT, +21%), though not explosive-contraction strength training (ECT). PT stiffness increased similarly after ECT (+20%) and SCT (+16%), yet neither induced tendon hypertrophy. SCT produced modest muscle (+8%) and aponeurosis (+7%) hypertrophy. Chronic strength trained (CST: >3 years) males had substantially greater muscle and aponeurosis size, but similar tendon size as untrained controls (UNT) and short-term (12 weeks) strength trained (STT) individuals. Between these groups, at the highest common force, MTU stiffness was indifferent, while PT stiffness was similarly greater in STT and CST than UNT. These results suggest FL and PA have little influence on muscle strength and tendon stiffness has no influence on RTD. Maximum strength negated any qualitative influence of MTU stiffness on in vivo RTD. Component MTU tissues (muscle-aponeurosis vs. external tendon) adapt differentially depending on the strength training regime. Specifically, free tendon appeared to adapt to high magnitude loading, while loading duration is also an important stimulus for the muscle-aponeurosis. However, chronic strength training was not concordant with greater higher force MTU stiffness, and does not further increase higher force PT stiffness beyond the adaptations that occur after 12 weeks of strength training. Finally, no evidence was found for tendon hypertrophy in response to strength training.

Bioengineering<br>Ph.D.<br>More than 350,000 anterior cruciate ligament (ACL) injuries occur every year in the United States. A torn ACL is typically replaced with an allograft or autograft tendon (patellar, quadriceps or hamstring), with the choice of tissue generally dictated by surgeon preference. Despite the number of ACL reconstructions performed every year, the process of ligamentization, transformation of a tendon graft to a healthy functional ligament, is poorly understood. Previous research studies have relied on mechanical, biochemical and histological studies. However, these methods are destructive. Clinically, magnetic resonance imaging (MRI) is the most common method of graft evaluation, but it lacks adequate resolution and molecular specificity. There is a need for objective methodology to study the ligament repair process that would ideally be non- or minimally invasive. Development of such a method could lead to a better understanding of the effects of therapeutic interventions and rehabilitation protocols in animal models of ligamentization, and ultimately, in clinical studies. Fourier transform infrared (FT-IR) spectroscopy is a technique sensitive to molecular structure and composition in tissues. FT-IR fiber optic probes combined with arthroscopy could prove to be an important tool where minimally invasive tissue assessment is required, such as assessment of graft composition during the ligamentization process. Spectroscopic methods have been used to differentiate normal and diseased connective tissues, but have not been applied to investigate ligamentization, or to investigate differences in tendons and ligaments. In the proposed studies, we hypothesize that infrared spectroscopy can provide molecular information about the compositional differences between tendons and ligaments, which can serve as a foundation to non-destructively monitor the tissue transformation that occurs during ligamentization.<br>Temple University--Theses

The purpose of this dissertation was to study the effects of graft pretension in anterior cruciate ligament (ACL) reconstruction through a series of experiments. First, an in vitro study of 5 human knees was conducted to determine if intact joint kinematics could be restored when using the ideal graft - the intrinsic ACL. The ACL tibial insertion site was freed, and pretensions of 0, 10, 20, 30, and 40 N were applied to the ligament using a custom designed load cell connection. Kinematics during a simulated active extension were compared to those of the intact knee. Intact knee kinematics were not restored. Pretensions that best restored tibial anterior/posterior translation and internal/external rotation ranged from 0-40 N. Furthermore, the pretensions that best restored these kinematic variables were widely disparate in two specimens. Second, the in vitro kinematics during a simulated active extension of human and porcine knees were compared and contrasted both prior to and following transection of the ACL. The ACL limited: (1) tibial anterior translation in both species, (2) tibial internal rotation in humans, and (3) tibial external rotation in pigs. Differences in kinematic patterns for tibial internal/external rotation and abduction/adduction between the species was explained by requirements for biped and quadruped stances. Third, the mechanical characteristics of porcine patellar tendon (PT) were investigated by uniaxial tensile testing at two strain rates. Patella-PT-tibia complexes from freshly sacrificed skeletally immature and mature animals were loaded to failure at elongation rates of 20 and 200 mm/min. Both strain rate and skeletal maturity significantly affected failure mode, tangent modulus, and ultimate stress of the tendons, and hence are important considerations in the mechanical evaluation of porcine PT. Fourth, ACL reconstructions were performed using pretensions of 10 or 20 N in an in vivo porcine model with a specially designed load cell/telemetry system to monitor graft load. Graft pretension was seen to increase during fixation with interference screws. Following sacrifice at 4 weeks, tissues were mechanically, histologically, and biochemically analyzed. A pretension of 20 N resulted in a tissue more similar to the intrinsic ACL.<br>Ph. D.

