Abstract:
People with long-term physical disabilities also suffer from different types and nature of tumors as well as general population. Unfortunately, the information about the frequency of these diseases among the specified contingent is very deficient.<strong>&nbsp;</strong>The objectives of the study were to determine the incidence of benign and malignant neoplasia among the adults and elderly with physical disabilities and to indicate the risk factors for the appearance and development of tumors in this population.<strong>&nbsp;</strong>In this descriptive survey, 186 people, living in the largest home on the Balkan Peninsula for adults and elderly people with long-term physical disabilities were studied in 2021. 82.79% of the residents were over 51 years of age and almost all of them are inhabited by two residents. 42.47% of the respondents were men and 57.52% &ndash; women. Data on the demographic characteristics of the respondents, including their personal lifestyle and self-reported diagnosis of presence and/or history of a tumor formation, were collected through a semi-structured interview. The verification of the collected information about the incidence of benign and malignant lesions among adults with physical disabilities was carried out by means of the application of the second research method &ndash; content analysis based on the medical records. For processing and analysis of the collected data, it was used statistical software &ndash; IBM SPSS Statistics V21.0. The results showed a significantly high rate in the prevalence of tumor formations in the sample &ndash; 24.19%. The pre-cancerous formations have the highest incidence among residents &ndash; 42.22% with the most common variant &ndash; chronic form of cholecystitis combined with cholelithiasis (37.77%). Prostatic hyperplasia and uterine leiomyoma are the leading benign lesions in male residents &ndash; 11.11%, in female respondents &ndash; 11.11% respectively. The predominant form of cancer is breast carcinoma &ndash; 8.88%. The research found a high prevalence of tumor formations among adult patients with long-term physical disabilities. The harmful impact of a number of behavioral habits and health determinants significantly contributes to this negative tendency.

Background:&#x0D; People living in old age with disabilities acquired in early adulthood or because of a congenital anomaly represent a growing population with unique medical needs. This study aims to investigate the incidences of chronic diseases and the prevalence of chronic polymorbidity among adults with physical disabilities, who are accommodated in the largest social home on the Balkan Peninsula.&#x0D; Methods:&#x0D; In this descriptive survey, 179 people with physical disabilities were studied during 2017- 2018. A structured questionnaire was used to obtain information on socio-demographic characteristics, personal lifestyle and self-reported diagnosis of the presence of chronic diseases. In addition to the above, second research was conducted on the available medical documentation for the health status of each person, which helped to verify the information about the incidence and prevalence of chronic diseases among adults with physical disabilities.&#x0D; Results:&#x0D; Most of the adults with physical disabilities have at least more than two chronic conditions (69.27 %). Arterial hypertension is the commonest chronic illness among them (47. 48 %). The study reported a raised incidence of chronic diseases of the digestive system and chronic diseases of the urinary system, as well as increasing prevalence of diabetes mellitus type II, chronic diseases of the pulmonary system and degenerative joints diseases correlated with the age of persons.&#x0D; Conclusion:&#x0D; The study showed an increased frequency of chronic diseases among adults with physical disabilities and a significant number of persons who have multiple chronic conditions, which determines the need for frequent and long-term health care.

Orang dengan disabilitas adalah orang yang memiliki kemampuan berbeda baik fisik, mental, intelektual atau sensorik dalam jangka waktu lama. Orang dengan disabilitas secara sosial seringkali mengalami keterbatasan dalam memenuhi kebutuhan hidupnya sebagai akibat dari persepsi dan sikap lingkungan sosial yang tidak tepat dalam berinteraksi dengan mereka. Sehingga peran serta orang dengan disabilitas seringkali diabaikan. Jumlah orang dengan disabilitas pada 9 provinsi di Indonesia sebanyak 299.203 jiwa, dan sekitar 67,33% disabilitas dewasa tidak memiliki keterampilan dan pekerjaan. Perusahaan melalui program corporate social responsibility dapat berperan dalam membantu meningkatkan aksesibilitas orang dengan disabilitas melalui peningkatan keterampilan kerja dan akses terhadap sumber-sumber penghidupan mereka. Lembaga Sinergi Foundation merupakan salah satu organisasi yang berupaya membantu memberdayakan orang dengan disabilitas (ODD). Tujuan dari program CSR dari Sinergi Foundation adalah agar ODD mampu memenuhid kebutuhan hidup secara mandiri, sehigga taraf hidup mereka meningkat secara sosial dan ekonomi. People with disabilities are people who have different abilities whether physical, mental, intellectual or sensory in the long term. People with social disabilities often experience limitations in meeting their needs as a result of perceptions and attitudes of social environment that are not appropriate in interacting with them. So the participation of people with disabilities is often ignored. The number of people with disabilities in 9 provinces in Indonesia is 299,203 people, and about 67.33% of adults with disabilities do not have skills and jobs. Companies through corporate social responsibility programs can play a role in helping to increase the accessibility of people with disabilities through improving work skills and access to their livelihoods. The Sinergi Foundation is an organization that seeks to help empower people with disabilities (ODD). The aim of the CSR program of the Sinergi Foundation is that ODD are able to fulfill their needs independently, so that their standard of living increases socially and economically.

Abstract A lack of evidence-based interventions for people aging with long-term physical disabilities exists. To examine the exclusion of people with disabilities in behavioral clinical trials, a scoping review was conducted. ClinicalTrials.gov was searched for interventional behavioral studies from the United States completed from 2008–2018, with results focused on adults (18–64) and older adults (65+). In total, 158 clinical trials were included. In 129 articles, health conditions were excluded 697 times. Seventy-one clinical trials excluded at least one health condition with strong justification, 11 with poor justification, and 115 without justification. There is strong evidence that people with disabilities are excluded from behavioral clinical trials, often without justification. To help close this gap, our presentation will discuss how translational research strategies, focused on adapting existing EB behavioral trials, can be used to increase the availability of interventions that address the needs of individuals aging with and into long-term disabilities. ObjectivesTo investigate prevalence of mental health conditions, sensory impairments and physical disability in children, adults and older adults with co-occurring intellectual disabilities and autism, given its frequent co-occurrence, compared with the general population.DesignWhole country cohort study.SettingGeneral community.Participants5709 people with co-occurring intellectual disabilities and autism, compared with 5 289 694 other people.Outcome measuresRates and ORs with 95% CIs for mental health conditions, visual impairment, hearing impairment and physical disability in people with co-occurring intellectual disabilities and autism compared with other people, adjusted for age, sex and interaction between age and co-occurring intellectual disabilities and autism.ResultsAll four long-term conditions were markedly more common in children, adults and older adults with co-occurring intellectual disabilities and autism compared with other people. For mental health, OR=130.8 (95% CI 117.1 to 146.1); visual impairment OR=65.9 (95% CI 58.7 to 73.9); hearing impairment OR=22.0 (95% CI 19.2 to 25.2); and physical disability OR=157.5 (95% CI 144.6 to 171.7). These ratios are also greater than previously reported for people witheitherintellectual disabilitiesorautism rather than co-occurring intellectual disabilities and autism.ConclusionsWe have quantified the more than double disadvantage for people with co-occurring intellectual disabilities and autism, in terms of additional long-term health conditions. This may well impact on quality of life. It raises challenges for staff working with these people in view of additional complexity in assessments, diagnoses and interventions of additional health conditions, as sensory impairments and mental health conditions in particular, compound with the persons pre-existing communication and cognitive problems in this context. Planning is important, with staff being trained, equipped, resourced and prepared to address the challenge of working for people with these conditions.

