Abstract:
Background: Acute respiratory tract infection are a major cause of global morbidity and mortality. Observational studies report consistent independent association between low serum concentration of 25-hydroxy-vitamin D and susceptibility to acute respiratory tract infection. This study was done to assess serum vitamin D level in children and correlation with respiratory tract infection.Methods: The present study was undertaken in department of paediatrics medicine, Patna Medical College and Hospital in patients attending out-patients department (OPD) as well as admitted in in-patients department (IPD). Assessment of serum 25(OH) Vitamin D was done by the enzyme immunoassay kit at Patna Medical College and Hospital.Results: The mean serum vitamin D level in cases was (20.52±5.64 ng/ml) where as in control group was (26.46±4.52 ng/ml) with a statically significant p value of <0.0001, hence cases are associated with vitamin D deficiency.Conclusions: The finding in this study showed significantly decreased mean serum vitamin D level in cases than that of control which implies that vitamin D deficiency predisposes to respiratory tract infection in children.

Abstract:
Background: Severe Acute malnutrition (SAM) with severe wasting remains a major killer of children. In Bihar 48% of children are stunted, 21 % are wasted and 7% are severely wasted. Even during the first six months of life, 31% are wasted. Under nutrition generally decreases with the increasing mother’s schooling, better nutritional status of the mother. Stunting and under nutrition are higher in rural areas than in urban areas.Methods: It is a hospital based observational study done between June 2017 to December 2017. A total of 55 SAM patients with medical complications in the age group of 6 months to 60 months admitted in Nutritional Rehabilitation center (NRC), Department of Pediatrics, Patna Medical College, Patna were included in the study. Socio-Economic profile, effectiveness of NRC in treating SAM children, and effect of timely initiation of complementary feed on nutritional status of children were assessed.Results: A total of 55 children were admitted in the NRC of PMCH, Patna during the period of June 2017 to Dec 2017. 56.4% were males and 43.6% were females. 36% o were in the age group of 12months to 24months. 78% belonged to below poverty line. Major medical complications were anemia (53%), LRTI (33%) and acute gastroenteritis (29%). Immunisation was complete in only 45%. In only 53% babies Complimentary feeding was initiated after 6 months of age. 67% of the mothers of SAM children were illiterate. 95% children were discharged after gaining proper weight. Defaulter rate was 4% and death rate was 2%. 78% of the admitted children showed good weight gain i.e.>10 gm/kg/day, whereas in 15% children weight gain was in the range of 5-10 gm/kg/day.Conclusions: Many factors such as literacy, income, age of marriage and sanitation facility indirectly or directly influence the nutritional status of children. NRCs provide life-saving care for children with SAM as demonstrated by the high recovery rate (95%).

Background: Perinatal asphyxia causes multi organ dysfunction resulting in renal (50%) and neurological (28%) compromise with 1.4% of hypoxic ischemic encephalopathy (HIE) and almost 20% death in India. Early recognition of acute kidney injury (AKI) is important in babies with HIE to facilitate appropriate fluid and electrolyte management for a stable biochemical milieu is vital.Methods: A prospective case control study was done in Patna Medical College and Hospital, Patna between January 2019 and March 2020. 70 term asphyxiated neonates with HIE as cases and 70 healthy neonates as control were taken. AKI on basis of p RIFLE criteria and HIE on the basis of 5 minute APGAR score were determined and correlated.Results: 58.6%cases of AKI with 73% pre renal and 61% non-oliguric type were found in asphyxiated neonates with HIE blood urea and serum creatinine values were significantly higher in asphyxiated babies than control group babies (p<0.0001).Conclusions: The extent of AKI is directly proportional to severity of HIE.

Abstract:
Background: Acute Encephalitis Syndrome (AES) is defined as a person of any age, at any time of year with the acute onset of fever and a change in mental status (including symptoms such as confusion, disorientation, coma, or inability to talk) and/or new onset of seizures (excluding simple febrile seizures). Viruses have been mainly attributed to be the cause of AES in India although other etiologies such as bacteria, fungus, parasites, spirochetes, leptospira, toxoplasma, rickettsia, chemical, and toxins have also been reported over the past few years. The causative agent of AES varies with season and geographical location, owing to wide range of causative agents and the rapid neurological impairment due to pathogenesis, clinicians face the challenge of a small window period between diagnosis and treatment. The present study is dedicated to knowing the present epidemiological pattern of AES in Bihar aiming to help in diagnosis and treatment.Methods: This is a prospective study conducted in the department of pediatrics, Patna Medical College and Hospital, Patna from January 1st to December 31st, 2016, in this study all cases which presented with acute onset of fever and a change in mental status including symptoms such as confusion, disorientation, coma or inability to talk and/ or new onset of seizures excluding simple febrile seizures were included. Demographic, etiological analysis and outcome of cases of Acute Encephalitic Syndrome as well as Japanese encephalitis were done.Results: The total number of patient diagnosed clinically with AES were 186 of them 105 were male and 81 were female. Number of cases were highest in the age group of >5-10 years amounting to 37.7% followed by 26.4% in >2-5 years age group, marked male predominance was seen in the age group 5 -10 years. A minor female predominance was observed in the age group >10 years. In May number of cases were maximum 36 (19.4%) followed by April 32 (17.2%), number of cases of AES were least in the month of December followed by November (8). Maximum number of cases were from the district of Patna and its neighboring district amounting to 58.5% with Nalanda district alone comprises 24.3%. Etiological analysis reveals that 36.5% children admitted with the clinical diagnosis of AES, 36.5% were diagnosed with Acute bacterial meningoencephalitis and 22.04% were diagnosed with Japanese Encephalitis, 7.5% Tuberculous meningitis, 6.4% Cerebral malaria, 5.4% Herpes simplex encephalitis and 3.2% with acute encephalitis syndrome unknown.Conclusions: Acute Encephalitis Syndrome remains an important cause of prolonged hospital bed occupancy with a high rate of mortality. Although in JE positive cases mortality were less, but morbidity in form of various motor deficit and cognitive impairment increases the burden on the family and society. With the introduction of effective JE vaccine and with rigorous surveillance of AES cases and social initiative taken by the Government, we can hope a better scenario. More and more extensive studies are the need of hour to know more about the etiopathogenesis of AES, so that future strategies to bring down the mortality and morbidity due to this disease can be carried out.

