Abstract:
ABSTRACT Objectives: This study aimed to (1) measure the degree of patient satisfaction among the clinical and nonclinical dental services offered at specialty dental centers and (2) investigate the factors associated with the degree of overall satisfaction. Materials and Methods: Four hundred and ninety-seven participants from five dental centers were recruited for this study. Each participant completed a self-administered questionnaire to measure patient satisfaction with clinical and nonclinical dental services. Analysis of variance, t-tests, a general linear model, and stepwise regression analysis was applied. Results: The respondents were generally satisfied, but internal differences were observed. The exhibited highest satisfaction with the dentists’ performance, followed by the dental assistants’ services, and the lowest satisfaction with the center's physical appearance and accessibility. Females, participants with less than a bachelor's degree, and younger individuals were more satisfied with the clinical and nonclinical dental services. The stepwise regression analysis revealed that the coefficient of determination (R2 ) was 40.4%. The patient satisfaction with the performance of the dentists explained 42.6% of the overall satisfaction, whereas their satisfaction with the clinical setting explained 31.5% of the overall satisfaction. Conclusion: Additional improvements with regard to the accessibility and physical appearance of the dental centers are needed. In addition, interventions regarding accessibility, particularly when booking an appointment, are required.

Abstract:
The Specialized Dental Care Centers (SDCC) have the mission to expand access to public medium complexity dental care and support the primary health care actions at this level of complexity. However, it is necessary to ensure the quality of services and to evaluate such services continuously to identify weaknesses and strengths that support the processes of leadership/management. Nevertheless, there is a dearth of studies on the assessment of oral health in specialized care that may indicate which factors should be investigated. Therefore, this integrated literature review sought to explore the plethora of publications on the evaluation of SDCC in the LILACS and MEDLINE data bases in October 2013 to identify factors possibly related to the performance of such health services. Thus, 13 references were included in this review pointing to forms of organization and management of work processes related to the creation of healthcare networks (operation of regulation centers and setting up of health consortiums). They include the contextual characteristics of the places where SDCCs are located (population size, Family Health Strategy coverage, Municipal Human Development Index, governance, governing capacity) were factors that influenced the SDCCs performance.

Abstract:
The aim of this study was to evaluate the performance of the Centers for Dental Specialties (CDS) in the country and associations with sociodemographic indicators of the municipalities, structural variables of services and primary health care organization in the years 2004-2009. The study used secondary data from procedures performed in the CDS to the specialties of periodontics, endodontics, surgery and primary care. Bivariate analysis by χ2 test was used to test the association between the dependent variable (performance of the CDS) with the independents. Then, Poisson regression analysis was performed. With regard to the overall achievement of targets, it was observed that the majority of CDS (69.25%) performance was considered poor/regular. The independent factors associated with poor/regular performance of CDS were: municipalities belonging to the Northeast, South and Southeast regions, with lower Human Development Index (HDI), lower population density, and reduced time to deployment. HDI and population density are important for the performance of the CDS in Brazil. Similarly, the peculiarities related to less populated areas as well as regional location and time of service implementation CDS should be taken into account in the planning of these services.

Abstract:
Objectives: To retrospectively assess radiographic data and to prospectively classify radiographs (namely, panoramic, bitewing, periapical, and cephalometric images), we compared three deep learning architectures for their classification performance. Methods: Our dataset consisted of 31,288 panoramic, 43,598 periapical, 14,326 bitewing, and 1176 cephalometric radiographs from two centers (Berlin/Germany; Lucknow/India). For a subset of images L (32,381 images), image classifications were available and manually validated by an expert. The remaining subset of images U was iteratively annotated using active learning, with ResNet-34 being trained on L, least confidence informative sampling being performed on U, and the most uncertain image classifications from U being reviewed by a human expert and iteratively used for re-training. We then employed a baseline convolutional neural networks (CNN), a residual network (another ResNet-34, pretrained on ImageNet), and a capsule network (CapsNet) for classification. Early stopping was used to prevent overfitting. Evaluation of the model performances followed stratified k-fold cross-validation. Gradient-weighted Class Activation Mapping (Grad-CAM) was used to provide visualizations of the weighted activations maps. Results: All three models showed high accuracy (>98%) with significantly higher accuracy, F1-score, precision, and sensitivity of ResNet than baseline CNN and CapsNet (p < 0.05). Specificity was not significantly different. ResNet achieved the best performance at small variance and fastest convergence. Misclassification was most common between bitewings and periapicals. For bitewings, model activation was most notable in the inter-arch space for periapicals interdentally, for panoramics on bony structures of maxilla and mandible, and for cephalometrics on the viscerocranium. Conclusions: Regardless of the models, high classification accuracies were achieved. Image features considered for classification were consistent with expert reasoning.

Abstract:
This study aimed to find the set of risk indicators best able to predict root caries (RC) incidence in caries-active adults utilizing data from the Xylitol for Adult Caries Trial (X-ACT). Five logistic regression models were compared with respect to their predictive performance for incident RC using data from placebo-control participants with exposed root surfaces at baseline and from two study centers with ancillary data collection (n = 155). Prediction performance was assessed from baseline variables and after including ancillary variables [smoking, diet, use of removable partial dentures (RPD), toothbrush use, income, education, and dental insurance]. A sensitivity analysis added treatment to the models for both the control and treatment participants (n = 301) to predict RC for the control participants. Forty-nine percent of the control participants had incident RC. The model including the number of follow-up years at risk, the number of root surfaces at risk, RC index, gender, race, age, and smoking resulted in the best prediction performance, having the highest AUC and lowest Brier score. The sensitivity analysis supported the primary analysis and gave slightly better performance summary measures. The set of risk indicators best able to predict RC incidence included an increased number of root surfaces at risk and increased RC index at baseline, followed by white race and nonsmoking, which were strong nonsignificant predictors. Gender, age, and increased number of follow-up years at risk, while included in the model, were also not statistically significant. The inclusion of health, diet, RPD use, toothbrush use, income, education, and dental insurance variables did not improve the prediction performance.