Background: People with disabilities are those with physical or mental limitations in long term. People with disabilities have some rights they deserve such as rights to education, occupation, health, social welfare, and accessibility. Method and Subject: This research was conducted on December 2019, in Colomadu Sub District using questionnaire with cross-section design. The subject used consisted of 23 quadriplegic persons. Result: Out of 23 quadriplegic persons, the result of analysis showed significant score of 0.236 between people with disabilities and right to education, of 0.167 between people with disabilities and right to occupation, of 0.313 between people with disabilities and right to health, of 0.150 between people with disabilities and right to social welfare, and of 0.762 between people with disabilities and right to accessibility, with p &gt; 0.05. Conclusion: The rights of people with disabilities (disability people) have not been fulfilled yet in Colomadu Sub District.

Abstract Individuals aging with long-term disabilities may need support to perform and participate in everyday activities associated with well-being and quality of life. Technology solutions have potential to effectively promote autonomy and aging-in-place for this population. This symposium will highlight technology research and development efforts from the Rehabilitation Engineering Research Center on Technologies to Support Aging-in-Place for People with Long-Term Disabilities (RERC TechSAge). TechSAge is dedicated to understanding the needs of, and developing supportive technologies for people aging with long-term vision, hearing, and mobility disabilities. Mumma et al. will present insights from interviews of adults aging with vision loss due to glaucoma and macular degeneration regarding their challenges and response strategies for everyday activities. Lee et al. will discuss findings from interview data exploring how wisdom is manifested from the lived experiences of individuals aging with disability (i.e., hearing, vision, and mobility). Mitzner et al. will describe characteristics of the participant sample from a clinical trial examining the effect of a Tele Tai Chi intervention on physical activity and social connectedness for adults aging with long-term mobility disabilities. Sanford et al. will present the findings from three TechSAge pilot projects focused on the use of technology to support in-home rehabilitation for individuals aging with long-term upper extremity disabilities: DigiHand, KeyStroke, and 3DP for AT. Together these studies illustrate the needs and coping strategies of older adults with disabilities and the potential of technology to support those needs. This is a collaborative symposium between the Lifelong Disabilities and Technology and Aging Interest Groups.

The aim of this cross-sectional study was to determine the importance of resilience and self-efficacy in explaining employment status for people with long-term physical disabilities when other sociodemographic and symptom variables were also examined. A multiple logistic regression with sequential predictor entry was used to predict employment status. Participants were individuals, 18 to 65 years of age, with a diagnosis of multiple sclerosis, muscular dystrophy, post-polio syndrome, or spinal cord injury ( N = 882). Results indicated resilience but not self-efficacy was uniquely predictive of employment status. The combined effects of resilience and self-efficacy, however, did not significantly contribute to the variance in employment status above and beyond the sociodemographic and symptom variables. Other variables uniquely predictive of employment included education, age, marital status, disability benefits, and anxiety. We found in this study preliminary evidence to suggest that rehabilitation counseling practitioners should consider a client’s resilience with respect to employment. Knowledge of sociodemographic and symptom factors in conjunction with psychometrically sound measures of resilience and self-efficacy may be used to identify individuals with long-term physical disabilities whose beliefs and behaviors may limit the extent to which they prepare for, obtain, or maintain employment.

According to the Law of the Republic of Indonesia Number 8 of 2016 persons with disabilities are any person who experiences physical, intellectual, mental and or sensory limitations in the long term who in interacting with the environment can experience obstacles and difficulties to participate fully and effectively with other citizens. based on equal rights. These rights include the right to life, free from stigma, education, health, social welfare, accessibility, public services to employment, entrepreneurship and cooperatives. According to the Pontianak City Social Service, in 2021 people with disabilities in Pontianak City will number 1,161 people with 12 different types of disabilities. The most types of people with disabilities are physical disabilities, amounting to 276 people. Of the total number of people with disabilities in Pontianak City, only 34 people have jobs while the others are unemployed. The Job Training and Skills Center is a place that accommodates and facilitates all activities related to the process to become better than before by learning something to become an expert, clever and proficient in the field that is occupied. The design of the Job Training and Skills Center for Persons with Physical Disabilities is one of the facilities in fighting for the rights of Persons with Disabilities in Pontianak City and its surroundings. The design theme used is using a biophilic architecture approach with an emphasis on accessibility.

Currently, studies have shown that one in three people infected with coronavirus disease-19 (COVID-19) is likely to have had long-term exposure to COVID-19, known as long-term COVID-19. Clinical studies indicate that many people infected with the severe acute respiratory syndrome Coronavirus-2 (SARS-CoV-2) COVID-19 pandemic have long-term COVID-19 exposure. According to the study, it has been said that people with diabetes and obesity, and people who have received organ transplants, are more likely to suffer from this long-term effect of COVID-19. In this article, the effects of long-term COVID-19 exposure on neurological disability patients are analyzed with the help of a neuromachine learning model. The proposed model also shows that this long-term COVID problem does not depend on the factors such as race, age, gender, and socioeconomic status of those people. According to the proposed model, people suffering from long-term COVID problems continue to suffer from physical fatigue and shortness of breath and are regularly monitored and classified as per the proposed instructions. Even after they recover from the disease, various side effects are seen.

