Abstract:
Osteoarthritis (OA) is a disorder that can affect any joint. It results in a high clinical burden in many patients. Patients with OA experience a wide range of symptoms and clinical signs such as pain, disability, stiffness, tenderness, crepitus, and decreased mobility and strength in their osteoarthritic joints, where the impact depends on the involved joint. Also general symptoms such as fatigue and psychosocial consequences are experienced by OA patients. The impact of symptoms and signs does not only depend on osteoarthritic abnormalities, but also on patient factors, such as coping strategies and illness perceptions, and co-morbidities. In this chapter, general and joint-specific symptoms and signs as well as the impact of patient factors are discussed.

Abstract:
This chapter presents a synthesis of major research themes and findings on patients’ subjective experiences and perceptions of coercion in community psychiatry. Covering the 20-year period from 1994 to 2014, it reviews studies of both formal (legislative) and informal (non-legislative) coercion, including patients’ experiences of mandated community treatment or community treatment orders (CTOs), assertive community treatment (ACT), and leverage. It begins by presenting four concepts that characterize patients’ perceptions: interventions, obligations, threats, and safety. It then reviews the contradictory evidence surrounding patients’ experiences of coercion, including patient comparisons of hospital and community coercion and the implications of coercion for self-determination, ‘normality’, care and services, and for wellness. It concludes by identifying gaps in the literature and recommending future research.

Immunocompromised patients are at a high risk of infection with resistant organisms due to their increased exposure to hospital environments, including the intensive care unit, their frequent need for invasive procedures, and increased antimicrobial use. To limit this growing trend, and due to the paucity of development of new antimicrobial agents with novel mechanisms of action, the judicious use of the agents currently available should be encouraged. A broad spectrum of possible infections combined with the diagnostic uncertainty, clinical condition, and the specialist teams’ perceptions make antimicrobial stewardship very difficult. However, evidence presented in this chapter illustrates how stewardship in the immunocompromised host may be achieved.