Abstract:
Background: Patient safety is a priority for achieving higher quality health care standards and human error> reduction. Objective: The aim of this study was to evaluate the level of appropriate infection control practices in a prosthodontic clinic in Jeddah, Saudi Arabia. Methods: This cross-sectional study included 460 dental students and dentists recruited from five centers. Data were collected by self-administered questionnaires> composed of 25 questions. The questionnaires> assessed infection control practices in prosthodontic clinics, including wearable barriers, disinfection measures among patients, disinfection of laboratory submissions, and proper infection control. Other questions assessed sources of knowledge, self-evaluation of knowledge, implementation, and satisfaction with knowledge application. Results: Total correct answers for proper practice (16 questions) had a mean of 12.5 with a standard deviation of 2.8. The most commonly practiced procedure was wearing a mask (99.8%) and the least was sterilizing the facebow before its use by the patient (53.7%). Participants from governmental colleges (m = 13.67, SD = 2.17) had significantly higher scores than participants from private colleges (m = 12.35, SD = 2.9), p <0.001. Only 2.8% had never attended a lecture, and only 13.8% had never had hands-on training for infection control. Despite 49.1% evaluating themselves as having very poor to poor knowledge, 90.7% had fair to very good valuation of their infection control implementations, and 87.6% were satisfied with their knowledge and performance levels. Conclusion: Dental students and dentists have high levels of adequate infection control practices in the prosthodontic clinic despite the moderate level of their satisfaction with their knowledge.

Tertiary pediatric medical centers disproportionately care for low-income, underserved children with significant dental needs. Long wait times for hospital operating room treatment increase tooth loss rather than restoration. Oral sedation has commonly been provided to avoid the long waits for operating room treatment. However, this can be challenging with young, anxious patients. High failure rates and repeat visits for oral sedation have resulted in continued waiting for definitive dental services in the operating room. The Division of Dentistry requested the Department of Anesthesiology to create a general anesthesia program in the dental clinic to increase the use of anesthesia services but align the cost of the anesthetic with the revenue stream. Our aim was to objectively measure the performance of a dental clinic anesthesia service by comparing the percentage of case completions, percentage of complete radiographs, and number of serious adverse events to clinic-based oral sedations. We were also interested in total number of cases completed. We retrospectively studied data regarding an in-office general anesthesia (IOGA) program for dentistry and compared it to oral sedations before and after instituting the IOGA program. Patients received either a general endotracheal anesthetic or nonintubated total intravenous general anesthesia. Successful case completion increased from 88.6% (oral sedation) to 99.5% (IOGA). One hundred percent of IOGA cases had complete radiographs, as opposed to 63.4% for oral sedation. This was an increase from 53.5% from the previous 2 years with oral sedation. Serious adverse event rate was 0% (0/508) for oral sedation and 0.2% (1/418) for IOGA. Comparing 2 years before and after IOGA revealed a decrease in oral sedations from 930 to 508, whereas IOGA increased from 0 to 418 cases. Anesthesia services in dental clinic increased complete dental care and complete radiographs, reduced failed sedations, and were performed safely.

Background: It is no doubt that longer wait times can affect patient care and patients’ willingness to seek health-care services. Not only does this disrupt the continuity of treatment and care, but it also negatively impacts patient outcomes. During the past few years, the concept of patient satisfaction has become a vital component in assessing the delivery and efficiency of care. Patient satisfaction is a performance indicator that measures the extent to which patient is content and satisfied with the level of care provided by health-care institutions and providers. Therefore, this research examined association between the wait times and patient satisfaction in selected primary health-care centers in Al Qassim region in the Kingdom of Saudi Arabia. Methodology: A patient satisfaction questionnaire was administered to 850 patients, which collected patient perceptions on the delivery of care at health-care centers in Al Qassim City. Outcome measures included wait times for: registration and payment, seeing the physician, performing radiation and assays, and dispensing the medications. Results: The response rate was 72.94% (n = 620). The study found that 27.90% of the participants stated that the wait time to see the physician ranged between 21 and 30 minutes. Overall patients were mainly dissatisfied for wait times in relation to medication dispensation, vital signs measurement, dental consultations, and radiological investigation. The study found a positive association between the patient satisfaction and their education, marital status, and job. A significant regression equation was established between the patient satisfaction and age-group and literacy. Conclusion: The study advocated the need for recent technology, sufficient staffing, and patient-centered friendly methods to reduce wait times.

Abstract:
Introduction. The backbone of Serbian health system forms the public healthcare provider network with 355 institutions and around 112,000 employees, owned and controlled by the Ministry of Health and financed mainly by the Republican Health Insurance Fund. The law recognizes private practice that was not included, till recently, in the public funding scheme. New Health Insurance Law (2005) decreased the number of entitlements in the basic health service package. It abolished the right to dental health care for adults (exceptions are: children, older than 65, pregnant women and emergency cases) as well as the right to compensate travel expenses. The aim of this study was to evaluate the effects of health care system of the Republic of Serbia and indicate parameters that determine the state of health of the population, on the ground of data obtained by the Institute of Public Health of Serbia. Results. In the period 2004-2012, cardiovascular diseases represented the main cause of illness in Serbia (50%). In 2012 digestive system diseases were on the second place. Neoplasm and nervous system diseases were on the third place. From 2007 to 2012 there was slight decline in the birth rate and number of deaths, but the death rate increased from 13.9 to 14.2. Health care system in Serbia is funded through the combination of public finances and private contributions. Primary care is provided in 158 health care centres and health care stations, secondary and tertiary care services are offered in general hospitals, specialized hospitals, clinics, clinico-hospital centers and clinical centres. Conclusion. A significant but not satisfactory progress has been achieved in the field of health status indicators as the most important outcome of the final performance of the health system. The transition of public health care system in Serbia since the communist period to present and slow integration with European Union is unfinished process.

Purpose – This study aims to examine and analyze the effect of individual characteristics and leadership skill on organizational performance with Teamwork as an intervening variable at accredited Community Health Centers in Pasuruan district. Design/ Methodology/ ApproachThis study examines the effect of individual characteristic and leadership skill on organizational performance with Teamwork skill as an intervening variable at plenary accredited Community Health Center in Pasuruan district. The sample of this study are employees who involved in accreditation process and quality improvement at two plenary accredited Community Health Centers totaling 157 people. Hypothesis testing used Structural Equation Modelling with Partial Least Square regression. Result and Discussion – The result of this study shows that individual characteristics and leadership skill have significant positive effect toward Teamwork skill. Individual characteristic and leadership skill also have significant positive effect toward organizational performance through Teamwork which means that improvement on individual characteristic and Teamwork skill lead to better organizational performance along with better Teamwork skill. Leadership variable dominantly affects organizational performance through Teamwork skill rather than individual characteristic. Leadership is second order variable with transformational leadership which has dominant measurement toward leadership skill rather than transactional leadership. Conclusion –The result of this study shows that there is positive and significant effect between individual characteristic and leadership skill toward organizational performance with Teamwork skill as an intervening variable. Leadership skill dominantly affects organizational performance through Teamwork rather than individual characteristic. Leadership is second order variable with transformational leadership dimension which has more dominant effect toward leadership skill rather than transactional leadership. Research Implication – This research has implications for assessing individual characteristics and leadership, as well as Teamwork on the organizational performance of the health center.

