Abstract:
Objective. Assessment of the effectiveness of pelotherapy in combination with phytoaromatic baths in women during menopause transition. Patients and methods. We examined 26 women aged 52 ± 2.4 years. All women with vegetative-vascular manifestations of the climacteric syndrome were randomly assigned to 2 groups for comparison: group 1 – 12 patients, who received full-body “rapa” baths, inhalation phytoaromatherapy with a complex composition including phytoestrogens; group 2 – 14 patients, who received pelotherapy with applications of Saki lake mud on the «panty» zone and hydrodynamic phytoaromatic baths with a complex composition including phytoestrogens. Results. After therapy, a significant improvement of the parameters of climacteric disorders was noted in group 2. Severe disorders were practically absent in the second group – 2% (65 ± 6.1 scores), whereas mild changes prevailed – 64% (18 ± 5.3 scores). Changes in the microcirculation parameter were found, which increased by 19%, an increased contribution of endothelial oscillations to the total flux motion level, an increase of the amplitude of neurogenic and myogenic oscillations, a decrease of respiratory rates on average from 9.2 to 5.51 perf. units. In the first group, no significant changes were recorded. Conclusion. The use of pelotherapy in combination with hydrodynamic phytoaromatic baths permits to improve the parameters of microcirculation, venous and lymph output, trophism and oxygenation of tissues, and also ensures an adaptogenic effect. AbstractIllitic-smectitic clays were matured with a ferruginous, bicarbonate-rich and sulphate-rich water for different periods of time (7, 15, 30, 60 and 90 days), and subjected to discontinuous stirring, to test their use in pelotherapy. A progressive degradation of the clay minerals was observed during the maturation process, with a reduction in the illite and smectite crystallinity, in the percentage of smectites and in the phyllosilicate content. The percentage of <2 μm particles also decreased. These modifications caused changes in the clays’ physicochemical properties, which are evident in an increase in their plasticity index and in slower cooling of the peloid, both of which improved the clays’ effect in pelotherapy.

Abstract:
The pyelonephritis is the most frequent disease of urinary system in children with long current and process synchronization. Up to the present time, the scientific studies on the effectiveness of pelotherapy at the chronic pyelo-nephritis in children with ecological burdened history aren’t carried out; there are no methods of its application for this population group of patients. 40 children, aged 7-14 years, with chronic pyelonephritis in ecological burdened history were examined. In the complex Spa treatment, the pelotherapy was held traditionally on the lumbar region or developed adaptive-reflex technique. The advantage of the proposed approach to pelotherapy been proven that confirmed the best elimination of the complaints of patients and more pronounced dynamics of the signs of micro-hematuria, pyuria, oxaluria and recovery antichristianity ability urine on calcium phosphates in all patients. In comparison with the conventional treatment, the proposed technique is less invasive and more adequate, providing a gradual and consistent involvement of various reflex zones, which is important in violation of the adaptation processes in patients with ecologo-hygienic history. In addition, it is proved that the appointment of mud applications on a delicate mode contributes to the stimulation of protective forces of an organism, mainly through regulatory and training effects on various systems of the child&#180;s body, without causing general and local pathological reactions.

Abstract:
To increase the effectiveness of the rehabilitation of patients after surgical treatment of a rotator cuff rupture, 98 patients were examined and treated. It was revealed that the inclusion of exercises with dynamic controlled muscle tension in combination with thin layer pelotherapy in the medical complex, has an impact on the main clinical symptoms of the disease, regeneration processes, and contributes to an earlier restoration of the impaired functions of the shoulder joint, which is more significant than basic therapy.