Background:We know very little about physical activity in children with long-term illness or disability compared with those children without disabilities. Previous studies indicate low physical activity levels among all adolescents.Methods:The sample consisted of Canadian (n = 2720) and Finnish pupils (n = 3459) approximately 13.5 and 15.5 years of age in general (mainstreamed) education. The study is a part of the Health Behavior in School-aged Children (HBSC) study. Nationally representative data were collected in 2002 using a standardized questionnaire. The moderate-to-vigorous intensity physical activity screening measure was used.Results:Approximately one-fifth of the pupils in both countries had a long-term disability, illness or medical condition. In both countries boys and girls with a long-term illness or disability were equally physically inactive, and adolescents with a long-term illness or disability were as physically active as those without disabilities.Conclusions:There is no difference between young people with and without long-term illness/disability, and between boys and girls, in relation to their physical activity. However, all of them fall short of recommended guidelines. This indicates that promoting a physically active lifestyle should be of high priority in the lives of young people.

Persons with disabilities were people who had long-term physical, mental, intellectual or sensory limitations. People with disabilities were divided into several types, namely physical disabilities, sensory disabilities, mental disabilities, and intellectual disabilities. Deaf people are included in sensory disabilities who have limited senses. So people with disabilities need special assistance, especially in starting swimming. The purpose of this study was to determine whether deaf swimmers need a swimming start aid. The research method used was a descriptive method with a survey technique. The time of study was carried out in 2022 starting from April to July. The location for data collection was carried out at the Senen Jakarta swimming pool. The population and samples used were athletes with hearing impairment in DKI Jakarta, totalling 20 people using total sampling. The research instrument used was a questionnaire to analyze the need for start aids for deaf swimmers by having three indicators as a decision maker on whether or not to use start aids for deaf swimmers. The data analysis technique used was the descriptive analysis technique. The results showed that deaf swimmers needed a start aid. Further research is expected to be able to make starting aids that can be used by deaf swimmers to achieve optimal performance.

People with severe psychosocial disabilities have a 20-years shorter lifespan due to chronic somatic comorbidities and the long-term consequences of the side-effects of antipsychotic drugs.They often are sedentary and show lower levels of physical activity, factors which can contribute to their shorter lifespan, because of the greater cardiovascular risk.An increasing amount of evidence, including clinical trials, pointed out that sport, physical activity and structured exercise programs improve physical and psychological wellbeing of people with psychosocial disabilities, playing also an important role against their social isolation and self-stigma.The NICE and APA guidelines include exercise and physical activity for the management of depressive symptoms.Safe and effective programs require multidisciplinary teams that should always include mental health professionals, able to recognize the psychosocial needs, the impact of symptomatology, the role of secondary effects of psychotropic medication, the effect of previous exercise history, the lack of motivation, the inexperience with effort intensity and the frustration of people with psychosocial disabilities.

Abstract:
Background: Older adults with intellectual disability often take multiple medicines with anticholinergic activity and sedative properties to manage multi-morbidity; the use of medication with anticholinergic activity has been found to be associated with various cognitive and physical impairments. However, there are limited studies that have examined the long-term impact of anticholinergic use among older adults. Therefore, this protocol is designed to conduct a scoping review to examine the available data on the long-term impact of anticholinergic use in older adults with intellectual disability. Aim and objectives: The aim of this scoping review is to a) map and b) examine the existing research literature to answer the research question: What is the impact on cognitive and physical outcomes of long-term exposure to medications with anticholinergic activity among older adults with intellectual disabilities? Methods and analysis: This scoping review will follow the methodology framework of Arksey and O’Malley and its developed version by Levac. The framework consists of a six-stage process to be conducted according to the Preferred Reporting Items for Systematic reviews and Meta-Analysis extension for Scoping Reviews (PRISMA-ScR) reporting guidelines. The search strategy will include the following electronic data bases: Pubmed, Cochrane library, EMBASE, Medline, Science Direct, CINAHL Complete and PsycINFO. The search will include terms related to ‘Anticholinergic burden’, ‘Intellectual disability’, ‘Adverse drug reaction’ and ‘long-term impact’ with Boolean operator ‘and’. The scoping review will include studies with at least 3 months exposure to anticholinergics. The collected data will be mapped as a tabular presentation of the various physical and cognitive adverse effects associated with long-term use of anticholinergics in this group of population.

People with intellectual disabilities have a comparatively low level of physical activity and are affected by associated numerous impairments and diseases. However, target group specific interventions and concepts aiming at sustainable physical activity-related behavior change in everyday life are rare and the needs and perspectives of the target group have hardly been considered so far. Therefore, a target-group oriented intervention was developed. The research and developmental process was characterized by a participatory approach, involving people with intellectual disabilities throughout the whole process. For example, the interview guideline, design of the interview situation and ways of communicating were developed in a participatory manner. Twenty-four qualitative interviews with people with intellectual disabilities were conducted to explore individual physical activity-related experiences and strategies. Individual and contextual influences on physical activity were identified. Interview results were used to conceptualize an intervention that aims at promoting physical activity in the everyday life of people with intellectual disabilities by strengthening their self-management strategies. The intervention was tested in practice and modified based on communicative validation methods. Good acceptance in the long term is also expected, as the intervention takes place in people’s living environment, is socially embedded and builds upon the needs of the target group.

The position of humans in the eyes of God is considered equal, but often it is humans themselves who create differences in treatment towards fellow humans. This differential treatment is particularly felt by individuals with physical, mental, or intellectual limitations, known as people with disabilities. Disability is defined as a condition in which someone experiences long-term physical, intellectual, mental, and/or sensory limitations, resulting in obstacles and difficulties in social interaction. This article discusses the challenges and opportunities faced by people with disabilities in the workplace, as well as the government's role in creating inclusive employment opportunities and laws that protect their rights in the workplace. Despite the existence of many laws regulating this matter, people with disabilities often still face discrimination in the workplace. The method used in writing this article is through literature review, involving understanding and learning theories relevant to the topic.

