Full textBackground: Communication takes place continuously in the health-care. To avoid that important information in the communication between care providers get lost, can the structured communication tool SBAR be used. The patients right to receive correct and proper care is largely due to the fact that correct information is shared in the hand-offs. Aim: The aim was to study how communication in hospitals were affected after an education in SBAR was introduced to the healthcare personnel. Method: A literature review with quantitative approach. The degree project was based on ten empirical articles including one or two interventions. Systematic searches were conducted in PubMed, Cinahl and PsycINFO. The authors have both individually and joint reviewed the ten selected articles. The results are presented in six categories. Result: The six categories are Time, Quality in communication, Structures with SBAR, Documentation, patient outcome and safety-thinking. The result shows that SBAR have an effect on content, structure and quality and also a decrease in time on shiftreports. Conclusion: The overall result presented in the literature review shows that structured communication with SBAR increase the quality of communication, better safety-thinking and safer shift-reports which increase the patient safety.

Full textMost health care damages are partially or totally caused by communication failures. One way to improve patient safety is to use a communication tool, of which the most common is called SBAR. A literature review was done where fifteen articles were reviewed with the aim to illuminate how SBAR influences patient safety. SBAR exerted its influence in several different ways. The communication between the different professions was given a standardized structure and less information was omitted. Both nurses and physicians experienced increased confidence in the communication situation. Teamwork was positively influenced and the care culture developed towards more problem focus and less of person focus at adverse events. Nurses experienced growing security awareness and found SBAR having positive side effects which developed the professional role. SBAR contributed to flattening of care hierarchies, by all team members assuming equal responsibility for communication. More research is needed from the European countries and not least Sweden. It would be interesting to learn what kind of influence SBAR has on patient safety in a country where teamwork is more common than in many other countries. Another interesting research topic is electronic uses of the SBAR tool.

Full textBackground: Within health care there is a constant flow of information regarding patients. To have a good communication system between hospital staff is important since patient safety always is at risk. One example of a communication model is SBAR, which stands for “situation”, “background”, “assessment” and “recommendations”. Aim To investigate to what extent nurses use SBAR at five wards at a university hospital in Sweden and to see if there is a correlation between numbers of years in the profession and attitude towards SBAR and to see in what situations nurses think SBAR is most useful. Method: Empirical descriptive cross-sectional study with a quantitative approach. 103 questionnaires were handed out to nurses at wards and at intensive care units at a hospital in Sweden. Results: Regardless of ward-type, a total of 56 percent of the nurses (n=33) thought that SBAR was most useful when reporting to emergency teams. Thirty-six percent of the nurses (n=26) at intensive care units used SBAR on a daily basis, compared to nurses at regular wards where 11 percent of the nurses (n=8) used SBAR daily. No correlation was found between number of years in the profession and attitude towards SBAR. Conclusion: Ninety percent of the participants used SBAR occasionally. Forty-six percent used SBAR on a daily basis. Most nurses thought that SBAR was most useful when reporting to emergency teams followed by reporting to doctors. Resarch supports the fact that SBAR is useful in non-acute situations. Despite that, the result showed that nurses don’t use SBAR on such occasions to the extent, which is possible. Continuous training is demanded for SBAR to be used more.

Full textBackground: A significant reason to many damages and announcements that is done within the healthcare is when verbal or written communication lacks on structure and effectiveness. This can also affect on the patient safety. SBAR is a communication model that is used in healthcare to structure information and present important facts on a fast and effective way, this will also improve on patient safety. Aim: The aim with the study was to examine what nurse students had for knowledge and understanding about the communication model SBAR. Method: The study was a quantitative descriptive study, where data have been collected through a questionnaire that consisted of eleven questions, seven of them were multiple choice, three open alternatives and one were the students had to estimate their answers. Results: The result of the study revealed a limited knowledge and understanding about the SBAR model, only three participants had knowledge and understanding of the model on a total of 25 participants. They considered that the model where easy to use, easy understandable, effective and that the model increased patient safety. Conclusion: In the health care sector, it is extremely important that there is a communication model that facilitates and organizes up communication, such a model is SBAR. The study indicates that the nurse students that participated in the study had a meager knowledge of the SBAR model.