Organizations design data center and service delivery with the aim of catering to maximize device provisioning & availability, improve application performance, ensure better server virtualization and end up securing data centers using security solutions at internet edge protection level. These security solutions prove to be largely inadequate in times of a DDoS cyber-attack. In this paper, traditional data center design is reviewed and compared to the proposed three tier data center. The resilience to withstand against DDoS attacks is measured for Real User Monitoring parameters, compared for the two infrastructure designs and the data is validated using T-Test.

Abstract:
Prediction models learn patterns from available data (training) and are then validated on new data (testing). Prediction modeling is increasingly common in dental research. We aimed to evaluate how different model development and validation steps affect the predictive performance of tooth loss prediction models of patients with periodontitis. Two independent cohorts (627 patients, 11,651 teeth) were followed over a mean ± SD 18.2 ± 5.6 y (Kiel cohort) and 6.6 ± 2.9 y (Greifswald cohort). Tooth loss and 10 patient- and tooth-level predictors were recorded. The impact of different model development and validation steps was evaluated: 1) model complexity (logistic regression, recursive partitioning, random forest, extreme gradient boosting), 2) sample size (full data set or 10%, 25%, or 75% of cases dropped at random), 3) prediction periods (maximum 10, 15, or 20 y or uncensored), and 4) validation schemes (internal or external by centers/time). Tooth loss was generally a rare event (880 teeth were lost). All models showed limited sensitivity but high specificity. Patients’ age and tooth loss at baseline as well as probing pocket depths showed high variable importance. More complex models (random forest, extreme gradient boosting) had no consistent advantages over simpler ones (logistic regression, recursive partitioning). Internal validation (in sample) overestimated the predictive power (area under the curve up to 0.90), while external validation (out of sample) found lower areas under the curve (range 0.62 to 0.82). Reducing the sample size decreased the predictive power, particularly for more complex models. Censoring the prediction period had only limited impact. When the model was trained in one period and tested in another, model outcomes were similar to the base case, indicating temporal validation as a valid option. No model showed higher accuracy than the no-information rate. In conclusion, none of the developed models would be useful in a clinical setting, despite high accuracy. During modeling, rigorous development and external validation should be applied and reported accordingly.

Abstract:
In this study, the efficacy of the automated deep convolutional neural network (DCNN) was evaluated for the classification of dental implant systems (DISs) and the accuracy of the performance was compared against that of dental professionals using dental radiographic images collected from three dental hospitals. A total of 11,980 panoramic and periapical radiographic images with six different types of DISs were divided into training (n = 9584) and testing (n = 2396) datasets. To compare the accuracy of the trained automated DCNN with dental professionals (including six board-certified periodontists, eight periodontology residents, and 11 residents not specialized in periodontology), 180 images were randomly selected from the test dataset. The accuracy of the automated DCNN based on the AUC, Youden index, sensitivity, and specificity, were 0.954, 0.808, 0.955, and 0.853, respectively. The automated DCNN outperformed most of the participating dental professionals, including board-certified periodontists, periodontal residents, and residents not specialized in periodontology. The automated DCNN was highly effective in classifying similar shapes of different types of DISs based on dental radiographic images. Further studies are necessary to determine the efficacy and feasibility of applying an automated DCNN in clinical practice.

Abstract:
An air turbine handpiece is a dental abrasive device that rotates at high speed and uses compressed air as the driving force. It is characterized by its small size, light weight, and painless abrading due to its high-speed rotation, but its torque is small and noise level is high. Thus, to improve the performance of the air turbine handpiece, we conducted a performance test of an actual handpiece and a numerical analysis that modeled the whole handpiece; we also analyzed the internal flow of the handpiece. Results show that experimental and calculated values were consistent for a constant speed load method with the descending speed of 1 mm/min for torque and turbine output. When the tip of the blade was at the center of the nozzle, the torque was at its highest. This is likely because the jet from the nozzle entered the tip of the blade from a close distance that would not reduce the speed and exited along the blade.

BackgroundTo compare performance of routing paradigms for patients with acute ischemic stroke using clinical outcomes.MethodsWe simulated different routing paradigms in a system comprising one primary stroke center (PSC) and one comprehensive stroke center (CSC), separated by distances representative of urban, suburban, and rural environments. In the nearest center paradigm, patients are initially sent to the nearest center, while in CSC first, patients are sent to the CSC. In the Rhode Island and distributive paradigms, patients with a FAST-ED (Facial palsy, Arm weakness, Speech changes, Time, Eye deviation, and Denial/neglect) score ≥4 are sent to the CSC, while others are sent to the nearest center or PSC, respectively. Performance and efficiency were compared using rates of good clinical outcome, determined by type and timing of treatment using clinical trial data, and number needed to bypass (NNB).ResultsGood clinical outcome was achieved in 43.76% of patients in nearest center, 44.48% in CSC first, and 44.44% in Rhode Island and distributive in an urban setting; 43.38% in nearest center, 44.19% in CSC first, and 44.17% in Rhode Island in a suburban setting; and 41.10% in nearest center, 43.20% in CSC first, and 42.73% in Rhode Island in a rural setting. In all settings, NNB was generally higher for CSC first compared with Rhode Island or distributive.ConclusionRouting paradigms that allow bypass of nearer hospitals for thrombectomy capable centers improve population level patient outcomes. Differences are more pronounced with increasing distance between hospitals; therefore, paradigm choice may be most impactful in rural settings. Selective bypass, as implemented in the Rhode Island and distributive paradigms, improves system efficiency with minimal impact on outcomes.