The high prevalence of this injury in jumping athletes may be related to hip extensor and ankle plantar flexor weakness, which could lead to a preferential use of the quadriceps to absorb the impact forces acting on the lower limbs during landing. However, no previous study has evaluated hip and ankle strength in subjects with PT. Therefore, the aim of this study was to evaluate the eccentric hip and knee extensor and ankle plantar flexor peak torques and the knee/hip and knee/ankle ratios in athletes with symptomatic PT (SPTG), athletes with asymptomatic patellar tendon ultrasonographic abnormalities (APTG) and healthy controls (CG). Methods: Seventeen athletes (volleyball, basketball and handball) between 16 and 35 years of were enrolled in this study. They were divided into three groups: SPTG (n=6), APTG (n=5) and CG (n=6). Eccentric peak torque was evaluated with an isokinetic dynamometer. Differences between groups were evaluated by multivariance analysis (MANOVA) and the Tukey post-hoc test (&#945;=0.05). Results: The SPTG had a lower eccentric knee extensor torque than the APTG (p=0.047) and the CG (p=0.035), and the APTG had a higher knee/ankle ratio than the SPTG (p=0.015). Conclusions: The SPTG had lower knee extensor torque than the other groups, which may have been due to pain inhibition. Additionally, the APTG had a higher knee/ankle ratio than the other groups, which may have been related to increased patellar tendon load. Prospective studies are needed to investigate whether these aspects are risk factors for PT.<br>Introdução: A tendinopatia patelar (TP) é uma lesão por sobreuso, mas não se sabe ao certo quais são os fatores que contribuem para a sobrecarga no tendão. A alta prevalência dessa doença em atletas saltadores pode estar relacionada a uma diminuição da utilização dos músculos extensores do quadril e flexores plantares do tornozelo, e consequente uso preferencial do músculo quadríceps para absorver as forças de impacto que atuam no membro inferior durante as aterrissagens de saltos, possivelmente devido ao déficit de força dos mesmos. Assim, o objetivo do estudo foi avaliar o pico de torque excêntrico extensor do quadril e do joelho e flexor plantar do tornozelo e suas razões joelho/quadril e joelho/tornozelo em indivíduos com tendinopatia patelar (GTPS), indivíduos com alteração de imagem no tendão patelar assintomáticos (GTPA), e indivíduos sadios (GC). Métodos: Participaram deste estudo 17 indivíduos com idade entre 16 e 35 anos, praticantes de voleibol, basquetebol e handebol, divididos em 3 grupos: GTPS (n=6), GTPA (n=5) e GC (n=6). O pico de torque excêntrico dessas articulações foi mensurado utilizando um dinamômetro isocinético. As diferenças entre os grupos foram avaliadas por meio da análise de multivariância (MANOVA) e o post-hoc de Tukey (&#945;=0,05). Resultados: O GTPS apresentou menor torque excêntrico extensor do joelho do que o GTPA (p=0,047) e o GC (p=0,035). Além disso, o GTPA apresentou maior razão de torque joelho/tornozelo do que o GTPS (p=0,015) e uma forte tendência a ter essa razão maior do que o GC (0,064). Conclusão: GTPS apresentou menor torque extensor do joelho, o que ocorreu possivelmente por uma inibição pela dor. Além disso, o GTPA apresentou maior razão joelho/tornozelo, podendo esse ser um fator relacionado ao aumento da carga no tendão patelar. São necessários estudos prospectivos para comprovação desse fator de risco.