Abstract:
Background: The aim of this study is to compare outcome of inguinal hernia repair by combined method of repair (Lichtenstein and Bassini repair) and its comparison with older techniques.Methods: A prospective study conducted on 158 patients reported in surgery OPD Patna Medical College and Hospital, Patna from July 2017 to June 2018 with inguinal hernia. All these patients were included in the study, operated by combined method of repair (Lichtenstein and Bassini repair) and followed up for 12 months. Data were entered and analyzed by SPSS version 10.0.Results: Study involved 158 patients who were operated by combined repair. Pain on 1st post-operative day was similar with previous study. Complication was erythema, scrotal swelling, neuralgia, superficial wound infection and seroma formation in the order of frequency. The average hospital stay was 5 days for combined approach. Recurrence rate was 0% in combined approach following 1 year of follow-up.Conclusions: As compared with previous studies of hernia repair by Modified Bassini’s and Lichenstein mesh hernioplasty, combined approach have comparatively better outcome.

Introduction: The aim of this study was to enumerate the causes of Fluid-attenuated inversion recovery (FLAIR) hyperintensity in the sulcal space which could be due to cerebrospinal fluid (CSF) or non-CSF related pathologies. Methods and Materials: This is an observational retrospective study done in 100 patients in the department of Radio-diagnosis of Patna Medical College and Hospital, Patna from September 2019 to September 2020. Results: The mean age of patients was 40.13 +/- 13.88 years (Range 19 to 75 years). Male to female ratio was 1.32:1.00 (57:43). The most common cause of FLAIR sulcal hyperintensity was infection in 66% cases followed by meningeal tumor deposits in 16%, vascular cause in 6%, subarachnoid hemorrhage in 5%, mass effect in 5%, dermoid rupture in 1% and hyperoxygenation in 1%. Conclusion: Presence of sulcal hyperintensity on FLAIR images is a very strong sign to an underlying brain pathology. A keen observation of the same on FLAIR sequence in association with other findings can increase diagnostic confidence and thus lead to better patient care.

The therapeutic efficacy of drugs depends upon the genuineness of raw material and right. Shilajeet is an important herbomineral drug among the Maharasa. Describing its importance Charaka has said, “there is hardly any curable disease which cannot be alleviated or cured with the aid of Shilajeet. Aim of study is to evaluate the Physico-chemical parameterof Salsaradi Gana Bhawit Shilajatu. Shilajatu and all raw drugs were obtained from P.G. Department, G.A.C.H. Patna and Shilajeet Shodhana was done by Triphala Kwatha and three times Bhawana by Salsaradigana (Dravyas) Kwatha. All samples were analyzed at Laboratory of Govt. Ayurveda College and Hospital Patna. It found that Shilajatu after Bhawana appeared as semi solid colour brownish black, soft in consistency and having typical smell.

Background: Medical method of abortion (MMA) is a safe and effective method of abortion. Combination of mifepristone and misoprostol is most widely used. However, this is being randomly used by women without proper prescription which can lead to life threatening complications.Methods: This is an observational study done at Patna Medical College and Hospital, Patna, a tertiary care teaching hospital in Bihar, in a period of one year from March 2019 to February 2020. This is an attempt to study incomplete abortion after medical method of abortion and to observe the method of taking it among patients, with prescriptions or without it. Hundred women with incomplete abortion following MMA in 1st trimester of pregnancy were included. Patients’ age, parity, gestational age, locality, complaints, complications and treatment were noted. An information regarding method of administration and prescription noted.Results: 96% women used combined mifepristone plus misoprostol drug. Only 12% took the medicine on prescription of MBBS doctor, rest were all either self-administered or advised by quacks. Blood transfusion was required in 60% patients.Conclusions: Medical method of abortion is safe and effective but complications can occur if not used in accordance with guidelines. Women should be taught about and motivated for contraception. Adequate training to health care providers about comprehensive abortion care should be given.

Abstract:
This chapter looks at the promise and peril of police use of big data analytics for inequality. On the one hand, big data analytics may be a means by which to ameliorate persistent inequalities in policing. Data can be used to “police the police” and replace unparticularized suspicion of racial minorities and human exaggeration of patterns with less biased predictions of risk. On the other hand, data-intensive police surveillance practices are implicated in the reproduction of inequality in at least four ways: by deepening the surveillance of individuals already under suspicion, codifying a secondary surveillance network of individuals with no direct police contact, widening the criminal justice dragnet unequally, and leading people to avoid institutions that collect data and are fundamental to social integration. Crucially, as currently implemented, “data-driven” decision-making techwashes, both obscuring and amplifying social inequalities under a patina of objectivity.