Abstract Despite the fact that simple interventions and emerging therapies can increase survival of SCD patients, under detection renders SCD a major cause of under age-5 mortality in rural regions of the developing world where it is prevalent. (Piel et PLoS Med DOI: org/10.1371/journal.pmed.1001484, 2013). Two major diagnostic challenges are: 1. A lack of affordable diagnostic methodology to obtain point-of-care results in low-resource environments. 2. Dispersion of SCD patients in vast rural hinterlands who rarely interface with healthcare services. To address the first impediment, we have developed and validated a simple, low-cost test to screen for SCD using aqueous multiphase systems (AMPS). AMPS are solutions of polymers and surfactants that form immiscible self-assembling step-gradients. An AMPS designed to target the density signature of sickle cell anemia forms the basis of this novel diagnostic technique. (Kumar et al, PNAS DOI:10.1073/pnas.14147391110, 2014). Building on a design previously tested in Zambia (Kumar et al, PLoS One DOI: 10.1371/journal.pone.0114540, 2014) an improved test has been developed and named "Mpana" for "Multi-Phase Analyzer." The Mpana test detects the presence or absence of cells with a high mass-density as an indirect method to test for the presence of sickled cells. The test uses 5 µL of fingerstick blood, can be performed in 15 minutes using an inexpensive ($150), battery-powered centrifuge, the results can be read with the naked eyes by a trained reader (Figure 1) and costs 50 cents per test. The improved density-based test was first verified on fingerstick samples in the U.S. before being validated in a field study in rural Zambia. To manage the access difficulty of large-scale rural SCD screening, we exploited a partnership with the National Zambian Dental Training School. We have previously demonstrated that provision of free toothbrushes, toothpaste and other minimal health services is a powerful magnet to attract subjects in rural regions of Zambia that have high rates of tooth decay. Once collected these individuals can receive other healthcare interventions such as SCD screening. We conducted a cross-sectional field test of this approach in August and September of 2017 in the rural Serenje and Chitambo Districts of Northern Zambia. A local drama group and radio and mobile broadcasts informed the target communities about the dental intervention that attracted over 700 subjects. Of these, 503 children aged 1 month to 18 years were symptom screened for SCD, tested with MPANA, and dried blood spot (DBS) samples collected. The DBS were analyzed by iso-electric focusing and high-performance liquid chromatography at the New England Newborn Screening Program. Of the screened subjects, 78 had sickle cell trait and 17 had SCD. A score was assigned to visual readings of the MPANA test to enable ROC analysis.The ROC curve shows good diagnostic performance with an area under the curve (AUC) of 0.84. (Figure 2) The MPANA test had a sensitivity of 71% (CI 47-88%) and a specificity of 99% (CI 98-100%), with an overall diagnostic accuracy of 98% (CI 97-99%). Importantly, the MPANA test had a negative predictive value of 99% (CI 98-100%). Combining Mpana results with a clinical symptom score yielded a slightly improved specificity of 100% (CI 99.5-100%) and yielded an overall diagnostic accuracy of 99% (CI 98-100%). In conclusion, a multidisciplinary consortium has achieved the first validation of a low-cost simple point of care test to diagnose SCD in a rural area of a developing nation and has demonstrated the ability of dental services to facilitate such screening. Offsetting the theoretical concern that the MPANA test might not detect SCD in neonates and young infants due to their high hemoglobin F content are the advantages that our SCD screening strategy is low cost, primarily utilizes local resources and immediately delivers positive subjects to care interventions (which could be anything in addition to or other than SCD screening). It is therefore amenable to scaling up for widespread application. The quality of health services is measured by 3 components, namely input (HR, Infrastructure,) process (reliability), and output (product results). If the program's achievements do not reach the target, the quality of primary health care services can be doubted. The research objective is to determine the achievements of the program with the level of accreditation of health centers. The study uses a descriptive-analytic method with a cross-sectional design, using secondary data essential nutrition programs. The population and sample are all primary health care in Aceh Province that are accredited or not accredited, data collection is carried out in the program and data section of the Aceh Health Office and the statistical tests used are chi-square and odds ratio using STATA 13 software. The results of the study indicate, that malnutrition that receives care with low performance, children under five are weighed with low performance and low LBW cases have a relationship with the level of accreditation (p-value <0,05). In conclusion, several nutrition programs related to primary health care accreditation are malnourished children receiving treatment, LBW, and weighing under five. While the achievements of other nutrition programs do not show a relationship with the accreditation of primary health care. Suggestions, to create excellently and quality service, the health center must be able to improve program achievements according to the Government's target either in accordance with one of the indicators namely the strategic plan and health indicators.

Out of the 20 respondents, 5% of themquite agreed, 15% agreed, and 80% strongly agreed that the SEIMUT PERSIA video was interesting to watch. Background: Late adulthood is an aging process characterized by a decrease in some bodily functions. Physical changesassociated with late adulthood are musculoskeletal disorders, changes in the cardiovascular system, changes in the immunesystem, and changes in physical activity. Physical exercise is one of the ways to improve elderly’ physical activities. Anepidemiological study on elderly under Jagir Public Health Center coverage area reported that periodontitis and dentalcaries could lower physical performance in the elderly, lowering physical activity. Purpose: In consideration of theproblem in question, a promotive and preventive program is needed. The Dental Health Physical Exercise (SKG) programwas expected to improve the overall physical condition of the elderly. Methods: The “Senam Kesehatan Gigi dan Mulut”took place on 19 July 2019 at Integrated Health Service Post for Elderly. Results: There were 26 elderly participated inphysical exercise. 7.7% of them were not able to follow the exercise, and the remaining 92.3% were able to follow theexercise. Conclusion: 92.3% of 26 elderly were able to follow the exercise properly and correctly. However, the remaining7.7% of them were not able to catch up with the physical exercise. In addition, most participants were able to perform 4out of 5 movements criteria.

The purpose of this study was to know the correlation between tutuor's performance and the learner's motivation. The study was conducted at the Learning Center of Nurul Fikri in Petukangan, South Jakarta as from July through November 2008. The population covered 115 with 50 learners selected by cluster technique. The data were collected by using questionairs, observation, and document study. Employing correlation formula of Rank Spearman, the study proved the positive correlation between tutor's performance and learner's learning motivation. Besides this study also indicated the tutor's low performance in instructional process, and the tutor's poor knowledge in tutor's functions and competencies. The study also provides a set of recommendations to improve the tutor's performance.

The number of patients visited Dental Service Center at Petanahan Community Health Center Kebumen inOctober-November 2016 went up and down. This reaserch is aimed to know patients' satisfaction level at the Dental ServiceCenter at Petanahan Community Health Center, Kebumen. The type of research was descriptive research. The researchwas conducted at Petanahan Community Health Center, Kebumen. The number of samples was 40 respondents withdetermination of the sample by purposive sampling. Criteria of respondent were 20-45 years old, graduated from junior highschool (minimum), and willing to be the research respondent. The patients' satisfaction level research was conducted afterthe patient received dental and oral health care and completed a questionnaire containing 20 statements. The resultsshowed the satisfaction level of patients in the category of very satisfied as much as 26 respondents (65%). Patientsatisfaction level seen from the comfort dimension showed 60% of respondents are very satisfied, seen from the dimensionsof reliability and professionalism officials 70% of respondents feel very satisfied and seen from the dimensions ofresponsiveness and empathy 67.5% of respondents feel very satisfied. It can be concluded that the level of patientsatisfaction on Dental Service Center Puskesmas Petanahan Kebumen entered in the category very satisfied as much as 26respondents (65%).

