COVID-19 infection affects many systems in the body including the coagulation mechanisms. Imbalance between pro-coagulant and anticoagulant activities causes a roughly nine times higher risk for pulmonary embolism (PE) in COVID-19 patients. The reported incidence of PE in COVID-19 patients ranges from 3 to 26%. There is an increased risk of PE in hospitalized patients with lower mobility and patients requiring intensive care therapy. Obesity, atrial fibrillation, raised pro-inflammatory markers, and convalescent plasma therapy increases the risk of PE in COVID-19 patients. Endothelial injury in COVID-19 patients causes loss of vasodilatory, anti-adhesion and fibrinolytic properties. Viral penetration and load leads to the release of cytokines and von Willebrand factor, which induces thrombosis in small and medium vessels. D-dimers elevation gives strong suspicion of PE in COVID-19 patients, and normal D-dimer levels effectively rule it out. Point of care echocardiogram may show right heart dilatation, thrombus in heart or pulmonary arteries. DVT increases the risk of developing PE. The gold standard test for the diagnosis of PE is CTPA (computerized tomographic pulmonary angiography) which also gives alternative diagnosis in the absence of PE. Therapeutic anticoagulation is the corner stone in the management of PE and commonly used anticoagulants are LMWH (low molecular weight heparin) and UFH (unfractionated heparin). Mortality in COVID-19 patients with PE is up to 43% compared to COVID patients without PE being around 3%.

Abstract:
Venous thromboembolism represents a true worldwide medical problem that is encountered within all realms of practice. Venous thromboembolism (VTE) occurs in approximately 100 patients per 100,000 population yearly in the United States and increases exponentially with each decade of life (White, 2003). Approximately one-third of patients with symptomatic deep vein thrombosis (DVT) experience a pulmonary embolism (PE). Death occurs within 1 month in 6% of patients with DVT and 12% of those with PE. Early mortality is associated strongly with presentation as PE, advanced age, malignancy, and underlying cardiovascular disease. An experience dating back several decades has provided a better understanding of disease states and conditions associated with VTE (Anderson and Spencer, 2003). Given the potential morbidity and mortality associated with VTE, it is apparent that prophylaxis represents an important goal in clinical practice. A variety of anticoagulants including unfractionated heparin, low-molecular-weight heparin (LMWH), and warfarin have been studied. More recently, two new agents have been developed that warrant discussion. Fondaparinux underwent a worldwide development program in orthopedic surgery for the prophylaxis of VTE. The program consisted mainly of four large, randomized, double-blind phase II studies comparing fondaparinux (SC), at a dose of 2.5 mg starting 6 hours postoperatively, with the two enoxaparin regimens approved for VTE prophylaxis—40 mg qd or 30 mg twice daily beginning 12 hours postoperatively. The results support a greater protective effect with fondaparinux, yielding a 55.2% relative risk reduction of VTE (Bauer et al., 2001; Eriksson et al., 2001; Lassen et al., 2002; Turpie et al., 2001, 2002; ). A European program of three large-scale clinical trials (MElagatran for THRombin inhibition in Orthopedic surgery [METHRO] I, II, and III, and EXpanded PRophylaxis Evaluation Surgery Study [EXPRESS]) (Eriksson et al., 2002a, b, 2003a, b) evaluated the safety and efficacy of subcutaneous melagatran followed by oral ximelagatran compared with LMWH for thromboprophylaxis following total hip replacement (THR) and total knee replacement (TKR) surgery.

<EM>ABSTRACT </EM>Two potential therapeutants, fumagillin, an antibiotic derived from the fungus <em>Aspergillus fumagatus</em>, and TNP-470, a superactive analog of fumagillin, were tested for efficacy to prevent <em>Myxobolus cerebralis </em>infection in rainbow trout. The study was conducted at the Wild Trout Research Laboratory in Bozeman, Montana utilizing rainbow trout <em>Oncorhynchus mykiss </em>fry (~2.0 g). Treatment groups included negative controls (no treatment, no exposure), positive controls (no treatment, exposure), fumagillin top-dressed on feed, fumagillin incorporated in feed, and TNP-470 incorporated in feed. The exposure dose was 1000 triactinomyxons (TAMs) per fish for 2 h. Medicated feed treatment was initiated 24 h after exposure to TAMs. All treated groups received medicated feed for 10 d, with the exception of a single TNP-470 treatment group that was fed for 26 d. Five replicate tanks of fish were used for each treatment group. Presence and level of <em>Myxobolus cerebralis </em>infection were determined by histology, spore counts, and electron microscopy evaluation conducted 150 d postexposure. Hematology samples were also collected to evaluate potential toxic effects of treatment. Spore count and histological evaluation indicated that fumagillin and TNP-470 treatment was not efficacious in preventing or reducing <em>Myxobolus cerebralis </em>infection. Although fumagillin and TNP-470 administered for 10 d or 26 d did result in a reduction in spore numbers, results were not significantly different than observed in positive controls. Furthermore, histological scores were similar for all treatment groups and the positive controls. Although electron microscopy revealed spore deformation in both fumagillin and TNP-470 treated groups, no treatment group was effective in preventing <em>Myxobolus cerebralis </em>infection. Fumagillin treatment and TNP-470 fed for 10 d did not appear to negatively impact fish performance. However, toxicity was observed in fish fed TNP-470 for 26 d. These fish were observed to be lethargic 30 d pe, and blood samples revealed low hematocrits, severely decreased lymphocytes, and reduced numbers of blast cells. Histological evaluation revealed abnormal cytology in both the kidney and thymus. Based on study results, fumagillin and TNP-470 did not appear to be effective therapeutants for use in the prevention or control of <em>Myxobolus cerebralis </em>infection in rainbow trout.