Abstract Background. Balneotherapy is a stimulation - adaptation treatment method applied in the forms of bathing, drinking, and inhalation cures performed with natural therapeutic factors, a method which is acting in three main ways: thermally, mechanically, and chemically. Mud or peloids are natural therapeutic factors formed by natural processes under the influence of biological and geological phenomena, which in a finely dissolved state and mixed with water (mud) are used in medical practice in the form of baths or local procedures. Objective. This systematic review aims to rigorously select related articles and identify within their content, the main possible uses of therapeutic mud and physiological mechanisms, to see the main region of scientific interest for pelotherapy, and to discuss the value of mud therapy in rehabilitation medicine. Methods. The working method is based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched for open-access articles published in English, between January 2015 and December 2020, from the following databases: Cochrane, Elsevier, NCBI/PubMed, NCBI/PMC, PEDro, and ISI Web of Knowledge/Science (the latter was also used to identify ISI indexed articles). The contextually searched syntax used was ”Pelotherapy/Peloidotherapy/Mud-therapy/ /Fango-therapy AND Rehabilitation”. The selected articles were analyzed in detail regarding pathologies addressed by mud therapy and country scientific relevance for this therapeutic method. The meta-analysis proceeded was designated to estimate the prevalence of various pathologies in the use of mud therapy. Results. Our search identified, first, 394 articles. Based on the successive filtering stages and, respectively, on the classification criteria of the Physiotherapy Evidence Database (PEDro), we finally identified/retained and analyzed 68 articles. Although, in principle, a rigorous method – and we have followed the PRISMA type paradigm – there still might be some missing works of our related article selection. On the other hand, to augment/ consolidate our documentation base, we have used also 40 papers freely found in the literature, and even – aiming, too, at an as exhaustive knowledge underpinning as possible – derogatively, we have also considered some articles which, probably being very new, couldn't yet have reached the PEDro threshold score we have settled. Conclusions. This paper overviews the current state-of-the-art knowledge in the approach of peloidotherapy in rehabilitation, with a focal point on the therapeutic properties of peloids. The method consists in the combined use of peaty mud applications on lower limb regions, magnetolaser therapy, and electrosleep. The investigation involved 88 patients with the verified hypertension diagnosis. It has been found that the combined medical rehabilitation exerts the positive effect of the main links of pathogenesis, improves the adaptation abilities of an organism, and thus increase the efficiency of treatment and the patients’ life quality.

Clinical, laboratory, functional, and radiological examination of 82 patients with slowly resolving pneumonia was performed. The average age of the patients was 42.0 ± 2.4 yrs, 34 (41.5 %) were males and 48 (58.5 %) were females. The patients were randomly divided into 3 comparative groups: the 1st and the 2nd groups received pelotherapy and naftalan, respectively, in combination with medications for relief of symptoms and physiotherapy. The 3rd group (control) patients received medications for relief of symptoms and physiotherapy. Implementation of these methods in treatment of patients with slowly resolving pneumonia could promote regress of residual inflammation, considerably shorten length of the disease, reduce medication load and need in healthcare resources that, in turn, could reduce healthcare expenditures for treatment of such patients. Clinical results were confirmed by short- and long- term results of morphologic and blood biochemical investigations, microscopic examination of sputum, lung function testing, immunological, bronchoscopic and radiological data, investigations of physical capacity and quality of life.

Abstract:
We report a developed treatment technique with the use of physical factors for treatment of bronchial asthma patients with accompanying gastrointestinal pathology. A total of 129 patients were divided into three groups by the method of treatment: 1st (main) group (53 patients) received peaty mud applications and traveling magnetic field (TMF), 2nd group (45 patients) received TMF, and 3rd group (31 patients) received peaty mud applications. The study of the dynamics of clinical pattern, biochemical, immunological, functional, and endoscopic data has shown the efficiency of the combined application of pelotherapy and TMF, which exert the positive effect on the main and accompanying diseases and provide high immediate and long-term results of treatment.

Abstract:
Evaluation of hygienic aspects of thermal mud microbiology is still neglected. This study evaluates the microbiological hygiene quality of thermal muds, providing a comprehensive assessment of the whole mud cultivation chain. Maturing mud, peloid and used mud samples were collected twice in a year from 30 SPAs of the Euganean Thermal District, NE Italy. Samples were processed with an ad hoc laboratory method. The following indicator parameters were assessed: Total Count at 22, 37 and 55 °C; total coliforms; Escherichia coli; enterococci; Staphylococcus aureus; Pseudomonas aeruginosa; sulfite-reducing clostridia; dermatophytes. Statistical significance of differences between the two sampling campaigns and correlation between temperature and indicator parameters were evaluated. One-hundred eighty samples were analyzed. Widespread presence of environmental species was found, as well as hints of possible microorganism transfer from the patient’s skin to the mud. Proper setting of thermal water temperature resulted critical, in terms of hygienic quality. Although optimal maturation should be granted (thermal water at 30–42 °C), a pasteurization step at 60–65 °C is strongly recommended to sanitize peloids before pelotherapy. Facilities re-using thermal muds should also implement a regeneration step at ≥65 °C. Core evaluation of thermal mud hygienic quality could encompass the following guidelines: absence (i.e., 0 colony forming units (CFU)/g) of E. coli, P. aeruginosa, S. aureus and dermatophytes.

In this investigation, we examined the chemical composition, physicochemical properties, and organic matter of therapeutic mud from Lake Noot in the Arkhangai province of Mongolia. The therapeutic mud from “Lake Noot” is used in the pelotherapy of local health care services for some time but without deep characterization. Results showed that the therapeutic mud belongs to the continental hydrogen sulfide sticky mud type. The total organic matter in the therapeutic mud of Lake Noot was 14.44%. Total organic matter contains a humic substance of 42.17%, lipid 16.62%, and carbohydrate 7.13%. A total of 172 compounds were identified in the dissoluble organic matter of therapeutic mud, mainly of natural origin, using the gas chromatography-mass spectrometry (GC/MS) method. The dominant compounds were saturated and unsaturated hydrocarbons with 13–44 carbon atoms and carboxylic acids and their ethers. Some of the identified organic compounds have been reported antimicrobial, anti-inflammatory, antibacterial, and antifungal properties.