Objek wisata agro adalah produk jasa yang ditawarkan oleh perusahaan untuk menarik banyak pengunjung. Penelitian ini bertujuan untuk menganalisis tingkat kepuasan pengunjung dengan tingkat kinerja dan kepuasan pengunjung terhadap Agro Hortimart Agro Center. Pendekatan yang digunakan adalah analisis deskriptif dengan metode survei dan kuesioner sebagai alat penelitian. Teknik pengambilan sampel menggunakan accidental sampling dengan 40 orang sebagai responden. Teknik analisis yang digunakan adalah analisis deskriptif, Importance Performance Analysis (IPA) dan Cartesian Diagram. Hasil analisis tingkat kepuasan pengunjung dengan metode Importance Performance Analysis (IPA) menunjukkan bahwa tingkat kinerja lebih tinggi dari kepentingan pengunjung (performance > importance) dengan skor rata-rata tingkat kinerja 4,06 dan skor rata-rata kunjungan pengunjung 4.02. Diagram kartesius menunjukkan bahwa kuadran I dan III membutuhkan perhatian khusus untuk menjaga dan meningkatkan kualitas layanan. Dengan demikian dapat disimpulkan bahwa pengunjung puas dengan kinerja yang dilakukan oleh Hortimart Agro Center.

Indicator of posyandu work performance in West Aceh was still far below the target determined and there was a decrease during 2011-2012. The results of the preliminary study showed that the motivation of village midwives were still low in posyandu cadre coaching. The aim of this research was to identify the factors associated village midwives motivation in posyandu cadre coaching. The study was conducted quantitavely and qualitatively. The quantitative research used survey method with a cross sectional design. The subjects were 146 village midwives selected by clustered random sampling at public healthcare center level. Data colection was conducted by interviews using structured questionnaire. The bivariate analysis used Chi Square tests, and the multivariate analysis used logistic regression. The qualitative data was collected by in-depth interviews to the village midwives and analyzed by content analysis. The results showed that 64.4% of the respondents had low performance, 54.8% had less responsibility, 62.3% had insufficient potential development, 57.5% felt inadequate compensation, and 52.7% had less supervision. There were associations between responsibility (C= 0.237, p=0.006), potential development (C= 0.268, p=0.001), compensation (C=0.518, p=0.0001), working condition (C=0.237, p=0.006), supervision (C=0.261, p=0.002,) and the village midwife performance in posyandu cadres’s coaching. Compensation had the most significant effect on the village midwive motivation in posyandu cadres coaching (Exp-B=17.383, p=0.0001). The obstacles experienced by the village midwives in posyandu cadres coaching were insufficient fund, unavailable special training for the posyandu cadre coaching and unavailable health facilities. Compensation has the most significant effect on village midwive motivation in posyandu cadres coaching. It is suggested to public healthcare centers and Health office to implement refreshing cadre coaching and improving village midwives performance in by giving appreciation to village midwives who conducted posyandu cadre.

This study was conducted at BP3K Jati Agung Regency, South Lampung District in 2017 with 17 respondents of Extension Workers. The research method used is a survey method. Methods of analysis used were descriptive analysis and Rank Spearman correlation. The results showed that Extension Workers have a high rate of ICT utilization and a high performance level. The use of ICT has a significant relationship with the performance of Extension Workers and factors related to the use of ICT are age, education, income, and cosmopolitan characteristics. Maka bangunan jenis ini perlu dirancang dengan sebaik mungkin, diantaranya ditandai dengan kenyamanan pengguna bangunan, serta kelayakan bangunan sebagai sarana umum. Thermal performance telah menarik perhatian bagi manusia dari semenjak manusia membuat tempat penampungan pertama yang dibangun untuk melindungi kita dari cuaca, musuh alami, dan bahaya lainnya. Kemajuan lebih lanjut dalam studi tentang thermal performance terjadi bersamaan dengan pengembangan ilmu bangunan sebagai suatu disiplin ilmu, dan dengan pengenalan komputasi personal (khususnya simulasi komputer) di lapangan. Untuk mengukur thermal performance yang pada kajian ini dikhususkan pada bangunan olahraga indoor secara kredibel, harus menggunakan prosedur pengukuran performa yang terstandarisasi internasional. Standar pengukuran yang sering digunakan adalah ASHRAE standard 55. ASHRAE standard 55 ini mengidentifikasi apa saja yang harus diukur, bagaimana pengukurannya, dan seberapa sering diukur melihat kenyamanan bangunan tersebut terhadap penggunanya.

Abstract:
The payment of capitation funds affects the motivation and performance of health workers in providing quality services at the Puskesmas. The work motivation of doctors can affect the quality of service and the performance of other health workers. The purpose of this study is to determine the effect of the distribution of capitation funds on the work motivation of doctors in Puskesmas in Indonesia. This study uses secondary data from Risnakes 2017 research results with cross-sectional design. The sample was all doctors in the puskesmas in Indonesia who were successfully interviewed and had independent variables is 4.012 sample. Data analysis was performed descriptively and statistically using chi-square test and logistic regression. From the results of the study found there is a relationship between the suitability of the distribution of capitation funds with the work motivation of doctors in urban health centers in Indonesia with a P value of 0,000 and PR 1,309 (95% CI 1,153 - 1,487). This means that the appropriate distribution and capitation have an influence of 1,309 times in increasing the work motivation of doctors in Indonesian health centers. The distribution of capitation funds must be in accordance with the workload of doctors at the puskesmas to increase motivation and performance of quality services at the puskesmas and the existing regulations must be regularly evaluated.

Objective. The purpose of this study was to examine the relationship between physical fitness, lifestyle, and academic performance of Chinese college students and investigate the differences among medical and dental students on their lifestyle. Methods. This study was conducted with 316 students enrolled from 2012 to 2014 at Tongji University. Scores from the college physical test were used to represent the students’ physical fitness condition. Lifestyle was measured by some variables extracted from the students’ behavior data provided by the university’s information center. Academic performance was measured by the average score of basic courses and the average score of professional courses. Demographic information, including age, gender, nation, and family background, was also obtained. Separate multiple linear regression analysis was performed for modeling academic performance and physical fitness with a p value threshold of 0.05. Results. A total of 212 (45.97% females) medical students and 104 (58.65% females) dental students participated in this study. Physical fitness score (medical: r = 0.34, p<0.001; dental: r = 0.47, p<0.001), library visiting frequency (medical: r = 0.30, p<0.001; dental: r = 0.62, p<0.001), number of books borrowed (medical: r = 0.19, p<0.01; dental: r = 0.37, p<0.001), frequency of waking up early (medical: r = 0.29, p<0.001; dental: r = −0.30, p<0.01), and times of eating breakfast (medical: r = 0.49, p<0.001; dental: r = 0.47, p<0.001) were all significantly associated with academic performance. Library visiting frequency (medical: r = 0.26, p<0.001; dental: r = 0.41, p<0.001) and eating frequency (medical: r = 0.48, p<0.001; dental: r = 0.42, p<0.001) were also closely related with physical fitness. Conclusion. Physical fitness, library usage, and the regularity of lifestyle are significant contributors to academic performance among Chinese medical and dental students. Moreover, medical students are shown to have less rest time compared to dental students.