To achieve a better condition in living, everyone has the rights to equal opportunity, one of which through providing rights to employment at formal work sector either government owened or private owned companies for people with physical disabilities. However, to fulfill the rights required long adaptation process, and involving various connecting factors on the process of social acceptance of the workforce with physical disabilities on the formal work sector. Using literature study, this article is describing the factors influencing the acceptance of the workforce with physical disabilities as well as the opportunities to gain employment at formal sector occupation. The study found that Indonesian government have attempt to fulfill the rights of people with physical disability through the issuance of several regulations ensuring the population of people with physical disabilities in equal opportunity to employment as well as protecting them from discrimination in the formal employment surrounding. In the process, there are various factors influencing the acceptance of workforce with physical disabilities which are internal and external factors. There is an acceptance quota of people with physical disabilities to be accepted that is serves as supporting factors, meanwhile, there is also preventing factors influencing the optimalization of workforce with physical disabilities in their workplace. This writing recommends the people with physical disabilities being facilitated in enhancing their skills and knowledge in order to be qualified with the requirements of the workplace. Abstrak. Dalam pencapaian kehidupan yang lebih baik setiap orang berhak akan kesempatan yang sama, salah satunya melalui pemberian hak untuk mendapatkan kesempatan kerja yang sama pada sektor kerja formal milik pemerintah maupun perusahaan swasta termasuk bagi orang dengan disabilitas fisik. Namun, ternyata untuk dapat mewujudkan hal tersebut harus melalui proses adaptasi yang cukup panjang, serta melibatkan berbagai faktor yang berkaitan pada penerimaan sosial tenaga kerja dengan disabilitas fisik pada sektor kerja formal. Menggunakan metode kajian pustaka, tulisan ini berusaha menggambarkan situasi penyebab dan peluang orang dengan disabilitas fisik dalam mendapatkan pekerjaan di sektor formal. Hasil penelitian menemukan bahwa pemerintah Indonesia telah berupaya untuk dapat memenuhi hak orang dengan disabilitas fisik dengan menerbitkan beberapa peraturan yang dapat menjamin populasi orang dengan disabilitas fisik dalam mendapatkan kesempatan kerja dan melindungi mereka dari diskriminasi di dunia kerja formal. Pada prosesnya terdapat beberapa faktor yang mempengaruhi penerimaan tenaga kerja dengan disabilitas fisik yaitu terdiri dari faktor internal dan faktor eksternal. Terdapat aturan kuota penerimaan orang dengan disabilitas fisik untuk bekerja yang menjadi faktor pendukung diterimanya orang dengan disabilitas fisik di tempat kerja formal, akan tetapi ada pula faktor penghambat orang dengan disabilitas fisik bekerja secara optimal di tempat kerja nya. Tulisan ini menyarankan agar orang dengan disabilitas fisik difasilitasi untuk meningkatkan keterampilan dan pengetahuan agar mereka dapat memenuhi persyaratan untuk penerimaan lowongan pekerjaan.

Ageing with long term physical disability is largely uncharted territory. This review focuses primarily on the experiences of the people who incurred some paralytic effects following polio or spinal cord injury. It is suggested that the concentrated medical debate on specific syndromes, such as the post-polio syndrome, might be unproductive and that more effective knowledge might be obtained by studying the implications of ageing on the emerging group of older people who have been physically disabled for decades. With such knowledge, lifespan health care models and health promotion strategies can be developed to assist people who are ageing and have physical disabilities to stay healthy and productive members of the community.

Abstract Technology designers often exclude individuals aging with diverse needs, capabilities, and disabilities from engaging in the design process, which can hinder the usability and usefulness of emerging technologies. In this symposium, investigators report on research and development efforts aimed at understanding the needs of, and developing supportive technologies for, people aging with long-term disabilities. This symposium features projects from the Rehabilitation Engineering Research Center on Technologies to Support Aging-in-Place for People with Long-Term Disabilities (RERC TechSAge), which is an interdisciplinary collaboration between Georgia Tech and the University of Illinois at Urbana-Champaign. First, Bayles et al. will discuss findings from the Aging Concerns, Challenges, and Everyday Solution Strategies II study, focused on Deaf older adults’ use of technologies as solution strategies for common everyday challenges. Next, Mitzner et al. will highlight the development of an evidence-based group exercise intervention (Tellewellness Tai Chi for Arthritis) aimed at promoting both physical exercise and social interaction for older adults with long-term mobility disabilities. Kadylak et al. will describe how voice-activated digital assistants can support older adults aging with mobility disabilities by reporting on findings from a longitudinal demonstration project with older adults in assisted and independent living communities. Exploring the potential for smartbathroom technology to promote aging in place, Sanford et al. will discuss how smartbathroom sensor data can be analyzed and vizualized to identify ways to communicate insight from sensor data to improve training of occupational therapy practitioners. Susy Stark from Washington University will serve as the discussant for the symposium.

People with disabilities have become a critical, international phenomenon. People with disabilities are those who have a physical or mental condition which limits their movements, senses, or activities. This article is a discussion of the factors which influence the empowerment of Qatari people with disabilities. The Qatari government has gone a long way towards making people with disabilities valued and appreciated by providing opportunities for education and work. Qataris are also keen to apply the literal teachings of Islam, although Qatar’s tribal culture has an effective influence in dealing with people with disabilities. The analysis of the findings from the interviews with charitable organisations’ staff members showed that Qatari people with disabilities still lack the empowerment to achieve their aims of being productive members of society. People with disabilities in Qatar desire a decent life which meets their basic needs such as healthcare, education, and job opportunities.

&#x0D; The existence of persons with disabilities in society is one of the important things to be considered by the state, or in this case the government, because persons with disabilities are part of the community who also have the same position, rights, obligations and roles. According to Law Number 8 of 2016 Article 1 paragraph 1, people with disabilities are defined as any person with physical, intellectual, mental, and/or sensory limitations in the long term who in interacting with the environment can experience obstacles. and difficulties to participate fully and effectively with other citizens on the basis of equal rights. However, the need for special treatment given to persons with disabilities is usually misunderstood by the general public which ultimately leads to acts of discrimination against groups of people with disabilities. Seeing the inequality in policies regarding people with disabilities in DI Yogyakarta, the CIQAL Foundation (Center for Improving Qualified Activity in Life of People with Disabilities) pays attention to fighting for the rights of people with disabilities by encouraging changes to laws and regulations in DI Yogyakarta. This research is a qualitative research with a literature study that uses books and other literatures as the main object (Hadi, 1995). Abstract National Core Indicators Aging and Disabilities (NCI-AD™), survey data support State Medicaid, aging, and disability agencies to conduct surveys with individuals receiving publicly-funded long-term services and supports and measure and track their own outcomes and performance using reliable data collection methods and tools. Established in 2015, NCI-AD is now in use by 23 states nationwide. The NCI-AD portfolio includes the flagship survey – the Adult Consumer Survey (ACS) and the State of the Workforce—Aging and Disabilities (SotW-AD) survey. The ACS collects data directly from older adults and people with physical disabilities to assess the outcomes of services provided to individuals. Indicators address key areas of concern including service planning, rights, community inclusion, choice, health and care coordination, safety and relationships. In 2022, NCI-AD piloted the SotW-AD is a first of its kind for the aging and disabilities field, this new effort collects and aggregates statewide information about wages, benefits, and turnover of the direct service workforce—as reported by the agencies that employ them. This session will provide an overview of the NCI-AD program, the type of data collected from each survey, and how state-level and research partners have utilized the data sets. Additionally, 18 NCI-AD measures are included in the newly released CMS HCBS Quality Measure Set. We will discuss how this impacts states, and potential ways that these and other measures will be implemented and used to support important aspects of services access and equity.

