Abstract:
Erectile dysfunction (ED) following treatment for clinically localized prostate cancer, particularly radical prostatectomy (RP), is a major quality of life issue that remains unsatisfactorily addressed. With the introduction and use of cavernous nerve–sparing procedures over the past 25 years, many men recover erections postoperatively that enable sexual intercourse unlike in the prior surgical era, when permanent ED postoperatively was certain. Despite this advance, 26–100% of these patients may never recover normal erectile function (EF). Recent advances in the understanding of ED after RP have stimulated great attention to develop penile rehabilitation programs and neuromodulation. The purpose of penile rehabilitation is to prevent adverse corpus cavernosal tissue structural alterations and thereby maximize the chances of recovering functional erections. Rehabilitation programs are common in clinical practice, but there is no definitive evidence to support their efficacy. Neuromodulation represents another strategy for promoting erection recovery postoperatively. This therapy involves the application of neuroprotective interventions, conceivably targeting biological elements involved in the erection response that are affected by neuropathic injury. Well-conducted, controlled trials with adequate follow-up are required in order to determine the erection preservative benefits of these therapeutic strategies. The purpose of this essay is to describe the mechanisms related to post-RP ED, assess the need for penile rehabilitation and neuromodulation following surgery, and analyze the basic science and clinical trial evidence associated with these applications for preserving EF following prostate cancer treatment.

Abstract:
Erectile dysfunction is the most common complication after pelvic radical surgery. Rehabilitation programs are increasingly being used in clinical practice but there is no high level of evidence supporting its efficacy. The principle of early penile rehabilitation stems from animal studies showing early histological and molecular changes associated with penile corporal hypoxia after cavernous nerve injury. The concept of early penile rehabilitation was developed in late nineties with a subsequent number of clinical studies supporting early pharmacologic penile rehabilitation. These studies included all available phosphodiesterase type 5 inhibitors, intracavernosal injection and intraurethral use of prostaglandin E1 and to lesser extent vacuum erectile devices. However, these studies are of small number, difficult to interpret, and often with no control group. Furthermore, no studies have proven an in vivo derangement of endothelial or smooth muscle cell metabolism secondary to a prolonged flaccid state. The purpose of the present report is a synthetic overview of the literature in order to analyze the concept and the rationale of rehabilitation program of erectile dysfunction following radical pelvic surgery and the evidence of such programs in clinical practice. Emphasis will be placed on penile rehabilitation programs after radical cystoprostatectomy, radical prostatectomy, and rectal cancer treatment. Future perspectives are also analyzed.

Abstract:
Introduction. Post-prostatectomy erectile dysfunction affects a considerable number of men and is a significant quality of life issue. There has been a substantial amount of research on the treatment of post-prostatectomy ED, and now there is a rising interest in the concept of penile rehabilitation. The goal of penile rehabilitation is to moderate the destructive processes that occur after prostatectomy in order to preserve erectile function, either through spontaneous or assisted means.Methods. We reviewed published data and experiences of post-prostatectomy penile rehabilitation using regimented interventions of phosphodiesterase inhibitors, vacuum erectile device, and intracavernosal agents, and we present and analyze the research conducted.Results. These studies show improved objective and subjective clinical outcomes in regards to physical parameters, sexual satisfaction, and rates of spontaneous erections.Conclusion. These studies are often limited by small size, study period, and study design. There continues to be a need for large, randomized, placebo controlled trials with adequate followup to fully evaluate the efficacy and cost-effectiveness of the various proposed penile rehabilitation regiments before a clear standard can be established.

