Abstract:
Background:Juvenile systemic sclerosis (jSSc) is an orphan disease with a prevalence in around 3 in a million children. Pulmonary involvement in jSSc occurs in approximately 40 % in the inception cohort. Traditionally in jSSc, pulmonary function testing (PFT) with FVC and DLCO are used for screening and computed tomography (HRCT) was more reserved for those with abnormal PFTs. More recently, it has become apparent that PFTs might not be sensitive enough for detecting ILD in children.Objectives:Utilizing a prospective international juvenile systemic scleroderma cohort (JSScC) [2], to determine if pulmonary screening with FVC and DLCO is sufficient enough to assess the presence of interstitial lung disease in comparison to CT evaluation.Methods:The international juvenile systemic scleroderma cohort database was queried for available patients with recorded PFT parameters and HRCT performed to determine sensitivity of PFTs detecting disease process.Results:Of 129 patients in the jSScC, 67 patients had both CT imaging and an FVC reading from PFTs for direct comparison. DLCO readings were also captured but not in as many patients with tandem HRCT (n =55 DCLO and HRCT scan). Therefore, initial analyses focused on the sensitivity, specificity and accuracy of the FVC value from the PFTs to capture the diagnosis of interstitial lung disease as determined by HRCT.Overall, 49% of the patients had ILD determined by HRCT, with 60% of patients having normal FVC (>80%) with positive HRCT findings, and 24% of patients having normal DLCO (> 80%) with positive HRCT findings. Fourteen percent (n = 3/21) of patients with both FVC and DLCO values within the normal range had a positive HRCT finding.Conclusion:The sensitivity of the FVC in the JSScC cohort in detecting ILD was only 39%. Relying on PFTs alone for screening for ILD in juvenile systemic sclerosis would have missed the detection of ILD in almost 2/3 of the sample cohort, supporting the use of HRCT for detection of ILD in children with SSc. In addition, the cut off utilized, of less than 80% of predicted FVC or DLCO could be too low for pediatric patients to exclude beginning ILD. Abstract:
Background:Juvenile systemic scleroderma (jSSc) is an orphan disease with a prevalence of 3 per 1 000 000 children. There are limited data regarding the clinical presentation of jSSc. The Juvenile Systemic Scleroderma Inception Cohort (JSSIC) is the largest multinational registry that prospectively collects information about jSSc patients.Objectives:Evaluation of the jSSc patients at the time of inclusion in the JSSIC.Methods:Patients were included in the JSSIC if they fulfilled the adult ACR/EULAR classification criteria for systemic scleroderma, if they presented the first non-Raynaud symptom before 16 years of age and if they were younger than 18 years of age at time of inclusion. Patients’ characteristics at time of inclusion were evaluated.Results:Until 15thof December 2019 hundred fifty patients were included, 83% of them being Caucasian and 80% female. The majority had the diffuse subtype (72%) and 17% of all jSSc had overlap features. The mean age of first presentation of Raynaud´s phenomenon was 9.8 years in the diffuse subtype (djSSc) and 10.7 years in the limited subtype (ljSSc) (p=.197). The mean age at first non-Raynaud’s symptoms was 10.0 years in the djSSc and 11.2 years in the ljSSc (p=0.247). Mean disease duration at time of inclusion was 3.4 years in the djSSc and 2.4 years in the ljSSc group.Significant differences were found between the groups regarding mean modified Rodnan skin score, 18.2 in the djSSc vs 6.2 in the ljSSc (p=0.02); presence of Gottron´s papulae (djSSc 30% vs ljSSc 13%, p=0.43);presence of teleangiectasia (djSSc 42% vs 18% ljSS, p=0.01); history of ulceration (djSSc 42% vs 18% ljSSc,p=0.008); 6 Minute walk test below the 10thpercentile (djSSc 85% vs ljSSc 54%, p=0.044), total pulmonary involvement (djSSc 49% vs ljSSc 31%, p=0.045), cardiac involvement (ljSSc 17% vs djSSc 