Metodologia: Foi realizada uma pesquisa computorizada nas bases de dados PubMed, Web of Science e EBSCO- Academic Search Complete de modo a identificar estudos randomizados controlados que comparassem os resultados clínicos dos enxertos do tendão rotuliano e semitendinoso/gracílis numa ligamentoplastia do LCA, após um período mínimo de 2 anos de follow-up, em publicações entre 2010 e 2020. Resultados: Foram incluídos 7 estudos clínicos randomizados envolvendo 714 indivíduos com a classificação metodológica de 8.2 em 10 na escala de PEDro, que compararam os diferentes enxertos através da implementação de diferentes programas de reabilitação e parâmetros de avaliação. Conclusão: A reconstrução do LCA através dos enxertos rotuliano e semitendinoso apresenta resultados efetivos e semelhantes entre si e garante um retorno seguro e eficiente do paciente à sua atividade desportiva.<br>Objective: This review aims to analyze the different clinical and functional results achieved after a surgical reconstruction of the ACL using the patellar and semitendinosus/gracilis grafts in the rehabilitation and return to sports activities processes. Methodology: A computerized research was carried out in the PubMed, Web of Science and EBSCO- Academic Search Complete databases in order to identify randomized controlled studies that compared the clinical results of patellar and semitendinosus/gracilis tendon grafts in an ACL ligamentplasty, after a minimum period of 2 years follow-up, in publications between 2010 and 2020. Results: 7 randomized clinical studies were included in this review, involving 714 individuals with a methodological classification of 8.2 out of 10 on the PEDro scale, comparing the different grafts through the implementation of different rehabilitation programs and assessment parameters. Conclusion: ACL reconstruction using patellar and semitendinosus grafts presents effective and similar results and guarantees a safe and eficient return of the patient to his sports activity.<br>N/A

Resultados: Nesta revisão foram incluídos 5 estudos clínicos randomizados envolvendo 344 indivíduos com a classificação metodológica de 9 em 10 na escala de PEDro, que compararam os diferentes enxertos com programas de reabilitação e instrumentos de avaliação. Conclusão: Constatou-se que a reconstrução anatómica artroscópica do LCA com enxertos de osso-tendão-osso (OTO) em comparação com semitendinoso e gracílis (ST-G) são ambas técnicas seguras e efetivas. Introduzione: le tendinopatie achillea e patellare sono le più frequenti a livello dell’arto inferiore e sono caratterizzate da sintomi persistenti quali dolore che si accentua col carico e compromissione della funzione. L’approccio riabilitativo tradizionale si basa su esercizi eccentrici isolati in carico, modalità che pare inefficace per circa il 45% dei pazienti. Stanno perciò emergendo nuove strategie riabilitative che includano altri tipi di contrazione muscolare. Si è ipotizzata una correlazione tra la persistenza dei sintomi e fenomeni di sensibilizzazione del SNC. Obiettivo: analizzare le nuove strategie basate sul carico nel trattamento della TA e TP - in particolare l’HSR – e le componenti di sensibilizzazione del SNC che sembrano ostacolare la riabilitazione. Materiali e metodi: consultazione delle principali banche dati. Si potevano includere studi primari e secondari. I soggetti dovevano presentare un quadro di TA o TP con eventuale associazione di alterazioni dell’elaborazione del dolore a livello centrale e dovevano essere sottoposti a un programma di esercizio eccentrico o ad HSR. Risultati: 5 studi hanno soddisfatto i criteri di inclusione. Tre studi confermano l’efficacia dell’HSR per il raggiungimento degli outcome desiderati, ma non in maniera statisticamente significativa rispetto all’esercizio eccentrico. Due articoli analizzavano se fosse presente un quadro di sensibilizzazione del SNC nei soggetti con TA e TP. Entrambi affermano la presenza di un’alterazione della capacità di modulazione centrale del dolore. Conclusioni: l’HSR risulta efficace per il trattamento della TA e TP in maniera sovrapponibile all’esercizio eccentrico tradizionale. L’alta percentuale di recidiva dei sintomi sembra causata da un’alterazione della capacità di modulare il dolore in questi pazienti. E’ necessario quindi ricercare nuove tipologie di trattamento - come il Tendon Neuroplastic Training - che tengano conto delle presenza di sensibilizzazione del SNC.