This study aims to identify factors that influence of job performance and career development at the Rehabilitation Center of the Ministry of Defence. Data were collected from 65 respondents using self-administration questionnaires, and path analysis was used to test the hypotheses. The results show that work experience, competencies, and training directly affect performance, but only work experiences that directly affect career development. Finally, performance directly affects employee career development. Implications and suggestions are explained in the paper.

This study aims to analyze the effect of role conflict on employee performance; spiritual intelligence on role conflict; spiritual intelligence on employee performance; and, role conflict mediated by spiritual intelligence on employee performance. Role conflicts include intrarole conflict, intrasender conflict, interrole conflict, and conflicting expectations and requests and incompatible evaluation standards. 108 ASN employees working in four health centers in XYZ city were selected as samples using a purposive sampling method. The results of the study found that conflict of intrasender and conflicting expectations and requests and incompatible evaluation standards had a significant effect on employee performance; and that spiritual intelligence partially mediates the relationship between conflicting expectations and expectations and incompatible evaluation standards on employee performance. Dominant factor that affect employee performance is conflicting expectations and requests and incompatible evaluation standards, so this factor should be a major concern for research objects in realizing their organizational goals.

Penelitian ini merupakan jenis penelitian kuantitatif, yaitu metode penelitian yang berlandaskan pada filsafat positivisme, digunakan untuk meneliti pada populasi atau sampel tertentu, pengumpulan data menggunakan instrument penelitian, analisis data bersifat kuantitatif statistik, dengan tujuan untuk menguji hipotesis yang telah ditetapkan. Penelitian ini dilaksanakan pada Tax Center Universitas Jember. Pada penelitian ini, instrumen penelitian berupa kuesioner. Kuesioner tersebut akan dibagikan kepada responden sejumlah 59 orang relawan pajak tax center Universitas Jember, kemudian akan diolah secara statistik dengan software SPSS. Penelitian ini menghasilkan temuan bahwa secara parsial dan simultan, kompetensi dan pelatihan berpengaruh secara signifikan dan positif terhadap prestasi kerja. Sehingga jika terjadi peningkatan kompetensi dan pelatihan, maka akan meningkatkan pula prestasi kerja. Begitupula sebaliknya, jika terjadi penurunan kompetensi dan pelatihan, maka akan diikuti pula dengan adanya penurunan prestasi kerja. Sebesar 59,8% perubahan yang terjadi pada prestasi kerja Relawan Pajak Tax Center Universitas Jember dijelaskan dengan baik oleh kompetensi dan pelatihan. Kata Kunci : Kompetensi, Pelatihan, Prestasi Abstract - This study aims to determine the effect of competence and training owned by Jember University Tax Center Volunteers on work performance generated. This research is a type of quantitative research, which is a research method based on the philosophy of positivism, used to examine populations or specific samples, collecting data using research instruments, analyzing quantitative quantitative data, with the aim to test the hypotheses that have been set. This research was conducted at the Jember University Tax Center. In this study, the research instrument was in the form of a questionnaire. The questionnaire will be distributed to 59 respondents from the Jember University tax center volunteer, then will be processed statistically with SPSS software. This research produces findings that partially and simultaneously, competence and training have a significant and positive effect on work performance. So that if there is an increase in competence and training, it will also increase work performance. Likewise on the contrary, if there is a decrease in competence and training, it will also be followed by a decrease in work performance. 59.8% of the changes that occur in Jember University's Tax Center Volunteer Tax work performance are well explained by competence and training. Pada tahun 2011, di Provinsi Daerah Istimewa Yogyakarta, Kabupaten Bantul mempunyai jumlah tenaga bidan konselor ASI terbanyak, tetapi cakupan ASI eksklusif masih menempati urutan terendah ketiga. Tujuan penelitian ini untuk menganalisis implementasi kebijakan pemberian ASI eksklusif melalui konseling oleh bidan konselor ASI berdasarkan faktor disposisi dan struktur birokrasi di puskesmas wilayah Kabupaten Bantul. Penelitian deskriptif kualitatif ini mengambil informan penelitian secara purposive dengan informan utama adalah empat orang bidan konselor ASI. Informan triangulasi adalah empat bidan koordinator, Kasie Gizi Dinas Kesehatan Kabupaten Bantul, empat kepala puskesmas, dan 12 orang ibu yang melakukan pemeriksaan kehamilan, nifas dan imunisasi bayi ke puskesmas terpilih. Data dikumpulkan dengan teknik wawancara mendalam. Pengolahan dan analisis data menggunakan analisis isi. Hasil penelitian menemukan implementasi kebijakan pemberian ASI melalui konseling ASI di puskesmas belum berjalan optimal, disposisi/sikap bidan konselor ASI adalah menyetujui tugas memberikan konseling ASI. Dinas Kesehatan Kabupaten Bantul disarankan menyusun SOP pelaksanaan konseling ASI dan puskesmas disarankan melaporkan kinerja bidan konselor ASI ke dinas kesehatan.Bantul is a district that has the highest number of midwives counselor of breastfeeding in Yogyakarta Province with 40 persons but the exclusive breastfeeding coverage of this district was the third lowest in 2011. The study aimed to analyze the policy implementation of exclusive breastfeeding counseling by midwives counselor of breastfeeding based on disposition and bureaucratic structure factors at Public Health Centers in Bantul district. Design of this study was descriptive qualitative using a purposive sampling. The main informants were four midwives counselor of breastfeeding in health centers and triangulation informants namely four head of health centers, four coordinator of midwives, a head of nutrition section and 12 mothers i.e. pregnant women, postpartum mothers and mothers of infants that are immunized at health center. Data were collected from in-depth interview. Processing and analysis of data by using content analysis. The result of this study shows that breastfeeding policy implementation through breastfeeding counseling in health centers is not optimal, the midwives counselor of breastfeeding agreed to do breastfeeding counseling. It is suggested to Bantul Regency Health to formulate standard operating procedures of midwife counselor of breastfeeding and for the health centers to reports the performance of midwives counselor of breastfeeding to regency health.