Physically disabled is a term used for individuals who have physical disabilities. Physical disabilities can be caused by life events or have been present since birth. People with disabilities are often considered to have a miserable life, but they can also feel happiness. Happiness can be seen through 5 indicators, including: (1) positive emotion; (2) engagement; (3) relationships; (4) meaning; (5) advancements. This study aims to obtain an overview of the happiness of early adulthood as a physically disabled person. There are 4 research subjects in their early adulthood, with characteristics aged 20-40 years in which 2 have a disability from birth and 2 others acquire physical disabilities through life events. This qualitative research uses in-depth interviews and interpretive phenomenological analysis. The research findings show that the four subjects have happiness, both which is a pleasant experience (hedonic) and that is achieved through meaning and purpose (eudaimonic). It was found that there was a relationship between the indicators that appeared in the four subjects, where all indicators led the subjects to feel happiness.

Introduction: Pedal Power is a Physiotherapist led, evidence based intervention, allowing people with acquired brain injury (ABI) to have regular social interaction, to be physically active, to cycle, attain cycling qualifications and to be skilled and equipped. This programme is delivered in partnership with Sustrans (NI) at their Active Travel Hub, at CS Lewis Square, Belfast. Method: Adapted trikes, helmets, cycling shoes and jerseys are provided as part of this adapted cycling programme, 'Pedal Power'. Participants cycle weekly around miles of traffic free paths in their community. It is known people with ABI and disabilities have much lower levels of physical fitness and physical activity. The medical, physical, psychological, cognitive and social benefits of cycling and exercise has been well established. A key influence for 'Pedal Power' was Dr Dave Saunders, University of Edinburgh, lead author of the Cochrane review ‘Physical fitness training for stroke’ (an ABI). More recent research has demonstrated that one of the most important factors in ensuring that people stay engaged in sport and physical activity in the long-term, is social support. Some of this research was carried out 20 years ago, so this adapted cycling programme is about putting that research into practice. Additionally, 'Pedal Power' seeks to reflect the UN Convention on the Rights of People with Disabilities, particularly articles 8 (awareness raising) and article 30 (participation in cultural life, recreation, leisure and sport). Results: Participants with a wide range of clinical needs report enhanced understanding of their ABI as well as improved mood, self-efficacy and social integration. Participants also feel supported across the lifespan and self-report developing new interests and skills. One person who suffered an ABI as a teenager, self-referred 20 years after injury, hoping: “…Just to have a purpose. Meet people with brain injury so I don’t feel alone.” Conclusion: Pedal Power supports the World Health Organisation's definition of rehabilitation i.e., "appropriate measures, including through peer support, to enable persons with disabilities to attain and maintain their maximum independence, full physical, mental, social and vocational ability, and full inclusion and participation in all aspects of life.” This innovative programme shows promise in providing long term community support and rehabilitation to those impacted by ABI, and ultimately offers invaluable support and friendship.

Life skills have been proven to help students overcome various problems. Therefore, they have been integrated into various student learning activities. However, have life skills also been integrated into physical education teaching for students with disabilities? This study aims to investigate the bibliometric publication of articles on teaching life skills to students with disabilities in physical education. The inspection was conducted on June 1, 2024, based on the Scopus platform using the term "teaching life skills to students with disabilities in physical education" with the doc-type article, review, and conference paper. In addition to using the Scopus analysis engine, researchers used metric analysis from Publish or Perish and visualization analysis from VOSviewer. Over the past 51 years, researchers have only managed to publish 18 Scopus documents on teaching life skills in physical education for students with disabilities. In addition, from the keyword corpus, only the keyword "disability" has networking with the keyword "daily life activity," unfortunately the networking is the furthest from other keywords. Meanwhile, from the title and abstract corpus, the term "life skills" was not found in the networking map. Thus, teaching life skills in physical education for people with disabilities has not received serious attention from researchers worldwide, so future studies on teaching life skills in physical education for people with disabilities have great potential to be explored. Future investigations can expand data collection platforms beyond Scopus, such as WoS, ERIC, DOAJ, and others, to gather more comprehensive data in discussing life skills teaching for students with disabilities in physical education. Most of today's optimization efforts aim to reduce costs, time or the number of resources used. However, optimization efforts should consider other factors as important as these, such as facilitating the lives of the disabled, elderly and pregnant and helping them in their daily lives. In this study, the Nuh Naci Yazgan (NNY) University (Kayseri/Turkey) personnel transport problems were discussed. The NNY University provides a shuttle service to bring employees to school at the start of the work and to leave them at home after work. In order to shorten the collection / distribution time and the total distance travelled, the service vehicle does not leave / pick up all employees in front of their homes. Instead, the employees are picked up / dropped at appropriate locations on an intuitively determined route. Since only the time and cost savings are taken into account when determining the service route, some employees have a long walking distance to the service route. This creates a very important problem, especially for the disabled and pregnant workers. In this study, a new mathematical model is proposed which takes into consideration the physical disadvantages and occupational positions of the employees in order to determine the shortest vehicle route. The results show that the proposed model can significantly reduce walking distances of physically disabled people without compromising the total distance travelled by the vehicle.

This study aims to find out the views and attitudes of people in St. Filemon community, Blimbing Parish (Malang Diocese) towards the fulfillment of the rights of persons with disabilities. This research is a survey, involving 37 respondents who representing 40 heads of families. The research data was collected using questionnaires. The results of the study were presented through visual graphics of histogram. While data analysis is done using percentages. The results showed that people's knowledge of the terms and meaning of disability correctly reached 94%, people's views on people with disabilities in church life 68%, people's attitude towards the fulfillment of rights in education 49%, people's attitude towards the fulfillment of rights in the field of health 57%, and people's attitude towards the fulfillment of rights in the field of livelihood 60%, while for the attitude of people towards the fulfillment of rights in the social field 57%.&#x0D; Indonesia:&#x0D; Penyandang disabilitas adalah mereka yang memiliki keterbatasan fisik, mental, intelektual, atau sensorik dalam jangka waktu lama yang dapat menghalangi partisipasi mereka secara penuh dalam masyarakat. Keterbatasan yang dialami oleh penyandang disabilitas sering menjadi lebih berat oleh karena pengetahuan, pandangan, dan sikap masyarakat termasuk umat katolik, yang tidak tepat. Penelitian ini bertujuan untuk mengetahui pandangan dan sikap umat di Lingkungan St. Filemon Paroki Belimbing terhadap pemenuhan hak penyandang disabilitas. Penelitian ini merupakan penelitian survey yang melibatkan 37 responden yang mewakili 40 kepala keluarga. Data penelitian dikumpulkan menggunakan angket. Hasil penelitian dipaparkan melalui gambar visual berupa grafik histogram. Sedangkan analisa data dilakukan menggunakan persentase. Hasil penelitian menunjukkan pengetahuan umat terhadap istilah dan arti disabilitas secara benar mencapai 94%, pandangan umat terhadap penyandang disabilitas dalam hidup menggereja 68%, sikap umat terhadap pemenuhan hak dalam bidang pendidikan 49%, sikap umat terhadap pemenuhan hak dalam bidang kesehatan 57%, dan sikap umat terhadap pemenuhan hak dalam bidang penghidupan 60%, sedangkan untuk sikap umat terhadap pemenuhan hak dalam bidang sosial 57%.

