It describes the field of pediatric ethics, a unique and important aspect of the discipline of bioethics, the study of moral conduct in healthcare and the rational process for determining the best course of action in the face of conflicting choices. The book begins with an exploration of what it means to be a child in America and the unique kinship relationships and obligations engendered by the decision of parents to have a child, and it examines ethical principles and professional obligations related to the care of children. Each of the chapters in the book focuses on important ethical concerns. It begins with ethical issues in creating babies using reproductive technologies, and then continues with an analysis of the ethical issues in labor and delivery of a child. The book continues with an in-depth analysis of the many hard choices faced by families and clinicians in the care of critically ill neonates, and then it goes on to describe current controversies in caring for older children who are dying and their families, as well as ethical issues concerning adolescents; research ethics as it relates to children; issues concerning genetic testing, screening and biobanking; and surgical and medical enhancement of children. Each chapter has case examples to illustrate the real-life concerns of patients, families, and clinicians. As robots leave the lab and are deployed in hospital or other healthcare settings, the community of users may become overreliant on and overtrust such technology. Thus, there is a pressing need to examine the tendency to overtrust and develop strategies to mitigate the risk to children, parents, and healthcare providers that could occur due to an overreliance on pediatric robotics. To overcome this challenge, we seek to consider the broad range of ethical issues related to the use of robots in pediatric healthcare. This chapter provides an overview of the current state of the art in pediatric robotics, describes relevant ethical issues, and examines the role that overtrust plays in these scenarios. We conclude with suggested strategies to mitigate the relevant risks and describe a framework for the future deployment of robots in the pediatric domain.

This chapter responds to the sparse attention paid to the connections between religion, spirituality, and pediatric quality of life in healthcare settings. Key points of spiritual distress in the context of pediatrics, including intensive care settings, diagnostic meetings, and oncology, are explored. In order to envision a robust embrace of religious and spiritual issues in pediatric care, this chapter analyzes core themes from the perspectives of caregivers and clinicians. Building on these themes as well as current best practices from the literature, this chapter proposes an expanded vision and accompanying concrete practices to improve attention to and integration of religion and spirituality in pediatric care. The chapter concludes that an expansion of creative engagement with the religious and spiritual components of pediatric healthcare would foster more interdisciplinary, holistic practices of care.

Over the last decade, the number of neuroimaging and other neuroscience studies on the developing brain from fetal life through adolescence has increased exponentially. Children are viewed as particularly vulnerable members of our society and observations of significant neural structural changes associated with behavioral anomalies raise numerous ethical concerns around personal identity, free will, and the possibility of an open future. This chapter provides a review of recent research in the pediatric neuroscience literature, common pediatric decision-making, and social justice models, and discusses the implications of this research for the future of pediatric ethics thinking and policy. New research presents challenges to professional and pediatric bioethicist views of the moral future of children in pediatric healthcare and opportunities to examine anew notions of how to consider the developing moral agency of children.

Sleep is important for children’s physical, cognitive, and social functioning. The most common sleep disorder contributing to inadequate sleep and daytime impairments in pediatric populations is insomnia. There are both biological and behavioral factors that can contribute to insomnia; therefore, understanding sleep processes, how sleep changes throughout development, and which common behaviors influence sleep is important. Measurement of sleep through both objective and subjective measures plays an important role in the identification, diagnosis, and intervention of insomnia. First-line treatment for insomnia includes psychoeducation, healthy sleep practices, and behavioral and cognitive strategies. Children with special needs are at especially high risk for sleep problems, and treatment of insomnia in these populations should aim to minimize sleep disturbances without increasing other comorbid symptoms. Moving forward, healthcare professionals require access to more comprehensive education on pediatric sleep, and greater public awareness of the importance of sleep and health is needed.

Physical health concerns are common among youth and are linked to mental health. Attention to the interplay between physical and mental health is critical for healthcare providers. This chapter highlights crosscutting issues, assessments, and interventions relevant to child health populations. To understand the interaction between medical and psychological health in youth, chronic pediatric conditions (Type 1 diabetes and sickle cell disease) are described as prototypes for understanding psychosocial issues (e.g., adherence, pain management) that affect youth with medical conditions. Evidence suggests that these children with medical conditions, especially those poorly managed or controlled, are at greater risk for psychosocial issues (e.g., stress, comorbid psychological concerns, family conflict) compared to the general population. Careful risk assessment and individual or family interventions are critical for these youth and are a focus here. Well-established interventions for diverse youth with medical conditions are discussed, and recommendations for future work in this area are provided.

Radiation considerations are an integral part of the practice of nuclear cardiac imaging. Concern regarding radiation has increased in recent years, reflected in statements by many professional societies, and likely attributable both to rapid growth in use of nuclear cardiology as well as high doses received by some nuclear cardiology patients. The fundamental principles of medical radiological protection are justification (ensuring that the right test is performed for the right patient at the right time), optimization (ensuring that the test is performed in the right manner), and dose limitation, which while applicable to healthcare workers is not operative regarding patients. Three "As" facilitate and serve as an organizing principle for justification: awareness, appropriateness, and audit. Awareness incorporates knowledge of the benefits and risks of testing involving radiation and effective communication of these to the patient. Appropriateness in nuclear cardiology can be assessed using the American College of Cardiology's appropriateness criteria. Methods that have been demonstrated to improve appropriateness include using a collaborative learning model, a point-of-order decision support tool, and a multifaceted intervention including threatened loss of insurance coverage. A variety of strategies should be considered for optimization to ensure patient-centered imaging. These including strategic selection of both the protocol, e.g. selecting a stress-first protocol and performing stress-only imaging in patients without a high pre-test probability of abnormal findings on stress imaging, or using PET, and also the administered activity, e.g. by using weight-based dosing and/or software- or hardware-based advances in camera technology. Special considerations are required for pregnant, nursing, and pediatric patients.
