This chapter covers the second stage of labour, from onset through to latent and active phases. It describes the physiology and diagnosis of the second stage of labour for low-risk women. It considers the mechanism of normal labour which underpins the principles of care and conduct of a normal vaginal birth. The care of the perineum in line with the current evidence base is also described. Categorization of perineal trauma is given and the performance of an episiotomy described. The incidence and management of female genital mutilation are discussed.

The principles of postnatal care within this chapter focus on the midwife’s role in monitoring the well-being of the mother through knowledge of the physiological processes of involution of the uterus and the return of the circulatory and excretory systems to their normal parameters. Care of the perineum management of perineal pain and monitoring for any signs of infection or sub-involution are included. The psychological and emotional aspects of postnatal recovery are incorporated, along with parent education, post-operative care, and the subsequent care of women who suffer from a pre-existing medical condition such as diabetes.

Abstract:
Fournier’s gangrene (FG) defines a life-threatening necrotizing fasciitis of the external genitalia and perineum with a peak incidence at 50 years of age. It is associated with a mixed bacterial infection with Escherichia coli and Bacteroides spp. predominating. The diagnosis is made clinically with symptoms of skin necrosis, swelling, pain, crepitus, and feculent odour, and local and systemic signs of severe sepsis. Initial resuscitation with fluid replacement, oxygen therapy, and broad-spectrum empirical antibiotics should be rapidly followed by complete and aggressive surgical debridement with postoperative placement of an occlusive dressing. Urinary and faecal diversions are often required. Debridement should be repeated until all necrotic tissue has been excised, allowing later reconstruction. Early diagnosis and immediate therapy is crucial to improve survival in patients with FG.

Abstract:
The third stage of labour and its various managements are considered in this chapter, initially looking at the importance of informed choice with regard to this period of labour and birth and then the risks associated with this time. The chapter then discusses active management versus physiological management of the third stage; these methods are considered with birth on dry land or in a birthing pool. The chapter then looks at the repair of the perineum, considering the various categories of tears and the care of the woman to be given. The repair is discussed stage by stage, along with diagrams to ensure the learner has clarity with regard to this technical procedure. Finally the examination of the placenta and membranes is covered, looking at the normal and the recognition of the abnormal.

Abstract:
This chapter comprises a midwifery definition and explains the midwife’s role, focusing on the challenges of a changing population demographic with increasing levels of complexity, ill health, and maternal morbidity. Highlights from the latest MBRRACE (2014) report are reflected as is new evidence of the value of midwifery-led care in all settings. Statutory supervision of midwifery in its current model is unique to UK midwifery and there are proposed changes to the regulatory framework pending in 2017. Meanwhile, current supervision arrangements remain in place until regulations are redrafted. Drug administration information is updated and the latest Human Medicines Regulations (2012) are cited along with the list of midwives exemptions. Independent prescribing is included here to indicate how by undertaking further approved training a midwife could be permitted to prescribe medicines promptly and without delay whilst giving care, e.g. prescription of antibiotics for mastitis, infected perineum, or urinary tract infection.