This case study is a performance measurement system on BNI with PMS (performance measuring system) method is a Balance Scorecard based. Since 2006, BNI's performance measurement system uses only 2 perspectives, namely financial and operational, so it has not presented the comprehensive and integrative performance of the organization. However, starting in 2011, BNI performs Balance Scorecard based performance process using 6 (six) perspectives namely: financial, growth, risk, customer, employee and process. Performance measurement is conducted at 6 (six) level of organization that is: BNI wide, sectoral, division, region, branch and center while for office service and cash office level measurements are still in developmen process. With the development of six perspectives and measurement of all levels, it is expected that BNI performance measurement becomes more objective and linkage. BNI wide performance measurement results at the end of each year with a balanced scorecard approach is expected to be a strategic planning of the bank in determining the performance improvement of the current year so that the scorecard can be maximized.

The purpose of this research is to give further description how leadership factor and organizationalr cultural factor can contribute or related to the performance gained by the tutors in the organization by looking at (a) the relation between leadership style and tutor’s performance, (b) the relation between organizational culture and tutor’s performance, and (c) the relation between leadership style and organizational culture with tutor’s performance. This positivistic research employed survey method and conducted in Community Learning Center (SKB) Limboto, Gorontalo District, as from September through November 2014. The data were collected by distributing questionnaires to the tutors, interviewing informant, and studying the document and then analyzed using path analysis. The result of the study indicates (a) leadership style has significant relation with the tutor’s performance, (b) organization culture has significant relation with tutor’s performance, and (c) between leadership style and organizational culture simultaneously has significant relation with tutor’s performance.

Telkomsel Call Center Office is the call center service office for 24 hours which is facilitated by the telecommunications operator service companies Telkomsel in order to optimize the quality of service to its customers. Service optimisation is certainly influenced by internal company both in terms of the work system as well as where its taken place. One case study is taken, i.e. Telkomsel Call Center Office in Gedung Infomedia Nusantara -Bandung. Interior design on this Office has not applied optimally where affect the satisfaction of workers, mainly call center agents. This often raises the boredom on the agents whom do their work repeatedly to serve the consumers who don't rarely give them complaints or criticism. Using a research method of qualitative descriptive (comparative analysis of the condition in existing interior design with the theory of psychological space (five aspects which can form a good workplace design)), it is evident that Telkomsel Call Center Office interior design is not applied optimally to support satisfaction of workers in order to optimize its performance.

The experimental design used was completely randomized design with three replications. The eel used for this study was <em>Anguilla bicolor bicolor </em>at stadia elver with average body weight of 3 g/fish. Fish were reared for 60 days. Analysis of the blood profile and blood glucose level were done every ten days, while cortisol measurement was performed three times on the day-0, 30, and 60. Results showed that stocking density affected biomass growth. The best stocking density was 4 g/L with the growth of 10.62 g biomass/day, the specific growth of 1.47%, 1.16 feed conversion, and survival of 96.24%. Rancangan percobaan yang digunakan adalah rancangan acak lengkap dengan tiga ulangan. Ikan sidat yang digunakan adalah spesies <em>Anguilla bicolor bicolor </em>stadia elver dengan bobot rata-rata 3±1 g/ekor. Pemeliharaan dilakukan selama 60 hari. Analisis gambaran darah dan glukosa darah dilakukan setiap sepuluh hari sekali, sedangkan pengukuran kortisol dilakukan sebanyak tiga kali yaitu pada hari ke-0, 30, dan 60. Hasil penelitian menunjukkan bahwa perlakuan padat tebar berpengaruh nyata terhadap laju pertumbuhan biomassa. Padat tebar terbaik adalah 4 g/L dengan laju pertumbuhan biomassa 10,62 g/hari, laju pertumbuhan spesifik 1,47%, konversi pakan 1,16, dan sintasan 96,24%. Optical coherence tomography (OCT) is a noninvasive, high-resolution, cross-sectional imaging technique. To date, OCT has been demonstrated in several areas of dentistry, primarily using wavelengths around 1,300 nm, low numerical aperture (NA) imaging lenses, and detectors insensitive to the polarization of light. The objective of this study is to compare the performance of three commercially available OCT systems operating with alternative wavelengths, imaging lenses, and detectors for OCT imaging of dental enamel. Spectral-domain (SD) OCT systems with (i) 840 nm (Lumedica, OQ LabScope 1.0), (ii) 1,300 nm (Thorlabs, Tel320) center wavelengths, and (iii) a swept-source (SS) OCT system (Thorlabs OCS1300SS) centered at 1,325 nm with optional polarization-sensitive detection were used. Low NA (0.04) and high NA (0.15) imaging lenses were used with system (iii). Healthy in vivo and in vitrohuman enamel and eroded in vitro bovine enamel specimens were imaged. The Tel320 system achieved greater imaging depth than the OQ LabScope 1.0, on average imaging 2.6 times deeper into the tooth (n = 10). The low NA lens provided a larger field of view and depth of focus, while the high NA lens provided higher lateral resolution and greater contrast. Polarization-sensitive imaging eliminated birefringent banding artifacts that can appear in conventional OCT scans. In summary, this study illustrates the performance of three commercially available OCT systems, objective lenses, and imaging modes and how these can affect imaging depth, resolution, field of view, and contrast in enamel. Users investigating OCT for dental applications should consider these factors when selecting an OCT system for clinical or basic science studies.

Kualitas pelayanan dapat diartikan sebagai upaya pemenuhan kebutuhan dan keinginan konsumen serta ketepatan penyampaiannya dalam mengimbangi harapan konsumen. Kualitas pelayanan dapat diketahui dengan membandingkan persepsi konsumen atas pelayanan yang mereka terima dengan pelayanan yang diharapkan terhadap atribut-atribut pelayanan suatu perusahaan. Jika jasa yang diterima atau dirasakan sesuai dengan yang diharapkan, maka kualitas pelayanan dipersepsikan baik dan memuaskan, jika jasa yang diterima melampaui harapan konsumen, maka kualitas pelayanan dipersepsikan sangat baik dan berkualitas. Sebaliknya jika jasa yang diterima lebih rendah dari yang diharapkan, maka kualitas pelayanan dipersepsikan buruk. Pada penelitian ini penulis melakukan analisis kepuasan konsumen dengan metode CSI (Customer Satisfaction Index) pada Viva Beauty Center Surabaya guna mengetahui tingkat kepuasan konsumen. Dengan menggunakan metode CSI didapatkan indeks kepuasan pelanggan sebesar 69,64% yang menggambarkan pelanggan Viva Beauty Center puas atas jasa pelayanan tersebut. Namun masih perlu dilakukan perbaikan lagi agar indeks kepuasan pelanggan bisa mendekati 100% demi mencapai loyalitas pelanggan. Untuk mengetahui atribut apa saja yang perlu diperbaiki dilakukan analisis Importance Performance Analysis (IPA) yang menghasilkan beberapa atribut yang harus diperbaiki yakni atribut C1, E4, C2 yang berada di kuadran A dan A2, A4, B1, D1, D2, E1 yang ada dikuadran C.

