Full textDetta examensarbete har genomförts vid Institutionen för produktionsekonomi på Linköpings tekniska högskola. Uppdragsgivare har varit Svenskt Näringsliv och arbetet har utförts på Södersjukhuset i Stockholm. Syftet med arbetet har varit att kartlägga flödet av patienter med sökorsak höft genom sjukhusets akutmottagning och ortopediska verksamhet, för att därefter analysera detta och föreslå åtgärder som kan minska väntetider och öka genomflödet.
På akutmottagningen har tre aktiviteter visat sig vara viktigare än de övriga, nämligen läkarundersökning, röntgen och inläggning på vårdplats. Anledningen till detta är att väntan till de aktiviteterna utgör cirka två tredjedelar av en patients tid på akutmottagningen. Därför har åtgärderna på akutmottagningen syftat till att reducera dessa väntetider och mildra dess konsekvenser.
Ett led i detta är att akutmottagningen anpassar antalet läkare bättre efter de varierande behoven över dygnet. Röntgenavdelningen bör kontinuerligt utvärdera behoven från akutmottagningen för att på så sätt balansera sin kapacitet. Patienter som är färdiga på akutmottagningen placeras i en separat lokal under tiden de väntar på vårdplats.
Inom den ortopediska verksamheten syftar åtgärdsförslagen till en förändrad hantering av resurser och ett bättre utnyttjande av desamma. De två resurser som arbetet koncentrerar sig kring är vårdplatser och operation, och avvägningen om dessa bör vara gemensamma eller dedicerade mellan elektiva och akuta patienter.
Det betydande antalet operationer som drabbas av störningar, och därför ej kan genomföras, talar för en uppdelning av operationsresurser mellan akuta och elektiva patienter. Däremot bör vårdplatserna vara gemensamma eftersom de elektiva patienterna kan jämna ut den totala efterfrågan på vårdplatser. Genom att dessutom se över utskrivningsrutinerna från vårdavdelningar kan flödet förbättras ytterligare. Åtgärder som föreslagits för att åstadkomma detta är tidig planering av utskrivningar, utskrivningskoordinator och att inrätta en lokal i väntan på hemfärd.

Full textThis thesis aims to examine the patient flow through the Swedish veterinary clinic Mälaren Hästklinik. The thesis also examines how this flow can be improved. The clinic has, as of today, long patient queues in certain parts of the process, and a mathematical model is made to improve this process. Firstly, an examination of the clinic is made and suitable mathematical theories are selected. The patient flow is then modelled from this mathematical perspective and this modelling is compared to the reality. Finally, a mathematical improvement of the patient flow is made. The results presented are that it is clearly possible to represent the current patient flow with mathematical models and theories. It is also shown that there is some theoretical optimization to be made that reduces the queuing time, and the total time in the system. Lastly, there are also presented methods and theories for how a Swedish SME active in the veterinary business should consider an efficiency process through a sustainability perspective.

Full textBackground. Patient flows to Swedish emergency departments are increasing. With an increased inflow of patients, the demands on the healthcare staff at the emergency department increase. With a discrepancy between in-, through- and outflow, risks of crowding are created, with consequences such as increased waiting times for patients, increased patient safety risks and suffering as a result. The emergency nurse's responsibility includes knowledge of patient flows, patient safety and alleviation of suffering. Nevertheless, there is little research about patient flows to the emergency department nationally. Purpose. The purpose of the study was to study the inflow of patients to the emergency departments at two regional hospitals. Method. The study was conducted as a quantitative point-prevalence study with an inductive approach. The analysis includes an analysis of data from two emergency departments in Region Dalarna, n = 201. Results. Over 70 percent of the patients that presented to the emergency departments had a previous healthcare contact. It was most common that younger patients used their own initiative and that elderly presented by ambulance. There were more men than women who came from community health centres with referral. Patients who had another healthcare contact before the visit had a higher medical priority or risk of admittance to the hospital than the patients who came on their own initiative. Conclusion. A physical meeting with healthcare staff can be of great importance for reducing the inflow of patients to the emergency department. It may be that there is a low capacity in the health care system to treat low-acuity patients.

Full textThe wait time at Södersjukhuset’s emergency room in Stockholm is amongst the longest in Sweden. The waiting time continuously increases and is therefore a growing issue. The wait time further deteriorates as the demand for emergency care increases. This results in a lower quality of patient care, patient safety and patient satisfaction. As a solution for the long waiting times a new emergency room is scheduled for completion by 2020, where the current emergency room for adults will be moved. As the demand for emergency care gradually increases, so will the load on the current emergency room. This requires examinations of patient flows and wait times during the construction period of the new emergency room. Consequently, the aim of this paper is to examine how Södersjukhuset’s current emergency room handles the increasing patient flows during the construction period. The examination takes emergency staff and number of care rooms into account. The methods used were dynamic simulation and a simple Jackson network. The results show that the so called patient rooms are bottlenecks. It is dif- ficult to come to more conclusions, as questions within care are charecterized by several perspectives that are in conflict. For that reason, perceived service and wait time management within health care management are examined, as a complement for the mathematical part of this paper.

Full textPatient flow related problems are a medial attention phenomenon, however, the debate is mainly focused on human healthcare (Delin, 2014). Through a focus on veterinary care and more specifically lameness exams on the equine vet clinic Mälaren Hästklinik AB this report intends to expand the scope of examination of patient flows. Mälaren Hästklinik is one of the leading clinics in Sweden and only within the department of lameness examination the clinic receives about 18 horses per day (Intervju, 2015). A primary problem for this vet clinic is the lack of a patient flow model and therefore it is common that customers have to queue upon arrival to the clinic. The main purpose of this project is to develop a clear picture of the patient flow through a mathematical process model with accompanying time for process throughput. A secondary target, an efficiency recommendation, is added based on the results from the mathematical process model. The mathematical process modelling is done via two separate models, both of which are based on Markov processes and queuing theory. The first model is based on a reality adaptation to a Markov queuing system, M/M/1- systems, through predetermined assumptions about the flow characteristics. The second model is an “accurate reality model” with no predetermined assumptions. It is most common that similar but fictional mathematical process modelling problems in textbooks are formulated by the assumptions that the reality has markov properties (Enger & Gran Dell, 2014). The choice of project methodology aims to examine and thus contribute to an illustrative example of what can happen with the results if the same assumptions are made on a reality-rooted problem. Therefore, the methodology has a side objective in addition to delivering the desired results of the project goal. The results of the modelling with M/M/1- systems prove to be illogical while the “accurate reality model” demonstrates valid results. The result from the latter model generates a result in line with the project objective. Finally, a recommendation is given to the clinic in form of an efficiency project as a result of the secondary project objective. The proposal compiles relevant business management models and lean theories and is adapted to the clinic's operations.