A peloid’s liquid phase can be mineral, sea or salt-lake water. This study examines the interactions among three materials, two special clays (bentonite and sepiolite) and one common clay, and three chemically different mineral-medicinal waters. In all clay–water mixtures, the hardness and adhesiveness decreased with the amount of water in the mixture. For a given hardness or adhesiveness, sepiolite retained more water than the other clays, especially in the presence of sulphate-rich mineral-medicinal water (73%). In contrast, the common clay retained the least amount of water (26%), and the bentonite samples returned an intermediate value for the amount of retained water (52–53%). These differences had a strong influence on the thermal behaviour of the mixtures and, hence, should be taken into account for their use in thermotherapy applications. There were no significant differences in the instrumental texture of the clay pastes according to the predominant anion in the mineral-medicinal waters.

Pelotherapy is the application of thermal muds (peloids) for recovering muscle, bone and skin pathologies. Specific criteria were established for the evaluation of the therapeutic suitability of peloids. Critical factors ruling the quality of the solid phase of peloids include granulometry, mineralogy, and physical-chemical properties. The aim of the present study was to investigate the textural, mineralogical and geochemical characteristics of the Pikrolimni Lake sediments (PLS), Kilkis district (N. Greece) in order to assess their suitability for application as raw material into mud therapy technologies. Representative PLS samples were collected and analyzed by means of different techniques at the analytical laboratories of the Hellenic Survey of Geology and Mineral Exploration (HSGME). Results showed that, texturally, the PLS were classified as sandy loam sediments. X-ray diffraction (XRD) study revealed that the PLS consist mainly of quartz, clay minerals (kaolinite, montmorillonite, illite), and muscovite, with minor feldspar and calcite. X-ray fluorescence (XFR) analysis showed that PLS samples have higher SiO2 and lower Al2O3 contents compared to the average shale composition. Among the analyzed, by means of inductively coupled plasma mass spectrometry (ICP-MS) technique, potentially toxic elements only As and Pb contents were found significantly higher than those of the average shale. Based on the calculated mean values of enrichment factor (EF) and geo-accumulation index (Igeo) PLS displayed minor enrichment and moderate contamination by Pb and As, respectively. In conclusion, an improvement of the grain size composition of PLS by means of a sieving process is needed. Besides, the study of the concentration and behavior of toxic elements in peloids formed by the mixing of the processed PLS with salty thermo-mineral water is proposed.

In Portugal there are several natural occurrences of clays/muds which are used for pelotherapy and/or geotherapy. These are carried out either indoors (thalassotherapy and thermal centres) or outdoors, in natural sites generally located near the seaside. The aim of this study is to assess the mineralogical and physicochemical properties of Portuguese raw materials for therapeutic purposes. These materials were collected from different Portuguese Mesozoic-Cenozoic geological formations located in the neighbourhood of thermal centres or at beaches known from their empirical applications. X-ray diffraction (XRD) and scanning electron microscopy (SEM-EDS) were used to assess the mineralogical composition of these clays. Physicochemical properties, such as specific surface area, cation exchange capacity, plasticity/abrasiveness indices and heat diffusiveness were also determined. Having distinct geological ages and genesis, the materials examined are mainly illitic. Less abundant kaolinite and smectite are also present. With respect to their physicochemical properties, all samples have good thermal properties which make them potentially suitable for therapeutic or aesthetic purposes.

AbstractThe empirical application of muds for therapeutic purposes is widely known. This is called pelotherapy and consists of the local or generalized application of a mixture of a solid phase and a liquid phase (peloid) for the recovery of arthro-rheumatic issues, bone-muscle traumatic damage and dermatological pathologies. During the time of mixing (maturation process) the mud is progressively colonized by thermophilic microorganisms that contribute to improvements in the peloid quality and endow the mud with organic substances. Several studies report diatoms as the main agent of thermal muds capable of producing anti-inflammatory sulphoglycolipid which renders the mud suitable for this use. The effect of the temperature is also considered important for therapeutic purposes.Recent studies assessed physical, chemical and technological properties of some peloids in use at spa centres. Some maturation surveys have also been performed in recent years. The need for quality criteria establishment and certification of clayey products intended to be used currently is evident, especially for peloids which have therapeutic action. Consequently, this study aims at the compilation and analysis of some characteristics of peloids from the literature in order to contribute to a major database that allows the establishment of regulations and quality criteria for suitable applications of peloids.