Abstract Direct care workers play a key role in supporting the health and wellbeing of older adults and people with disabilities across care settings—yet their own health risks are largely overlooked. The four papers in this symposium address this critical knowledge gap. First, McCall and colleagues will present a comparative analysis of the health status, health insurance coverage, and healthcare experiences of direct care workers across long-term care using National Health Interview Survey data. Next, Lee et al. will present the trends and characteristics of occupational injuries and illnesses among California’s long-term care workers from 2019 to 2020 using California Workers’ Compensation data, assessing the impact of COVID-19 on their occupational health. Sterling will characterize the physical and mental health of the direct care workforce before and during COVID-19 using data from the CDC's Behavioral Risk Factor Surveillance Survey, as well as drawing on qualitative and survey-based studies of unionized, agency-employed home care workers in New York. Sterling will also present findings from a pilot feasibility study of an intervention aimed at improving home care workers’ well-being. Finally, Quinn et al. will synthesize findings on home care workers’ occupational hazards—including needlesticks, musculoskeletal strain, violence and infections—and examine how preventing these risks can improve safety for both workers and clients. The discussant will draw out themes and implications from across these complementary studies, highlighting the importance of safeguarding direct care workers’ health as a key step toward improving care quality and outcomes for older adults and people with disabilities.

In 2020, COVID-19 spread throughout the world, and international measures such as travel bans, quarantines, and increased social distancing were implemented. In Japan, the number of infected people increased, and a state of emergency was declared from 16 April to 25 May 2020. Such a change in physical activity could lead to a decline in physical function in people with disabilities. A retrospective study was conducted to determine the impact of the pandemic on the physical function of disabled persons living in the community. Data were collected at four points in time: two points before the declaration of the state of emergency was issued and two points after the declaration period had ended. Time series data of physical function at four points in time were compared for 241 people with disabilities. The mean age was 72.39 years; 157 had stroke, 59 musculoskeletal disease, and 26 other diseases. Overall, there was a long-term decrease in walking speed (p &lt; 0.001) and a worsening of the Timed Up-and-Go (TUG) score (p &lt; 0.001) after the period of the state of emergency. The TUG score worsened only in the group with a walking speed of 1.0 m/s or less before the state of emergency (p = 0.064), suggesting that this group was more susceptible.

The study determined differential psychological vulnerability of people with disabilities across different age groups in worst affected areas in tsunami disaster. A total of 275 tsunami affected people with various disabilities aged between 16 and 85 years were included in the final sample. Self Reporting Questionnaire (psychological distress), Impact of Event Scale (post-traumatic stress), and quality of life (QOL) were measured. People in their late adulthood and elderly years reported significantly higher psychological distress when compared with the teens and early adults and the young adults. People across different age groups reported equal posttraumatic stress. Increase in age resulted in more psychological distress and poor physical, social, and environmental QOL. Age and severity of disability were found to be significant predictors of psychological distress. The findings primarily supported the “differential vulnerability hypothesis” in the context of a disaster; therefore, implications are far reaching for the policy makers and planners, administrators/disaster managers, and mental health/psychosocial service providers. Long-term psychosocial and psychiatric interventions are suggested to be provided till the reconstruction and rebuilding phase continues.

The city of Olomouc is one of the most active operators in the long term municipal level, which addresses support for people with disabilities or physical handicaps comprehensively. In 2012, the APA Center began an intense collaboration (Department APA, FTK UP in Olomouc) with the Department of Social Affairs to address accessibility of cultural and sports facilities, public spaces, playgrounds, etc for people woth special needs. As part of efforts to unify methodologies and sharing information we intend to use in the monitoring of barrier-free collaboration with Kazuist company. Background: Older adults with intellectual disability are exposed to a higher anticholinergic burden compared to general older adults. This is due to a higher rate of both mental and neurological disorders among people with intellectual disability. The use of medications with a high anticholinergic burden is associated with adverse effects including daytime dozing, constipation and higher dependence level in the Barthel index for measuring activities of daily living. This scoping review aims to map and examine the existing research on physical and cognitive adverse effects associated with the long-term impact of anticholinergics among people with intellectual disabilities. Methods: The search was conducted in: PubMed, Cochrane library, EMBASE, Medline, Science Direct, CINAHL Complete and PsycINFO. Preliminary studies, grey literature, and conference papers were searched in related electronic databases. The search terms included terms related to ‘anticholinergic’, ‘long-term exposure’, ‘intellectual disability’ and ‘adverse drug reaction’ with Boolean operator ‘and’. Studies with at least three months’ exposure to anticholinergics were included. The search was restricted to research papers on people with intellectual disability aged 40 or over and publication in the English language only. Initially, it was conducted in May and June 2021 and covered the publication period between 1970 and 2021. It was re-run in October 2021. Results: The conducted search provided 509 records of publications and grey literature. Duplicates were removed using EndNote 20 and resulted in 432 remaining records. Then, 426 further records were excluded because they were deemed irrelevant, or non-longitudinal studies or conducted on different populations. Only six full articles were retrieved to assess their eligibility and all were excluded due to different study populations. This resulted in no studies meeting the stated inclusion criteria. Conclusions: Further research is urgently required to examine the long-term adverse effects associated with higher anticholinergic scores among older people with intellectual disability.