Abstract:
Aims: To assess the diagnostic accuracy of four undiagnosed Type 2 diabetes mellitus risk scores accounting for erectile dysfunction status. Methods: This was a population-based cross-sectional study. Type 2 diabetes was defined according to a oral glucose tolerance test and self-reported physician diagnosis. Erectile dysfunction was defined according to the answer to the question, ‘Have you had difficulties obtaining an erection in the last 6 months?’ (yes/no). The risk scores used were the FINDRISC, LA-FINDRISC, American Diabetes Association score and the Peruvian Risk Score. A Poisson regression model was fitted to assess the association between Type 2 diabetes and erectile dysfunction. The area under the receiver-operating characteristic curve was estimated overall and by erectile dysfunction status. Results: A total of 799 men with a mean (sd) age of 48.6 (10.7) years were included in the study. The overall prevalence of Type 2 diabetes was 9.3%. Compared with healthy men, men with Type 2 diabetes had 2.71 (95% CI 1.57–4.66) higher chances of having erectile dysfunction. Having excluded men aware of Type 2 diabetes status (N=38), the area under the receiver-operating characteristic curve of three of the risk scores (not the American Diabetes Association score) improved among those who had erectile dysfunction in comparison with those who did not; for example, the area under the receiver-operating characteristic curve of the LA-FINDRISC score was 89.6 (95% CI 78.7–99.9) in men with erectile dysfunction and 76.5 (95% CI 68.5–84.4) overall. Conclusions: In a population-based study, erectile dysfunction was more common in men with Type 2 diabetes than in the otherwise healthy men. Screening for erectile dysfunction before screening for Type 2 diabetes seems to improve the accuracy of well-known risk scores for undiagnosed Type 2 diabetes.

Abstract:
This work consists in an attempt to investigate the possibilities and contradictions when schooling education for transformation of the individuals punished is constituted as one of the aspects pertaining to the prison operation. As a point of deflection after a ten-year professional trajectory working for the Educational Program in the penal system of São Paulo State, this work intends to systematize several thoughts and questionings formerly oriented to consolidate an education proposal particularly driven to the population incarcerated. The prisons, its rules, procedures and values observe absolute primary attention to the domination and control of the mass incarcerated. Therefore, the maintenance of internal order and discipline is transfigured to the essential goal of the penal organization. The so-called re-educational programs and activities are inserted in this functional logic, having their actions and objectives based on the need of subjugating the people punished, by adapting them to the social system inside the prison. Nevertheless, prisoners resistance to the control is evident. By no means education is neutral in such a process (battle) of subjugation and resistance. Its methodological basis and daily practises can contribute to sediment school as a subsequent resource for the preservation and formation of individuals in the interstices of the domination processes. This research aims to draw possibilities so that the prerogatives of prison administration shall not invade the educational practice, ruling its actions. Therefore, theres need to introduce the Education Program for Adult Inmates along with its congenial programs in a national approach, with the view of effecting its organization on educational and not incarcerating basis.

Abstract:
ABSTRACT: The prison is currently a place with a very wide range of action, far from the eight century ideals that provided the foundations for the birth of this institution as an instrument of deprivation of liberty. Today, criminal systems tend to develop a more humane character in the treatment of inmates, betting on rehabilitation and seeks greater effectiveness of its social reintegration and combating criminal recidivism, that makes the prison an increasingly dynamic place for its programmatic plurality. The aim of this thesis is to project a new prison facility in Setúbal (EPS), responding to the urgent need for a modernization of the portuguese prison system, not only for the overcrowding in it’s prisons, but especially when we look at their status quo, far from the political discourse that attempts to disguise the precarious conditions of advanced state of degradation, demonstrating the decadent scenery of the institution and the inhumane conditions in which it lives. In agreement with the place of intervention, the EPS is one of the eight establishments in this situation of decay that will be gradually deactivated until the construction of five new prisons, of which one will be erected in the same district. In an approach to the prison context, it seeks to develop a reflection and critical discourse regarding the valences of prison as a measure par excellence in the Defense of society against the prevaricators, showing how the penitentiary models were developing innovative control and surveillance strategies to the current conception of prison architecture. Thus, we intend to understand how the architecture influences the atmosphere of the prison space and how it represents a determinant factor in the rehabilitation process of the prisoner, being imperative to recognize the mechanisms and techniques used on these institutions that are effective as a machine of the penal system, seeking, throught there, the valences that will serve as guidelines in the projectual conception of the prison space. Finally, the theoretical concepts analyzed here will serve as a motto for the development of the architectural proposal, seeking the conception of a prison establishment that respects human dignity and promotes the rehabilitation of prisoners as well as the Appreciation of the plurality of activities as a reinsertion process.