The Osservatorio Termale Permanente (OTP) operated from 1996 to 2014 in the Italian and international therapeutic thermalism field. The main aim of OTP was the development of a multidisciplinary approach for the characterization of the quality of peloids for cutaneous application and their maturation process using different qualitative and quantitative analytic techniques opening at the Integrated Analytical Approach in the field of therapeutic thermalism. The main innovation introduced by OTP were rheology and surface tensiometry applications on peloids. The introduction of surface tensiometry included the skin as a fundamental element for the development of mud therapy protocols based on the bioadhesive properties of peloids. The interfacial investigations between peloids and skin led to the characterization of the exchange activity occurring during pelotherapy on the cutaneous surface in a non-invasive and fast way. On these bases, our work linked each other the multidisciplinarity approach to the study of hydrothermal resources performed by OTP with the concept of the participative methodology commonly applied to the development of projects in the field of development cooperation. This combined approach led us to develop a scientific proposal for the development of therapeutic thermalism in countries not yet improved in this field and characterized by the presence of hydrothermal resources. Clays are natural ingredients used to prepare therapeutic cataplasms suitable for topical application. The knowledge about these formulations and their preparations to be applied on humans and animals has been orally transmitted since ancient times. Several empirical methods using clays have demonstrated fast and effective results in the reduction of the inflammatory response and the formation of edemas in horse limbs. The use of traditional and alternative medicine, such as pelotherapy, is now becoming more popular in veterinarian medical practice, alone or combined with other therapies in horse muscle and tendon rehabilitation. This study characterizes the use of commercial equine clays and an old therapeutic clay cataplasm formulation, using acetic acid, to treat tendon injuries in horses. This work might contribute to a major database characterization of clays used empirically on equine health, the potential of dermal absorption, the risks of exposure to some toxic elements, and safety assessment for these formulations. The present study was carried out to characterize the suitability of four commercial equine clays (Group II) and a protocoled healing mixture: “clay acetic acid cataplasm”, (Group III), to treat tendon injuries in horses. In this mixture, three conventional “green” clays (Group I) without any mineralogical specificity were used and blended with acetic acid. The mineralogical composition was determined through X-ray powder diffraction and X-ray fluorescence data. To determine the performance of the samples, cooling kinetics, oil absorption, expandability, and specific surface area were measured. According to the mineralogical composition, Group I was mainly composed of carbonates and silicates, while Group II was much richer in silicates with the main clay minerals kaolinite and illite. Group II exhibited the highest values for As, Pb, Cr, Ni, and Zn, considered potentially toxic. Both groups showed low cation exchange capacities and exchanged mainly Ca2+, with the exception of VET.1 and VET.7, which also highlight Na+, and VET.5 and VET.6, which have K+ as an exchangeable main cation. The addition of acetic acid (Group III) does not reveal any significant chemical changes. The results confirm that both clay groups are adequate for the therapeutic propose. They have good plastic properties (skin adherence), good oil absorptive capabilities (cleaning), and exchange an essential physiological element, calcium. Group II has prior industrial preparation, which is probably why it showed better results. Group I presented lower heat retention capacity and higher abrasiveness, which could be improved using cosmetic additives. The clinical benefit of the “clay acetic acid cataplasm” (Group III) could be the systemic anti-inflammatory effect established by the acetic acid.

AbstractIn this study we aimed to compare the mineralogical, thermal, physicochemical, and biological characteristics of recent organic carbon-rich sediments (‘sapropels’) from three geographically distant Romanian lakes (Tekirghiol and Amara, SE Romania, and Ursu, Central Romania) with distinct hydrogeochemical origins, presently used for pelotherapy. The investigated lakes were classified as inland brackish Na-Cl-sulfated type (Amara), coastal moderately saline and inland hypersaline Na-Cl types (Tekirghiol and Ursu, respectively). The settled organic matter is largely composed of photosynthetic pigments derived from autochthonous phytoplankton. Kerogen was identified in the sapropel of coastal Tekirghiol Lake suggesting its incipient maturation stage. The mineral composition was fairly similar in all sapropels and mainly consisted of quartz, calcite, and aragonite. Smectite, illite, mixed layer smectite/illite appeared as major clay components. Potentially toxic elements were found in low concentrations. The physical properties (i.e., specific heat, thermal conductivity and retentivity) and cation exchange capacity are comparable to other peloids used for therapy. This study is the first comprehensive multi-approached investigation of the geochemical nature of recent sapropels in Romanian saline lakes and thus contributes to expanding our knowledge on the origin and physicochemical qualities of organic matter-rich peloids with therapeutic uses.
