Narrative psychiatry empowers patients to shape their lives through story. Rather than focusing only on finding the source of the problem, in this collaborative clinical approach psychiatrists also help patients diagnose and develop their sources of strength. By encouraging the patient to explore their personal narrative through questioning and story-telling, the clinician helps the patient participate in and discover the ways in which they construct meaning, how they view themselves, what their values are, and who it is exactly that they want to be. These revelations in turn inform clinical decision-making about what it is that ails them, how they'd like to treat it, and what recovery might look like. The Art of Narrative Psychiatry is the first comprehensive description of narrative psychiatry in action. Engaging and accessible, it demonstrates how to help patients cultivate their personal sources of strength and meaning as resources for recovery. Abstract:
The co-authors of this chapter—an internist and a psychoanalyst—examine the situation of a patient whose care was enhanced by narrative methods of collaborative writing by physician and patient. In turns, the co-authors discuss psychoanalytic and narratological aspects of this clinical case. The analyst proposes that, in the care of medically ill patients, transference may occur not to the analyst but to the illness itself and that a stabilizing transitional space, as described by D.W. Winnicott, may open up within routine medical practice. The internist reviews concepts of creativity, reflexivity, and reciprocity as aspects of a narratively fortified practice. Together, they can unite patient and clinician in care for a specific episode of illness and can also create both participants’ reciprocal awareness of the inevitability of death to come, binding them in a powerful and lasting alliance as fellow humans facing mortality.

Abstract:
This chapter summarizes the clinical practices that have been developed worldwide by those trained in narrative medicine. It describes innovations in interviewing techniques that improve the development of narratively fortified clinical relationships, including examples from clinical genetics and fetal cardiology. The chapter describes the increasing call for disciplined training in creative and reflective writing as a means to enrich and strengthen the clinical skills of individual practitioners. Interprofessional education and practice is a growing arena for the application of narrative medicine methods, as their collaborative nature helps to overcome divisive conflicts between specialized professions that can hamper effectiveness of care and threaten patient safety. Discoveries in clinical witnessing and clinical writing are presented, with descriptions of narrative medicine practices of including patients as readers and writers of the electronic medical record.