This thesis sought a analysis of the rehabilitation process of criminals convicted of theft crime – what is the political action aimed at an individual convicted of a nonviolent crime in the city of Juiz de Fora, Minas Gerais. We seek the understanding of the role of criminal enforcement network to establish an analytical description about the gaps found between the State planning (both at federal level as at state level) for the criminal policy and the implementation of their proposals. We propose an analysis of the participation of the three spheres of executive power, as well as the articulation between the Secretaries, in the process of the criminal rehabilitation, consisting of preventative strategies and corrective strategies. During the research project, however, we maintain our focus of analysis on corrective actions, i.e., in the process of criminal enforcement in implementation while criminal policy towards criminals rehabilitation. We understand that the criminal rehabilitation is not a result, but a process to be promoted, exclusively, by the State. This process involves technical issues and political issues, both under discussion during this thesis. We refer to our discussion by non-violent criminal, convicted of a crime recognized by interviewees as "permeated by a high rate of specific recidivism", i.e., recurrence in the same type of crime. We take as a benchmark for our analysis the program offered by the State of Minas Gerais for the prevention of recurrence as well as the impact of punitive strategies offered to such individuals. In our discussion we oppose the rehabilitation not exactly at the incapacitation, but at the production of delinquency; and identified it as a localized process in the government‟s action, i.e., in the field of public policy. We understand the rehabilitation process as an alternative to the sentence, which interrupts the possible formation of criminal careers.

The resocializing purpose of the penalty, in the course of history, has been placed as its primary purpose. This claim is supported by the fact that some of the dogmatic theories on the subject never exclude it as the ultimate goal of the sanction or even place it as a limitation on other purposes, as for example in Claus Roxin's Dialectic Theory. Moreover, the Brazilian legal system, if analyzed in a systematic way, prioritizes re-education and social reintegration of the convicted in society, which demonstrates crystal proof because the Law on the Execution of Criminal Sanctions contains numerous resocializing mechanisms to be applied. The purpose of this study is to analyze, through the hypothetical-deductive method, whether the hypothesis of resocialization of the condemned prisoner is a myth, besides being essentially obedient to the guiding values of the national legal system and likewise being a motivating purpose of numerous legal institutes included in the Law on the Execution of Criminal Sanctions. Thus, it was verified that social integration is not really implemented, either because it is not in fact the purpose of the intended penalty or because of the dysfunctional use of the available legal instruments, it became only a legitimating speech of the existence of the sentence and has as a consequence the mistaken production of legal and legislative decisions, impacting significantly on the criminal policy adopted by the State

Abstract:
Anglophone probation has strongly influenced probation agencies in France and Israel, in spite of their different legal frameworks. During the Second World War, Human Rights were overall maintained in both England and Wales and in Palestine under British Mandate. This enabled probation services to continue their activities. At the same period, the projects for the establishment and development of probation services were halted in occupied France during the Vichy regime. In the three jurisdictions, juvenile supervision played a key role in the development of adult probation. The period following WWII – essentially the Welfare State –, is characterised, on the one hand, by the influence of the medical model on probation and, on the other hand, by the belief that the support and assistance given to probationers was sufficient to prevent reoffending. At the end of the Welfare State, Martinson’s resounding report, marked the end of the rehabilitation ideal in anglophone jurisdictions and caused wide-spread scepticism regarding the efficiency of probation, leading to a punitive policy era. Consequently, research and practice sought new answers in order to limit this punitive turn. At the time, French probation was not involved in this debate. With regard to Israel, its strong roots in social work as well as general support for probation kept it away from a rethinking of probation. At the end of the 1970s, changes emerged in probation methods and in public services, which were similarly contrasted in the study countries