3%, p=0.002). djSSc patients had significantly worse scores for Physician Global Assessment of disease activity compared to ljSSc patients (VAS 0-100) (40 vs 15) (p=0.001) and for Physician Global Assessment of disease damage (VAS 0-100) (36 vs 17) (p=0.001).There were no statistically significant differences in the other presentations. Pulmonary hypertension occurred in approximately 6% in both groups. No systemic hypertension or renal crisis was reported. ANA positivity was 90% in both groups. Anti-Scl70 was positive in 35% in djSSc and 36% in the ljSSc group. Anticentromere positivity occurred in 3% in the djSSc and 7% in the ljSSc group.Conclusion:In this unique large cohort of jSSc patients there were significant differences between djSSc and ljSSc patients at time of inclusion into the cohort regarding skin, vascular, pulmonary and cardiac involvement. According to the physician global scores the djSSc patients had a significantly more severe disease. Abstract:
Nella poesia italiana contemporanea si possono rintracciare diverse linee di tendenza. Per quanto riguarda la veste metrica, ad esempio, spicca, da un lato, l’inclinazione al recupero di forme chiuse tradizionali, ossia il neometricismo, cui si contrappone, dall’altro, l’apertura alla discorsività rappresentata dalla poesia in prosa. Entrambi gli indirizzi sono stati recepiti come una risposta ad una crisi poetica (già da Contini e Mengaldo), mentre la loro interpretazione come possibili manifestazioni letterarie del postmoderno appare piuttosto controversa. Per ciascuno dei due orientamenti formali si è proposta l’analisi di parte della produzione di un autore solitamente ritenuto rappresentativo della corrente entro cui è inserito: Patrizia Valduga per il neometricismo, Stefano Dal Bianco per la poesia in prosa. Lo studio delle rispettive opere è incentrato in particolare sulla metrica, il cui diverso trattamento oppone i due scrittori, e sulla sintassi, in quanto sistemi differenti di organizzazione del discorso. L’approfondimento della poesia di Valduga e Dal Bianco ha evidenziato, oltre alla prevedibile antitesi per ciò che concerne soprattutto il versante formale (ad esempio, il recupero o il rifiuto della metrica, uno stile sofisticato o, per converso, privo di escursioni significative), e ad alcune analogie puntuali, due rimarchevoli elementi di affinità. Ambedue i poeti, innanzitutto, dimostrano sì di appartenere alla tendenza di cui sono esponenti, ma non vi aderiscono pedissequamente, ciascuno rendendo la propria traiettoria poetica originale, interessante perché frutto di una consapevolezza maggiore della problematicità del dato formale. Valduga e Dal Bianco, infine, pur partendo da presupposti distanti ed elaborando produzioni molto differenti, paiono approdare entrambi ad una riabilitazione del concetto di classicismo, benché questo venga naturalmente declinato in modi diversi, secondo le rispettive esperienze poetiche.

Il lavoro si presenta come un esempio propedeutico a un'eventuale proposta di traduzione. Nel caso specifico, si tratta di approcciarsi alla trasposizione dall'italiano al francese di "Requiem", raccolta di poesie di Patrizia Valduga dedicata alla morte del padre. Fulcro dell'elaborato non sarà, paradossalmente, la traduzione stessa, quanto l'analisi del testo in tutti i suoi fondamentali elementi - dalle figure retoriche alle principali tematiche, dalle immagini evocate ai riferimenti culturali - da cui è assolutamente necessario partire per comprendere il contenuto in maniera completa e coesa, prima di poter azzardare una traduzione che resta, comunque, costantemente in divenire. La traduzione proposta, infatti, non è altro che il prodotto risultante da questa complessa operazione, e che tiene in considerazione tutte le caratteristiche con cui si è misurato, tra cui quella, non di poco conto, dell'incapacità del dire, dell'esprimere appieno un dolore lacerante, straziante, con il quale convivere, costretti ad andare avanti nella vita di tutti i giorni.