Ability-based design is a useful framework that centralizes the abilities (all that users can do) of people with disabilities in approaching the design of assistive technologies. However, although this framework aspires to support designing with people with all kinds of disabilities, it is mainly effective in supporting those whose abilities can be clearly defined and measured, in particular, physical and sensory attributes of ability. As a result, the ability-based design framework only provides limited guidance to design with users with intellectual disability, whose cognitive, physical, sensory, and practical abilities vary along a spectrum. In this article, we reflect on a long-term co-design study where we leveraged what we termed “competencies,” i.e., the representative practical skills people develop from their participation in life activities, in particular, mainstream technologies, such as social media and the Internet. Our reflection is based on our experience in designing SkillsTube, a web application we co-designed with young adults with intellectual disability to support them to learn life skills through videos. The app's design, which explored and leveraged their social media participation competencies, supported the fundamental participation of all participants and their peers. Their familiarity with the app's social media-inspired design features fostered confidence in their participation, usability, and engagement. Drawing on the findings and design process of the app, we discuss a Competency-based approach to designing with people with disabilities that extends upon ability-based design, by grounding it in user competencies.

Abstract People living with dementia in long-term care (LTC) are a diverse group with varied physical and cognitive abilities and backgrounds. Previous research has shown that virtual reality experiences can bring joy and foster the health and well-being of people living with dementia. A tailored approach based on cultural preferences, needs, abilities, disabilities, and contextual limitations is needed to foster practical implementation, uptake, and sustainability. This qualitative study harnessed the experiential knowledge of people with dementia, care partners and frontline staff to co-build a novel Virtual Reality Program in two Canadian LTC homes. We conducted focus groups and interviews with 10 residents living with dementia, 10 family care partners and 12 staff to explore their preferences on the video content and delivery methods. We also explored participants’ experiences in the co-creation process. Our interdisciplinary team, including people with dementia and family partners, researchers, clinicians, and trainees, analyzed the data thematically. We identified four themes: 1) Significance of culturally relevant and diverse videos to address heterogeneous population, 2) Acknowledgement of residents’ autonomy and choice in the co-creation process, 3) Feelings of satisfaction and motivation through contributing and learning, and 4) Appreciation of a respectful co-design environment. The results offer useful insights to inform future directions in co-designing safe and accessible virtual programs with people living with dementia in LTC research.

AbstractNeglected tropical diseases (NTDs) are a diverse group of infectious conditions that vary in their epidemiology, impact and control. They are among the most common conditions globally, affecting approximately one billion people. Many NTDs have long-term consequences, such as visual and physical impairments. As a result, people with NTDs may have difficulties in carrying out activities or participating in society—in other words, NTDs can cause disabilities. Additionally, NTDs are often strongly linked to stigma and can have mental health consequences. It is therefore important to incorporate rehabilitation within NTD programmes. Rehabilitation can be conceptualized narrowly in terms of the provision of clinical services (e.g. physiotherapy and assistive devices) or, more broadly, including efforts to improve employment, overcome stigma and enhance social participation of people with disabilities. Approximately 15% of the global population has a disability, and this large group must be considered when designing NTD programmes. Improving the inclusion of people with disabilities may require adaptations to NTD programmes, such as making them physically accessible or training staff about disability awareness. Without incorporating disability within NTD programmes, the quality of life of people with NTDs will suffer and global targets for elimination and management of NTDs will not be met.

Every individual requires care and support across the life-course, though the nature of the services and intensity may vary and generally increase with age and disability. Long-term care (LTC) services are required by people with (one or several) physical and/or mental health conditions to support them to live well. As children and young adults with disabilities are living longer, a life-course perspective is increasingly imperative when planning and delivering LTC. This 90-minute workshop is a collaboration between IFICs Ageing and Frailty SIG and Laurel, a new European project that will consider the current economic constraints, the accessibility, affordability and quality of LTC services, workforce shortages, and the systemic reforms required in the face of the growing demand for LTC. This interactive workshop will be of interest to policy makers, researchers, advocates, beneficiaries, carers and professionals who experience, plan, commission, fund, provide or regulate LTC services. Members of the Laurel consortium will share current perceptions, experience and expectations of older people and carers to ensure the dialogue is grounded in lived experience of people who receive LTC. Then, through the interaction with the attendees to the workshop we expect to hear other voices from regions across Europe and beyond to enrich the insights on more integrated approaches to LTC. Participants will be invited to discuss and prioritize policy action in five main domains: 1.Lack of access to LTC and / or social protection; need for more person-centredness, coordination, prevention and community engagement. Physical education and sports for people with disabilities are the areas of physical and social rehabilitation. The im- plementation of this can be possible through the use of an inclusive approach in the training process of athletes with disabilities. Anthropometric measurements and testing of athletes with disabilities were conducted to determine the impact of an inclusive approach in the construction of the training process. The main purpose of this research was to demonstrate the positive impact of inclusive training sessions within the developed program on the development of the physical abilities of athletes with disabilities. The research used theoretical (analysis of scientific and methodologi- cal literature; analysis of sports programs for athletes with disabilities), empirical (pedagogical observation; pedagog- ical experiment) methods, as well as methods of mathematical statistics. The results of the research showed a positive impact from the training on the developed karate program for people with disabilities. It also presented the increase of the testing results in the exercises “Sit and reach test”, “Standing long jump” and “Abdominal crunches”.

The impact of long-term unemployment is not confined to the vocational sphere but also includes a psychosocial component. Many who are out of work for long periods of time not only report deskilling but also lack of support, poor self esteem and low self confidence. This paper examines some of these issues by reference to both the Australian government's ‘Working Nation’ paper on unemployment and research carried out with 77 unemployed persons in Sydney, Australia. While it is not unusual for rehabilitation counsellors to receive referrals of people with disabilities who have been unemployed for considerable periods, contact with the general long-term unemployed population has been less common. The rehabilitation counselling profession is closely examined in terms of the degree to which its skill and knowledge base matches the needs profile of this general unemployed population. Rehabilitation counselling was found to be an appropriate profession to deal with the complex mix of vocational and psychosocial issues confronted by persons who have remained unemployed for long periods of time.

In the universe of thirteen subjects, nine constituted the sample. Used as instruments form and semi-structured interviews. In the interviews we used the Content Analysis Categorical Theme ruled in the Theory of Social Representations. In the results, enunciated three categories are: representations on disability from anatomic changes; representations of feelings experienced disability and lack of representation of the term disability. The disability acquired in adolescence or youth as a result of violence brings indelible mark, representing biopsychosocial difficulties and limitations, compromising the quality of life.

Many underlying physiological changes occur with age, and in elderly people, there is an increased risk of chronic disease and care dependence. Globally, the number of elderly people increases considerably in a relatively short period of time and the need for further support is also increasing exponentially. As the number of elderly people continues to grow along with their longevity, the need for long-term health care will increase significantly. Ultimately, the health care system faces a number of challenges, including long-term care, which includes activities related to the risk of age-related disabilities associated with chronic diseases. We examine the status of the elderly program in Oman, the achievements, and gaps in achieving the Ministry of Health's vision in 2050. The review eluded that the elderly program is headed in the right direction, however, the program still has unmet needs of physical fitness-related care, psychological health care, community support, and the leaving the environment. The findings of this review also underlined the importance of care models and support services specifically based on the needs of the elderly.