Abstract:
The dissertation examines how offenders deal with “critical rethinking” on their crimes. According to art. 27 d.P.R. 30 June 2000, n. 230, it consists in a dialogical reflection on the wrongdoings they committed, their motivations, the consequences that follow on for the offenders themselves, and the possible reparations during the post-sentencing phase. The study is divided into five chapters. The first chapter focuses on the research for a constitutional basis of the “critical rethinking” and for a renewed understanding of the “finalismo rieducativo” (equivalent to the rehabilitative goal). The second chapter highlights the points of intersection between the “critical rethinking” and the “general theory of crime”. The third chapter summarizes the obstacles and the operative problems that hamper the implementation of this dialogical reflection and describes hints for a possible reform of the criminal justice system, particularly with regard to the post-sentencing phase. The fourth chapter proposes an in-depth analysis of some of the basic key-concepts for the introduction of a dialogical-restorative model of justice. Finally, the fifth chapter investigates the relationship between “rehabilitation” and “restorative justice” and takes into consideration a justice model that is inspired by “responsivity” [John Braithwaite] and “restorative justice dialogue” [Mark S. Umbreit]. By means of that, the study aims at providing a framework for an active assumption of responsibility in a more dialogical and inclusive culture.

Abstract:
This research aims to analyze the limits and possibilities of professional qualification for economic and social rehabilitation of prisoners, with reference to the Penal Colony Agroindustrial of Paraná. It builds on theoretical and methodological foundations of the Historical and Dialectical Materialism and uses as data collection tool semi-structured individual interviews conducted with seventeen imprisoned participants of educational and professional activities. The main results show that the socioeconomic profile of the prison population, characterized by low education, lack of professional qualification, and the occupation of precarious jobs, contributes to entry into the crime and hence incarceration. The second aspect was noted the contradictory nature of the arrest at his punitive and ressocializadora purpose, with the simultaneous goal punish and reinsert the condemned to society. Finally the qualification, widely understood as a social relation, articulating general education, professional and practical, reduce the short and streamlined courses, regardless of education and integration between general and vocational education, even taking into account the market demands. The evidence presented allow us to infer that vocational training programs are inefficient and ineffective in the economic and social rehabilitation of prisoners. Prison labor, productive and educational, only fulfills the productive function, predatory operated by private companies, meaning for the imprisoned very much a possibility to ease prison conditions and redemption pen, your professionalism.

Abstract:
This practicum explores potential for a new prison typology for federally sentenced women within Canada. The Koza Centre is an alternative to designs and practices currently in place that have the potential to create barriers to successful rehabilitation and reintegration. The primary intention of this work is to understand the issues and needs that are shared by many women who have come into conflict with the law and respond with an appropriate design. The methodology for this includes a brief history of women’s imprisonment in Canada and current political contexts, an analytical framework focused on overall well-being, and precedent studies. From this, programming and strategies have been developed that inform an evidence-based design. The result is a design focused on rehabilitation through the building of healthy relationships, ties to the community, programming that fosters successful reintegration, and supportive interior design that facilitates the well-being of those within the space.

Abstract:
AbstractThe purpose of this chapter is to explore how the humane Norwegian policy principles and values may impact the prison-based practices and the implications of these for the collaborative work undertaken by front-line personnel. Humane traditions are considered a substantial focus of the Norwegian prison system and its policies. The approach in Norway is supported by increasing empirical evidence that shows the system to centralise the welfare of inmates. It emphasises offender rehabilitation and reintegration rather than merely punishment as a fundamental means to reduce reoffending. However, the collaborative practice that may arise as a consequence of these traditional values is underexplored. Two case studies were undertaken with front-line staff working in a Norwegian prison transitional residence (Overgangsbolig). This is the final phase of an inmate’s sentence while being reintegrated back into society. The study found that staff in their collaborative working practices adhered either to the aim of normalising the lives of their inmates after long periods of incarceration (normalisation ideals) or reparative ideals that reflected the system’s humane focus on rehabilitation and reintegration takes precedence over punishment. We conclude that irrespective of differing professional disciplines, there seems to be compatibility between the overarching principles and values of penal policy and front-line ideals to promote collaborative practices at the reintegrative phase of the Norwegian prison system. The shared application of these humane ideals promoted collaborative practice among the front-line workforce while focusing upon the provision of welfare to inmates and their impending reintegration back into society.