Full textBackground: Healthcare is a high risk activity. Each year a large number of patients worldwide suffer from preventable harm that could have been avoided and in some cases leading to patients death. Scientific research suggests that the most common cause of incidents and adverse events in health care is the communication failure between healthcare professionals´. Factors that can influence the basis of differences in handover could be heavy workload and stressful working conditions. The aim: The aim of this literature review was to explore the effects of the SBAR-communication model for handover in healthcare with regard to the SBAR model’s impact on the nursing staff communication and patient safety. SBAR is a structured communication model that stands for (Situation, Background, Assessment and Recommendation). Method: A systematic approach of Goodman's seven steps wear used in the preparation and implementation of the study. The database search was performed primarily in PubMed and CINAHL. Results: The results were based on eight scientific studies that show that the SBAR-model improves communication, the handover-communication, teamwork and communicative self-confidence of healthcare professionals and students. The healthcare professionals´ improved communication also resulted in increased patient satisfaction. Conclusion: The studies findings supports the use of a structured communication model for handover in healthcare. Because the SBAR-model is a relatively new method of reporting, there is a need for increased knowledge and training to integrate this method in nursing. Furthermore it is recommended that SBAR-communication should be included in nursing education. Further research is required regarding the SBAR-model´s function and effect in healthcare in order to increase the evidence for the SBAR-model´s effect on nursing staff communication and patient safety.

Full textBackground: Poor professional communication and information transfer are main causes for medical error in the health care systems. The complexity in health care organisations in combina-tion with human factors issues raises a demand for structured and standardised communication. One challenge is to implement standardised communication models and study its effect on pa-tient safety and working conditions. SBAR (Situation, Background, Assessment and Recommen-dation) is a knowledge based communication model, developed to transfer critical information in complex work environments, both in normal and stressful conditions. Aim: To study implementation of SBAR on a Cardiology ward with focus on strategies that could facilitate the process. Method: Ten qualitative interviews with members of staff during the implementation process and quantitative data (130 anonymous questionnaires among staff, Safety Attitude Questionnaire, SAQ, during the implementation of SBAR have been collected (March- December 2011) Results: 95 % of the nurses and 70 % of the physicians have received SBAR education. In the interviews, the nurses describe an improved intra-professional communication after the imple-mentation of SBAR. The nurses also describe the communication to be more focused on the patients’ care, follow the SBAR process. However, only a few physicians explain that they are using SBAR in the intra- and/or inter professional communication. Strategies (leadership, role definitions within the team, structure for multi-professional cooperation, identification and prevention of hierarchical structures and introduc-tion of improvement knowledge) and hinders (limited clinical analyses before the project, unclear assignment, limited resources e.g. time and methodology, handling critical voices, limited input from physicians and external impact factors) for implementation of the SBAR model were identi-fied. The response rate for the baseline SAQ questionnaire was 94 % among nurses and 69% among physicians. No differences were found in nurses and physicians’ attitudes on safety climate and teamwork within their own groups. Some significant differences however were found concerning cooperation and communication between the two groups; nurses were less satisfied with cooperation and communication compared to the level of satisfaction in the group of physi-cians. Discussion: The implementation process has been meaningful in regards to an increase in knowledge trough the analytic outlook of the study. The result indicates that the actual imple-menting of SBAR requires further steps to create optimal conditions to achieve better results in communication and collaboration in cross-professional teams. The study confirms the communi-cation gap that exist between doctors and nurses and highlights the need for a better understand-ing of each other`s roles and responsibilities, in order to achieve an integral patient-centred ap-proach. It is crucial to identify and to prevent existing structures of hierarchy and to clarify the meaning of leadership within the team. The process of implementing SBAR is an effort to ad-dress one of the most important issues in patient safety and requires firm and committed leader-ship.

Full textLack of communication is the single biggest reason for errors in healthcare. By standardizing the communication, the variation is reduced and logic and mutual agreement are created. The purpose of the improvement work was to increase patient safety by simplifying communication by introducing the SBAR communication tool. The subsequent study aimed to investigate how SBAR was experienced in the elderly care team, and to describe success factors and difficulties at the time of implementation. The author was inspired by action research. A content analysis has been applied on the data collected. There was an improvement in the information transfer, but the unit did not reach the set targets. SBAR was found useful by the professionals after practical training. The professionals felt greater security with a combination of oral and written report. The project created common learning through participatory observations and by clarifying the surrounding structures for the professionals. The project contributed to changed focus from an individual to a system level concerning the information process. The SBAR structure was slightly simplified to fit the multilingual context. New areas for improvement were identified and an activity board for the elderly was created. Keeping consistent education and training temporary staff was a challenge.