Pertumbuhan pusat perbelanjaan di kawasan Tangerang menunjukkan adanya tren pusat perbelanjaan berskala regional center. Mall Alam Sutera sebagai pusat perbelanjaan berskala regional dinilai mampu bersaing dengan pusat perbelanjaan lain di kawasan Tangerang. Berdasarkan hasil observasi dan wawancara terhadap pihak pengelola Mall Alam Sutera diperoleh dugaan bahwa adanya faktor-faktor yang mempengaruhi penurunan tingkat kepuasan pengunjung mau pun penyewa, di antaranya image yang kurang menarik, minimnya event/ promosi, tenancy mix, serta faktor keamanan. Dalam hal ini, diperlukan penelitian yang menguji tingkat kepentingan terhadap kepuasan pengunjung serta penyewa demi meningkatkan kinerja pelayanan Mall Alam Sutera. Studi ini bersifat deskriptif kuantitatif dengan menggunakan metode pengukuran likert scale, crosstab, serta importance performance analysis. Jumlah sampel pengunjung ditentukan berdasarkan jumlah populasi rata-rata pengunjung Mall Alam Sutera per hari, sedangkan jumlah sampel penyewa ditentukan berdasarkan populasi penyewa aktif di Mall Alam Sutera pada Bulan September 2016 yang keduanya menggunakan metode Slovin. Ada pun alat bantu yang digunakan dalam penelitian ini adalah lembar kuesioner bagi pengunjung dan penyewa, serta perolehan data dan interview dari pihak pengelola Mall Alam Sutera. Hasil akhir dari penelitian ini diperoleh faktor yang paling dominan terhadap penurunan tingkat kepuasan pengunjung adalah Diversity yaitu minimnya ketersediaan beragam jenis makanan dan minuman, sedangkan untuk penyewa adalah Performance dan Renew yaitu masalah traffic pengunjung yang rendah.Hasil penelitian diharapkan menjadi rekomendasi bagi pihak pengelola Mall Alam Sutera untuk dapat lebih meningkatkan kinerjanya sesuai dengan faktor kepuasan pengunjung dan penyewa.

Various activities have been implemented in the prevention and control of disease in the city of Balikpapan, but based on the Health Profile 2012 field program is known that the achievement of disease control and environmental sanitation is still not maximal. This corresponds to an increase in cases of environment-linked diseases such as DBD, tuberculosis, diarrhea, ISPA and pneumonia each year. The quality of programperformance officer holder is a factor that affects the extent of success in achieving thehealth programs that have been established.The purpose of this study was to determine the relationship between co-operation, in return, infrastructure and support superior to the performance of holders of Disease Prevention program in the city of Balikpapan. This type of research is analytic surveywith cross sectional approach. The population in this study were all employees of the holder of health programs in 27 health centers with a total sampling Balikpapan 54 people. The method used is the analysis of univariate and bivariate.The results showed no significant relationship between co-operation with the performance of the holder of the eradication program (ρ value 0.002), and betweenrewards to the performance of the holder of the eradication program (ρ value 0.027).Suggestions to increase cooperation and support employers through training soft skillsand personality, as well as leadership training management organizations, as well as considering pemberiaan remuneration policy. Thus achieving the health program can be run according to plan.

6516 Background: Medical treatment at the end-of-life (EOL) should be responsive to patient values, but research shows it may instead reflect the regional supply of health services and/or physician practice patterns. We sought to determine whether patients' preferences are associated with EOL medical care and place of death. Methods: Coping with Cancer is an NCI/NIMH-funded, multi-site prospective, longitudinal cohort study of patients with advanced cancer, conducted from September 2002-February 2008. Analyses were based upon 317 deceased patients interviewed at baseline and followed until death, a median of 4.4 months later. EOL care was obtained from chart review, and regressed on patients' baseline preferences for life-extending therapy. Analyses were adjusted for significant confounds; i.e., socio-demographics, cancer type, patient denial, EOL discussion, do-not resuscitate (DNR) order, and treatment center. A sub-analysis examined EOL care within three different medical centers to determine whether patients' preferences and treatment intensity varied by region/practice type. Results: 90 of 317 patients (28.4%) reported a desire to receive life-extending therapy at baseline. Patients' preferences did not differ by proximity to death or performance status, but patients who preferred life-extending therapy had higher rates of denial (75.0% vs. 55.6%, p = 0.002) and were less likely to report having an EOL conversation (24.4% vs. 48.9%, p = 0.0002) or a DNR order (24.4% vs. 48.9%, p < 0.0001). These patients were more likely to undergo ICU admission (AOR 4.6, 95% CI 1.9–11.1) and ventilation (AOR 3.0, 95% CI 1.2–7.3), and die in an ICU (AOR 6.7, 95% CI 2.4–18.7) instead of at home (AOR 0.6, 95% CI 0.3–0.9). A sub-analysis of patients treated at Yale, Parkland, and New Hampshire Oncology-Hematology confirmed that patients' preference for life-extending treatment was associated with higher rates of ICU admission in all three centers. Conclusions: Advanced cancer patients' treatment preferences may play a more important role in determining the intensity of medical care at the EOL than previously recognized. Future research should determine whether these preferences are informed and thus reflect patients' true values for EOL care. No significant financial relationships to disclose.

This study aimed to verify oral health, treatment needs, dental service accessibility, and impact of oral health on quality of life (QL) of subjects from settlement in Pontal do Paranapanema/SP, Brazil. In this epidemiological survey, 180 10-to 19- years old adolescents enrolled in the school that attend this population in settlement underwent oral examination, to verify caries index (DMFT- decayed, missing and filled teeth) and periodontal condition (CPI), and were interviewed using the World Health Organization Quality of Life (WHOQOL-Bref) and Oral Impact Daily Performance (OIDP) instruments to evaluate QL, and the Global School-Based Health Survey (GSHS) about dental service accessibility. DMFT average was 5.49 (± 3.33). Overall, 37.2% of participants showed periodontal problems, mainly CPI = 1 (77.7%). Treatment needs were mainly restorations. GSHS showed that the last dental consultation occurred > 1 year previously for 58.3% of participants at a public health center (78.9%). The average WHOQOL-Bref was 87.59 (± 15.23). Social relationships were related to dental caries and health service type. The average OIDP was 6.49 (± 9.15). The prevalence of caries was high and observed periodontal problems were reversible. The social relationships of adolescents from settlement were influenced by caries and health services type.