Abstract:
This chapter reiterates events that lead to the founding of the Mettray, which was instigated by concerns on increase in juvenile crime by penal reformers, philanthropists, and jurists during mid-nineteenth-century France. Mettray was founded during a period that invited experimentation in corrections, as evidenced by the allure of carceral innovations abroad. With the advent of the Second Empire, the focus of justice policy shifted to emphasize reinforcing mechanisms of repression. Louis Napoléon conceived of punishment as a retributive and intimidating force and relegated rehabilitative corrections to a secondary position. Rather than improving conditions inside prisons and adapting punishments to facilitate rehabilitation, Louis Napoléon shored up the police forces and manipulated the penal codes to assure more prompt, certain, and harsh punishments.

Abstract:
Abstract Patients suffering from medical conditions resulting in hand impairment experience difficulty in performing simple daily tasks, like getting dressed or using a pencil, resulting in a poorer quality of life. Rehabilitation attempts to help such individuals regain a sense of control and normalcy. In this context, recent advances in robotics have manifested in multiple designs of hand exoskeletons and exosuit gloves for assistance and rehabilitation. These designs are typically actuated using pneumatic, shape memory alloys and motor-tendon actuators. The proposed Motor Tendon Actuated Exosuit Glove (MTAEG) with an open palm is a soft material glove capable of both flexion and extension of all four fingers of the human hand. Its minimally invasive design maintains an open palm to facilitate haptic and tactile interaction with the environment. The MTAEG achieves flexion-extension motion with joint angles of 45° at the metacarpal joint which is 57% of the desired motion; 90° at the proximal interphalangeal joint which is 100% of the desired motion; and 50° at the distal interphalangeal joint which is 96% of the desired motion. The paper discusses the challenges in achieving the desired motion without the ability to directly model human tendons, and the inability to actuate joints individually.

Abstract:
The human being in the environments of modern technology has to endure stresses of many and varied kinds of vibrations [1]. Measuring vibration is an important tool in rehabilitation and biomechanical fields of research. We have proposed image processing as a new method to record and determine the frequency response of human body. The subjects were exposed to whole body periodical vibration while standing on a shaking table. Two digital camcorders were used to capture the motion of colored pencil-dot markers on the skin of human body (forehead) and on the edge of the shaking table. After color spotting each frame, the binary image results were processed using new circle factor criteria proposed in this work, for fast finding circles based on second order statistics. The extracted points were calibrated using our own extended version of the direct linear transformation (DLT) method. We subsequently used Borland Delphi 5.0 language to develop useful software for measuring and analyzing human body vibration. As a result, it was clear that the proposed method was lower noise-sensitive in comparison to accelerometer. In order to investigate the validity of the software, the obtained mechanical impedance of the body were compared with other investigations in literature and showed to be compatible. The main advantage of this method is working with a simple user-familiar hardware with no external device attached to the subject and also a user-friendly-software.

Abstract:
A large number of people are exposed to whole body vibration in their occupational life. Measuring vibration is an important tool in rehabilitation and biomechanical fields of research. We have proposed image processing as a new method to record and determine the frequency response of human body. The arranged set up for forced vibration consisted of an AC motor, a variable speed drive unit and a shaking table for producing one directional sinusoidal vibration. Volunteers were asked to stand on the shaking table at a relaxed posture. Subsequently Vibration measuring software has been completely developed in Borland Delphi 5.0. Finally obtained displacement function of the body and the shaking table has been used in conjunction with Matlab 6.5 to prepare a proper algorithm for analyzing human body vibration. We discussed mechanical characteristics of the body by obtaining mechanical impedance and transmissibility from the shaking table to the head as example applications of the conducted software. The important point is the fact that all devices used in our developed measurement system are usually available in a biomechanics laboratory where a Gait system is functioning. This gives the opportunity for such laboratory to add vibration measurement to its capabilities without much excessive costs. The system has the advantage of lower noise sensitivity in comparison to accelerometer. The main advantage of this method is working with a simple user-familiar hardware with no external device attached to the subject and also a user-friendly-software.