À partir des années soixante-dix les femmes poètes s'affirment avec énergie dans le panorama littéraire italien. Bien que leur présence dans les anthologies les plus reconnues soit encore assez réduite, leurs écritures poétiques se caractérisent par une puissante originalité. L'objectif de cette thèse est d'étudier ces productions poétiques tout en s'interrogeant sur la possibilité d'identifier des éléments communs. Il ne s'agit pas de définir une spécificité féminine dans une perspective essentialiste, mais d'interpréter certaines données textuelles en se référant à un contexte historique et culturel bien défini. Le corpus de travail comprends les œuvres des femmes poètes suivantes: Amelia Rosselli (1930-1996), Alda Merini (1931), Jolanda Insana (1937), Patrizia Cavalli (1947) et Patrizia Valduga (1953). Afin de respecter les singularités de chaque écriture poétique et d’éviter des simplifications réductrices, nous avons accordé la priorité à l'analyse textuelle et organisé les chapitres selon une approche monographique plutôt que thématique. L'analyse révèle que ces femmes poètes abordent avec obstination trois nœuds problématiques: elles développent une réflexion sur la valeur de la langue poétique, ont tendance à structurer leur poésie autour d'oppositions fortes et irréductibles, et, enfin, font souvent appel à des éléments de type théâtral. Ces caractéristiques sont interprétées en considérant le rapport ambigu et contradictoire que le sujet féminin entretient avec le code poétique: il ressent un fort besoin d'inscription dans la langue poétique, mais aussi un sens d'extranéité. Cette tradition qui lui est chère ne lui appartient pas complètement car il a été exclu de son élaboration
As from the seventies women poets assert themselves with energy in the Italian literary survey. Even if their presence in the most famous anthologies is curtailed, their poetic writings are characterized by a powerful originality. The aim of this thesis is to study their poetic production inquiring into the possibility of common elements. It’s not about defining a female specificity from an essentialist point of view, the purpose is rather to interpret some textual data with reference to a well-defined historical and cultural context. The study corpus includes works of the following Italian woman poets: Amelia Rosselli (1930-1996), Alda Merini (1931), Jolanda Insana (1937), Patrizia Cavalli (1947) and Patrizia Valduga (1953). In order to respect the particularities of every writing and to avoid hasty and restrictive simplifications, a great importance has been conferred to the textual analysis and the chapters have been arranged according to a monographic approach rather than a thematic structure. The result of the research demonstrates that contemporary Italian women poets attend insistently three knotty problems: they develop a reflection about the power of poetic language, they display a tendency to structure their verse on strong oppositions without appeasing synthesis and they often resort to dramatic elements. These characteristics can be interpreted referring to the double and conflicting position of the female subject towards poetic code: this new subject needs to inscribe himself in the poetic language, but he also feels a sense of extraneousness. The tradition he loves doesn’t belong completely to him as he was left out of its elaboration

A partire dagli anni Settanta le donne si affermano con energia all'interno del panorama poetico italiano. Sebbene la loro presenza nelle antologie più note sia ancora abbastanza ridotta, i loro testi poetici sono caratterizzati da una spiccata originalità. L'obiettivo di questa tesi è quello di studiare queste produzioni poetiche interrogandosi sulla possibilità di identificare alcune costanti. Non si tratta di definire una specificità femminile in prospettiva essenzialistica, ma d'interpretare alcuni dati testuali facendo riferimento a un contesto storico e culturale ben preciso. Il corpus di riferimento comprende le opere delle seguenti poetesse: Amelia Rosselli (1930-1996), Alda Merini (1931), Jolanda Insana (1937), Patrizia Cavalli (1947) e Patrizia Valduga (1953). Al fine di rispettare la singolarità di ogni scrittura poetica ed evitare semplificazioni riduttive, grande importanza è stata attribuita all'analisi testuale e i capitoli sono stati organizzati secondo un approccio monografico piuttosto che tematico. L'analisi rivela come le poetesse affrontino con ostinazione tre nodi problematici: sviluppano una riflessione sul valore della lingua poetica, hanno la tendenza a strutturare la loro poesia attorno a opposizioni forti e irriducibili e, infine, fanno spesso ricorso a elementi di tipo teatrale. Queste caratteristiche vengono interpretate prendendo in considerazione il rapporto ambiguo e contraddittorio che il soggetto femminile intrattiene con il codice lirico: prova un forte bisogno d'iscrizione nella lingua poetica, ma anche un senso di estraneità. Questa tradizione che gli è cara non gli appartiene completamente perché è stato escluso dalla sua elaborazione.

Le corpus de travail comprend les œuvres des femmes poètes suivantes: Amelia Rosselli (1930-1996), Alda Merini (1931), Jolanda Insana (1937), Patrizia Cavalli (1947) et Patrizia Valduga (1953). Afin de respecter les singularités de chaque écriture poétique et d'éviter des simplifications réductrices, nous avons accordé la priorité à l'analyse textuelle et organisé les chapitres selon une approche monographique plutôt que thématique. L'analyse révèle que ces femmes poètes abordent avec obstination trois nœuds problématiques: elles développent une réflexion sur la valeur de la langue poétique, ont tendance à structurer leur poésie autour d'oppositions fortes et irréductibles, et, enfin, font souvent appel à des éléments de type théâtral. Ces caractéristiques sont intérprétés en considérant le rapport ambigu et contradictoire que le sujet féminin entretient avec le code poétique: il ressent un fort besoin d'inscription dans la langue poétique, mais aussi un sens d'extranéité. Cette tradition qui lui est chère ne lui appartient pas complétement car il a été exclu de son élaboration.