Background. The process of a comprehensive rehabilitation of the disabled person may be more complicated when the diagnosis becomes vague and uncertain. Unknown data about the patient’s past, mental, physical or vocational background, may cast a different light on the process. The patient’s diagnosis sometimes becomes an enigma. Using the model of Venn, the patient’s situation shifts from one circle to another. The only logical solution in these cases is to use the Occam’s Razor. Purpose. To draw the attention of rehabilitation medicine professionals to the problem of clients whose diagnoses are vague and uncertain and to share with the readers my personal thoughts on this topic. Methods. Reviewing literature and sharing with the readers the author’s 43 years of clinical experience in the field of rehabilitation medicine. Results and Conclusions. A PubMed query revealed 1479 articles with the words „diagnostic enigma”. The search term „diagnostic enigma in rehabilitation medicine” resulted only in four articles. In the long process of rehabilitation of disabled patients, some patients’ diagnoses change during the hospitalization phase and some diagnoses become unclear. The rehabilitation process becomes complicated and sometimes incomplete. Implications for Rehabilitation Medicine: 1. In most instances, patients are referred to rehabilitation medicine facilities with definite diagnoses. 2. The long process of a comprehensive rehabilitation of the disabled person may be more complicated when, prior to the discharge, the final diagnosis becomes vague and uncertain. 3. Facts about the patient’s past mental, physical or vocational background may cast a different light on the process. The patient’s diagnosis sometimes becomes an enigma.

Abstract Introduction Acute leukemia is a fatal hematologic malignancy. Because the clinical features of patients with acute leukemia can deteriorate very quickly, an early diagnosis and appropriate treatment are important for these patients. People with disabilities tend to be physically and mentally vulnerable and often have serious economic issues as well. Such situations can be linked to suboptimal diagnoses and treatments in case of acute leukemia. The purpose of this study is to analyze diagnosis rates, treatment patterns and prognoses of acute leukemia patients with disabilities and compare these findings with those of patients without disabilities. Method The major data sources were the National Disability Database, the Korean Central Cancer Registry, and the Korean National Health Insurance claim database. We built a cohort of 2,776,450 people with disabilities from the Korean National Disability Database. To establish a non-disabled cohort of 8,329,350 as a comparative group, frequency-matched sampling with the disabled group was performed using a merged database, with individuals grouped by age and sex at a ratio of 1:3. From this population, adult patients who were diagnosed with acute myeloid or lymphoblastic leukemia were selected for analysis. Disabilities were categorized into the following types: physical, communication, and intellectual/psychological. Results Among the 8,329,350 people without disabilities and 2,776,450 with disabilities, 3,171 and 873 were diagnosed with acute leukemia, respectively. The prevalence rates of acute leukemia were 38.07 per 100,000 without disabilities and 31.44 per 100,000 with disabilities. Among the acute leukemia patients with disabilities, those in the lowest income group amounted to 13.4% of the total, which was significantly higher than the rate of 4.3% of the patients without disabilities (P &lt; 0.0001). When analyzed by disability type, the numbers of patients with intellectual/psychological disability (50.37 per 100,000) and a communication disability (31.93 per 100,000) was exceeded the number of those with a physical disability (28.34 per 100,000). Chemotherapy for acute leukemia was performed in 61.7% of patients with disabilities, which was significantly lower than the rate of 68.0% for acute leukemia patients without a disability (p = 0.0006). Patients with communication (53.7%) or intellectual/psychological disabilities (54.2%) received chemotherapy less often than those with a physical disability (68.6%). Allogeneic hematopoietic stem cell transplantation was performed in 11.2% of patients without a disability and 8.0% of disabled patients (p = 0.0026). The number of hospitalization days per patient during the first year after diagnosis was 109.3 days for the disabled and 126.1 days for the non - disabled (P &lt;0.0001). The median survival was 14.0 months for patients with disabilities, which was significantly shorter than those without a disability (21.4 months) (P &lt;0.0001). Patients with a communication disorder (8.1 months) and an intellectual/psychological disability (13.1 months) had significantly worse survival rates than patients with a physical disability (15.2 months) (P &lt; 0.0001). Conclusion People with disabilities overall have a relatively low diagnosis rate of acute leukemia and undergo less active treatment of acute leukemia. This tendency is particularly evident in patients with intellectual/psychological and communication disabilities. Patients with acute leukemia with disabilities have significantly lower survival rates than those without a disability. Various measures including financial support are needed to ensure that acute leukemia patients with disabilities receive appropriate treatment and achieve long-term survival. Figure. Figure. Abstract Despite an increased number of community-based services, people with intellectual and developmental disabilities (IDD) continue to struggle with meaningful community participation and often feel they are not valued community members. This study examined how community support services, which support physical placement and community integration, were allocated in Medicaid 1915(c) HCBS waivers, the largest provider of long-term services and supports for people with IDD in the United States. Community support services totaled $447.5 million of projected funding in fiscal year 2014. While community transition services funded expenses required to establish a basic household for the initial transition, community integration services promoted a successful community placement by utilizing natural supports and developing skills necessary to maintain community integration.

BackgroundPolicy makers need disability projections for planning adequate services and measures for health promotion. The aim of this study is to provide projections on severe mobility limitations up to year 2044 and illustrate how the projected prevalence and the number of persons with severe mobility limitations are affected by potential changes in the modifiable risk factors, namely excess weight, physical inactivity and smoking.MethodsWe analysed the nationally representative, repeated measures Health 2000 and 2011 Surveys (BRIF8901) with 8615 and 6740 participants, respectively, aged 18 years and older. Severe mobility limitations were defined as major difficulties or unable to walk about half a kilometre. We applied a multistate model on repeated measures to account for both individual risk factors and their changes over time.ResultsThe number of people with severe mobility limitations was projected to double by the year 2044 in Finland, due to the rapid ageing of the population. Eliminating half of the excess weight would reduce their number by one-fifth, while reductions in the prevalence of smoking and physical inactivity would have a minor impact. Even if excess weight, smoking and physical inactivity were completely eliminated, the number of persons with severe mobility limitations is projected to increase.ConclusionsDesigning and implementing strategies to promote healthy weight are important to slow down the rapid increase in mobility limitations due to population ageing. Providing adequate health and social services for the increasing population with disabilities will nevertheless be an increasing national challenge.
